1. Chemical constituent analysis of EtOAc fraction of Tithonia diversifolia (HEMSL.) A. Gray

Objective: To isolate and identify the chemical constituents of the EtOAc fraction of Tithonia diversifolia. Methods: The dried aerial parts of Tithonia diversifolia were extracted with 80% EtOH,and the chemical constituents of the ethyl acetate portion from the extract were isolated by column chromatography with silica gel, Sephadex LH-20 and MCI. The isolated compounds were identified by NMR and MS analysis. Results: Ten compounds were isolated from EtOAc extract, and identified as, vanillin (1), ergosterol (2), ergosterol peroxide (3), 3-methoxy-4-hydroxy-trans-cinnamaldehyde (4), 3-hydroxybenzaldehyde (5), 7-methylesculetin (6) , 5-hydyoxymethyl furfural (7), nepetin (8) , docosanoic acid 2, 3-dihydroxypropyl ester(9) and 13-hydroxyeupatorochromene(10). Conclusion: Vanillin, ergosterol, ergosterol peroxide, 3-methoxy-4-hydroxy- trans-cinnamaldehyde, p-hydroxybenzaldehyde, 7-methylesculetin, 5-hydyoxymethyl furfural, docosanoic acid 2, 3-dihydroxypropyl ester, and 13- hydroxyeupatorochromene have been isolated from Tithonia diversifolia for the first time.
2.Effect of swimming and caressing on neonatal jaundice and weight

Objective To explore the effect of swimming and caressing on neonatal jaundice and weight.Methods Two hundred and forty newborns were divided equally into control and observation group with random digits table.The former received care with caressing and the latter with swimming and caressing.The two groups were compared in terms of weights and jaundice indices at birth and 7d after birth.Results Seven days after birth,the jaundice index in the observation group were lower than that of the control group and the weights of the observation group were larger(P<0.05).Conclusion Swimming and caressing are helpful for the development of neonates by C-sect and the reduction of jaundice duration.

Objective To study the solute clearance effect of t he new concentrated anticoagulation hemodialysate of citrate for hemodialysis in patients with high risk of bleeding.Methods During the period of 2002-06~2003-12,42 patients with h igh risk of bleeding were divided into two groups according to hemodialysis mann ers.Group Ⅱ were hemodialyzed with new citrate anticoagulation hemodialysate wi thout any other anticoagulant and group Ⅰ were hemodialyzed with bicarbonate he modialysate with dalteparin anticoagulation.The BUN and Cr levels were determine d before and after dialysis and Kt/V and URR were calculated.In addition,ACT and iCa 2+ were also measured at the artery and venous end.Results ACT was extended and iCa 2+ decreased significantl y at the venous end compared with that at the artery end in group Ⅱ (P0.05).Conclusion The solute clearance effect of the new concentrated anticoagulation hemodialysate of citrate for hemodialysis in patients with high risk of bleeding is excellent.
5.CT diagnosis of lung cystic echinococcosis with infections

Objective To analyze the CT findings of lung cystic echinococcosis with infections, and to assess the value of CT examination. Methods The CT characters of 23 cases of pathologically proved lung cystic echinococcosis were analyzed and CT findings were compared with pathology. Results All 23 cases of lung cystic echinococcosis appeared as single cystic lesions. Fifteen cases appeared in the right lung, and the other 8 in the left. The lesions located in lower lobe were more than those in the upper lobe. According to the CT examination, it could be divided into two types: (1) simple lesion (7 cases) appeared as irregular solid mass with different size, crude edge, and lobulation, and with fiber shadows in the lung field around the lesions. In 6 cases, curvilinear or spotty calcification occurred in the lesions. (2) echinococcosis as a bag (16 cases) appeared as a mass with cystic changes in lung tissue with different size and off-center cystic cavity. The lesions took the form of ball or irregular shape. The edge was crude. Around the focus, prickle-like or patchy changes were detected. Ruptured lesions showed “water snake” or “flow ribbon” signs in 4 cases. In 7 cases, calcification occurred on the cystic wall or extended into the inside contents. Conclusion CT could accurately demonstrate the location, appearance, internal structure, and adjacent situation of the cystic echinococcosis, providing valuable information for correct diagnosis and treatment.
6. Identification and bioactivities analysis of Halomonas sp. from seawater

Objective: To determine the 16S rDNA sequences of 36 strains of Halomonas sp. Isolated from the seawater of the East China Sea and to study their bioactivities. Methods: The 16S rDNA sequences of all isolated strains were amplified by PCR, and the obtained sequences were subjected to phylogenetic analyses using MEGA4.1 and multiple sequence alignment with Clustal X1. 8 software. MTT assay was applied to assess the cytotoxic activities of their fermentation broth; ABTS and DPPH antioxidant models were applied to test the antioxidation activities of their fermentation broth. The HLE inhibition activities of the fermentation broth were tested by a stable HLE in hibitor screening model. Results: Analysis of 16S rDNA sequence showed that the 36 strains had a high affinity (96%-99%) to the Halomonas sp. Fermentation broth of the 36 strains showed different degrees of cytotoxic activities, antioxidation activities, and HLE inhibition activities. Eleven strains inhibited the growth of HepG2 cells, with the inhibition rate ranging (4.40±1.2)%to(24.90±3.5) %; 20 strains inhibited the growth of HL-60 cells, with the inhibition rate ranging (1.70±1.1)% to (50.90±4.2) %; 7 strains showed ABTS radical scavenging capacity, with the scavenging rate ranging (4.49±2.1) % to (58.43±4.4) %; 3 strains showed DPPH radical scavenging capacity, with the scavenging rate ranging (5.68±3.7) % to (59.06±3.2) %, and 3 strains showed HLE inhibition activities, with the inhibition rate ranging (12.71±1.81)% to (71.19±5.62) %. Conclusion: All the 36 Halomonassp. Strains show different degrees of cytotoxic effects, antioxidation activities, and HLE inhibition capacities. Several strains with high activities possess the potential for medicinal application.
7.Relationship between the prothrombotic state and leukoaraiosis in elderly patients with metabolic syndrome

Objective To investigate the relationship between the prothrombotic state and leukoaraiosis(LA)in elderly patients with metabolic syndrome.Methods A total of 350 metabolic syndrome patients in the affiliated hospital of Zhejiang University who received treatment from January 2013 to January 2015.According to the results of head MRI,patients were divided into LA group(n=203)and non-LA group(n=147).The severity of LA was graded by its MRI appearance and the Blennow scale.The plasma levels of fibrinogen (Fib),prothrombin time (PT),thrombin time (TT),activated partial blood coagulation time(APTT),D-dimer (D-D) and yon willebrand factor (vWF) were measured and compared between the two groups.Results The levels of Fib,vWF and D-D were higher in LA group than in non-LA group(all P<0.05).Spearman correlation analysis showed that the severity of leukoaraiosis was positively correlated with Fib,vWF and D-D(r=0.014,0.089,0.215,respectively,all P < 0.05).Multivariate linear regression analysis showed that Fib,course of hypertension,vWF and HbA1c were the main influencing factors with the standardized regression coefficient of 2.516,0.312,0.206 and 0.167 respectively for leukoaraiosis in elderly patients with metabolic syndrome.Conclusions The course of hypertension and HbAlc level are positively related with LA.The prothrombotic state markers including Fib and vWF can be considered as the predictors for LA severity in elderly patients with metabolic syndrome.
8.Construct a Harmonious Physician-Patient Relationship to Achieve the Win-Win Pattern between Doctors and Patients

The currently unfavorable physician - patient relationship has lead to a total loss of interests from multiple sides of hospitals,patients,medical staff,medical education and the society,which specifies in the wellbeing loss of patients and the whole population.The social stability has also been affected.The health care reform should aim to minimize the conflicts between doctors and patients,thus the above mentioned losses will be diminished and a harmonious physician - patient relationship will be built up and the win - win pattern of benefits between doctors and patients will also be realized.Multiple efforts from the government,society,hospitals,medical staff,and patients are all needed to achieve such a goal.
9.Effect of Meibomian gland massage before phacoemulsification on ocular surface after surgery in patients with Meibomian gland dysfunction

?AIM:To study the changes of ocular surface in cataract patients with Meibomian gland dysfunction ( MGD) who treated with Meibomian gland massage before surgery.?METHODS: Totally 90 patients ( 93 eyes ) with cataract and MGD were randomly divided into two groups. Patients in experimental group were treated with hot compress and Meibomian gland massage every day before surgery, while the patients in the control group were not taken. Two groups of patients received phacoemulsification combined with intraocular lens implantation. Lid margin abnormalities, secretion characteristics, Schimer I test ( SⅠt) and tear film break-up time ( BUT ) were recorded and compared between two groups using slit lamp microscope inspection preoperatively and postoperatively 1wk.?RESULTS:In the control group, the postoperative score of eyelid margin and Meibomian gland secretion was increased significantly than preoperative, while the value of BUT and SⅠt was down significantly. Compared with control group, the postoperative score of eyelid margin shape and Meibomian gland secretion in the experimental group was decreased significantly, while the value of BUT and SIT improved significantly (P<0. 05).?CONCLUSION: Phacoemulsification can aggravate the Meibomian gland dysfunction and have some extent of effects on the ocular surface. Meibomian gland massage before surgery can significantly improve the function of Meibomian gland and the state of ocular surface in patients with MDG.
10.Expression of bone morphogenetic protein in sclera of form deprivation myopic eye

Background It is well known that sclera remodeling occurs during axial elongation in myopia under the control of growth hormone or its downstream effectors.The role of transforming growth factor-β (TGF-β) in myopia has been determined in previous studies.Bone morphogenetic protein (BMP) is one of members of the TGF-β superfamily,but if it plays an important role in the genesis and development of myopia is not completely clear.Objective This study was to identify the presence of BMPs in normal guinea pigs sclera and investigate the change of BMPs in the sclera in form-deprivation myopia (FDM) of guinea pigs.Methods Thirty young guinea pigs were randomized into normal control group and experimental group using table of random number.FDM models were established by occluding unilateral eyes of guinea pigs with a translucent lens for 14 days in the experimental group,and the fellow eyes served as the controls.Diopter of all eyes was tested by retinoscopy optometry,and ocular axial length was measured by A-sonography before and after modeling.Posterior sclera tissue of the animals was obtained on 14 days,and the relative expression level of BMPs mRNA and protein were assayed by reverse transcription PCR (RT-PCR) and Western blot.The use and care of the animals complied with ARVO Statement.Results On 14 days after occluding of unilateral eyes,the refraction diopter of the experimental group was (-0.48±0.51) D,and that of the fellow eyes was (3.22 ±0.34) D,showing a significant difference between them (t =-12.814,P =0.000).Also,a significant difference in the diopter was seen between the experimental group and normal control group ([-0.48±0.51]D vs.[2.97±0.70]D,t =-11.878,P=0.000).Axial length was (8.30 ± 0.05) mm in the experimental group,(8.11 ±0.06) mm in the fellow eyes and (8.06±0.06) mm in the normal control group,showing a significant increase in the experimental group compared with the fellow eyes and normal control group (t =7.230,P =0.000 ; t =9.084,P=0.000).The expressions of BMP-2 mRNA,BMP-4 mRNA,BMP-5 mRNA in posterior sclera were detected in the normal guinea pigs.Fourteen days after the induction of myopia,the relative levels of BMP-2 mRNA and BMP-5 mRNA in sclera were 0.41 ± 0.11 and 0.65 ± 0.06 in the experimental eyes,which were significantly lower than 0.62 ± 0.07 and 0.84 ± 0.03 in the fellow eyes with the descent range of 34.48% and 23.67% respectively (t=2.838,P=0.017; t=2.524,P=0.028).The relative values of BMP-2 protein and BMP-5 protein were 0.44±0.06 and 0.70±0.05 in the experimental eyes,and those of the fellow eyes were 0.61±0.05 and 0.82±0.03,showing significant decline in the experimental eyes with the lowing range of 23.42% and 15.21%,respectively (t =2.465,P =0.030;t =2.445,P=0.031).No significant differences were found in the expression of BMP-4 mRNA and protein in posterior sclera between the experimental eyes and the normal control eyes (mRNA:t =0.704,P=0.460;protein:t=0.987,P=0.365).Conclusions The expressions of the BMP-2 and BMP-5 in sclera down-regulate significantly in FDM eyes,which suggest that BMP-2 and BMP-5 participate in sclera remodeling during myopia induction.
