1.Laparoscopic cholecystectomy versus open cholecystectomy: Clinical efficiency and costs analysis

Objective To compare the costs and effectiveness between the laparoscopic cholecystectomy (LC) and the open cholecystectomy (OC). Methods Clinical data of 97 patients treated either by LC (n=41) or OC (n=56) were retrospectively reviewed. Results The hospital stay, postoperative hospital stay, hospitalization expenditure and medical incomes in Group LC and Group OC were (11.4?3.0) days and (15.1?4.2) days (t=-4.811,P=0.000), (6.1?0.9) days and (10.7?2.4) days (t=-11.673,P=0.000), (8 723.8?1 077.2) yuan and (6 423.4?1 398.4) yuan (t=8.791, P=0.000), (6 236.4?591.0) yuan and (3 426.6?683.2) yuan (t=21.162,P=0.000), respectively. There were no statistical significances between the two groups in preoperative hospital stays and occurrences of complications (t=1.684, P=0.095;?2=0.434,P=0.510). Conclusions Compared with the OC, health resources were more properly utilized but costs were higher in the LC.

The public welfare of public hospitals is determined by the social attributes of its public sector,which is a manifestation of public welfare in health management.The public welfare of public hospitals is conceptual,which is existed by the specific form,thus affects people's awareness about the public welfare in public hospitals.The paper introduced the concept of public welfare of public hospitals,analyzed the characteristics of public welfare in public hospitals,and summarized the manifestations of public welfare and the intervention policies on public welfare in public hospitals since the reform of public hospitals in China.Then the paper put forward the suggestions on the development path of public welfare in public hospitals.

5.A review on reporting guidelines of clinical research in evidence based medicine.

Good clinical practice should be based on evidence. Evidence quality should be based on critical appraisal in evidence based medicine (EBM). Evaluation of evidence quality plays an important role in evidence level clarifying, which is the core of EBM. Different recommendations for clinical practice often derive from evidence levels. Thus evidence quality evaluation is the first and most important step in EBM. There are lots of standards to evaluate evidence quality in the world. However there are two aspects of the evaluation, one is methodological evaluation and the other is reporting evaluation. This article collected a series of standards for clinical trials quality evaluation according to different research designs. It is hoped that the resource and introduction about the quality evaluation of clinical trials be helpful for medical researchers in China. Only being familiar with all kinds of standards of methodology and reporting, researchers could publish high quality scientific papers.

7.Effects of electroacupuncture on Wnt-β-catenin signal pathway in annulus fibrosus cells in intervertebral disc in rats with cervical spondylosis.

OBJECTIVETo observe the effects of electroacupuncture (EA) at "Dazhui" (GV 14) on Wnt-β-catenin signal pathway in annulus fibrosus cells in intervertebral disc in rats with cervical spondylosis.
METHODSForty SD rats were randomized into a control group, a model group, an EA group and a medication group, 10 rats in each one. Rats in the control group were treated with sham operation, only incision on local skin; rats in the remaining groups were made into cervical spondylosis models. After model establishment, rats in the control group and model group received fixed treatment under identical condition; rats in the EA group were treated with EA at "Dazhui" (GV 14), 30 min per treatment; rats in the medication group were treated with intragastric administration of meloxicam tablets. Treatments were both given once a day, and 14 days were taken as one session; there was an interval of 2 days between two sessions, and totally two sessions were given. After the treatments, immunohistochemistry was applied to measure the expression of Wnt, glycogen synthase kinase-3β (GSK-3β) and Axin in annulus fibrosus cells; western blot was used to test the expression of P-β-catenin.
RESULTSIn the control group, there were more positive cells of Wnt, GSK-3β and Axin, which were intensively distributed, deeply colored, and strongly positive; In the model group, there were less positive cells of Wnt, GSK-3β and Axin, which were sparsely distributed and weakly positive. The expression of Wnt, GSK-3β, Axin and P-β-catenin in the model group was less than that in the control group (all P < 0.05); expression of Wnt, GSK-3β, Axin and P-β-catenin in the EA group and medication group was higher than that in the model group (all P < 0.05); expression of Wnt, GSK-3β, Axin and P-β-catenin was not significantly different between EA group and medication group (all P > 0.05).
CONCLUSIONEA could delay the degeneration of intervertebral disc, which may be related to EA inhibiting signal pathway of Wnt-β-catenin.

8.A magnetic resonance imaging study of the gray matter configuration in amnestic mild cognitive impairment patients

Objective To investigate the cortex gray matter configuration in amnestic mild cognitive impairment(aMCI)patients using MRI technology, FMRIB software library(FSL)and Freesurfer software.Methods Twenty aMCI patients and 20 normal control subjects were recruited and studied. They were matched by age, sex and education. All the patients and healthy volunteers underwent MRI scan using SEMENTS trio 3.0 T MRI. The subtile three-dimensional brain images were obtained using high resolution scanning technique. The imaging data was processed and analyzed with FSL and Freesurfer software. The cortex gray matter density and thickness in different brain areas of aMCI patients and normal control subjects were calculated and compared using statistic analysis. Results Compared to that in the controls, cortex gray matter density in the aMCI patients showed remarkable decreases in left frontal lobe, temporal lobe,parietal lobe, and slight decrease in right thalamus, temporal lobe and island lobe; For cortex thickness,aMCI patients showed significant decreases in left anterior cingulate gyrus((2. 19 ±0. 24)mm), inferior parietal lobe((2. 27 ± 0. 15)mm), bilateral parahippocampal gyrus((2. 03 ± 0. 15),(2. 04 ±0. 17)mm), precentral gyrus((2. 20 ± 0. 11),(2. 31 ± 0. 19)mm), postcentral gyrus((1.88 ± 0. 11),(1.82 ± 0. 09)mm), superior frontal gyrus((2. 42 ± 0. 34),(2. 40 ± 0. 28)mm), middle frontal gyrus ((2.31±0.31),(2.33 ±0.29)mm), supramarginal gyrus((2.53 ±0.33),(2.55 ±0.23)mm),temporal pole((3.41 ±0.68),(3.30 ±0.56)mm)and transverse temporal gyrus((2.04 ±0. 12),(2. 01 ± 0. 11)mm; t = 2. 13-3.75, P < 0. 05), no significant changes in the other areas(t = 0. 09-1.88, P > 0. 05). Conclusions Our results suggest that there are significant changes in gray matter configuration in cortex of aMCl patients. The changes of cortical thickness is earlier than the changes of gray matter density.
10.Therapeutic Observation of Suspended Moxibustion plus Chinese Medication for Icteric Hepatitis

Objective To observe the clinical efficacy of suspended moxibustion plus Chinese medication in treating icteric hepatitis. Method Ninety-two patients with icteric hepatitis were randomized into a treatment group and a control group, 46 in each group. The treatment group was intervened by suspended moxibustion plus oral administration of Chinese medication for supplementing yang, tonifying spleen, expelling dampness, and activating blood circulation;the control group was intervened only by the same Chinese medication. The symptom and sign score, and liver function items including TBil and ALT were observed, and the clinical efficacy was compared. Result The symptom and sign scores were significantly changed in both groups after intervention (P<0.05). After intervention, there was a significant difference in comparing the symptom and sign score between the two groups (P<0.05). The TBil and ALT levels were significantly changed after intervention in both groups (P<0.05). The recovery rate and total effective rate were respectively 87.0%and 95.7%in the treatment group versus 76.1%and 82.6%in the control group, and the inter-group differences were statistically significant (P<0.05). Conclusion Suspended moxibustion plus Chinese medication is an effective method in treating icteric hepatitis.
