1.The value of Fisher discriminant analysis in the diagnosis of ectopic pregnancy

Objective To discuss the value of Fisher discriminant analysis of serum progesterone and the growing rate of β-human chorionic gonadotropin in the prediction of early ectopic pregnancy. Methods 66 patients with ectopic pregnancy (11 cases were successfully treated expectantly and 55 cases were treated surgically including 40 cases of rupture of fallopian tube and 15 cases of tubal abortion) and 55 patients with intrauterine pregnancy and 50 patients with threatened abortion were chosen. Serum progesterone,β-HCG,48 hβ-HCG and the 48 h growing rate of β-HCG in each group were measured and a Fisher discriminant analysis was used. Results The serum progester-one was (30.27± 18.20) nmol/L in ectopic pregnancy group,( 108.44±23.27 ) nmol/L in intrauterine pregnancy group and (91.68±34.90) nmol/L in threatened abortion group. The first β-HCG was ( 3767.63 ± 3530.38 ) U/L in ectopie pregnancy group,(29 028.65 ± 10 874.01 )U/L in intrauterine pregnancy group and (13 457.47±16 367.65)U/L in threatened abortion group. The second β-HCG was (4349.24±3536.22)U/L in ectopic pregnancygroup,(56 139.46 ± 23 296.87 ) U/L in intrauterine pregnancy group and (23 270.63 ± 23 811.68 ) U/L in threat-ened abortion group. The growing rate of β-HCG ( β-HCG/the first serum β-HCG) was 1.29 ± 0.28 in ectopic preg-nancy group,1.93 ± 0.36 in intrauterine pregnancy group and 1.97±0.28 in threatened abortion group. There was significant difference in serum progesterone,the first β-HCG and the second β-HCG as well as the growing rate of β-HCG among the groups(P<0.05 or <0.01). Fisher discriminant analysis of combing progesterone and the growing rate of β-HCG were connected with diagnosis of ectopic pregnancy,however,the only one serum β-HCG was not con-nected with diagnosis of ectopic pregnancy. 98.5% of ectopic pregnancy,65.6% of intrauterine pregnancy and 64.0% of threatened abortion were correctly classified in the Fisher discfiminant analysis,with overall correct rate of 77.8%. Conclusion Fisher discriminant analysis of combing progesterone and the growing rate of β-HCG can bet-ter predict the early ectopic pregnancy.
2.Treatment of iatrogenic biliary-tract injury: 22 cases report

ObjectiveTo explore the methods of prevention and treatment of the iatrogenic biliary-tract injury.Methods22 cases with the iatrogenic biliary-tract injury were analyzed retrospectively.ResultsModified Roux-en-Y choledochojejunostomy was performed in 11 cases, end-to-end anastomosis of bile duct in 4 cases, repair of bile duct in 2 cases, a drainage of the inter bile duct and the abdominal cavity in 5 cases. Only 1 case of common bile duct injury after laparoscopic cholecystectoy was death from the toxic shock, another case of bile duct injury after conventional cholecystectoy with the drainage of abdominal cavity was death from multiple system organ failure, all of the other patients were cured.ConclusionThe iatrogenic biliary-tract injury may be causes variously, and should be discovered as soon as possible and be operated in time.
3.The effect of ART in the prevention of dental caries in Chinese school children

Objectives: To assess the effects of ART in the preventio n of dental caries in school children.Methods: Standard instrument s and procedures of ART were used in 191 fissure seal in 140 children aged 12~1 3 years. The material used was a high-strength glass-ionomer material (Ketac-M olar, ESPE). The treatments were evaluated annually after ART by clinical examin ation.Results: The cumulative 1-,2- and 3-year retention rat es of the sealant were 89.6%,78.8% and 71.9% respectively. The incidence of recu rrent caries 2 years and 3 years after ATR was 1.6% and 2.8% respectively. Conclusion: ART approach for preventing tooth decay in school childre n is effective.
4.The value of quantitative parameters of diffusion weighted imaging in differentiating the nature of ovarian epithelial tumors

Objective To investigate the value of minimum apparent diffusion coefficient(ADCmin)and delta apparent diffusion coefficient(dADC)of quantitative parameters of MR diffusion weighted imaging(DWI)in differentiating benign from stage Ⅰ malignant ovarian epithelial tumors.Methods A total of 112 patients with primary ovarian epithelial tumors confirmed by operation and pathology(43 benign tumors and 69 stage Ⅰ malignant tumors)were retrospectively selected.All patients underwent routine MRI and DWI before operation.The ADCmin and dADC of solid portion were measured and compared between the two groups.Logistic regression anal-ysis and receiver operating characteristic(ROC)curve were used to evaluate diagnostic performance of each parameter alone and in combination in differentiating ovarian epithelial benign and stage Ⅰ malignant tumors.Results The ADCmin was greater in benign tumors than that in stage Ⅰ malignant tumors,while dADC was smaller in benign tumors than that in stage Ⅰ malignant tumors(P<0.05).Logis-tic regression analysis showed that ADCmin and dADC were independent risk factors for stage Ⅰ malignant tumors.With threshold of 1.183 × 10-3 mm2/s for ADCmin and 0.579 × 10-3 mm2/s for dADC,the area under the curve(AUC)was 0.802 and 0.857,sensitivity and specificity of 0.885,0.808,0.714,0.857,respectively.According to regression equation logit(P)=7.154+0.267 × dADC-1.016 X ADCmin,the AUC(0.951)was greater than that of single parameter,the sensitivity was 0.890,the specificity was 0.995.Conclusion ADCmin and dADC are useful for differentiating ovarian epithelial benign from stage Ⅰ malignant tumors,and ADCmin combined with dADC can improve the diagnose accuracy before operation.
5.Current situation and performance evaluation of nursing human resource management in 37 public hospitals in China

Nursing human resource is an important part of health human resource and the basis of ensuring nursing quali-ty.Effective nursing performance management can mobilize nurses'subjective initiative,improve work efficiency and enhance nursing human resource management level.At present,the exploration of nursing performance management in public hospitals in China is in the initial stage,and the management mode still refers to the enterprise human resource management mode,without fully considering the particularity of medical work,leading to many problems in the current nursing performance salary manage-ment.Through questionnaire survey,this paper understands the current situation of nursing human resources and performance man-agement methods of comprehensive public hospitals in different regions of the country,analyzes the problems existing in nursing per-formance management and their causes,and puts forward reasonable suggestions for reform direction.In order to provide reference for the reform of nursing performance management in public hospitals,make the distribution of nursing performance much fair and reasonable,improve the quality of nursing service,and promote the sustainable development of nursing management.
6.Three applications and the challenge of the big data in otology.

With the expansion of human practical activities, more and more areas have suffered from big data problems. The emergence of big data requires people to update the research paradigm and develop new technical methods. This review discussed that big data might bring opportunities and challenges in the area of auditory implantation, the deafness genome, and auditory pathophysiology, and pointed out that we needed to find appropriate theories and methods to make this kind of expectation into reality.
