1.Relationship between mathematical characteristics of cardiotocography in electronic fetal monitoring and neonatal acidemia

Objective To explore the relationship between mathematical characteristics of cardiotocograph in electronic fetal monitoring (EFM) and umbilical blood acidosis,and help to evaluate the clinical significance of indeterminate EFM.Methods A total of 85 vaginal delivery cases (or transfer to caesarean section during labor) hospitalized in Peking University Third Hospital during February 4 to December 5,2011 was enrolled in this retrospective study.EFM monitored 0.5-1 hour before delivery was recorded and the mathematical characteristics as the baseline,deceleration area per unit time (cm2/min),deceleration duration ratio and some other variables of EFM were analyzed.All cases were divided into acidosis (n=12) and non-acidosis group (n=73) based on the umbilical blood gas results.Rank sum test or t-test were used to compare the difference on mathematical characteristics between these two groups.Results The deceleration duration ratio and deceleration area per unit time in acidosis group were 0.56±0.17 and 0.45 cm2/min (0.38-0.79 cm2/min),which were higher than those in non acidosis group [0.42±0.14 and 0.30 cm2/min (0.19-0.40 cm2/min)],the differences were statistically significant (t=2.889,Z=2.587,both P<0.05).There were no statistically significant differences in the number of mild variable deceleration and severe deceleration per unit time between acidosis group and non acidosis group [(0.44± 0.14)/min vs (0.41±0.13)/min,t=1.318,P>0.05; 0.033/min (0.016-0.062/min) vs 0.016/min (0.008-0.033/min),Z=-1.596,P>0.05].Conclusions The deceleration duration ratio and deceleration area per unit time can be used to assist the interpretation of indeterminate EFM.
2.Analysis of ROC curves for the effect of predicting hand grip strength based on morphological indicators in high school girls

Objective:
To evaluate and compare the accuracy of body mass index (BMI), waist circumference (WC), waistcorrected BMI (wBMI), and waist to height ratio (WHtR) in predicting hand grip strength (HGS) in high school girls, so as to provide the reference for evaluating their overall physical fitness and health status.
Methods:
From May to December in 2022, a total of 1 600 girls aged 15 to 18 years old were selected by cluster random sampling from 12 high schools in Xuzhou, Jiangsu Province. Measures of HGS, waist circumference, height and weight among high school girls were taken, and their BMI, wBMI and WHtR were calculated. Pearson correlation analysis and Logistic regression were used to analyze the association between BMI, wBMI, WC, WHtR and HGS Receiver operating characteristic (ROC) curves were constructed to evaluate the accuracy of different indicators in predicting HGS
Results:
Pearson correlation analysis showed that the BMI, wBMI, WHtR and HGS expression of high school girls were positively correlated ( r =0.31, 0.27, 0.32, 0.14, P < 0.01 ). Logistic regression analysis showed that BMI, WC and wBMI were correlated with HGS expression ( OR=2.00, 0.47, 1.22, P <0.05), while the association of WHtR and HGS expression had no statistical significance ( OR=0.51, P >0.05). The area under the curve (AUC) of BMI for predicting HGS expression in high school girls was the largest (AUC=0.65, 95% CI =0.61-0.69), followed by the AUC of wBMI for predicting HGS expression (AUC=0.65, 95% CI =0.57-0.66)( P <0.05). The most sensitivity was wBMI (71.5%), followed by BMI (69.4%), and the least sensitivity was WHtR (52.3%); and WC had the best specificity (61.1%), whereas wBMI had a relatively low specificity (52.5%).
Conclusion
For high school girls, BMI, WC and wBMI could predict the strenth of HGS expression, but BMI and wBMI have the highest accuracy and sensitivity, while wBMI has the lowest specificity in predicting the strenth of HGS.
3.Virtual reduction and design of 3D printed guide template for comminuted patellar fractures

Objective:To explore the role of virtual reduction and design of 3D printed guide template in assisting reduction and internal fixation of comminuted patellar fractures.Methods:A retrospective study was conducted of 12 patients who had been treated for closed patellar fractures from March 2016 to April 2019 at Department of Orthopaedics, Jiangxi Provincial People's Hospital. They were 8 males and 4 females with an average age of 35.4 years (from 22 to 51). All their fractures were type 34-C3 (more than 3 fragments) according to the AO/OTA classification. Their preoperative CT data were imported into software Mimics to print a full patella, upper and lower halves of the patella, and a guide template for reduction. During operation, bone fragments were first temporarily fixated with a fine Kirschner wire after combined together with the assistance of reduction templates for upper and lower hemi-patellas. Next, the whole patella was precisely reset and definitely fixated using patellar ring ligation and tension band wire with the assistance of the reduction guide template for full patellar. Recorded were operation time, fluoroscopic frequency, and visual analogue scale (VAS), knee range of motion (ROM) and knee function B?stman scores at the last follow-up.Results:In this group of patients, operation time averaged 90 minutes (from 75 to 120 minutes), fluoroscopic frequency 4.5 times (from 3 to 8 times), follow-up duration 19 weeks (from 16 to 22 weeks), and fracture healing time 14 weeks (from 11 to 17 weeks). At their last follow-up, X-ray showed good patellar morphology, fine internal fixation positions and an average VAS pain scores of 0.3 (from 0 to 1), their knee joints could be fully straightened, their knee ROM averaged 130°(from 100° to 138°), and their knee B?stman scores 28.8 points (from 21 to 30 points), giving 9 excellent and 3 good cases.Conclusion:3D printed reduction guide templates can significantly improve the efficiency and quality of surgical reduction in comminuted patella fractures, enhancing fixation strength and facilitating early and full range exercise of the knee joint to achieve excellent recovery.
4.Application progress of Yttrium-90 microsphere selective internal radiation therapy on metastatic liver cancer

Yttrium-90 microsphere selective internal radiation therapy (90Y-SIRT) has been widely used in the treatment of malignant liver cancers, and its safety and efficacy have been fully validated. As an important local therapeutic method, 90Y-SIRT has been utilized for the treatment of multiple metastatic liver cancers in many cancer diagnosis and treatment centers at home and abroad, demonstrating significant therapeutic potential. This article reviews the basic principle, development history, indications and contraindications of 90Y microsphere, as well as the specific applications in treatment of different types of metastatic liver cancers, aiming to provide references for further research and optimization of treatment strategies.
5.Virome in immunodeficiency: what we know currently.

Over the past few years, the human virome and its complex interactions with microbial communities and the immune system have gained recognition as a crucial factor in human health. Individuals with compromised immune function encounter distinctive challenges due to their heightened vulnerability to a diverse range of infectious diseases. This review aims to comprehensively explore and analyze the growing evidence regarding the role of the virome in immunocompromised disease status. By surveying the latest literature, we present a detailed overview of virome alterations observed in various immunodeficiency conditions. We then delve into the influence and mechanisms of these virome changes on the pathogenesis of specific diseases in immunocompromised individuals. Furthermore, this review explores the clinical relevance of virome studies in the context of immunodeficiency, highlighting the potential diagnostic and therapeutic gains from a better understanding of virome contributions to disease manifestations.

6.Exploring the Essential Factors of Applying the Consensus Methods in the Development of Traditional Chinese Medicine Guidelines: A Qualitative Interview

This study delves into the pivotal factors influencing the consensus process within traditional Chinese medicine guideline development, with the objective of augmenting the quality of this process through methodological recommendations aimed at elevating standardization. Semi-structured qualitative interviews were used to interview guideline leaders, working groups and consensus groups to explore the pertinent elements impacting the credibility of consensus and gather insights into the constitution and progression of the consensus methodology. The study encompassed interviews with 26 participants, yielding 212 codes that were subsequently categorized into five domains: establishment of the consensus group, integration of patient participation, adeptness of the meeting moderator, preparation for consensus formulation, and overarching factors influencing consensus. The research distilled three fundamental phases for forming a consensus group and delineated 17 fundamental tenets for applying the consensus methodology. In forthcoming guideline development endeavors, it is advisable to bolster methodological training ahead of the consensus process while ensuring comprehensive engagement of methodologists. Encouraging experts to navigate differences judiciously and prioritizing meticulous methodology and evidentiary groundwork are recommended. The process should involve openly disclosing the selection of consensus group members, heightening the involvement of patients, and effective management and disclosure of conflicts of interest. This collective approach helps curtail bias, enhance transparency, bolster reliability, and fortify the scientific rigor of consensus outcomes.
