1.Appraisement of the operative therapeutical effects of salpingocatheterism for refractory otitis media

Objective:The therapeutical effects of salpingocatheterism and the states of tubal function were studied after different middle ear operations and within different middle ear pathological changes.Method:Total 71 patients with tubal occlusion, 18 of myringoplasty, 20 of reconstruction of ossicolar chain followed tympanoplasty, 14 of combined approach tympanoplasty, 10 of radical mastoidectomy followed tympanoplasty, 9 of re-radical mastoidectomy followed tympanoplasty were implemented salpingocatheterism. The patients were followed up for evaluation of the postoperative therapeutical effects and the state of tubal function. Result:The causes of operation failure in different middle ear operations or different middle ear pathological changes had no significantly difference. The major cause for postoperative tympanic membrane perforation and tubal occlusion was the pathogenic damage being not cleared up thoroughly and/or there being nasal cavity or pharyngeal cavity pathogenic changes accompanied.Conclusion:Salpingocatheterism is effective for preventing eustachian tube postoperative adhesion.
2.Nitinol shape memory alloy to construct auditory canal in aural atresia surgery of 8 cases

BACKGROUND: Methods to effectively overcome re-stenosis or atresia following reconstruction of external auditory canal are explored.OBJECTIVE: To explore the effect of NiTi shape memory alloy netlike bracket to prevent the restenosis or atresia of newly constructed external ear canal in external canal atresia surgery. DESIGN, TIME AND SETTING: Case analysis of aural atresia patients from Department of Otorhinolaryngology Head and Neck Surgery, Third Hospital of Sun Yat-sen University was performed from January 2001 to June 2003. PARTICIPANTS: Eight cases of aural atresia were selected, including 5 patients with congenital aural atresia, and 3 with postnatal aural atresia. All cases had different degrees of conductive hearing loss. METHODS: All cases underwent external canals reconstruction by using NiTi shape memory alloy netlike brackets. MAIN OUTCOME MEASURES: Diameter of newly constructed aural canal and pure tone auditory threshold. RESULTS: All the eight ears had a smooth and wide external ear canal during follow up. No restenosis of canal or cicatricial contracture was found. Pure tone auditory threshold of 7 ears had improved postoperatively. CONCLUSION: NiTi shape memory alloy netlike bracket has definite effect to prevent the restenosis or atresia of new constructed external ear canal.
3.Statins decreases expression of five inflammation-associated microRNAs in the plasma of patients with unstable angina

Objective:To explore the influence of treatment with HMG-CoA reductase inhibitors ( sta-tins) on the expression profile of microRNAs ( miRNAs) in the plasma of patients with unstable angina ( UA) .Methods:The Taqman low-density miRNA array ( TLDA) and significance analysis of microar-rays ( SAM) were used to identify distinct miRNA expression profiles in the plasma of UA patients treated with long-term and regular statins ( UA receiving statins , n=6 ) compared with UA patients who had not received statins therapy before ( UA received no statins , n=6 ) .These differentially expressed miRNAs discovered in the profiling were further validated by real-time PCR in another 20 controls with non-cardiac chest pain , 26 UA patients received no statins , and 19 UA patients received statins .Results: By using TLDA and SAM , significantly decreased expression levels of 21 miRNAs were observed in the UA pa-tients receiving statins compared with those who received no statins ( fold change >3 and false discovery rate<0 .0001%) .The unsupervised hierarchical clustering based on miRNA expression clearly separa-ted the UA patients receiving statins from those who received no statins .Consistent with the profiling da-ta, the levels of 5 inflammation-associated miRNAs (miR-106b, miR-21, miR-25, miR-451, and miR-92a) were down regulated (P<0.05) in the UA patients receiving statins compared with those who re-ceived no statins.Conclusion: A group of inflammation-associated miRNAs, consisting of miR-106b, miR-21, miR-25, miR-451, and miR-92a, could be decreased by treatment with statins and may be used as a novel biomarker for effectiveness of statins therapy in patients with UA .
4.Production of coenzyme Q10 by metabolically engineered Escherichia coli.

Coenzyme Q10 (CoQ10) is a lipophilic antioxidant that improves human immunity, delays senility and enhances the vitality of the human body and has wide applications in pharmaceutical and cosmetic industries. Microbial fermentation is a sustainable way to produce CoQ10, and attracts increased interest. In this work, the native CoQ8 synthetic pathway of Escherichia coli was replaced by the CoQ10 synthetic pathway through integrating decaprenyl diphosphate synthase gene (dps) from Rhodobacter sphaeroides into chromosome of E. coli ATCC 8739, followed by deletion of the native octaprenyl diphosphate synthase gene (ispB). The resulting strain GD-14 produced 0.68 mg/L CoQ10 with a yield of 0.54 mg/g DCW. Modulation of dxs and idi genes of the MEP pathway and ubiCA genes in combination led to 2.46-fold increase of CoQ10 production (from 0.54 to 1.87 mg/g DCW). Recruiting glucose facilitator protein of Zymomonas mobilis to replace the native phosphoenolpyruvate: carbohydrate phosphotransferase systems (PTS) further led to a 16% increase of CoQ10 yield. Finally, fed-batch fermentation of the best strain GD-51 was performed, which produced 433 mg/L CoQ10 with a yield of 11.7 mg/g DCW. To the best of our knowledge, this was the highest CoQ10 titer and yield obtained for engineered E. coli.

5.Modulating expression of key genes within β-carotene synthetic pathway in recombinant Escherichia coli with RBS library to improve β-carotene production.

β-carotene belongs to carotenoids family, widely applied in pharmaceuticals, neutraceuticals, cosmetics and food industries. In this study, three key genes (dxs, idi, and crt operon) within β-carotene synthetic pathway in recombinant Escherichia coli strain CAR005 were modulated with RBS Library to improve β-carotene production. There were 7%, 11% and 17% increase of β-carotene yield respectively after modulating dxs, idi and crt operon genes with RBS Library, demonstrating that modulating gene expression with regulatory parts libraries would have more opportunities to obtain optimal production of target compound. Combined modulation of crt operon, dxs and idi genes led to 35% increase of β-carotene yield compared to parent strain CAR005. The optimal gene expression strength identified in single gene modulation would not be the optimal strength when used in combined modulation. Our study provides a new strategy for improving production of target compound through modulation of gene expression.

6.Treatment and prognosis of differentiated invasive thyroid carcinoma

Objective To evaluate the clinical characters, management and prognostic factors of patients with differentiated invasive thyroid carcinoma (DITC). Methods The data were analyzed retrospectively for 114 DITC patients treated at Department of Head and Neck Surgery of Sun Yat-sen University Cancer Center. Survival analysis was performed by Kaplan-Meier method, comparison among/between groups was performed using log-rank test, and multivariate analysis was carried out using Cox proportional hazard model. Results After surgery, 68 patients were with tumor residue. The 5-year and 10-year overall survival rate were 91.9% and 80.1% respectively in all patients, while the 10-year overall survival rate were 88.5% 、78.5% and 53.1% in no tumor residue group, micro-residue group and grossresidue group respectively. This study failed to prove that radiotherapy might improve the survival rate in patients with postoperative tumor residue. Multivariate analysis indicated that age, invasion to esophagus and recurrence predict the prognosis. Conclusion DITC may be treated mainly by surgical operation. Radical resection is the key factor in the treatment of DITC. Patients with DITC have a relatively poor prognosis.Age, esophagus invasion and status of tumor residue are the most important factors affecting the prognosis.
7.Surgical treatment of anorectal fistula in patients of acute leukemia

Objective To evaluate treatment and operation timing of anal fistula in acute leukemia patients.Methods Clinical data of 42 acute leukemia patients with anal fistula from 2010 to 2015 in First Hospital of Xinjiang Medical University were evaluated according to the choice of operation time.Patients were divided into two groups undergoing surgery before chemotherapy, (group A).and surgery after chemotherapy (group B).Clinical analysis was performed usingx2 or t test and correlation index.Results Neutrophil count in group B was lower than that in group A (x2 =8.077, P =0.004).Chemotherapy before surgery regime reduces the incidence of infection.Conclusions Chemotherapy before surgery may improve the prognosis of anorectal fistula patients of acut leukemia.
8.Downregulation of thioredoxin-interacting protein attenuates early brain injury after subarachnoid hem- orrhage of rats

Objective To explore the possible mechanism by which thioredoxin-interacting protein (TXNIP) par?ticipated in early brain injury (EBI) of subarachnoid hemorrhage (SAH) via examination of the expression of TXNIP and its downstream apoptotic factors before and after intervention. Methods Subarachnoid Hemorrhage (SAH) was performed by endovascular perforation. Total 97 adult male SD rats were randomly divided into 6 groups:sham-operation (17), SAH (32), control siRNA (12), TXNIP siRNA (12), resveratrol control (12) and resveratrol injection (12). Western blot was used to examine the expression of TXNIP, p-ASK-1, Caspase-3 before and after intervention. Laser scanning confocal microscopy (LSCM) was used to detect the expression of TXNIP in neurons. The co-localization of TXNIP with apoptotic cells was examined by using fluorescent TUNEL. Mortality, behavior score and cerebral edema were also evaluated. Re?sults Mortality, behavior scores and brain edema were improved after TXNIP siRNA and resveratrol injection(P<0.05). LSCM showed that TXNIP was widely expressed in brain and mainly located in cytoplasm of neurons in SAH rats. Fluo?rescent TUNEL revealed the co-localization of TXNIP with apoptotic cells. The expression level of TXNIP was signifi?cantly higher in SAH group than in sham operation (P<0.05, n=3). The expression level of TXNIP gradually increased at 12h and still remained at high level at 72h (P<0.05). This increase was simultaneously accompanied by the increase in downstream apoptosis factors, p-ASK-1 and Caspase-3. Inhibition of TXNIP by siRNA or resveratrol significantly re?duced the expression of TXNIP, p-ASK-1 and Caspase-3 (P<0.05, n=3). Conclusion TXNIP gradually increases in ear?ly period after SAH and aggravates brain damage through activation of ASK-1 apoptosis signaling pathway, whereas inhi?bition of TXNIP may attenuate EBI through reduction of p-ASK-1 and Caspase-3 after SAH.
9.Central precocious puberty with primary pigmented nodular adrenocortical disease: One case report

To report the clinical, imaging, and pathological feature of a rare case of central precocious puberty with primary pigmented nodular adrenocortical disease(PPNAD), and to conduct a retrospective analysis of PPNAD with relevant literatures. The pubic hair was found in the child for more than one year. Physical examination showed Cushing′s syndrome. ACTH in blood decreased, cortisol rhythm was disordered, 24-hour urine free cortisol increased and the paradoxical increase of urine free cortisol after high dose dexamethasone suppression test. Adrenal enhancement computed tomography(CT)showed multiple small nodular shadows in bilateral adrenal glands. Gonadotropin releasing hormone(GnRH)stimulation test supported central precocious puberty and GnRH analogue was used to control the sexual development. PPNAD was supported by pathology result. The symptoms of Cushing′s syndrome were relieved partially after left adrenalectomy.
10.Abdominal electroacupuncture for post-stroke constipation:a randomized controlled trial

Objective:To observe the effect of abdominal electroacupuncture(EA)on intestinal function in patients with constipation after stroke. Methods:A total of 100 patients with post-stroke constipation were divided into an observation group and a control group by the random number table method,with 50 cases in each group.The control group was treated with oral lactulose,10 mL each time,3 times a day,for 2 consecutive weeks.The observation group was treated with additional abdominal EA at Tianshu(ST25),Fushe(SP13),and Daheng(SP15)on the basis of oral lactulose,30 min for needle retention each time,once a day,for 2 consecutive weeks.The total effective rate after treatment,constipation symptom score,the score of patient assessment of constipation quality of life scale(PAC-QOL),serum motilin(MTL)level,and the score of mini-mental state examination(MMSE)were compared between the two groups. Results:After 2 weeks of treatment,the total effective rate was 93.5%in the observation group and 76.1%in the control group,and the difference between the two groups was statistically significant(P<0.05).The constipation symptom score,PAC-QOL score,MMSE score,and MTL level changed significantly after treatment in both groups(P<0.05).After treatment,the constipation symptom score and PAC-QOL score in the observation group were lower than those in the control group,while the MMSE score and MLT level were higher than those in the control group,and the differences between the two groups were statistically significant(P<0.05). Conclusion:On the basis of oral lactulose,EA at abdominal points can significantly improve the intestinal and cognitive function of stroke patients simultaneously.
