1.Ultrasonographic screening of neonatal developmental dysplasia of hip

Objective To assess the role of hip ultrasonography in the early screening and following up of developmental dysplasia of the hip (DDH). Methods A total of 1324 hips of 662 infants younger than 6 months underwent ultrasonography with Graf method. The sonographic appearances of bony roof, superior bony rim, cartilaginous roof, as well as α and β angle measurements were classified according to the Graf method. Risk factors such as gender, sides, fetal position, and swaddling used were assessed. Following up were performed with ultrasonography in the cases of immaturity, dysplasia and dislocation that needed to be monitored or treated. Results Detection rate of selective ultrasound in screening of DDH of the hip was 7.42%. Both gender and fetal position was the risk factor of DDH. Twenty-seven infants with dysplasia hip and five with dislocation hip were followed-up with ultrasonography during treatment process. Thirteen infants with dysplasia turned to normal after abduction exercises, while other fourteen received treatment in Pavlik harness for no improvement in ultrasonography. All infants of dislocation received operation eventually because of failing to Pavlik harness. Conclusion Ultrasonography can objectively assess the development of DDH, monitor the course of following-up and treatment of DDH, and serve as a main tool in the early screening and following up of DDH in infants.
2.Change of matrix metalloproteinase-9 in rats with multiple organ dysfunction syndrome

Objective To investigate the changes in the serum MMP-9 (matrix metalloproteinase-9) and the expressions of MMP-9 in lung, kidney and intestine in rats with multiple organ dysfunction syndrome (MODS) and confirm extracellular matrix injuries being the mechanism in MODS in order to propose a novel theoretical basis for cfinical treatment of MODS. Method Forty wister rats were randomly divided into two groups: control group (n=8) and MODS model group (n=32). The rats of model group were further divided into four subgroups ac-cordingto the time elapsed after modelling: 12 h (n=8), 24 h(n=8) ,48 h(n=8) and 72 h (n=8), and were modelled by celiac injection of mixed liquid of zymosan-paraffin (4 mL/100 g) after blood loss (1mL/100 g) by extirpating their left eyes. Blood,lung, kidney and intestine were sampled 12,24,48 and 72 hours after models were established. The histological changes in the lung, kidney and intestine of the rats were observed by light mi-croscope. The serum MMP-9 were measured by enzyme-linked immunosorbent assay (ELISA). The immunohisto-chemistry was used to observe the expression of MMP-9 in lung,kidney and intestine during different phases of MODS. The data were processed by one-way ANOVA and Bivariate analysis. Results Compared with control group, the organs were injured by congestion, edema and inflammatory cells infiltration to a certain extent in model groups. The serum MMP-9 increased markedly 12 hours after modelling (P<0.01 ) and peaked 48 hours later. The expressions of MMP-9 in lung, kidney and small intestine significantly increased from 12 h to 72 h after mod-elling (P<0.01 or 0.05). Conclusions The MMP-9 increased both in serum and tissue are closely associated with the pathological process of MODS. The mechanism of organ damage probably attributes to the damage of extra-celluar matrix and tissue construction.
3.Effect of Acupuncture Cooperating with Bobath Approach on Spasticity after Stroke

Objective To explore the affect of acupuncture cooperating with Bobath approach on the spasticity after stroke.Methods 83 patient were divided into treatment group (42 cases) and control group (41 cases). On basis of conventional treatment, the treatment group accepted acupuncture and Bobath approach while the control group accepted a Bobath approach only. They were assessed with Modified Ashworth Scale before and 8 weeks after treatment. Results 8 weeks after treatment, the spasticity reduced in both groups, but more in the treatment group (P<0.05).Conclusion Acupuncture can facilitate the efficacy of Bobath approach on spasticity after stroke.
4.Efficacy and safety of hydroxychloroquine in the treatment of obstetric antiphospholipid syndrome:a Meta-analysis

Objective To systematically evaluate the efficacy and safety of hydroxychloroquine(HCQ)in obstetric antiphospholipid syndrome(OAPS).Methods PubMed,Embase,Cochrane Library,Web of Science,SinoMed,Wanfang Data,CNKI,and VIP databases were searched electronically to collect clinical research on HCQ treatment for OAPS from inception to January 31,2023.Two researchers independently screened the literature,extracted data,and assessed the risk of bias of the included studies,Meta-analysis and GRADE evaluation were performed using RevMan 5.4 software and GRADE Profile 3.6 softwares.Results Five cohort studies and three randomized controlled trias(RCTs)were included,with a total of 644 OAPS patients(732 pregnancies).The results of Meta-analysis showed that compared with conventional treatment,HCQ supplementation significantly increased the live birth rate of OAPS(RR=1.29,95%CI 1.10 to 1.51,P=0.001),the negative conversion rate of lupus anticoagulant(RR=1.29,95%CI 1.13 to 1.47,P<0.001),the anticardiolipin antibody negative conversion rate(RR=1.27,95%CI 1.12 to 1.45,P<0.001)and the anti-β2 glycoprotein I antibody negative conversion rate(RR=1.31,95%CI 1.12 to 1.52,P<0.001),the rate of early abortion(<10 weeks)was significantly reduced(RR=0.31,95%CI 0.10 to 0.93,P=0.04).However,there was no significant difference between the two groups in reducing the rate of premature birth,late abortion(>10 weeks)and the incidence of preeclampsia(P>0.05).In terms of safety analysis,two studies described HCQ adverse effects including skin reactions and dry eyes,symptoms are mild.Three RCTs were used to compare the incidence of adverse reactions between the two groups,the incidence of adverse reaction of HCQ group was lower than that of control group(RR=0.40,95%CI 0.25 to 0.66,P<0.001),and no serious adverse reactions occurred in both groups.The sensitivity analysis results were robust and reliable.The results of GRADE evaluation showed that the quality of index evidence included in this study were low or very low,with weak recommendations.Conclusion HCQ can significantly improve the live birth rate of OAPS and the negative conversion rate of antiphospholipid antibody,and reduce the fetal abortion rate before 10 weeks with fewer adverse reactions,but there is insufficient evidence to reduce the incidence of premature birth,fetal abortion after 10 weeks and preeclampsia.Due to the limited number and quality of included studies,the above conclusions need to be confirmed by more high-quality studies.
5.Feasibility of endoscopic submucosal dissection for the treatment of large hiatal hernia combined with refractory gastroesophageal reflux disease (with video)

Objective:To study the safety and efficacy of hiatal hernia-endoscopic submucosal dissection (HH-ESD) for the treatment of giant hiatal hernia (>3 cm in length) complicated with refractory gastroesophageal reflux disease (GERD).Methods:Patients with giant hiatal hernia complicated with refractory GERD who voluntarily received HH-ESD at the Digestive Endoscopy Center of the Affiliated People's Hospital of Inner Mongolia Medical University from April 2018 to March 2020 were included in the clinical study. The completion of HH-ESD and the occurrence of complications were observed, and the changes of indicators before and after the treatment were observed, including gastroesophageal reflux disease-health related quality of life (GERD-HRQL) score, gastroesophageal reflux disease questionnaire (GERD-Q) score, endoscopy results, 24 h esophageal pH monitoring results, esophageal high-resolution manometry results and proton pump inhibitor (PPI) usage.Results:Data of 10 patients were collected during the study, with a medical history of 2-10 years. All patients successfully underwent HH-ESD treatment. No adverse events such as perforation and massive bleeding occurred during the operation, and the hospital stay was 6-12 days. Dysphagia occurred in 3 cases after the operation, which was relieved spontaneously within 3 or 6 months. The preoperative GERD-HRQL scores ranged from 19 to 29, which were reduced to 0-14 and 0-8 at 3 and 12 months after the operation, respectively. The preoperative GERD-Q score was 9-17, and the scores at 3 and 12 months after the operation were all 6-9, which were significantly lower than those before. Gastroscopy showed that esophagitis was improved in all patients, hernia sac was reduced, and Hill grade was reduced compared with that before. The preoperative DeMeester score was 30.3-247.1, and the postoperative 12-month score was 0.2-29.9, which was significantly lower than that before. The long diameter of hiatal hernia was 3.0-6.0 cm before the operation and 0-5.0 cm at 12 months after, which was smaller than that before. At 12 months of the follow-up, 7 patients had stopped PPI, and the remaining 3 had changed to intermittent oral PPI.Conclusion:Preliminary results show that HH-ESD is safe and effective for the treatment of giant hiatal hernia complicated with refractory GERD.
6.Clinical Experience Applying the "Kidney-supplementing and Blood-invigorating" Method to Treat Infertility of Repeated Implantation Failure

kidney deficiency and blood stasis are believed to be the key pathogenesis of recurrent implantation failure (RIF), and therefore, it is proposed to take kidney-supplementing and blood-invigorating as the basic treatment method, and use it by stages to assist the controlled superovulation program of in vitro fertilization-embryo transfer (IVF-ET). The focus should be put on nourishing kidney yin, soothing the liver and invigorating blood during down-regulation period, using kidney yin-nourishing medicinals and modified Qinghao Biejia Decoction (青蒿鳖甲汤). During the ovulation induction period, the focus should be put on nourishing kidney yang, replenishing blood, and promoting blood circulation, with self-made Bushen Culuan Formula (补肾促卵方) in modifications. After transplantation, it is suggested to put focus on tonifying the kidneys and replenishing essence, and self-made Wuzi Yanzong Guchong Decoction (五子衍宗固冲汤) in modifications can be used. At the same time, fetal protection treatment should be carried out for pregnant women, and modified Shou Tai Pill (寿胎丸) can be used to tonify the kidneys and calm the fetus.
