1.Diagnostic value of serum HE4 and YKL-40 for the ovarian cancer

Objective To study the significance of serum human epididymis protein 4 ( HE4 ) and human cartilage glycoprotein-39(YKL-40) in the diagnosis of ovarian cancer .Methods 100 patients with ovarian cancer , 90 patients with benign tumor of ovary and 50 healthy people were enrolled .Serum HE4 and YKL-40 were detected . Results The positive rates of serum HE 4 in ovarian cancer group , ovarian benign tumor group and healthy group were 86.0%,5.5%,2.0%.The positive rate of serum HE4 in ovarian carcinoma was significantly higher than that in benign ovarian tumor group and the healthy group (χ2 =31.06,30.82,all P<0.01).The positive rates of serum YKL-40 in ovarian cancer group ,ovarian benign tumor group and healthy group were 82.0%,3.3%,2.0%.The positive rate of serum YKL-40 in ovarian carcinoma was significantly higher than that in benign ovarian tumor group and the healthy group(χ2 =30.92,32.06,all P<0.01).Serum HE4 in the diagnosis of ovarian cancer ,the sensitivity was 86.0%,specificity of 95.7%,accuracy of 91.7%.Serum YKL-40 in the diagnosis of ovarian cancer ,the sensitivity was 82.0%,specificity of 97.1%,accuracy of 90.8%.For the joint detection,the sensitivity for diagnosis of ovarian cancer was 92.0%,and the specificity was 93.6%,accuracy was 95.0%,higher than those of the single detection . Conclusion Combined with serum HE 4 and YKL-40 detection can significantly improve the diagnostic accuracy of ovarian cancer patients .
2.Characteristics and the value of renal solid lesions' boundary at contrast-enhanced ultrasonography

Objective To investigate the characteristic and the value of renal solid lesions' boundary at contrast-enhanced ultrasonography(CEUS). Methods The study included 225 patients (124 males, 101 females) with renal 239 solid lesions [133 renal cell carcinoma(RCC) and 106 benign lesions]. The enhanced mode of lesion boundary at CEUS was observed. The histopathologic pseudocapsule of RCCs was analysed.Results Enhanced modes of all lesions' boundary at CEUS were classified as: type Ⅰ , iso-enhanced boundry in whole phase, 82.85 % (198 of 239) ;type Ⅱ , a perilesional annular highly-enhanced signal at early phase,4.18% (10 of 239);type Ⅲ ,perilesional annular highly-enhanced signal in whole phase,9.62% (23 of 239) ;type Ⅳ, perilesional annular highly-enhanced signal in midium and late phase, 1.25 % (3 of 239) ;type Ⅴ ,iso-enhanced boundry in the mdium and late phase with no enhancment at early phase, 2. 09% (5 of 239). The distribution of types Ⅰ , Ⅱ , Ⅲ between groups were significant different( P=0.000, 0.046,0. 000), the type Ⅳ and Ⅴ was not ( P = 0.256,0.068). The distribution of perilesional annular highlyenhanced signal between benign and RCC groups was statistically different (x2=29. 681, P=0.000).Regared it as a diagnostic criteria of RCC,the sensitivity was 26.32% (35/133) ,the specificity was 99.06%(105/106) ,the positive predictive value was 97.22% (35/36),the negative predictive value was 51.72%(105/203) ,and the accuracy was 58.58% (140/239). The perilesional annular highly-enhanced signal was not correlated with the pseudocapsule in pathology ( P = 1. 000). Conclusions The boundary enhancement mode of renal solid lesions at CEUS was divided into five types. The perilesional annular highly-enhanced signal was important in diagnosis of RCC,which was not correlated with the pseudocapsule in pathology.
3.Distribution of a polymorphic site of stromal cell-derived factor 1 gene in She Ethnic Group in the south of China

BACKGROUND: Stromal cell-derived factor 1 (SDF1), a potential inhibitor of infection by lymphophilic HIV-1 strains, can help to block the pathway of HIV-1 invasion into the human body.OBJECTIVE: Genotype and polymorphism of SDF1-3 'A allele associated with HIV-1 infection were investigated in She Ethnic Group in the south of China so as explore the possible causes of uninfection by HIV-1 strains among this population.DESIGN: Single sample study.SETTING: Department of Biochemistry and Molecular Biology, Gannan Medical College.PARTICIPANTS: Totally 186 She Ethnic subjects without HIV-1 infection collected randomly from those whose three generations belonged to She Ethnic Group, and inhabited in Qianshan County of Jiangxi Province,Ningde area of Fujian Province and Jingning She County of Zhejiang Province, from January to December 1995.METHODS: The whole blood samples from 186 She Ethnic subjects were collected randomly, and then their genomic DNA samples were extracted respectively. Allelic polymorphism was examined by the polymerase chain reaction and restriction-fragment-length polymorphisms (PCR-RFLP).MAIN OUTCOME MEASURES: The distribution of SDF1-3'A allele in She Ethnic Group in the south of China.RESULTS: The data of 186 She Ethnic subjects entered the result analysis without any loss in the midway. The frequency of SDF1-3 'A allele in She Ethnic Group samples was 19.6%, and the allelic distribution of the gene was in accordance with Hardy-Weinberg equilibrium. No difference was found between male and female individuals.CONCLUSION: The frequency of SDF1-3 'A allele of She Ethnic Group in the south of China was similar to that of Dai Nationality in Yunnan.Based on its slow-down effect on clinical course of AIDS, the mutation of SDF1-3'A is significant in the prevention and treatment of AIDS in She Ethnic Group in the south of China.
4.Photo-activated DNA binding and antimicrobial activities of alkaloids from Glycosmis pentaphylla.

In our screening for photosensitizers from natural resources, four alkaloids were isolated from Glycosmis pentaphylla by various chromatography techniques. Their structures were identified as glycoborinine (1), glybomine B (2), carbalexin A (3) and N-p-coumaroyltyramine (4) by spectral analysis. Their photoactivated antimicrobial activities were evaluated by thin-layer chromatography (TLC) agar overlay assay against Staphylococcus aureus and Bacillus subtilis. It was found that compounds 1 and 4 showed photo-activated antimicrobial activities. Meantime, photo-activated DNA binding activities of these compounds were also assessed by using a specially prepared 1.8 kb DNA fragment and restriction enzymes. Under UVA irradiation, compound 1 showed moderate inhibition on Nde I, Xba I, Nco I and Bcl I which have either 5'-TpA or 5'-ApT and trace or no inhibition on other restriction enzymes. It showed a similar inhibition pattern with the reference 8-methoxypsoralen. However, compounds 2-4 showed no inhibition against any of the restriction enzymes.
5.Emergency treatment of aortic dissection and clinical pathway discussion

Objective To investigate the clinical features of aortic dissection (AD) and emergency treatments. Methods Data from 784 patients with aortic dissection were collected in the Department of Emergency from January 2000 through December 2009. A retrospective analysis was carried out to determine the survival rate, mortality rate and treatment efficiency. Results Pain was the most common onset symptom (77.7% , 609/784). The majority of patients (86.5%) had essential hypertension (678/784). All the patients with preoperative diagnosis of aortic dissection underwent emergency medical intervention by internists resulting in 81.5% survival rate (639/784) and 18.5% mortality rate (145/784). There were 157 patients without improvement (20.0% ) and the total efficiency rate was (83. 1% ). The efficiency rate of conventional treatment was 76.4% , while the efficiency rate of triple four-procedure treatment was 89. 8% (P<0.05). Of them, 139 patients (17. 7% ) died in the hospital. Among them,. 26 patients died within 24 hours (18.4% ) and 47 cases died within 48 hours (33. 8% ) and 66 patients died within 72 hours (47.2% ). There were 92 patients who refused treatments after diagnosis, and among them, 81 patients died within 72 hours (88.04% ). The difference in mortality rate between two groups was significant (P<0.05). Conclusions The diagnosis of aortic dissection depends on detailed history, physical examination and CT or MRI imaging. Analgesia, sedation and control of blood pressure are essential for emergency treatments. Early diagnosis and effective emergency treatments are the critical strategy for the early surgical intervention and time window for further treatment to improve the survival rate of AD.
6.Effect of FTO gene on type 2 diabetes mellitus treated by Roux-en-Y gastric bypass

Objective To investigate the effect of FTO gene on laparoscopic Roux-en-Y gastric bypass for type 2 diabetes.Methods From Jan 2014 to 2015 Jun,32 T2DM patients received LRYGB in Beijing Shijitan Hospital.According to single nucleotide polymorphism of FTO rs9939609 gene,patients were divided into TF genotype (20 cases),and AT/AA genotype (12 cases).Results Following the degression of BMI from (36.4 ±3.3) kg/m2 to (28.1 ±2.8) kg/m2,fasting blood glucose and HbA1c in group A significantly improved [(8.9 ± 1.6) mmol/L vs.(5.6 ± 1.2) mmol/L,t =8.274,P =0.000;(8.5% ± 1.9%) vs.(6.2% ± 0.8%),t =5.032,P =0.000].Following the degression of BMI from (39.5 ±5.6) kg/m2 to (29.7 ± 5.1) kg/m2,fasting blood glucose and HbA1c in group B significantly improved [(10.8 ± 2.8) mmol/L vs.(4.9 ± 0.6) mmol/L,t =7.589,P =0.000;(9.0% ± 1.8%) vs.(6.1% ±0.9%),t =5.324,P =0.000].Insulin resistance index in both groups significantly improved [(12.6±10.7) vs.(4.9±5.6),t=5.402,P=0.000;(16.0±5.6) vs.(1.7±1.3),t=9.025,P=0.000].Fasting blood glucose and the insulin resistance index in group B patients was significantly lower than that in group A patients (P < 0.05).Conclusion T2DM patients with FTO rs9939609 gene phenotype AT/AA have better prognosis than those with TT in postoperative diabetes improvement.
7.Experience in Application of the Lotus Medicinal Herbs for Female Reproductive System Diseases

As one of notable works of HUANG Yuanyu, a renowned physician from the Qing Dynasty, The Indications of Classics by Four Medical Sage emphasize the theoretical paradigm of “one qi circulation” with a particular focus on the middle qi. Building upon disease differentiation and treatment, HUANG Yuanyu attaches great importance to understanding the underlying cause of the disease. It is believed that the imbalance of the six climatic factors is the fundamental cause of diseases. The concept of mutual interaction between the physiological and pathological aspects of the six climatic factors is proposed to grasp the basic pathogenesis. Based on the individual's specific condition of six climatic factors, excess or deficiency, specific prescriptions are formulated. The diagnosis and treatment system can be summarized as a “differentiation of diseases → disease etiology → differentiation of the condition of six climatic factors → individualized pathogenesis of traditional Chinese medicine (TCM) → prescription of TCM” model based on the holistic view. HUANG Yuanyu' s diagnosis and treatment system is highly practical and can provide reference for clinical diagnosis and treatment of diseases in TCM.
8.Treatment of Psoriasis Based on Theory of Ascending and Descending of Center Qi

Based on the theory of ascending and descending of center qi, it is believed that yang deficiency and water dampness, as well as abnormal circulation of center qi, are the core pathogenesis of psoriasis. The common pathogenic evolution of psoriasis includes wood constraint and blood stagnation, lung metal fluid exhaustion. In the later stage of psoriasis, a mixture of deficiency and excess patterns, as well as cold and heat in complexity, are often observed. Treatment focuses on warming yang and resolving dampness to restore proper qi ascending and descending, and the therapeutic formulas such as Ganjiang Lingzhu Decoction (甘姜苓术汤), Zexie Decoction (泽泻汤), Zhenwu Decoction (真武汤), and Linggui Zhugan Decoction (苓桂术甘汤) are suggested based on the nature and location of the disease. To unblock the qi movement through discharging the liver and rectifying the lung, Xiaochaihu Decoction (小柴胡汤) and Danzhi Xiaoyao Powder (丹栀逍遥散) are commonly used as the foundation with modifications. To balance yin and yang through clearing the heart and nourishing the kidneys, a self-designed Changzhong Decoction (畅中汤) is utilized. Following the theory of ascending and descending of center qi, it is suggested to combine cold and warm medicinals and mediate the center earth, so as to promote the circulation of the center qi, restore the clear and the turbid, and keep four-dimensional smooth flow of qi, providing a reference for traditional Chinese medicine treatment for psoriasis.
