1.Social support status and influencing factors among schizophrenics in remission in Northeast Sichuan

ObjectiveTo investigate the social support status and influencing factors of schizophrenics in remission in Northeast Sichuan, and to provide ideas for improving their social support. MethodsFrom May to September 2020, a total of 533 patients who met the diagnosis criteria of the International Classification of Diseases, tenth edition (ICD-10) for schizophrenics in remission at the mental health institutions in Guangyuan, Bazhong and Dazhou cities were selected for the survey, and patients were assessed by self-made demographic and clinical data inventory and Social Support Rating Scale (SSRS). Then the social support status of schizophrenic in remission and influencing factors were analyzed, meantime, the impact of the second round reimbursement policy of medical insurance benefits on their social support was addressed particularly. Results①The SSRS total score, objective support, subjective support, and utilization of support scores of schizophrenics in remission were lower than those of the national norm (t=5.065~30.382, P<0.01). ②Univariate analysis showed that SSRS score was relatively high among patients with female gender (t=-3.632), retired status (F=5.951), married status (F=5.951), spouse as primary caregiver (F=23.841), annual household income >5 000 yuan (F=15.892), patient's economic income (t=4.083), and outpatient or online follow-up (F=3.954), with statistically significant differences (P<0.05 or 0.01). ③The total and dimensional scores of SSRS in patients with access to the second round medical insurance reimbursement were significantly higher than those without (t=10.195~25.103, P<0.01). ④Multiple linear regression analysis denoted that gender, work status, marital status, primary caregivers, annual family income, economic income, follow-up visits and the second round medical insurance reimbursement were the factors influencing social support status of schizophrenics in remission (β=0.201~2.115, P<0.05 or 0.01). ConclusionThe social support of schizophrenics in remission in Northeast Sichuan is below the national average, furthermore, their social support levels are affected by the gender, work status, marital status, primary caregivers, annual family income, economic income, follow-up visits and the second round medical insurance reimbursement, and the second round medical insurance reimbursement may ameliorate the social support status of patients.
2.The effect of low-dose aripiprazole addition therapy on adiponectin and leptin in schizophrenia patients with olanzapine-induced weight gain

Objective To investigate the efficacy of aripiprazole addition therapy on adiponectin and leptin in schizophrenia patients with olanzapine-induced weight gain. Methods Schizophrenia patients with clinically significant weight gain induced by olanzapine were randomly divided into two groups:treated with olanzapine+aripiprazole (5 mg/d, n=48) or olanzapine+placebo (n=46). The level of FBS, TG, TC, adiponectin, leptin, the body index (BMI) and the insu?lin resistance index (HOMA-IR) were measured before and 4, 8 and 12 weeks after treatment. Results Adiponectin in?creased and leptin as well as HOMA-IR decreased in olanzapine+aripiprazole group after 4, 8 and 12 weeks treatment (P<0.05). However, FBS, TG and TC decreased and BMI increased only after 8 and 12 weeks treatment(P<0.05). In olanzapine+placebo group, the dadiponectin decreased and leptin, HOMA-IR as well as BMI increased after 8 and 12 weeks treatment(P<0.05). In olanzapine + aripiprazole group the adiponectin, leptin and HOMA-IR of 4, 8 and 12 weeks and BMI of 8 and 12 weeks were different from olanzapine + placebo group(P<0.05). Conclusions In schizo?phrenia patients with olanzapine-induced weight gain, aripiprazole addition therapy can ameliorate the HOMA-IR and BMI probably through the regulation of adiponectin and leptin.
3.DETERMINATION OF SERUM FREE AMINO ACIDS IN NORMAL ADULTS——THE COMPARISON OF THE METHODS FOR PHYSIOLOGICAL FLUID AND PROTEIN HYDROLYZATE ANALYSIS

The free amino acid concentrations in serum were determined in 65 normal adults by use of the physiological fluid analysis in Hitachi 835-50 Model Amino Acid Autoanalyer. 29 specimens were determined by physiological fluid analysis and protein hydrolyzate analysis method simultaneous-ly.The results of Serine, Glycine, Valine, Isoleucine, Leucine, Tyrosine, Phenylalanine, Lysine, Tryptophan, Proline, and the molar ratio of the bran-ched-chain amino acids/aromatic amino acids were very similar. By hy-drolyzate method Aspartic acid was not detected and Threonine, Glutmic acid, Cystine, Methionine and Arginine were higher than physiological fluid analysis.The reasons for such higher values were discussed.
4.The application of standardized patients in cardiothoracic surgery teaching

5.Influence of nasal septal deviation on incidence of bilateral sinusitis.

OBJECTIVE:
To investigate the influence of nasal septal deviation on the incidence of bilateral sinusitis.
METHOD:
Two thousand and seventy-four cases of nasal septal deviation with chronic sinusitis available from January 2006 to March 2012 were reviewed retrospectively.
RESULT:
Of these 2074 cases, 1 687 (81.3%) patients had sinusitis in narrow side and 1582 (76.3%) in wide side, the difference was statistically significant (P < 0.01) and the relative risk value in narrow side (1.4) was higher than wide side (0.7) associated with the incidence of sinusitis. There was significant difference (P < 0.05) between the two group counting datas analyzed by rank sum test, and the mean rank order in narrow side was 1656.396, significantly greater than wide side (1 612.184). The incidence of sinusitis in men was 50.6% (1049/2074), in women was 49.4% (1025/2074), and there was no significant difference between them. Deviating leftward was 1148 cases (55.4%), deviating rightward was 926 cases (44.6%), and the difference was statistically significant (P < 0.05). Anatomic variations in narrow side and wide side were uncinate process hypertrophy (7.0%, 27.0%), middle turbinate hypertrophy (7.8%,38.0%), ethmoidal bulla hypertrophy (26.2%, 39.0%), paradoxical middle turbinate (5.4%, 15.5%), hypertrophic inferior turbinate (52.9%, 67.0%) and mucosa pachynsis (0.5%, 2.8%), respectively.
CONCLUSION
Sinusitis incidence in narrow side is higher than wide side. There is no significant difference between the incidence of sinusitis in men and women. The majority of nasal septum deviate leftward. Higher incidence of sinusitis in wide side is the result of compensatory mechanism. Only septoplasty may not restore the normal venting function in nasal cavity. Hypertrophic inferior turbinate are closely correlated with sinusitis.

6.Clinical study of 34 patients with extranodal NK/T cell lymphoma-nasal type.

OBJECTIVE:
To explore the clinical feature, the reason of misdiagnosis and mistreatment, influential factor of prognosis in patients with extranodal NK/T cell lymphoma-nasal type.
METHOD:
A retrospective study was made on the clinical data of 34 patients with extranodal NK/T cell lymphoma-nasal type. Among them, 10 cases were staged I(E) intra-cavity, 15 cases were I(E) ex-cavity, 6 cases were II(E) and 3 cases were IV(E). Among them, 29 cases were in nasal cavity, 5 cases were outside nasal cavity; 14 cases were treated with single chemotherapy or radiation therapy, 20 cases were treated with radiation therapy add chemotherapy.
RESULT:
The total rate of misdiagnosis and mistreatment were 58.8% (20/34), 52.3% (18/34), respectively. The 5-year survival rate of the I(E) intra-cavity group were 60.0% (6/10), and those of I(E) ex-cavity group were 26.7% (4/15), and those of II(E) group and IV(E) group were 16.7% (1/6), 0% (0/3), respectively There was significant difference between 3 groups by statistical analysis (P < 0.01). The 5-year survival rate of I(E) ex-cavity group treated with single therapy were 0% (0/6), and those of I(E)-cavity group treated with combined therapy were 50% (1/2).
CONCLUSION
The early clinical manifestation of extranodal NK/T tell lymphoma-nasal type is atypical and which is hard to diagnose and treat. Diagnosis depends on pathologic biopsy and immunohistochemistry, there are many factors that influence the prognosis of this disease, in which the clinical stage is a major factor. It is crucial for diagnosing and treating early.

7.The impact of aerobic exercise on cardiorespiratory function in patients with obstructive sleep apnea/hypopnea syndrome

Objective To investigate the effects of aerobic exercise on cardiorespiratory function in patients with obstructive sleep apnea/hypopnea syndrome (OSAHS).Methods A total of 84 patients with OSAHS were randomly divided into a control group and an aerobic exercise group (42 in each group).The control group was given only training in the activities of daily living (ADL).The acrobic exercise group was given ADL training combined with aerobic exercise training.The regime was maintained for 16 weeks.Each patient's VO2max,maximum heart rate (HRmax),exercise time to exhaustion,6-minute walking distance and quality of life (QOL) were evaluated before and after treatment.Results VO2max,HRmax,VO2max/HRmax,maximum exercise time,6-min walking distance and QOL all increased significantly more in the aerobic exercise group compared with the control group.Conclusion Aerobic exercise training can significantly improve cardiorespiratory function in patients with OSAHS.

9.Study on Action Mechanism of African Traditional HerbHypoxis Hemerocallidea Extracts on AMPK Signal Pathway of Skeletal Muscles in Diabetes

This study was aimed to investigate the action mechanism ofHypoxis Hemerocallidea (African Potato, AP) on the AMPK signal pathway of skeletal muscles in diabetic rats. Among 40 male SD rats, 10 rats were used as the normal group, and the other 30 rats were fed with high-fat food for one month, and then injected with STZ for the model establishment. After the successful model establishment, rats were divided into the model group, pioglitazone hydrochloride group and the AP group. Intragastric administration was given for 5 weeks in each group. Then, the skeletal muscle tissues were embedded and sliced for immunohistochemistry test. The protein expression of p-AMPKα, p-AS16 and GLUT4 in skeletal muscles was detected by western blot. The 100 mmol·L-1 glucose was used in the establishment of C2C12 skeletal muscle cells insulin resistance model. AP drug-containing serum was used in the establishment of the treatment group. The control group was the normal cells. Glucose consumption, cell proliferation, SOD content, and MDA content were detected. And the protein expressions of p-AMPKα, p-AS160, GLUT4 were detected with the western blot and RT-PCR. The results showed that compared with the normal group, AP can up-regulate p-AMPKa protein express (P < 0.01), increase skeletal AS160 phosphorylation level (P < 0.01), and up-regulate the GLUT4 level (P < 0.01). Compared with the normal group, the high glucose caused the decrease of C2C12 skeletal muscle cell activity and the decrease of glucose consumption (P < 0.05), decrease of SOD, increase of MDA (P < 0.01), and the decrease of p-AMPKα, p-AS160, GLUT4 protein expression (P < 0.01). After 48 h intervention, the SOD of C2C12 skeletal muscle cells in the AP drug-containing serum group was significantly increased (P < 0.01), the MDA content was decreased (P < 0.05), the AMPKa and AS160 phosphorylation levels were increased (P < 0.01), the GLUT protein expression was increased (P < 0.01). It was concluded that the induced AMPKa and AS160 phosphorylation promoted GLUT 4 expression may be one of the action mechanism of insulin resistance of skeletal muscles in diabetes.
10.Application of nutrition gastrostomy in the retrosternal route for esophageal reconstruction after operation

Objective:Esophageal cancer has high morbidity in China, and surgery is the main treatment for this disease. Postop-erative nutrition is also important for the patients. In this article, we discuss the possibility of retrosternal route gastrostomy feeding (RGF). Methods: The data of 127 esophageal cancer patients between 2011 and 2013 were retrospectively analyzed. RGF was per-formed in all the patients, and post-operation complications were studied. Results: Bowel obstruction, catheter displacement, and wound infection did not occur. Conclusion:RGF is a safe and effective nutrition method for patients who underwent retrosternal recon-struction.
