OBJECTIVE To investigate antimicrobial resistance of Acinetobacter baumannii and detect metallo-?-lactamases (MBLs) in clinical isolates from ICU. METHODS Forty-two strains of A. baumannii were isolated from sputum samples between Jul 2005 and Mar 2007 in Affiliated Hospital of North Sichuan Medical College. Bacteria identification and antimicrobial susceptibility test were performed by VITEK-32 system and K-B disk method. Meanwhile,MBLs were detected by Etest. RESULTS Cefoperazone/sulbactam with low resistance accounted for 2.4%. The resistance to imipenem was 66.7%. The resistance to other antibiotics ranged from 69.4% to 100%. Nine MBLs-producing strains were detected by Etest. CONCLUSIONS Metallo-?-lactamases produced by A. baumannii are one of important mechanisms which caused resistance to imipenem. Cefoperazone/sulbactam and polymyxin can be chosen to treat resistant A. baumannii.

Objective To explore the effect of tumor necrosis factor-α antagonist (Remicade) on the patients with active moderate to severe ulcerative colitis (UC) who failed to response to mesalazine or sulfasalazine Sah'cyloyl treatment for six months period.Methods The patients were collected in our hospital,who diagnosed by colonoscopy as moderate to severe ulcerative colitis and failed to response to mesalazine or sulfasalazine Salicyloyl treatment for six months period;These patients were randomly divided into control group(22 cases) and treatment group(28 cases) ;glucocorticoid was used alone in control group while it was combined with Remicade in treatment group.After treatment for six months,they were suffered from colonoscopy and clinical evaluation.Results There were significant differences between control and treatment group of the recover rate concerning the clinic symptoms such as abdominal pain,diarrhea,and mucous bloody stool.There were 7 cases complete remission,13 cases effective outcome and 9 cases ineffective,with complete remission rate of 31.67% .efficiency rate 59.09% in control group.There are 17 cases complete remission,25 cases effective outcome and 3 cases ineffective,with complete remission rate of 60.71% ,efficiency rate 89.28% in treatment group.There was significant difference (P<0.05) between two groups of complete remission rate and efficiency rate.There were 3 cases recurring in control group,with the recurrence rate of 13.67% ,while there were 2 cases recurring in treatment group with the recurrence rate of 7.14%.There was significant dif-ferene( P<0.05) between two groups with regards to recurrence rate;Besides,there were no significant adverse effect.Conclusion The combination with Remicade and glucocorticoids in the treatment of refractory severe ulcerative colitis was more effective and can also reduce the relapse rate.

Objective To investigate the relationship between the expressions of Twist and clinical pathological parameters in gastric carcinoma,to explore its significance in judging the prognosis of patients,and to analyze the expression of Twist proteins in the epithelial-mesenchymal transition (EMT) phenomenon.Methods Immunohistochemistry was used to detect the expressions of Twist and EMT relative proteins E-cad,N-cad in 120 cases of gastric carcinoma specimens and corresponding adjacent normal tissue.Analysis The relationship between Twist expression and clinical pathological parameters,and the Twist expression and E-cad,N-cad were analyzed.Results In gastric carcinoma,Twist,E-cad,N-cad positive rates were 59.1％(71/120),36.6 ％ (44/120) and 47.5 ％ (57/120),and in adjacent tissues,the positive rate were 17.5 ％ (21/120),93.3 ％ (112/120) and 11.6 ％ (14/120).There were significant differences in each index between tumor tissues and adjacent tissues (P ＜ 0.05).Patients with Twist positive expression showed higher incidence rates of lymph node metastasis (P ＜ 0.05) and distant metastasis (P ＜ 0.05) than those with Twist negative expression.The expression of Twist was significantly correlated in gastric carcinoma accompanied by low expression of E-cad and abnormal high N-cad expression (P ＜ 0.05).Conclusions Compared with the adjacent tissue,the expression of Twist,E-cad and N-cad have significant differences,and the expression of Twist was closely related to the biological characteristics in gastric carcinoma.The expression of Twist in gastric cancer have negative correlation with the EMT relative factor E-cad,while positive correlation with the N-cad.

[Objective]To investigate the effect of three-dimensional computered tomographic(3DCT)reconstruction on the classification of operation for congenital elevation of the scapula and choice of congenital elevation of the scapula.[Method]Twenty-two patients with congenital elevation of the scapula were scanned with serial spiral CT and the images were three-dimensionally reconstraucted before operation.The scapular altitude difference of both sides were measured with the reference of scapular spine.Different appropriate operations which based on these results and Cavendish's classification were chosen for different patients.[Result]Twenty-two patients were followed-up for 2 years to 4 years,the appearance and the range of motion were improved without injuries of nerve,vessel and spine.[Conclusion]The pathological changes of congenital elevation of the scapula could be displayed clearly by 3DCT,so it is beneficial to choose the methods of operation and avoiding complications.

Objective:To construct the eukaryotic expression vector of interleukin-18 gene and carry out analysis of pVAX1-IL-18 sequence . Methods:The recombinant eukaryotic expression vector pVAX1-IL-18 was constructed by inserting interleukin-18 gene into the eukaryotic expression vector pVAX1 with molecular cloning technique . It was confirmed by restrictive enzymes(BamHⅠ/ EcoRⅠ) digestion and analysis of DNA sequencing . Similar nucleotide sequences encoding IL-18 of pVAX1-IL-18 were looked up by Blast means in NCBI GenBank. Results:Restrictive enzymes digestion analysis and DNA sequencing results revealed that the interleukin-18 gene was cloned into the eukaryotic expression vector pVAX1 successfully .The sequence of encoding IL-18 of pVAX1-IL-18 was exactly the same as the reported sequence of encoding human IL-18 in GenBank . Conclusion: The recombinant eukaryotic expression vector pVAX1-IL-18 has been constructed successfully, which lay the foundation for pVAX1-IL-18 as a genetic adjuvant of DNA vaccine.

Objective To explore the possibility of human neutrophil peptide 1 (HNP1) gene engineering, we construct the eukaryotic expression vector carrying HNP1 gene. Methods With RNA extracted from human neutrophil cell as template, cDNA encoding mature HNP1 was amplified by RT-PCR, and then it was inserted into vector pMD18-T. After restriction endonuclease digestion and DNA sequencing confirmation the gene was subcloned into eukaryotic expression plasmid pcDNA3.1/V5-His-TOPO to construct a recombinant expression plasmid pcDNA3.1/V5-His-TOPO/HNP1, then the recombinant plasmid was transfected into COS-7 cells by lipofectamine, and the expressed product was identified by biotin-avidin enzyme-linked immunosorbent assay ( BA-ELISA). Results The sequence of HNP1 completely matched those published in GenBank, thus eukaryotic expression vector pcDNA3.1/V5-His-TOPO/HNP1 was constructed correctly. The ELISA results showed that the eukaryotic expression plasmid pcDNA3.1/V5-His-TOPO/HNP1 could temporarily express HNP1 in COS-7 cells. Conclusion The successful construction and expression of pcDNA3.1/V5-His-TOPO/HNP1 pave the way for the stable expression HNP1 in mammalian engineering cells.

ObjectiveTo observe the efficacy of simethicone, mosapride combined with flupentixol-meiltracen in the treatment for functional dyspepsia(FD). MethodsThe diagnosis of functional dyspepsia patients treated according to Zung Self-rating Anxiety Scale (SAS) ＞ 40, Hamilton Depression Scale(HAMD) ＞ 17 for inclusion criteria,a total of 64 patients were selected and randomly divided into two groups. The treatment group( simethicone,mosapride combined with flupeentixol-meiltracen) in 33 cases and the control group ( single flupenthixol melitracen) in 31cases. 2 groups of the period of treatment was 4 weeks. Evaluation before and after treatment with mood disorder scale score changes and syndrome efficacy and adverse reactions. ResultsThe treatment group after treatment, regardless of scale score change of mood disorders, syndrome treatment efficacy were significantly better than the control group,especially in the syndrome efficacy difference was significant( P ＜0.05 ,P ＜0.01 ) ;after the treatment without obvious adverse reaction. ConclusionThe simethicone, mosapride combined with flupentixol-meiltracen treatment with anxiety and depression in patients with functional dyspepsia could effectively relieve the symptoms of functional dyspepsia,and relieve the patient's psychological disorder;certainly result in fewer adverse reactions.

Objective To analyze the correlation between the metal components and MRI signal intensities of gallstones, to investigate the causes of gallstone signal features on MRI. Methods The MRI data and the metal components of the gallstones in vivo and in vitro of 30 patients were retrospectively analyzed. The MR sequences, namely 3D fast spoiled gradient-echo with fat saturation T1-weighted imaging (3D-FSPGR-FS T1WI), fast spoiled gradient-echo with fat saturation T1-weighted imaging(FSPGR-FS T1WI), fast imaging employing steady-state acquisition(FIESTA)and fast spin-echo T2-weighted imaging with fat saturation(FSE-FS T2WI), were performed on the gallstones in vivo and in vitro. According to the characteristics of the surface and cross section, gallstones were divided into pigment gallstones( n=16) and cholesterol gallstones(n=14). The MR signal characteristics of the gallstones were observed and the signal intensity rates of the gallstones were calculated. Preoperatively, the signal intensity rates of cholesterol and pigment stones of each MR sequence were compared by using t test and Mann-Whiteney U test. Postoperatively,the signal intensity rates of the fresh,dried and re-soaked gallstones were compared by using paired t test and Wilcoxon test. The correlation between the signal intensity rates of gallstones on the 3D-FSPGR-FS sequence and their metal components was analyzed by using Linear Regression analysis. Results The pigment gallstones showed high signal intensity on the 3D-FSPGR-FS T1WI. The signal intensity rates of the pigment gallstones were higher than the rates of the cholesterol gallstones on the 3D-FSPGR-FS T1WI, which were 2.02 ± 0.53 and 0.51 ± 0.24 (t=10.26,P< 0.01), respectively. On the 3D-FSPGR-FS T1WI, the signal intensity rates of the drying pigment stones were significantly lower than the rates of the fresh ones, the rates of the two states of the pigment gallstones were 0.21±0.06 and 1.42±0.35(t=13.49,P<0.01),respectively. The signal intensity rates of pigment gallstones showed significant rebound after re-soaking, the rates of the two states of the pigment gallstones were 0.21±0.06 and 1.68±0.86(t=-6.63,P<0.01),respectively. The metal components of pigment gallstones were significantly higher than the cholesterol stones. In the pigment gallstones and cholesterol gallstones, the medians of the Calcium were 28.186 and 2.347 mg/g(Z =-4.66,P< 0.01),respectively.For pigment gallstones, there was a significant correlation between the calcium and the signal intensity rate on 3D-FSPGR-FS T1WI. The regression equation of linear regression analysis was SI=65.40 logCa-166.67. Conclusions The pigment gallstones containing much more water and metal showed high signal intensity on 3D-FSPGR-FS T1WI. The Calcium in the pigment gallstones may be the main cause for this MR appearance.

Objective To explore the cellular localization of chemokine (C-X-C motif) ligand 1 (CXCL1) in brain tissue and its expres-sion in brain tissue and blood in patients with severe traumatic brain injury (TBI), as well as its correlation with the injury severity. Methods From September, 2013 to October, 2015, 78 cases of TBI with craniotomy admitted to our hospital were involved as TBI group. A total of 78 peripheral blood samples and 19 brain tissue samples were studied. According to the scores of Glasgow Coma Scale (GCS) at admission, the TBI group was classified as severe TBI group (6~8, n=35) and particularly severe TBI group (3~5, n=43). Ten cases of control brain tissue were taken from patients with cerebral aneurysms or benign tumor and also undergoing craniotomy during the same time. Peripheral blood from ten healthy people were involved as the healthy control group. Immunofluorescent double staining was used to detect the cellular local-ization of CXCL1 in brain tissues, and ELISA was used to detect the expression of CXCL1 in brain tissue and blood. The relationship be-tween the level of CXCL1 in peripheral blood at different time and the score of Glasgow Outcome Scale (GOS) was analyzed with Spear-man correlation analysis. Results In normal brain tissue, CXCL1 mainly localized in astrocytes. For severe TBI, CXCL1 mainly expressed in neurons and astrocytes. The level of CXCL1 was higher in brain tissue in the particularly severe TBI group than in the severe TBI group (t=-12.58, P<0.05). In the severe TBI group, the level of CXCL1 in blood reached a peak before surgery, then gradually decreased, and was still higher than that in the healthy control group 14 days after surgery (P<0.05), however, no significant difference was found 30 days after surgery compared to the healthy control group (P>0.05). In the particularly severe TBI group, the level of CXCL1 in blood reached a peak before and one day after surgery, then gradually decreased, and was still higher than that in the healthy control group 30 days after surgery (P<0.05). The level of CXCL1 in blood was higher in the particularly severe TBI group than in the severe TBI group at all time points (P<0.05), and the level before surgery was negatively correlated with the score of GOS in the particularly severe TBI group (r=-0.351, P<0.05). Conclusion The CXCL1 protein of injury brain tissue was mainly colocalized in neurons and astrocytes in severe TBI patients, and the ex-pression was associated with injury severity and outcome.

Objective According to the level of serum cystatin C (Cys-C)in patients with multiple myeloma (MM),to explore its significance of early assessment to renal damage.Methods To select 42 patients with MM(MM group) and 30 healthy subjects(control group),and measure serum Cys-C,creatinine (SCr) and β2-microglobulin (β2-MG).Results The level of serum Cys-C in MM group was significantly higher than that in control group [(2.34 ± 0.57) mg/L vs.(0.66 ± 0.12) mg/L],which increased with ISS stage[Ⅰ,Ⅱ,Ⅲ stage was(1.77 ± 0.38),(2.11 ± 0.46),(3.14-± 1.07) mg/L](P＜ 0.05).Cys-C and β 2-MG,renal function had a positive correlation (r =0.760,0.694,P ＜ 0.05).Conclusion The increasing of Cys-C in MM patients is related to impaired renal function and tumor burden,which can be used as an early assessment indicators.
