1.Coblation plasma surgery system in the treatment of perennial allergic rhinitis

OBJECTIVE To compare the effect and safety of coblation plasma surgery and beclomethasone dipropionate nasal spray in the treatment of perennial allergic rhinitis. METHODS A stratified,randomized,controlled trial of surgery and medicament was performed in the treatment of perennial allergic rhinitis. A total number of 102 patients were involved in the trial,including 56 cases with swelling of inferior turbinate,28 cases in surgery group,28 cases in medicament group; and 46 cases without swelling of inferior turbinate,23 cases in surgery group,23 cases in medicament group. Coblation plasma surgery was performed in all surgery groups. Each patient was given 50?4?2 ?g/d of beclomethasone dipropionate nasal spray in all medicament groups,Treatment time was 3 months. The symptom scores (SS)and visual analogue scale(VAS)were recorded. RESULTS Effect assessment showed that both methods had significant repressive effect on allergic symptoms,the effect of surgery group was better than that of medicament group at the 14th day,28th day and 3th month in cases with swelling of inferior turbinate. Difference between the surgery group and medicament group in cases without swelling of inferior turbinate had no statistic significance. CONCLUSION Both coblation plasma surgery and beclomethasone dipropionate nasal spray are effective to treat perennial allergic rhinitis. But coblation plasma surgery is better than medicament in cases with swelling of inferior turbinate.
2.Multicenter clinical study on patients with inflammatory bowel disease in Kunming city

Objective To retrospectively investigate the characteristics of patients with inflammatory bowel disease (IBD)in last 10 years in Kunming city. Methods Four hundred and thirty consecutive patients with IBD, who had hospitalized in 7 hospitals between January 1998 to March 2007, were investigated. Among them, 379 patinets had ulcerative colitis (UC) and 51 had Crohn's disease (CD). The patients who received coloscopy, histopathological and bariam enema examination accunted for 98.2%, 56.2% and 2.6% in UC group,respectively, and 72.5%,78.4% and 31.4% in CD group,respectively. The gender, age, occupation and the clinical manifestation of the patients, and the results of the colonoscopy and pathological examination were analyzed. Results Most of the UC patients were aged 30 to 39 years and 50 to 59 years with average age of (46.9±15.8) years, whereas CD patients were aged 20 to 29 years with average age of (41.6±17.2)years. The male and urban patients were predominat in both UC and CD groups. Mental workers were common in UC group. The symptoms of diarrhea (302,79.7%), abdominal pain (285,75.2%) and bloody stools (290,76.5%) were major events in UC patients, and those of abdominal pain (44,86.3%), diarrhea (28,54.9%) and weight loss (28,54.9%) were major events in CD patients. The coincidence ratio among colonoseopy, histopathology and bariam enema examinations was 88.4 % ( 329/372 ), 24.4 % ( 52/213 ) and 4/10 in UC patients, respectively, and 86.5%(32/37), 27.5%(11/40) and 75%(12/16) in CD patients, respectively. All of the UC patients were in active stage of disease,including 38.3 % in mild,42.2% in moderate and 19.5% in severe. In CD group, 7.8% was in relief stage and 92.2% in active stage,including 15.7% in mild,43.1% in morderate and 41.2% in severe.Conclusion The understanding of prevalence and clinical characteristics of IBD in last 10 years in Kunming city will be helpful in diagnosis and treatment of the disease.
3.Study on extration process of Naolisu Granules

AIM: To optimize the milling and extraction rate for Naolisu Granules (Rhizoma Acori Tatarinowii, Radix Notoginseny, Pheretima Radix et Rhizoma Rhei, Radix Curcuma, Radix Ginserg Rubra, etc.). METHODS: The granularity of powder was determined by different micropowder processes. The process was studied by orthogonal design with the content of volatile oil, the yield of extract and the content of emodin. RESULTS: The optimum process of milling was that coarse powder was micropulverized for 20 min at -8℃ ~ -12℃ . The optimum extraction of volatile oil was distillated for 4 hs after volatile medical materials were macerated for 1h with 7 times amount of water. The optimum process of percolation was percolated by 300mL of 75% alcohol with 3.0mL?kg -1 ?min -1 after the medical material powders were macerated for 6 hs. CONCLUSION: These optimum processes are suitable for preparation of Naolisu Granules.
4.MR guided focused ultrasound for treating uterine fibroids:a Meta analysis

Objective To investigate the recent effect and safety of magnetic resonance guided focused ultrasound (MRgFUS) for treating uterine fibroids with a Meta analysis. Methods Such databases as Cochrane Library,PubMed,EMbase, CBM,CNKI,CQVIP and Wanfang were searched from their establishment. After thestudy selected, assessment and data extraction conducted by two reviewers independently, Meta-analysis were performed by using the Comprehensive Meta-analysis V2 software. The volume of non-perfusion ratio (NPVR), fibroid shrinkage, clinical symptom severity (SSS score) and adverse reactions were analyzed.Results Nighteen retrieved articles were included, with a total of 1 549 patients. The results of Meta-analysis showed that:(1)nonperfused volume ratio (NPVR),Mean=57.1%(95%CI 45.0% —69.2%).(2)Percentage of FBs shrinkage:mean=31.4% (95% CI 29.2% —33.6%).(3)Symptom severity score (SSS):baseline mean=51.00(95%CI 41.61—60.40),3 months mean=32.50 (95%CI 25.90—39.10),6 months mean=28.62(95%CI(23.57—33.66),12 months mean=24.46(95%CI 15.44—33.49), the average of the severity score of the patient symptoms had a tendency to reduce.(4)The degree of adverse reactions were mild and could relieve itself, there was fewer serious adverse reactions. Conclusions The current evidence indicates that MRgFUS in treatment of uterine fibroidsis reliable and safe. MRgFUS can significantly reduce the volume and the symptom severity score ofthe patients.
5.Imaging manifestation of primary leiomyoma of the liver

Objective To explore the imaging characteristics of hepatic primary leiomyoma.Methods The CT and MR imaging findings of three pathologically proven primary leiomyoma of the liver were analyzed,and the correlation between the imaging features and pathological results was analyzed.Two patients underwent CT scanning.Two patients underwent MRI.Results The lesions appeared hypodense in CT images,and with elongated T_1 time and elongated T_2 time in MRI.One lesion was homogenous both in CT and MRI,with significant homogenous enhancement.The other two lesions were inhomogeneous,with inhomogeneous enhancement in one lesion.All the three lesions had the sign of pseudocapsule.Neither hepatic cirrhosis nor venous tumor emboli were present.Pathological examination showed proliferation of spindle- shaped neoplastic cells and hyperplasia of capillary.One tumor had central hyalinization and another tumor had central necrosis.Conclusion Primary leiomyoma of the liver is one kind of hypervascular tumors with pseudocapsule.CT and MR can provide helpful information to the diagnosis.
6.Analysis of intracranial cystic disease on ~1H MR spectroscopy

Objective To analyze the features of intracrayze the features of intracranial cystic diseases on ~1H MRS,and to evaluate the applicable value of ~1H MRS.Methods Fifty-two patients were examinated by single voxel ~1H MRS,included Ⅰ-Ⅱ graded astrocytomas(n=8),glioblastomas(n=9),metastasis(n=13),brain abscesses(n=10),epidermoids(n=4),anachnoid cysts(n=5),brain Cysticerciasis(n=3).Results(1)There was only Lac in Ⅰ-Ⅱ graded astrocytomas.But in glioblastomas,as were as all showed Lac,4 cases showed low Cho and NAA and 2 cases Lip.(2) In(13 metastasis) patients,7 cases only presented Lac,6 cases showed Cho,3 cases showed low NAA and(6 ones) Lip.(3) In brain abscesses,all showed Lac,9 cases displayed AA,6 cases showed Ace,5 cases appeared Suc and Ala,2 cases had Lip.(4) Four cases of epidermoid showed Lac,one showed Lip.In(4 arachnoid) cysts,2 cases had low Lac,one showed Lip.Three cases of cysticerciasis showed Lac,Ace,Suc,Ala and no AA,Cho,NAA.Conclusion Lac is the most common resonance peak on ~1H MRS in intracranial cystic diseases and for no diagnostic specificity.AA combined Ace,Suc,and Ala are highly specificity for brain abscess.Ace,Suc and Ala appearance cues the the possible existence of cysticerciasis.~1H MRS is helpful for the qualitative diagnosis of intracranial disease.
7.Analysis of the ADC Values on Intracranial Epidermoid Cysts

Objective To analyze the DWI characteristics of intracranial epidermoid cyst (ECs),and to evaluate “T2 shine-through” effect. Methods There were 15 cases of ECs confirmed by pathology, in which 3 cases had the residual tumor after surgery. The ADC value was measured in ECs,deep white matter and CSF,and the comparison was made between DWI and FLAIR in showing the tumors. Results (1) The lesions in 15 cases were high intensity on DWI, their ADC value was from 0.80?10 -3mm2/s to 1.23?10 -3mm2/s, the average was (1.05?0.14)?10 -3mm2/s. But the ADC value of deep white matter was from 0.74?10 -3mm2/s to 1.09?10 -3mm2/s, the mean was (0.88?0.09)?10 -3mm2/s. The ADC value of CSF was from 2.98?10 -3mm2/s to 3.78?10 -3mm2/s,the mean was (3.31?0.21)?10 -3mm2/s. The ADC value of ECs was significant lower than that of CSF ( P
8.Analysis on Imaging Characteristics of Gastric Lipoma

Objective To analyze the imaging characteristics of gastric lipoma.Methods The clinical appearances and imaging characteristics such as size,margin,density or intensity,ulceration and so on of gastric lipomas in 13 cases were retrospectively analysed.Results 13 gastric lipomas were all solitary and intracavitary growth,origined from submucosal of every part of stomach.6 gastric lipomas showed submucosal filling defect and soft gastric wall,3 cases with ulcer presenting "bull eye sign" on barium meal.8 gastric lipomas on CT showed variable-size round or oval low-density masses with-50~-120 HU CT value.2 gastric lipomas on MRI were high intensity on T1/T2WI and low intensity on fat supression T2WI.Conclusion X-ray,CT and MRI are of respective value in diagnosing the gastric lipoma.
9.Characteristic manifestations of disseminated peritoneal adenomucinosis on multislice spiral CT

Clinical data and manifestations on muhi-slice spiral CT (MSCT) of 11 patients with disseminated peritoneal adenomucinosis (DPAM) were retrospectively reviewed.The CT manifestations were also compared with surgical and histopathological findings.MSCT findings showed a large amount of gel-like ascites in 9 cases and local cystic masses in 2 cases.Among 9 cases with a large amount ascites,abdominal multiple cystic masses were shown in 5 case,and infiltration of the greater omentum and mesentery in 5 cases.Hepatic scalloping was found in 6 cases ; parenchymal invasion of the liver or spleen were showed in 5 cases; calcification of the cystic masses in 5 cases.Ovary mueinous cystadenoma was presented in 3 female patients.Enlarged lymph nodes and omental cake were not found in all cases.The results indicate that the characteristic MSCT manifestations of DPAM include diffuse gel-like ascites,multiple cystic masses with or without calcification,hepatic scalloping and parenchyma invasion.
10.Clinic retrospective study on treating acute and chronic hepatitis B with extract of Silybum marianum Gaertn

Objective:To conduct a clinical tretrospective study on the effect of Silymarin Meglumine Tab on acute and chronic hepatitis B and to explore the related clinical significance. Methods: 41 cases and 205 cases of hospitalized patients pathologically diagnosed as acute and chronic hepatitis B respectively by liver tissuse puncture were divided into three group: ① Silymarin Meglumine Tab group; ② Ganyanling group; ③ complex group consisting of Silymarin Meglumine Tab and Gangyanling and diammonium glycyrrhizinate. To detect the index of routine liver function test before and after the treatment and analyze the hospitalization treatment course. Results: Compared with before treatment, liver functions of acute and chronic hapatitis B were improved to certain extent within each group after treatment (P0.05). The hospitalization treatment time of acute hepatitis B of the three group were almost the same, but the hospitalization treatmeng time of chronic hepatitis B of the Silymarin Meglumine Tab group was 2-4days less than that of the Ganyanling group and the complex group. Conclusion: As far as Silybini Meglumine Tab was concerned, it was better for chronic hepatitis B than acute hepatitis B because it was less complicated to administer the medicine, relieving the liver's burden of metabolism, and hospitalization treatment time was shorter, relieving patients' financial burden,which suggested that it was important to administer proper amount of medicine while taking into consideration protecting the liver's compensation function. If consideration was given to stimulating liver microcirculation with small dosage of TCM, so as to promote the exchange of the liver metabolite in blood, or detoxication and expulsion of toxin, it was likely to be more advantageous to the restoration of liver function and the liver tissue structure.
