1.Protective effects of octreotide on ischemic-reperfusion injury for resection of liver

Objective To study the protective effect of Octreotide against ischemia-reperfusion injury when liver was resected Methods Using the routine Pringle's maneuver hepaticvaseular occlusion,thirty patients for resection of liver with cirrhosis were random divided into two groups:octreotide group(n=15)and physiologic saline group(n=15).In octreotide group,the patients with hepatecto15),the patients were injected 1ml saline in the same way.The variations of postoperative liver function were measured.Plasma MDA and TNF-α after reperfusion for 1h were measured and the hepatic histopathologic alterations were also examined.Results The levels of ALT and AST from the octreotide group increased less than those of the physiologic saline group(P<0.01).Meanwhile,Plasma MDA and TNF-α from the octreotide group was slightly increased(P<0.01)and the hepatic histopathologic alterations were significantly decreased.Conclusions Given octreotide by subcutaneous injection before blocking portabepatis show all important protective effect against the hepatic isehemia-repedusion injury for resection of liver.
2.The relationship between the single nucleotide polymorphisms of multidrug resistance gene 1,brain derived neurotrophic factor gene and childhood drug resistance epilepsy

Aim To explore the relationship of the single nucleotide polymorphisms of multidrug resistance gene 1 and brain derived neurotrophic factor(BDNF) gene to childhood drug resistance epilepsy.Methods Two single nucleotide polymorphisms,T-129C polymorphism in multidrug resistance gene 1 and C270T polymorphism in BDNF gene,were conducted with PCR-restriction fragment length polymorphism analysis.The distribution of genotypes and allele frequencies of two single nucleotide polymorphisms in childhood drug resistance epilepsy were compared to those in drug respond epilepsy and controls.Results The distribution of TT genotype and T allele frequencies of multidrug resistance gene 1 in drug resistance epilepsy differed significantly from those in drug respond epilepsy and controls(P0.05).Conclusions The findings suggested that the T-129C polymorphism of multidrug resistance gene 1 maybe associated with childhood drug resistance epilepsy and played some role in the etiology of drug resistance epilepsy,but C270T polymorphism of BDNF gene was not confirmed to relate to childhood drug resistance epilepsy.
3.Transabdominal Total Gastrectomy and Gastro-intestinal Reconstruction by Substitution of "P" Shaped Jejunum for the Treatment of Cardial Carcinoma

Objective To explore the new operation approach and the effect of surgical therapy on cardial carcinoma. Methods A total of 78 patients with cardial carcinoma received surgical therapy by the transabdominal route from January 1992 to January 2002, of which 30 cases were treated by total gastrectomy by substitution of P shaped jejunum and 48 cases were treated by subtotal gastrectomy. All patients were followed-up postoperatively from 6 months to 5 years. Results The average length of resected lower part of the esophagus above the tumors was over 5cm, without cancer tissue retained in resected edge. The rate of intramediastinal lymph node metastasis was 18.4%. 5-year survival rate of the patients with total gastrectomy was higher than that of the patients with subtotal gastrectomy, and the morbidity rate of reflux esophagitis of the former was lower than that of the latter. Conclusions The patients of cardial carcinoma in Ⅲ stage or Ⅱstage with lymph node metastasis of 5 or 6 groups should be treated by radical total gastrectomy. The P shaped reconstruction of stomach with jejunum as a replacement satisfies the physiological needs and causes less compications.
4.Biomechanical properties of the newly-designed femoral self-locking nail with bevels

Objective To introduce a new design of interlocking system which is easy in use, makes the distal fracture fragments well aligned and rigidly fixated, reduces exposure to radiation for the surgeon and patient, and allows for a decrease in operating time. Methods The new interlocking system was designed to be a self-locking nail with bevels (b-SLN). According to the bevel principle, the axial force of the nail should be turned into a transverse force so as to control the transverse nail's movement. 10 dry-bone simples were made into the fracture model of the middle-femur to test the prototype nail. The retention of the nail in the intramedullary cavity was evaluated by the radiogram. The mechanical properties of a simulated model of single leg loading were tested on the instron-1342 type MTS. Results The prototype nail performed well as designed. The process was stable and reliable. The retention in the intramedullary cavity was fine. The shearing force between the transverse nail and main nail was enough to cut anything around them. The mechanical properties of b-SLN were similar to those of the Grosse-Kempf nail, better than auto fork compress locked intramedullary nail, and much better than MHUA nail and Ender's nail. Conclusions The structure of b-SLN is simple and reliable. There is no focus-point under stress. Its biomechanic properties are satisfactory. It is easy to use with no need of fluoroscopes in the operating theatre.
5.Effects of Triptolide on Proliferation and Apoptosis of Cyst-lining Epithelial Cells of Polycystic Kidney Disase

Objective To investigate the effect of triptolide (TP) on proliferation and apoptosis of cyst-lining epithelial cells with autosomal dominant polycystic kidney disease ( ADPKD). Methods Primary cultured cyst-lining epithelial cells were treated with TP at different concentrations for 12 h,24 h,48 h and 72 h, respectively.The proliferation activity of the cells was evaluated by Brdu assay. The cell cycle distribution was determined by flow cytometry. The apoptotic and apoptotic ratio were determined by FITC-AnnexinV binding/ PI. The morphological changes of cyst-lining epithelial cells were observed under transmission electron microscope. Results TP significantly inhibited the proliferation of cyst-lining epithelial cells and induced apoptosis in a dose- (10-40 ng?mL-1 )and time-dependent(12-48 h) manner. Typical ultrastructural changes of apoptotic cells were observed under electron microscope. Conclusion TP significantly inhibited the proliferation of cyst-lining epithelial cells and induced the apoptosis of cyst-lining epithelial cells, thus inhibited cyst forming and delayed cyst developing. The mechanism may involve several targets and pathways.
6.Impact of number of implanted fiducials on image target localization accuracy

Objective To find a proper number of implanted fiducials in order to reconcile both more accurate image tacking and less pain for the patients.Methods The phantom was made of two parts of different materials.The inner part was a ball-cube phantom developed by Accuray Corporation, which could be regarded as a rigid body, with 5 fiducials inside using as the reference to align the whole phantom in the experiment.The outer part was made of additional silicone rubber with two components to simulate soft tissue as a non-rigid body, which was implanted with 8 fiducials inside.All combinations of different number of fiducials were tracked to aquire the target location information, which were then compared with the reference position that was set by the inner 5 fiducials to obtain the deviations of the translation and the rotation parameters.Thus the impact of the number of fiducials on image target localization accuracy could be analyzed easily.Results When 4 fiducials are used for tracking, the decline of translation and rotation errors for every fiducial is largest (0.086 mm and 0.033°).The translation error was decreased by 0.343 mm from 1 to 4 fiducials, while by only 0.077 mm from 4 to 8 fiducials;the same decline was observed for rotation errors (0.131° for 3 to 4 fiducials , and 0.09° for 4 to 8 fiducials , respectively) .Conclusions When 4 fiducials are used for tracking, the decline of errors for every fiducial is maximum.When one more fiducial is adding, the image target localization accuracy can be enhanced obviously from 1 to 4 fiducials, but not as obvious for more than 4 fiducials.

8.Expression of Notch1 protein and its clinlcopathological significance in colorectal carcinomas

Objective To investigate the expression of Notch1 protein and its clinicopathologieal characteristics in colorectal carcinomas. Methods The expression and location of Notch1 protein were determined using immunohistochemical S-P method in 30 colorectal carcinomas and 18 normal colorectal tissues. Results The average positive rate of Notch1 protein was 45.5 ± 0. 7% in colorectal carcinomas and 6. 8 ± 0. 9% in normal colorectal tissues. There was significant difference between colorectal carcinomas and normal colorectal tissues (P < 0. 05). The expression of Notchl protein in different age, sex, tumor site, nodal involvement and Dukes stage had no significant difference. The expression of Notch1 protein in different histology grade had significantly statistical difference. Conclusion Notch1 expression was associated with the development of colorectal carcinoma. Notch1 was the potential oncagenic gene. Nothe1 would become the new target of the colorectal tumor therapy in the future.
9.The influence of traumatic subarachnoid hemorrhage on prognosis of head injury.

OBJECTIVETo study the influence of traumatic subarachnoid hemorrhage on secondary intracranial damage in GCS 13-15 head injuries and prognosis.
METHODSOne hundred and twenty-eight patients with mild head injury, including 64 with subarachnoid hemorrhage and 64 without subarachnoid hemorrhage, were selected and analyzed according to the changes of their conditions after injury.
RESULTSIntracranial abnormality was found in 14 patients (21.87%) with subarachnoid hemorrhage and only in 4 patients (6.25%) without subarachnoid hemorrhage (P<0.01). In the 14 patients, 4 were given surgical treatment. Mild disability was in 2 patients and 2 completely recovered. The rest were conservatively treated and achieved complete recovery at last.
CONCLUSIONSTraumatic subarachnoid hemorrhage, as a factor of intracranial complications in mild head injury should be given much attention. Early drainage of bloody cerebrospinal fluid by lumbar puncture is an effective method for prevention and treatment of complications in mild head injury.

Objective To investigate the utilization of ultrasound monitoring the renal blood flow during extracorporeal membrane oxygenation (ECMO).Methods Twentry one cases,who received veinartery ECMO for heart failure,were examinated by bed-side ultrasound before the ECMO initiated,right after the ECMO initiated,each day after the ECMO initiated,and right after the ECMO weaned.The renal interlobar artery peak velocity (Vmax) was measured,and the renal interlobar artery resistant index (RI) was calculated,as well as the values of the serum creatinine (SCr) and blood urea nitrogen (BUN) were recorded.All the data were compared.Results Compared to the variables right after the ECMO initiated,thc Vmax incrcascd (P < 0.05) two days after ECMO initiated and right after the ECMO weaned,while RI (P < 0.05),SCr (P < 0.05) and BUN (P < 0.05) decreased,there being significant differences (P < 0.05).Conclusions While treating patients with extracorporeal membrane oxygenation,the ultrasound can monitoring the renal blood flow effectively,and provide important parameters for the clinical doctors as the basis of the diagnosis and treatment.
