Objective The aim of this study is to elucidate the current status of hilar cholangiocarcinoma with regard to its epidemiology,clinical features,diagnosis,Bismuth-Corlette typing,pathologic categories,treatment,complication and prognosis in China.Methods Original articles published form January 1991 to August 2008 were searched in the CBM disc,VIP information and CNKI.Clinical appraisal and data extraction were conducted independently by 2 reviewers.The outcomes of 2280 procedures from 22 retrospective studies were analyzed.Results When radical resection was compared with palliative resection,the 1-yr survival rate (84.0％ vs.46.8％,OR=5.71,95％CI 3.99～8.17,P＜0.01) and 3-yr survival rate (31.5％ vs.13.5％,OR=4.43,95％CI 2.07～9.47,P＜0.01) were significantly higher.For the palliative resection group,the 1-yr survival rate of the drainage group (27.2％ vs.38.7％,OR=0.47,95％CI 0.31～0.72,P＜0.01) was significantly higher than the palliative resection group.The rate of tumor resection rose after 2002 (x2=35.9588,P＜0.01),but the rate of radical resection did not significantly rise ((2 =2.1052,P=0.1468).Conclusions Radical resection is still the optimal treatment for hilar cholangiocarcinoma.If radical resection is technically not possible,palliative drainage is a reasonable treatment.Palliative resection do not improve survival and it is not recommended.

MicroRNA-494 (miR-494) is one of the microRNAs from 14q32.31 miR-gene cluster.Recently, miR-494 was found to closely relate with tumors and other diseases .This article reviews the expression changes , roles and possible regulatory mechanisms of miR-494 in multiple tumors and other hypoxia/ischemia diseases .Recent studies demon-strate that the expression of miR-494 is affected by many factors , and miR-494 could be a biomarker of diagnosis , staging and prognosis in tumors and other diseases , and a target of therapy in future .

Objective To establish a loop-mediated isothermal amplification (LAMP) assay for rapid detection of the West Nile virus (WNV).Methods WNV genome (position nt 1 021 to nt 1 240) was synthesized by a PCR-based gene synthesis method.The synthetic fragments included 6 pairs of LAMP primer recognizing 8 primer sites of WNV genome.The LAMP gene amplification was carried out using a real-time PCR system at 63℃ for 60 min,then the amplification was terminated at 80℃ after 2 min.The amplification products were observed by agarose gel electrophoresis.The sensitivity and specificity of LAMP assay were compared with those of conventional PCR.Results The LAMP assay took less than 20 min,and the amplification product took on a ladder-like electrophoresis pattern.The sensitivity of LAMP assay was 10-fold higher than that of conventional PCR,and the detection limit of LAMP was 9.23 copies/?l.The specificity of WNV-specific LAMP assay was demonstrated by the negative amplification results from dengue virus and Japanese encephalitis virus,both were closely related members of the Flavivirus family.Conclusion LAMP assay is rapid,cost-effective,highly sensitive and specific in detecting genes of interest,and is of great significance for WNV surveillance,especially for grass root units and on-sport surveillance.

In many medical instrumentation systems, there exists a long distance between manipulation platform and host unit. Wireless data transferring between the platform and the host could be a feasible way to set parameters. This paper introduces wireless transmitter and receiver circuit based on encoder chip PT2262 and decoder chip PT22720, and presents some typical applications in medical instrument development.

Objective To observe the effect of pure Chinese traditional medicine preparation Fuzheng Anti-poisonous capsule and Kang Aibao Lives capsule to treat HIV/AIDS. Methods 140 examples confirmed by the Yunnan Province Centres for Disease Control and Prevention were treated by Fuzheng anti-poisonous capsule and Kang Aibao Lives Capsule,according to the Chinese medicine syndrome differentiation. 83 cases taking the medicine for more than 6 months were collected. The changes of Chinese medicine clinical symptoms and signs,the clinical symptoms and signs,the integrals,body weights,cold-catching numbers and CD4 were observed. Results The effective rate,steady rate and ineffective rate of Chinese medicine clinical symptom were 79.52%,10.84% and 9.63%,respectively. Increase,stable and reduces of integral were 48.19%,45.78% and 6.02%,respectively. Weight gain,stable and reduce were 56.62%,19.28% and 24.10%,respectively. Reduce,stable and increase of cold-catching number were 46.99%,43.37% and 9.64%,respectively. Effective,stable and ineffective of CD4 was 56.00%,10.67% and 33.33%,respectively. Conclusion The Chinese medicine has the remarkable curative effect in treating AIDS,and also has the obvious superiority.

Objective To investigate the genotype and gene mutation of hepatitis E virus isolated from the serum and stool samples of the patients with epidemic outbreak hepatitis E in certain recruit barracks of Guangzhou. Methods The reverse transcription polymerase chain reaction (RT-PCR) was performed to amplify the partial ORF2 nucleotide acid sequences of hepatitis E virus isolated from 34 and 46 inpatients with epidemic outbreak and sporadic hepatitis E respectively. The PCR products of the positive samples were cloned and sequenced. Results The 14 strains, including 12 epidemic strains and 2 sporadic strains, were isolated from the total 80 inpatients. The homology of nucleotide acid and amino acid sequences of 12 epidemic outbreak strains is 95.3%～100% and 94.0% ～100% . The homology between epidemic outbreak strains and sporadic strains is 95.3%～99.3% and 94.0%～100%. Compared with the standard different genotypes of HEV, these strains have the highest homology to the Jap1 strain which belongs to genotype Ⅳ, with homology of 96.0%～100%, respectively. Phylogenetic analysis indicated that these the nucleotide acid sequence homology of 92.0%～95.3% and amino acid sequence stains and Jap1 strain share the same cluster. Conclusion[KG1]Epidemic outbreak strains isolated from the patients in recruit barracks of Guangzhou belong to the genotype Ⅳ of HEV and the nucleotide acid sequences had partial mutation.

Objective To explore the relationship between the family conditions and internet addiction of medical college students with limb disabled. Methods 400 medical college students were investigated by the University Student Going-online Investigation table, EMBU and Chinese Internet Addiction Scale. Results There was significant correlation between parental occupations and internet addiction total score of medical college students with limb disabled( r= 0.195, P < 0.01 ; r = 0.163, P < 0.05 ). There was significant correlation and prediction between negative parental rearing patterns such as the parents' punishment-severity and rejection-denying and dimensions of interpersonal relations, health, forced going-online behaviors, time management( P < 0.05, P< 0.01 ), but there was no significant correlation and prediction between parents' warmth-comprehension and all dimensions. Conclusion Family conditions play a important role in influencing internet addiction of medical college students with limb disabled.

Objective To observe level of thyroid and corticosteroid hormone of peacekeepers in South Sudan the harsh environment under stress and explore the changes of hormone and its related causes the body to suffer stress.Methods 25 peacekeepers were drawn blood before going abroad,after going abroad 1 months,4 months and 7 months,respectively.Methods of immune by magnetic separation were adopted and determined the changes of thyroid hormone and cortisol levels.They were compared with before going abroad.Results After going abroad 1 months T3 (1.780 ±0.345)ng/mL,T4 (88.609 ±14.486)ng/mL,FT3 (3.758 ±4.287)pg/mL,FT4 (15.152 ±6.422)pg/mL than before going abroad (1.458 ±0.206)ng/mL,(2.060 ±0.642)ng/mL,(11.672 ±3.627)pg/mL,(96.958 ± 12.286)pg/mL were significantly increased (t =3.670,2.410,2.019,2.798,all P <0.0 ).TSH (1.320 ± 0.518)μIU /mL after going abroad 4 months was lower than before going abroad(1.872 ±0.821)μIU /mL decreased significantly(t =3.175,P <0.05).Conclusion In the role of the environment change of stress factors,changes of thyroid hormone and cortisol levels of body were obvious.We suggest to make countermeasure as soon as possible and appropriate intervention.At the same time the peacekeepers are detected comprehensive hormone detection and psy-chological counseling before going abroad,it is significant for the successful completion of peace -keeping task.
