1.DYNAMIC OBSERVATION OF PERIPHERAL T CELL SUBGROUP IN PATIENTS WITH RECTAL CANCER PREOPERATIVELY AND POSTOPERATIVELY

Objective The changes of peripheral T cell subgroup in patients with rectal cancer preoperatively and postoperatively were studied dynamicly.Method Peripheral T cell subgroup of 46 patients with rectal cancer was measured dynamicly by the monoclonar antibody enzyme labelling techniques during the preoperative and postoperative period.Result (1)CD3 cell and CD4 cell decreased markedly,the CD4/CD8 rate increased apparently in patients with rectal cancer.(2)Depressed immunity was related to the term of cancer.(3)The immunity of patients treated by radical rectal resection may recover gradually,the immunity of patients treated by palliative rectal resection,colostomy and laparotomy was depressed constantly.Conclusion The depressed immunity of patient with rectal cancer was related to the tumour preponderance.T cell subgroup dynamic surveillance was important to juide the possibility of recurrence and prognosis in patient with rectal cancer.
2.STUDIES ON THE ETIOLOGIC RELATIONSHIP OF MOLYBDENUM DEFICIENCY TO KESHAN DISEASE

A series of experiments designed in accordance with the biogeochemi-cal hypothesis on the etiology of Keshan disease was performed. It was shown that the pathogenic factors of Keshan disease existed in the grains cultivated in the endemic regions ("endemic regions" mean those localities where Keshan disease has been prevalent). These factors had an ability to elicit a metabolic derangement of calcium in the myocardium and indu- ce some latent damages in myocardial cells. Furthermore, there were definite relations between these factors and the nitrite. With the aid of nitrite, more serious myocardial necroses were produced by these pathogenic factors. It was also proved that the addition of ammonium molybdate to the grains from the endemic regions produced some prophylactic effect in reducing the myocardial injury in expermental animals. After an observation on fertilizing the soil in endemic region with ammonium molybdate for four years, the preliminary result showed that this measure seemed to be effective in preventing Keshan disease.Based on the results cited above, we considered that molybdenum deficiency and nitrite excess might play an important role in the occurrence of Keshan disease, but the exact pathogenic factors in the grains remained to be studied further.
3.Minocycline hydrochloride slow-release antibacterial ointment in the treatment of combined periodontal-endodontic lesions

BACKGROUND:The role of minocycline hydrochloride in adjuvant therapy of chronic periodontitis and
periodontal abscess has been widely reported, but detailed study of minocycline hydrochloride for combined periodontal-endodontic lesions is rare at present.
OBJECTIVE:To investigate the effect of minocycline hydrochloride in the treatment of combined periodontal-endodontic lesions.
METHODS:A total of 68 teeth with combined periodontal-endodontic lesions were treated with periodontal mechanical treatment and root canal therapy. According to the patient’s wiling, 68 patients were divided into experimental group (n=24; local injection of minocycline hydrochloride after periodontal mechanical treatment), positive control group (n=24; local injection of iodine glycerol after periodontal mechanical treatment), and
negative control group (n=20; no injection after periodontal mechanical treatment). These three groups were reviewed 1 month after periodontal mechanical treatment, and the differences before and after treatment were compared, including periodontal probing depth, attachment loss and gingival bleeding index.
RESULTS AND CONCLUSION: Probing depth, attachment loss and gingival bleeding index in three groups were significantly improved after 1 month of periodontal mechanical treatment (P< 0.05); both probing depth and
sulcus bleeding index in experimental group and positive control group were better than that of the negative
control group (P < 0.05), and probing depth and sulcus bleeding index in experimental group were better than that
of positive control group (P < 0.05). Experimental findings indicate that, in the clinical treatment of combined
periodontal-endodontic lesions, in addition to periodontal mechanical treatment and root canal therapy, local application of minocycline hydrochloride can effectively reduce the depth of periodontal pocket, improve gingival inflammatory
conditions and consolidate curative effect.
4.Application of 128 slice spriral CT in the measurement of anatomy of facial nerve canal before cochlear implantation

Objective To take the imaging anatomy and measurement of facial nerve canal for cochlear implantation patients,and to provide the reference for how to avoid the injury of facial nerve in the opreration.Methods 35 patients which would be given to cochlear implantation were taken 128 thin-slice CT scan preoperative,then using multi planar reformation (MPR) and curved planar reformation (CPR) techniques,the three-dimensional reconstruction were carried out,and referring the facial nerve anatomy of posterior tympanic facial recess approachartificial cochlear implantation,the relationship between the vertical segnent of facial nerve canal and facial recess,tympanic cavity structure was key displayed.The facial nerve canal sections in temporal bone were measured,and compared with measurement results of 35 cases of cochlear implant patients.Results 35 patients underwent 128-slice CT scan three-dimensional reconstruction of MPR and CPR techniques,the facial nerve segment was clearly displayed,and its vertical section of the posterior wall of the external auditory canal,facial recess distance was (3.31 + 0.88)mm,(1.89 + 0.29) mm,respectively,and the vertical segment of the facial nerve canal wall,the facial recess distance of intraoperative measurement was (3.22 + 0.69) mm,(1.85 + 0.26) mm,respectively,and there were.no significant difference between the the both results(all P > 0.05).35 patients had no case of face paralysis.Conclusion 128-slice spiral CT thin scan combined with MPR and CPR techniques can understand the facial nerve canal and surrounding anatomical landmarks of the fine relationship,measurable facial nerve canal length,and provide basis for how to avoid facial nerve injury in the cochlear implant,temporal bone,and the other side of the skull base surgery.
5.Effects of gene interference binding immunoglobulin protein on stimulation of osteoblast differentiation induced by fluoride

Objective To observe the effect of RNA interference binding immunoglobulin protein (BiP) on expression of bone markers and keytranscription factors in osteoblast exposed to fluoride.Methods MC3T3-E1 cells were used as osteoblast model in vitro.The cell viability was test with cell counting Kit-8 after cells were administrated with varying concentrations of fluoride [0.0 (control),0.1,1.0,2.0,4.0,8.0,16.0,20.0,32.0 and 64.0 mg/L] for different duration.Cells transfected with small interfering RNA (siRNA) BiP were exposed to fluoride (2.0,8.0 and 20.0 mg/L) for 2 days.Real-time PCR and Western blotting technique were used to determine the gene and protein levels of BiP.Meantime,the mRNA expression of bone markers and key transcription factors was investigated by real-time PCR.Results The difference of all viability in fluoride-dose groups was statistically significant exposed for 1,3,7 and 14 days (F =46.7,118.6,214.6,325.6,all P < 0.05).Expression of BiP significantly increased in cells exposed to 20.0 mg/L compared to that of control (11.22 ± 3.25 vs.7.94 ± 1.31,P < 0.05).The expression of alkaline phosphatase (ALP) elevated in cells exposed to 2.0 mg/L of fluoride (12.81 ± 3.62 vs.6.86 ± 2.13,P < 0.01);conversely,it significantly reduced in cells exposed to 20.0 mg/L of fluoride (0.89 ± 0.17 vs.6.87 ± 2.14,P < 0.01).Cells transfected with siRNA BiP significantly decreased the ALP expression in cells exposed to fluoride compared to that of cells only exposed to the same concentration of fluorine (12.81 ± 3.62 vs.7.43 ± 2.06;5.92 ± 2.38 vs.3.96 ± 0.21,all P < 0.05).Cells transfected with siRNA BiP and administrated with 2.0 mg/L significantly reduced the osteocalcin expression (4.29 ± 0.99 vs.1.29 ± 0.86,P < 0.01).Similarly,expression of runt-related transcription factor 2 (Runx2) significantly increased in cells exposed to 2.0 mg/L.However,expression of Runx2 significantly decreased in cells transfected with siRNA BiP and administrated with fluoride.Cells transfected with siRNA BiP significantly decreased the Runx2 expression in cells exposed to 2.0 mg/L and 20.0 mg/L compared to that of cells only exposed to the same concentration of fluorine (1.13 ± 0.22 vs.6.61 ± 0.48;0.02 ± 0.02 vs.1.50 ± 0.38,all P < 0.01).As the downstream of Runx2,the expression of osterix in cells treated with different concentrations of fluoride was similar to that of Runx2.Conclusion BiP is not directly involved in the process of osteoblast differentiation induced by fluoride;instead,it affects the expression of bone markers and key transcription factors in osteoblast exposed to fluoride.
6.Effects of homocysteine on matrix metalloproteinase-1 and tissue inhibitors of metalloproteinase-1 in vascular smooth muscle cells

BACKGROUND: Homocysteine (HCY) has been verified as an independent risk factor of atherosclerosis and atherothrombosis of cardiovascular disease.OBJECTIVE: To observe the effects of HCY on the secretion and activity of matrix metallopotinase-1 (MMP-1) and tissue inhibitors of metalloproteinase-1 (TIMP-1) in vascular smooth muscle cells (VSMCs).DESIGN : Auto-control observation.SETTING: Pathology Room, Institute of Regeneration Medical Sciences, Jilin University.MATERIALS: In vitro cultured vascular smooth muscles cells (VSMCs) of rats were obtained from male Wistar rats with the body mass of about 150 g from weeks 4-6 supplied by Laboratory of Animals, Norman Bethune Medical Sciences Division, Jilin University.METHODS: The experiment was performed at the Pathology Room, Institute of Regeneration Medical Sciences, Jilin University from May 2001 to May 2003. VSMCs ofin vitro cultured rats were adopted and divided into 5 groups, 0(control group), 0.10, 0.25, 0.50 and 1.00 mmol/L HCY were added, respectively for 48 hours. Effect of HCY on activity of MMP-1 was observed with zymography. The secretions of MMP-1 and TIMP-1 and their mRNA expressions were studied with Western blot and semi-quantitative reverse transcription polymerase chin (RT-PCR).MAIN OUTCOME MEASURES: Activity of MMP-1, secretions of MMP-1 and TIMP-1 and their mRNA expressions.RESULTS: ①Secretions of MMP-1 and TIMP-1: The secretion of MMP-1 in the 0.25, 0.50 and 1.00 mmol/L HCY groups was lower significantly than that in the control group (P < 0.05-0.01). The secretion of TIMP-1 in the 0.10, 0.25, 0.50 and 1.00 mmol/L HCY groups was higher than that in the control group (P< 0.05-0.01). ②The MMP-1 activity decreased with the increase of HCY, but reduced obviously in the 0.50 and 1.00 mmol/L HCY groups (P < 0.01). ③The expression of MMP-1 mRNA in the 4 HCY groups was lower markedly than that in the control group (P < 0.01). The expression of TIMP-1 mRNA in the 0.25 mmol/L HCY group was higher than that in the control group (P < 0.05), and it was higher remarkably in the 0.50 and 1.00 mmol/L HCY groups than that in the control group (P < 0.01).CONCLUSION: HCY can inhibit enzyme activity, decrease collagen degradation and induce collagen accumulation by inhibiting the secretion of MMP-1, which indicates that reduction of collagen degradation induced by HCY is one of the pathogenesies of atherosclerosis.
7.The rapetic analysis of 48 cases with subcutanous fistula excision to long-path low anus fistula

Objective To investigate the clinical effects of subeutanous fistula excision for the long-path low anus fistula.Methods Operation for long-path low anus fistula,fistula length>5cm,divided the pafients into two groups.The therapeutic group:using subcutanous fistula excision;the control group:using fistula icision.Comparing the healing time,arIus malformation,recurrence rate.Results Therapeutic group 47 cases heal,the healing time was 12 days;no anus malformation;no arms ineontenience.In the control group,the healing time was 23 days;anus malformation 23 cases;anus gas incontenience 12 cases.Conclusion To use subcutanous fistula excision for the longpath low anus fistula has the advantages of short healing time,no anus malformation,less pain,and low reccurrent rate.
8.Effect of endoplasmic reticulum stress in renal injury of fluorosis rats

Objective To observe the change endoplasmic reticulum stress in renal injury of fluorosis rats,and explore the effect of endoplasmic reticulum stree in the mechanism of renal injury of fluorosis.Methods 48 Wistar rats were divided into 4 groups:control,fluoride-treated,low calcium,and low calcium plus fluoride-treated.The rats in fluoride-treated and low calcium plus fluoride-treated groups were treated with sodium fluoride (NaF,221 mg?L-1) through drinking water for 3 months.The pathological slice of rat kidney was made and the morphological changes were observed under optical microscope.The transcription levels of Grp78,Xbp1,CHOP and PDI in the kidney tissues were determined by reverse transcription-polymerase chain reaction (RT-PCR).Results Edema and vacuolar degeneration of the proximal tubule and distal tubule cells in the kidney tissues in fluoride-treated group were observed;and cell edema,vacuolar degeneration,scattered necrosis,minor regeneration repair and interstitial hyperemia in the kidney tissues in low calcium plus fluoride-treated group were found.The mRNA expressions of Xbp1 in the kidney tissues in fluoride-treated and low calcium plus fluoride-treated groups were markedly higher than that in the responding control groups(P
9.Induction chemotherapy combined with three-dimensional conformal radiation ther a py for locally advanced non small cell lung cancer

Objective To evaluate the effect and complication of inductio n chemot herapy combined with three-dimensional conformal radiation therapy (3DCRT) for l ocally advanced non small cell lung cancer (NSCLC). Methods Ninety-two such pa t ients were randomized into radiation therapy alone group(RT-, 50 patients) and i nduction chemotherapy combined radiotherapy group (CMT-, 42 patients). The indu c tion chemotherapy consisted of 2-4 cycles of platinum-based regimen. Results Th e overall median survival time was 15 months with 12 months in the RT group and 18 months in the CMT group(P=0.014)respectively. The 1-year o verall survival rates were 48.6% and 71.2% in RT and CMT group,respectively (P=0.004). The 2-year survival rates w ere 20.8% and 37.6% in RT and CMT group, respectively (P=0.0 41). Treatment was w ell tolerated and the toxicities were similar in either group. C onclusion The ad dition of induction chemotherapy to 3DCRT takes a survival advantage over 3DCRT alone for Stage Ⅲ NSCLC without increasing toxicities.
10.Control study of buprenorphine sublingual tablets and methadone for heroin addicts on out patients treatment during recovery period

ObjectiveTo observe the effect of buprenorphine sublingual tablets (Bup) and methadone for heroin addicts on out patients treatment during recovery period. MethodsHeroin addicts who completed acute drug detoxification treatment were assigned in the order of cross to the observation group and control group. The observation group and the control group were treated with Bup and methadone, respectively for six months. Both groups were observed on the time of keeping heroin abstinence during the treatment and were assessed with Anxiety Scale before and after Bup or methadone treatment. ResultsBup group kept abstinence from heroin longer than methadone group ( P <0.01 ) ,and had lower anxiety scores( P <0.05). ConclusionBupernorphine sublingual tablets used for out patients at recovery period could better alleviate heroin addicts' anxiety and prolong their abstinence time.
