1.Practice and discussion of the quality control for injection pump

Objective:To discuss the quality control method of injection pump and analyze the factors that affect the quality control of injection pump.Methods: Flow error and blocking alarm error were tested by the INFUTEST 2000E dual-channel infusion pump analyzer according to the national calibration specifications.Results: The syringe specifications, injection speed and injection solvent could influence the test for accuracy of injection pump. The lower injection speed set, the bigger flow test error was, while the smaller the blocking alarm test error was. On the other hand, the higher the injection speed set, the smaller the flow test error was and the bigger the blocking alarm test error was. Besides, the higher qualified rate of flow test was, the lower the qualified rate of blocking alarm test was.Conclusion: The quality control of injection pump should abide by the principle of controllable risk, and should not pursuit too much accuracy.
2.Development of Telemedicine System in Community Medical Care

Objective To utilize the expert resource in central hospital for helping doctors in community hospital to make suitable diagnosis and treatment,improve the technical level of community medical care. Methods The Telemedicine Diagnosis System is developed based on B/S mode and Microsoft Visual Studio.net C#2005 in Windows platform. Results The Telemedicine Diagnosis System has been developed, and realized the function of interface display, information management, medical image transmission and telemedicine diagnosis. Conclusion The system has friendly interface, simple operation, more reliability and quick data transmission, and it can accurately realize the experts' telemedicine diagnosis in time.

4.Experimental Study :the Preparing of Targeted-directed Probe (Hab18-SPIO) and Testing of Its Phisichemical Characteristics

Objective To prepare the Target-directed probe(Hab18-SPIO) and to test its phisichemical characteristics. Methods The McAb Target-directed probe(Hab18-SPIO) was prepared through hepatocellular carcinoma(HCC) McAb(Hab18) conjugated toSPIO using chemical method,while negative McAb probe(SED-SPIO) was also prepared using the same method.These agents weretested by high-performance liquid chromatography(HPLC) and electron microscope. Results The results of HPLC and electronmicroscope proved that Hab18 was conjugated SPIO effectively.The diameter of particles was between 50 and 70 nm.The binding rate of Hab18 was 97.5%. Conclusion The probe targeted to membrane-associated antigen of HCC(Hab18g) has been successfully prepared,which has novel physiochemical property.It is of great value in specific MR diagnosis of early HCCs.
5.Old femoral neck fractures with senile osteoporosis: choice of femoral prosthesis according to femoral calcar-medullary cavity ratio

BACKGROUND:Old femoral neck fractures with senile osteoporosis have more surgical difficulties.There are a lot of intraoperative and postoperative complications and long-term effects are poor.OBJECTIVE:To explore the clinical outcomes of hip arthroplasty in patients with old femoral neck fractures with senile osteoporosis.METHODS:From October 2012 to July 2014,20 patients with old femoral neck fractures and senile osteoporosis were enroled in this study.Posterolateral approach was used to perform hip arthroplasty.Of them,14 patients received primary total hip arthroplasty with biological prosthesis,and 6 patients were subjected to semi-hip arthroplasty with straight shank cylindrical renovated biological prosthesis.RESULTS AND CONCLUSION: The patients were folowed up for 3 to 24 months.Harris score was apparently increased after arthroplasty in patients with old femoral neck fractures and senile osteoporosis compared with pre-arthroplasty.Postoperative radiographs revealed that femoral stem biological fixation was good.Bone fixation was visible in radiographs at 3 months after arthroplasty.These findings suggested that old femoral neck fractures and senile osteoporosis could be treated with hip arthroplasty.To select the type of prosthesis and surgical methods according to the femoral calcar-medulary cavity ratio in patients with old femoral neck fractures and senile osteoporosis can effectively restore the function of hip joint.
6.Construction of a rabbit model of steroid-induced osteonecrosis of the femoral head by combining different concentrations of glucocorticoids with horse serum

BACKGROUND: Glucocorticoid has been shown to be a major factor of osteonecrosis of the femoral head (ONFH), so constructing a reliable, effective and low mortality ONFH model will be helpful for searching for a better treatment strategy of ONFH.OBJECTIVE: To construct a rabbit model of early ONFH by intravenous injection of different concentrations of glucocorticoids and horse serum.METHODS: Thirty healthy male New Zealand rabbits were randomly allotted to six groups, followed by given the injection of 10 mg/kg horse serum combined with 5 mg/kg dexamethasone (group A), 10 mg/kg horse serum combined with 10 mg/kg dexamethasone (group B), 20 mg/kg horse serum combined with 5 mg/kg dexamethasone (group C), 20 mg/kg horse serum combined with 10 mg/kg dexamethasone (group D), 10 mg/kg dexamethasone (group E), and 2 mL/kg normal saline (control group) via ear veins, respectively.RESULTS AND CONCLUSION: Abnormal MRI signal of the femoral head appeared in the group D at postoperative 2 weeks, while abnormal signal was seen at postoperative 4 weeks in the other groups except the controls. Six weeks postoperatively, 80% rabbits in the group D showed abnormal signals, which were significantly more than those in the groups C (50%), B (40%), A (25%), and E (20%) (P < 0.05). The serum levels of triglyceride and total cholesterol in the groups A, B, C, D were significantly higher than those in the control group at 3, 7, 14 and 30 days after injection (P <0.05). Compared with the control group, the ratio of empty lacuna sigmificantly increased in the group D (P < 0.05).These results indicate that the injection of high concentration of horse serum combined with the high concentration of dexamethasone is successful and safe to make an animal model of early ONFH.
7.Antitumor effect on gastric cancer in vitro of selective replicative-competent adenovirus CNHK300-mIFN-γ

Objective To compare the expression of mIFN-γ, replicative activities and anti-tumor activities of CNHK300-mIFN-γand Ad-mIFN-γin normal and gastric cancer cells. Methods The replicative activities of viruses in cells were measured by viral replication assay. CPE assay was used to detect the antitumor effect of viruses. The expression level of mIFN-γ in cancer cells was detected by ELISA. Results The infection of CNHK300-mIFN-γled to an obvious expression of mIFN-γin gastric cancer cells. The vector system CNHK300-mIFN-γpossessed more replicated potential than Ad-mIFN-γ, and could specifically kill most of BGC-823 cells at MOI value of 0.1, which was much better than that by the traditional adenoviral vector. Conclusion CNHK300-mIFN-γcan selectively replicate and effectively express mIFN-γ in tumor cells, and specifically kill gastric cancer cells, suggesting a splendid future as a new anticancer agent.
8.Endoscopic diagnosis and treatment of early occult biliarypancreatitis

Objective To explore early endoscopic diagnosis and treatment of the clinical value of biliarypancreatitis. Methods From January 2007 to December 2009 completed 93 cases acute pancreatitis of endoscopic treatment,routining for endoscopic incision nipple,observed remission of symptoms and recurrence after treatment. Results In these cases acute biliarypancreatitis was 62.4% (58/93),occult biliarypancreatitis diagnosed by endoscopy was 27.6% (16/58),remission rate was 93.8% (15/16),no surgical operation and recurrence. Conclusion Occult biliarypancreatitis occupies a certain proportion of acute pancreatitis,also easily missed diagnosis and recurred,endoscopy is currently the most effective method.
9.Preparation and characterization of dual-modality molecular probes for imaging angiogenesis of gastric cancer

Objective To develop an MR optical dual-modality probe targeting angiogenesis of gastric cancer and to study its physical characteristics , in vitro cytotoxicity and magnetic effects of different pulse sequences on 3 T clinical MR scanner.Methods We conjugated GX1-Cy5.5, a novel gastric cancer neo-vasculature targeted peptide labeled with Cy 5.5, to the surface functionalized magnetic nanoparticles according to different molecular weights (1∶100, 1∶500),resulting in dual-modality probe DPs100 and DPs500 (named DPs).The hydrodynamic size and zeta potential of DPs and DPs 500 were analyzed by nano-ZS.The human umbilical vein endothelial cells (HUVECs) and BGC-823 cells were treated with DPs for 24 h, and methyl thiazol tetrazolium ( MTT) method was used to detect the survival rate of cells.DPs with different concentrations were scanned on different MR sequences , and then the relative signal intensity was observed.The absorbance of HUVECs and BGC823 cells treated with DPs of different concentration (0.00, 1.25, 2.50, 15.00, 50.00, 100.00 and 150.00 μg/ml) were compared with single factor analysis of variance.Relative signal intensity of different MR sequences was compared using a paired Wilcoxon signed-rank test.Results The dual-modality probe targeting angiogenesis of gastric cancer was successfully constructed.The hydrodynamic size of iron oxide nanoparticles , DPs100 and DPs500 was (35.23 ±0.07), (39.49 ±0.16) and (40.43 ±1.70) nm and the Zeta potential was (0.31 ±0.20), ( -4.15 ±0.79) and ( -10.51 ± 2.37) mV.The coupled rates of DPs 100 and DPs500 with polypeptide were 92%and 94% respectively.The absorbance of HUVECs and BGC823 cells treated with DPs of different concentrations were 0.76 ±0.04, 0.80 ±0.03, 0.79 ±0.05, 0.75 ±0.06, 0.74 ±0.05, 0.77 ±0.01,0.71 ±0.04 and 0.38 ±0.04, 0.43 ±0.04, 0.41 ±0.03, 0.43 ±0.07, 0.44 ±0.04, 0.41 ±0.07 and 0.40 ±0.04, there was no statistical significance ( F=0.94, 0.51;P>0.05).The signal intensity increased first and then decreased following the increasing concentrations of DPs on T 1WI,especially on FSPGR T1WI (Z =-3.294,P <0.05), while the signal intensity decreased on T2WI or T2*WI.There was no significant differences in signal intensity on FSE T2 WI and SSFSE T2*WI with iron concentration >10μg/ml( Z=-7.110,P>0.05).With iron concentration≤10 μg/ml,the signal intensity on SSFSE T 2*WI was significantly decreased compared to FSE T2 WI ( Z =-2.023, P <0.05 ) .Conclusions DPs may be potential dual-modal probes for characterization of tumor angiogenesis by MR and optical imaging noninvasively , without causing significant effects on the cell activity in vitro , and SSFSE T2*WI may be the most sensitive sequence for DPs evaluation on MR.
10.Surgical treatment and perioperative management of gastric cancer with liver cirrhosis

Objective To explore the operative procedure and perioperative management of gastric cancer with liver cirrhosis.Methods Clinical data of 28 cases of gastric cancer with liver cirrhosis who were surgically treated in our hospital during the last seven years were retrospectively analyzed.Preoperatively,a detailed examination,and improvement of their nutritional status,liver function,and prothrombin time were aggressively made,and the possibility of tumor removal was estimated.Seven patients underwent radical proximal subtotal gastrectomy,and 4 of these cases underwent splenectomy and pericardial devascularizaion simultaneously.Five cases underwent total gastrectomy,and 3 of them had splenectomy and pericardial devascularizaion simultaneously.Twelve patients underwent radical distal subtotal gastrectomy,and 5 of these cases had splenectomy and suture of pericardial varices simultaneously.Four patients had palliative distal subtotal gastrectomy.The support of liver function and intense monitoring and management of complications were carried out postoperatively.Results Varying degrees of ascites occurred in all the 28 patients postoperatively,and other complications such as early liver coma occurred in five patients,extensive wound bleeding in three patients,left subdiaphragmatic abscess in one patient and wound infection in two patients.The morbidity rate was 100%(28/28).However,all the complications were recovered by non-surgical treatment.There were no perioperative deaths in this series.Conclusions Gastric cancer patients with liver cirrhosis are at a significant risk of developing postoperative complications.The surgical procedure should the "individual" principle based on liver function,history of upper gastrointestinal bleeding and the location of gastric carcinoma.Good hepatic reserve and perioperative care,meticulous hemostasis during operation,and prevention and treatment of postoperative complications are the likely determinants of operative prognosis.
