Objective To investigate the extubated timing in acute severe organophosphorus pesticide poisoning patients. Methods Seventy-five patients with mechanical ventilation for acute severe organophosphorus pesticide poisoning were divided into early group (34 cases) and late group (41 cases) by extubation time.Mechanical ventilation time,ICU retention time, tracheal intubation time,ventilator-associated pneumonia (VAP) incidence and extubation success rate of both groups were observed. Results The mechanical ventilation time,ICU retention time between two groups showed no significant difference [(110.66±28.59)h vs.(114.47±34.21)h,(140.63±28.52)h vs.(138.44±34.74) h,P>0.05].The tracheal intubation time in late group was significantly longer than that in early group [(135.27 ± 28.84) h vs.(119.03 ± 35.05) h,P < 0.05],but the VAP incidence was nostatistically significant difference between two groups [29.27% (12/41) vs.20.59% (7/34),P > 0.05].The success rate of extubation in late group was significantly higher than that in early group [97.56% (40/41) vs.79.41% (27/34),P < 0.05].Conclusion Delayed extubation can significantly improve the chances of successful extubation, and no increase in VAP risk and more secure in acute severe organophosphorus pesticide poisoning patients.

Methods Application blood gas analysis and PaO2/PAO2,QS/QT,PaO2/FiO2,A-ac-DO2,RI and so on many indexes of blood gas analysis,to 2001~2005 year period were hospitalized in my courtyard NICU makes clear diagnoses the ARDS parallel breath machine treatment 20 examples trouble carries on monitors. Results First time when blood gas analysis 60% trouble prompt low oxygen blood sickness (PaO2<50 mm Hg).Parameter the and so on RI,QS/QT,A-acDO2,PaCO2/FiCO2,PaCO2/PaO2 values separately were:3.32±4.18,10±9.85,149.06±159.54,94.96±50.47,0.12±0.05. 72 hours ventilation after the breath machine. Conclusion Carries on the clinical appraisal using these targets to ARDS, to early strives for the breath machine treatment, the judgement condition and estimates pre-after has the vital significance.

Objective To assess the reliability and validity of the Pittsburgh sleep quality index (PSQI) .Methods The cross-sectional survey was conducted in the outpatient department of this hospital .544 insomnia and non-insomnia voluntary participants (18-65 years old) from July to October 2011 were assessed with PSQI and the insomnia severity index (ISI) ,which were randomly delivered to them .Results The test-retest reliability of PSQI was 0 .994 .The split-half reliability coefficient of PSQI was 0 .824 . The overall Cronbach′s alpha coefficient was 0 .845 .The confirmatory factor analysis showed that the fitting indicators for the χ2/df ,root mean square error of approximation(RMSEA) ,non-normed fit index(NNFI) ,comparative fit index(CFI) and goodness-of-fit index(GFI) were 4 .83 ,0 .09 ,0 .96 ,0 .98 and 0 .97 respectively .The correlation coefficient between the PSQI and ISI was 0 .842 . The correlations coefficients between PSQI with the questionnaire type ,interview date and interview department were -0 .046 , 0 .276 and -0 .331 .Grouping by individuals with or without insomnia ,the area under the receiver operator characteristic curve (ROC) for PSQI was 0 .944(95% CI:0 .925-0 .963) .Conclusion PSQI has good reliability and validity ,and could be used for the sleep quality comprehensive evaluation in the patients with insomnia .

Objective To explore the value of PET-CT and CT in the assessment after radiofrequency ablation (RFA) in lung cancer. Methods Eighteen patients were randomly divided into short-term group ( 8 cases) and long-term group ( 10 cases).Patients in short-term group received PET-CT and CT examination within 10 days after RFA therapy, whereas those in long-term group got PET-CT and CT examination within 2 months after RFA therapy. The value of PET-CT and CT after RFA therapy through the result of the image was compared between two groups. Results All patients were classified as stable disease in CT,while in PET-CT assessment,33.33% (6/18) of the patients presented complete remission and 66.67% (12/18)presented partial remission.37.50% (3/8) of short-term group presented complete remission,62.50% (5/8) of short-term group presented partial remission.30.00% (3/10) of long-term group presented complete remission,70.00%(7/l0) of long-term group presented partial remission. Conclusion PET-CT is superior to CT in the assessment of the effectiveness of RFA therapy, and there is no difference between short-term and long-term PET-CT examination, therefore early PET-CT is more with clinical significance.

This study was aimed to explore the efficacy of establishing and implementing individual healthy di-ets on patients with early diabetic nephropathy ( DN ) . A total of 120 patients with early DN of our hospital were randomly divided into two groups , which were the experimental group and the control group . There were 60 cases in each group . All patients received routine treatment . The fasting blood glucose and 24-hour uri-nary protein were measured pre-treatment , 2-week , 2-month , and 6-month after treatment . Patients in the experimental group were required to receive individual healthy diet therapy as well as routine treatment . The fasting blood glucose of both groups was compared in each stage . The fasting blood glucose of the experimental group was lower than that of the control group after 2-week treatment . After 6-month treatment , the fasting blood glucose of the experimental group was lower than that of pre-treatment . However , there was no obvious difference in the control group compared to that of pre-treatment . There were significant differences on the number of cases with fasting blood glucose below the level of 7.0 mmol/L in different stages of both groups. The 24-hour urinary protein of both groups was also compared in each stage . After 2-week treatment , there was no distinct difference on the 24-hour urinary protein in each stage of both groups . However , after 6-month treatment , the urinary protein of the experimental group , compared to pre-treatment , indicated striking difference , while the control group did not show any difference . It was concluded that individual healthy diet has a significant efficacy in the treatment of early DN , especially in reducing and controlling fasting blood glucose. After carrying out the treatment for 6 months, healthy diet also takes on distinct effects in cutting down 24-hour urinary protein and keeping the glycosylated hemoglobin below 6 . 5%.

Objective To explore the clinical feature of severe enterovirus 71 (EV71) associated hand foot and mouth disease (HFMD),and genotype of EV71.Methods Fluorescent quantitation PCR was done for detecting EV71.RT-PCR was performed to amplify VP1 for sequencing and identifying genotype.A retrospective analysis was performed based on the clinical data of 15 cases with severe EV71 infection. Results EV71 nucleotide was positive in all 15 cases. The genotype of EV71 was C4.All cases had abnormal temperature and followed with nervous symptoms in the early stage.Average time was 1.26 days from onset to severe complications appearance.Eleven cases progressed to neurogenic pulmonary edema.Four cases accepted nasal continuous positive airway pressure.Eleven cases accepted oral trachea cannula mechanical ventilation.Except for 3 cases died, one case abandoned, others 11 cases were cured. Conclusion The isolated strains of EV71 in this study are all C4 genotype. All cases with severe EV71 infection were followed with nervous symptoms in the early stage, most of whom would progress to neurogenic pulmonary edema. The mortality would be cut down by using mechanical ventilation in early stage.
