1.Advances in Permanent Pacemaker Requirement after Transcatheter Aortic Valve Implantation

Cardiac conduction abnormalities and/or atrioventricular (AV) block have been reported as one of the most frequent complications of transcatheter aortic valve implantation (TAVI). The impairment of AV conduction was resulted from the balloon and valve-induced injury. Patients with right bundle branch block (RBBB) before TAVI operation, receiving the Medtronic CoreValve Revalving System (MCRS) implant and the implantation of valve prosthesis too deep in the left ven-tricular outflow tract have been reported to require permanent pacemaker implantation.
2.Blunted response of L-type calcium current to simulated ischemia and reperfusion in ventricular myocytes from diabetic rabbits

AIM: To evaluate the effects of simulated acute ischemia and reperfusion on L-type calcium current (ICa,L) in ventricular myocytes from diabetic and non-diabetic rabbits.METHODS: Using whole-cell patch clamp techniques, ICa,L was measured in left ventricular myocytes isolated from 6-week alloxan-induced diabetic rabbits and age-matched control ones at baseline, 5 min of simulated ischemia, and 5 min of reperfusion.RESULTS: There were no significant differences on baseline maximum ICa,L densities between diabetic and control ventricular myocytes. In control cells (n=11), maximal ICa,L densities of baseline, ischemia and reperfusion were (-8.36±1.63)pA/pF, (-5.90±1.75)pA/pF and (-4.22±1.02)pA/pF, respectively. The ICa,L of ischemia was less than that of baseline (P<0.01), while the ICa,L of reperfusion was less than those of baseline (P<0.01) and ischemia (P<0.05). In diabetic cells (n=9),the ICa,L of baseline, ischemia and reperfusion were (-7.55±1.62)pA/pF, (-6.05±1.58)pA/pF and (-5.12±1.13)pA/pF, respectively. Only ICa,L of reperfusion was less than that of baseline (P<0.01), while ICa,L of ischemia was not significantly different from that of baseline (P>0.05) or reperfusion (P>0.05).CONCLUSION: ICa,L in diabetic ventricular myocytes represents blunted response to acute ischemic injury, being decreased more slowly than that in control cells. Post-ischemic reperfusion is still a potent inhibitor against ICa,L in both diabetic and non-diabetic cells. This study may be indicative of the mechanism about ischemia-reperfusion injury to diabetic myocardium and the therapy for diabetic patients with ischemic heart disease.
3.Detection of Japanese B Encephalitis Specific Antibody in pig's serum with Single Radical Hemolysis

This paper is to report our study of using single radical hemolysis(SRH) technique to detect the specific antibody for Japanese B encephalitis (JBE) virus in 101 samples of pigs' serum at Chongqing area. It was found that SRH was more sensitive and more specific in the detection of the JBE virus antibody in the pig's serum than CF or HI. SRH is simple in its technique and easy to perform. In addition, it is very sensitive and specific and it can be reproduced easily. It is suggested that SRH be used in clinical diagnosis and in seroepidermic survey of JBE virsus infection.
4.Study on Chemical Constituents of Essential Oil from Acorus calamus from Two Different Habitats

OBJECTIVE: To analyze the chemical constituents of essential oil of Acorus calamus from different habitats in order to provide scientific basis of exploitation of A.calamus.METHODS: The essential oil was extracted from A.calamus by steam distillation.The chemical constituents were analyzed by GC-MS.The relative contents of chemical constituents were calculated using normalization method.RESULTS: 39 kinds of chemical components were identified in A.calamus from Yunnan province,accounting for 80.51% of total amount of essential oil.37 kinds of chemical components were identified in A.calamus from Sichuan province,accounting for 81.96% of total amount of essential oil.There were 31 kinds of same chemical constituents in A.calamus from Yunnan province and Sichuan province.The content of ?-asarone was highest in A.calamus from Yunnan province (13.46%) and that of ?-asarone was highest in A.calamus from Sichuan province (10.01%).CONCLUSION:The study can provide scientific basis for the further development of A.calamus.
5.Percutaneous Laser Hypothermia in the Treatment of Lumbar Discogenic Pain

Objective To evaluate the efficacy of percutaneous laser hypothermia in the treatment of lumbar discogenic pain. Methods From June 2006 to March 2008,35 patients with lumbar discogenic pain were treated by low-frequency laser hypothermia in our hospital with 1 second impulse time,1 second interval time,3.5 w power,and 30 minutes duration. The effectiveness of the therapy was evaluated afterwards. Results Among the cases,15 patients were treated by one operation,20 patients received a second therapy,and 6 underwent the therapy for totally 3 time; the treatment was effective in 32 cases (excellent in 7,good in 24,and fair in 1). The therapy is ineffective in 2 cases. One case was converted to open surgery for unsatisfying outcomes of the laser hypothermia. The total effective rate was 91.4%(32/35),and fineness rate was 88.6%(31/35). Conclusions Percutaneous laser hypothermia is an effective treatment for lumbar discogenic pain. The indications for the procedure should be selected strictly.
6.Diagnostic Value of HLA-B_(27) for Ankylosing Spondylitis

Objective To evaluate the diagnostic value for ankylosing spondylitis (AS).Methods Ninety-two patients with confirmed diagnosis of AS were allcated to Group A and 70 non-AS cases served as the control(Group B).Human leucocyte antigen B 27(HLA-B 27)was determined by micro-lymphocytoxicity test.Results The sensitivity and specificity of HLA-B 27 in diagnosing AS were 87.0% and 91.4% respectively.In a randomized human group,the morbidity rate of AS was 2.57% for those with HLA-B 27 being positive.In clinic,when the assumed morbidity rate of AS was 50%,the morbidity rate arrived at 91.01% for the cases of HLA-B 27 being positive.Conclusion HLA-B 27 is an important accessory diagnostic index for AS,but it can not be ued as a confirmed diagnostic index for AS.
7.Efficacy analysis of sequential surgery after the DCF scheme induction chemotherapy in T2-3 laryngeal carcinoma and hypopharyngeal carcinoma.

OBJECTIVE:
To study clinical efficacy of surgery after the DCF induction chemotherapy in T2-3 laryngeal carcinoma and hypopharyngeal carcinoma.
METHOD:
To analyze clinical data about 102 cases of T2-3 laryngeal carcinoma and hypopharyngeal carcinoma retrospectively, 62 cases were divided into experimental group and 40 cases in control group, the experimental group completed three cycles of the DCF induction chemotherapy, then the efficacy was evaluated, surgery were carried out in the end, the control group received operation directly. In two groups, tumor response rates, toxicities, positive surgical margin rates and survival rate differences were observed and compared.
RESULT:
Tumor response rate was 61.3% in experimental group, the laryngeal carcinoma rate was significantly lower than that in hypopharyngeal (44.4% VS 84.6%, P < 0.05). Compared to its control, there were no significant differences about the positive surgical margin rate, the median survival time, 3-year and 5-year survival rate of laryngeal carcinoma(P > 0.05). The foci edges of 0. 3 cm and 0. 5 cm positive margin rate of hypopharyngeal carcinoma in experimental group were significantly lower than in its control (P < 0.05), but among the 1 cm edge' positive rate, the median survival time, 3-year and 5-year survival rate no significant differences were found (P > 0.05). During induction chemotherapy in the experimental group, no serious adverse reactions were found.
CONCLUSION
Induction chemotherapy of DCF regimen can not only narrow T2-3 laryngeal carcinoma and hypopharyngeal carcinoma' area, but also ensure an adequate safety margin.

8.The investigation of the aged deformity in longevous area of Yunnan Province

Objective To investigate on out the deformity in the elderly of longevous area and the characteristics of the regions in Yunnan Province. Methods According to the forth census information, using multistage sample typical survey to the 18 counties (cities) with centenarians over 1/100 000. The deformity types of people aged 60 and over and the deformity rate of various age groups and its distribution were investigated. Results Among the 6 477 people aged 60 and over investigated, a total of 1 928 people were in deformity, the deformity rate was 29 8%. The elder the people, the higher the deformity rate was. The highest deformity rate was in Kunming and Chuxiong prefectures. The audition deformity was highest among all types of deformity in elder people. The occurrence of fracture was highest in Miao Nation in Honghe prefecture, reaching 18.1%. About 14 % of the centenarian people sufferred from dementia. Conclusions Effective control in blood pressure and osteoporosis are important method to protect the elder people and to promote the level of quality of life of the elderly.
9.Detection of lymphocyte subsets in the application of the infectious diseases

Objective Investigate the different changes of host cellular immunity in patients with different infection by compa-ring peripheral blood lymphocyte subsets.Methods Flow cytometry was used to detect the peripheral blood lymphocyte subset a-mong 96 patients including 31 bacterial infection cases,13 fungal infection cases and 62 virus infection cases.Results Compared with control group the ratio of CD3 + T cell was increased in virus group,and the ratio of CD4 + T was decreased in fungi and virus group and the fungus is lower than the virus group,while the ratio of CD8 + T cell was increased in the two groups,and fungi group is higher than the virus,CD4 +/CD8 + ratio in the two groups were reduced.Three groups of NK cells (of CD16 + CD56 + )ratio was reduced to some extent,and bacteria group was lower than that of other two groups.The ratio of B cell was increased in Bacteria group and it was more obvious in the G+ bacteria.NK cells in G+ bacteria and G- bacteria group were relatively lower than control group,but the difference between the two groups was not significance.In virus infection cases,the ratio of CD3 + T in HBV group was higher than that of dengue fever group.And the ratio of CD3 + and NK cells were increased in the two groups.The ratio of CD4 + was decreased in dengue fever group.Conclusion Detection of lymphocyte subset can objectively reflect and help to under-stand the state of the immune function of patients with infectious diseases,providing laboratory basis for the diagnosis and preven-tion in order to reduce the infectious risk and side effect.
10.Influence of general anesthesia on postoperative cognition function in the elderly patients by using different methods

Objective To evaluate influence of general anesthesia on postoperative cognition function in the elderly patients by using different methods and choose a better anesthesia method for elderly patients. Methods Forty elderly patients undergoing selective abdominal surgery were selected and divided into two groups: anesthesia was maintained with general anesthesia combined with epidural anesthesia in observe group and with general anesthesia singlely in control group. Vital signs were observed and recorded during operation and blood pressure varied within the extent of 20% of basic level. Cognition function of each patient 24 hours before and 24,48 hours after operation was e-valuated by MMSE method. Results All the patients maintained steady vital signs and there was no significant differ-ence in MMSE scores between two groups at all the time points. There were 11 cases(55% ) who had acute cognitive dysfunction in observe group and 13 cases(65%) in control group 24h after operation(P>0.05) ,5 cases(25%) in observe group and 9 cases(45%) in control group 48h after surgery(P>0.05). Conclusion Compared with single general anesthesia, general anesthesia combined with epidural anesthesia uses less general anesthetics and has less negative effect on postoperative cognition funetion in the elderly patients,it maybe better in elderly patients undergoing non-cardiac operation.
