1.Study of MRI features in the lession and brain atroplny of cerebral multiple sclerosis and its correlated factors

Objective To study MRI feature in the lession and brain atrophy of cerebral multiple sclerosis (MS), and to analyze the relationship and the its correlated factors between cerebral MS and brain atrophy. Methods The MRI data from 80 patients with cerebral MS were collected and these patients were divided into two groups according to age. Each patient received T1-weighted and T2-weighted scanning. The number of lesion, characteristics of lesion and brain atrophy were evaluated and compared with control group. The correlated factors of brain atrophy were analyzed. Results (1)The most focal demyelinating lesions of cerebral MS were orbicular-ovate or similar round like with distinct boundary. Typical lesions presented with equal or long T1 and long T2 signals. The macroaxis of lesion was vertical to tangent line of lateral cerebral ventricle. (2)Compared with control group, the cerebroventricular anfractuosity was longer and lateral fissure was wider on MRI in cerebral MS group. The diameter of brain parenchyma was shorter. Statistic differences were found between two groups. (3)Among correlated factors, EDSS was the main predictive factor for cerebral atrophy. Conclusions The most lesions of cerebral MS are mainly located around lateral cerebral ventricles, orbicular-ovate or similar round like with distinct boundary, equal or slight long T1 and T2 signals on MRI.Brain atrophy is generally in cerebral MS and progress gradually, it is related to the course of disease, the number of lesion, the diameter of lesion and EDSS score. Measurement of brain atrophy may regard as an index about progression of MS.
2.Effects of isoflurane on expression of genes for proinflammatory cytokines in alveolar macrophages of patients undergoing partial hepatectomy

Objective To determine the changes in interleukin-8 (IL-8) and interleukin-1?(IL-1?) mRNA expressions in alveolar macrophages during isoflurane anesthesia. Methods Twenty-four ASA Ⅰ-Ⅱ male patients undergoing partial hepatectomy were randomly divided into two groups: group Ⅰ isoflurane; group Ⅱ general combined with epidural anesthesia. The age ranged from 43 to 67 years and body weight from 50 to 74kg. The patients were unpremedicated. Anesthesia was maintained with propofol infusion (4-6mg? kg-1?h-1 ) fentanyl and vecuronium in both groups. In addition 1% isoflurane was inhaled in group I and continuous epidural anesthesia with a mixture of 1 % lidocaine + 0.2% poutocaine (5ml/h) was performed in groupⅡ . ECG, SpO2, BP and HR were continuously monitored during anesthesia. Alveolar macrophages were harvested by bronchoalveolar lavage immediately and 4h after induction of anesthesia. RNA was extracted from harvested cells and cDNA was synthesized by reverse transcription. Expressions of IL-8 and IL-1? were measured by semiquantitative polymerase chain reaction using ?-actin as an internal standard. Results Gene expression of IL-8 and IL-1? in alveolar macrophages increased significantly at 4h after induction of anesthesia. The increase was greater in group Ⅰ than in group Ⅱ( P
3.Clinical observation on acupuncture combined with tuina manipulation for thoracic facet joint disorder

Objective:To observe the clinical effect of acupuncture combined with tuina manipulation for thoracic facet joint disorder. Methods:A total of 93 eligible cases were randomly allocated into an observation group and a control group. Patients in the observation group (n=46) were treated with acupuncture combined with tuina manipulation, whereas patients in the control group (n=47) were treated with tuina manipulation alone. The treatment was done once a day, for a total of 3 times. Therapeutic efficacies were then evaluated according to scores on signs and symptoms.
Results:After treatment, there were intra-group statistically significant differences in scores of eight signs and symptoms (all P<0.01); and the between-group differences were statistically significant in scores of eight signs and symptoms (all P<0.05). After three times of treatment, the recovery rate in the observation group was 54.4%, versus 25.4% in the control group, showing a statistical significance (P<0.05).
Conclusion:Acupuncture combined with tuina manipulation can obtain better effects than tuina manipulation alone for thoracic facet joint disorder and is therefore worth further clinical application.
4.The relationship between endothelial microparticles in patients with ST-elevated myocardial infarction and myocardial injury

Objective To observe the level of circulating CD31 +/CD42b-endothelial microparticles in patients with ST-elevated myocardial infarction (STEMI),and discuss the correlation between CD31 +/CD42b-and traditional myocardial injury index.Methods A total of 22 healthy subjects and 44 patients with angiographically confirmed coronary atherosclerotic lesions collected from January 2010 to December 2010 were studied prospectively.The patients were divided into SAP (stable angina pectoris,Canadian Cardiovascular Society,CCS Ⅱ to Ⅲ ) group (n =22) and STEMI group (n =22).The level of circulating CD31 +/CD42b-endothelial microparticles was detected by flow cytometric device after admission; creatine kinase (CK) and its isoenzymes (CK-MB) were detected by using biochemical analyzer; C-reactive protein was determined by a highly sensitive latex-enhanced turbidimetric immunoassay with a low detection limit of 0.25 mg/dl (IMMAGE,Beckman Coulter; Reagent from Orion Diagn Co.Ltd.,Vantaa,Finland).Cardiac troponin I was measured by chemiluminescence immunoassay (ACCESS2 from Beckman Coulter).In 22 STEMI patients,blood sample was taken not only after admission but subsequently at 4-hour intervals during the first 48 hours.Peak levels of myocardial enzymes after injury ( creatine kinase,CK; creatine kinase MB isoenzyme,CK-MB; c troponins I,cTnI) and high sensitive C reactive protein (hs-CRP) values were determined.The correlation between circulating level of CD31 +/CD42b(-) and peak levels of myocardial biomarkers after injury were analyzed.Results The endothelial micro-particles were significantly higher in STEMI patients than those in either SAP group or normal group ( P < 0.01 ),whereas there was no difference between the latter two groups.In STEMI patients studied in these cross sectional study,circulating CD31 +/CD42b-microparticles were positively correlated with peak level of myocardial biomarkers after injury.Moreover,the correlations between myocardial biomarkers after injury ( CK,CK-MB,cTnl and hs-CRP) and circulating CD31 +/CD42b-microparticles were r =0.489,P =0.021; r =0.501,P=0.018; r=0.491,P=0.02; r=0.612,P=0.002.Conclusions The level of circulating blood CD31 +/CD42b-endothelial micro-particle was expected to become a predictive marker in STEMI.
5.A mandibular canine with two roots and two root canals: A case report

The mandibular canine usually has a single-root with a single canal. A case of a patient who has a mandibular canine with two well-defined roots and two root canals was reported. Size of the lingual root and buccal root was equal. The buccal root length was 24.5 mm. The lingual root length was 23.0 mm. Clinicians should be aware of the anatomical variation that exists in mandibular canines in practice.
6.Analgesia effect of magnesium sulfate in ultrasound-guided interscalene brachial plexus block in limb sur-gery

Objective To observe the anesthesia and analgesia effect of ultrasound guided inter-scalene brachial plexus block (ISPSP)on upper extremity surgery.Methods Fifty-four patients (male 34 cases,female 20 cases,aged 37-73 years,ASA grade Ⅰ or Ⅱ)scheduled for elective upper limb surgery under ISPBP combined with general anesthesia were randomly assigned into two groups:magnesium sulfate group (group M,n =27)and control group (group N,n =27).0.5% ropivacaine 8ml (40 mg)+10% magnesium sulfate 2 ml (0.5 g)were used in group M ,0.5% ropivacaine 8 ml+ normal saline 2ml (40 mg)were used in group N.After the effects of ISBPB were confirmed,pa-tients were inserted laryngeal mask under intravenous induction.Anesthesia was maintained by inha-ling sevoflurane(MAC 0.8)during the operation.The onset time of sensory and motor block,duration of sensory and motor block,duration of analgesia,pain visual analogue digital score (VAS)4,8,12, 24 hours after operationand complications were recorded.Results Duration of sensory block and du-ration of analgesia of group M was significantly longer than those of group N (P <0.05).The patient`s VAS score of 8,12,24 hours after surgery in group M was significantly lower than that on group N (P <0.05).Onset time of sensory and motory block of both groups was similar.Conclusion 0.5%ropivacaine combined with magnesium sulfate in ultrasound-guided ISPBP can extend the duration of sensory block,reduce postoperative pain,as well as prolong analgesia time.

8.Susceptibilities of Ureaplasma Urealyticum and Its Different Serotypes to Niaoluqing Decoction

Objective To study the in-vitro antibacterial activity of Niaoluqing Decoction on Ureaplasma Urealyticum (Uu) . Methods Sixty-three clinical strains of Uu were identified to determine their serology and antibiotic susceptibilities by the metabolic inhibition test(MIT). These strains were subcultured for 3 generations and then cultured with Niaoluqing Decoction with a series of concentrations from 250 mg/mL to 0.48 mg/mL. The minimal inhibitory concentration (MIC) was determined when the color of culture medium did not change from yellow to red in 72 h.Results For 63 clinical strains of Uu, the range of MIC of Niaoluqing Decoction was from 0.48 mg/mL to 15.63 mg/mL,MIC50≤1.95 mg/mL,MIC90≤3.91 mg/mL. Clinical strains of Uu had a higher sensitive to Niaoluqing Decoction than to tetracycline(?2=18.38,P 0.05).Clinical strains of Uu including all of its serotypes had a higher antibiotic susceptibility to Niaoluqing Decoction (MIC≤3.91 mg/mL) except for serotype 11, 1, 2 in this experiment contains (MIC≥7.81 mg/mL). Conclusion Niaoluqing Decoction showed a significant in-vitro antibacterial activity on clinical Uu strains with different serotypes.
9.Initial assessment of the reliability and value of dual-source computed tomography in the evaluation of coronary artery disease

Objective:The aim of this study was to assess the diagnostic accuracy of dual-source computed tomography(DSCT) in the evaluation of coronary artery disease(CAD).Methods:Fifty-two patients(44 male,8 female,mean age 70.59) with a high pre-test probability of CAD underwent DSCT coronary angiography and selective coronary angiography(SCA).Two physicians independently assessed the images of all coronary segments.Results:The overall sensitivity,specificity,positive and negative predictive value for evaluating CAD were 92.31%,99.82%,90.57%,and 99.32%,respectively.Conclusion:The result indicates that DSCT coronary angiography provides high diagnostic accuracy for assessment of CAD.And it is a better choice for CAD evaluation.
10.The study of CT features in pancreatic carcinoma and inflammatory pancreatic mass

Objective To compare various CT signs of pancreatic carcinoma (PC) and inflammatory pancreatic mass (IPM), and to study the diagnostic value of these signs for distinguishing two diseases. Methods Eigty-five patients with PC and IPM were proved by surgery, fine needle aspiration or other comprehensive methods. These patients underwent non-enhanced and enhanced CT scans. CT findings were analyzed retrospectively. The occurrance rates of various CT signs in these two diseases were analyzed with Fisher test and were compared with the corresponding clinical and operational results as welL Results Among the 85 patients, 66 patients were proved to have PC, and 19 were proved to have IPM. In PC group,58 were corerectly diagnosed with CT, 3 (4. 5% ) were misdiagnosed, and 5 (7.6%) were omitted. In IPM group, 9 were correctly diagnosed with CT and 10 (52. 6% ) were misdiagnosed. The CT findings were as follows: (1) Pancreatic mass with liver metastases, lymph node metastases, encased celiac arteries, and cancer emboli in portal veins just occurred in PC group. (2) The occurrence rates of mass over 3 cm in diameter, clear boundary, low-density area within the mass, pseudocysts, peripancreatie infiltration, ascites, and slight and moderate pancreatic-bile duct dilation in PC group were 90. 91% (60/66), 15.15% ( 10/66), 54. 55% ( 36/66 ), 10. 61% ( 7/66 ), 4. 55% ( 3/66 ), 22. 73% ( 15/66 ), 24. 24% ( 16/66 ), 45.45% (30/66), and 27. 27% (18/66) respectively, the occurrence rates in IPM group were 94. 74% ( 18/19), 15.79% ( 3/19 ), 52. 63% ( 10/19 ), 15.79% ( 3/19 ), 15. 79% ( 3/19 ), 21.05% (4/19), 31.58% (6/19) ,21.05% (4/19), and 5.26% (1/19) respectively. There was no statistical difference for these CT findings between two groups(P >0. 05). (3) Pancreatic head mass with atrophy of pancreatic body and tail, mass calcification, pancreatic duct-penetrating sign, pancreatic head mass with hypertrophy of pancreatic body and tail, biliary stones with inflammation , and thickening of pre-kidney fascia in PC group were 48.48% ( 32/66 ), 3.03% ( 2/66 ), 1.52% ( 1/66 ), 10. 61% ( 7/66 ), 6. 06% ( 4/66 ) and 3.03% (2/66) respectively, the occurrence rates of those in IPM group were 5. 26% (1/19),47.37% (9/19), 15.79% ( 3/19 ), 84. 21% ( 16/19 ), 36. 84% ( 7/19 ) and 21.05% ( 4/19 ) respectively. There was statistical difference for these CT findings between two groups ( P < 0. 05 ) . Conclusion Accurate evaluation of various CT signs in PC and IPM is of great importance in the diagnosis of the two diseases.
