3.Operative treatment of acetabular fractures

Objective To discuss the method for operative treatment of acetabular fractures and assess corresponding results.MethodTotally 34 cases of acetabular fractures treated operatively from February 2000 to December 2005 were reviewed. There were 22 males and 12 females, with an average age of 39 years (16 to 61).On the basis of X-ray and computed tomography,all fractures were classified according to the Letournel-Judet classification,13 cases were simple fractures and 21 complicated ones. Kocher-Langenbeck approach(23 cases),illioinguinal approach(6 cases) and anterior combined posterior approach(5 cases) were adopted for different fractures with reconstruction plates and screws.The mean operation time was 150 min.The average blood loss was 600 mL.ResultAll cases were followed up for average 31 months(12 to 64 months).According to the criteria of Matta radiographicgrade,there were 18 cases with anatomic reduction, 9 with satisfactory reduction,3 with unsatisfactory reduction and 4 with joint contour reduction.Based on the modified Merle D' Aubigne and Postel clinical grading system for joint functions,11 cases(32.4%)showed excellent results,15(44.1%)good,4(11.8%)fair and 4(11.8%)poor. 4.Application of artificial shoulder prosthesis in proximal humeral fracture

OBJECTIVE:TO summarize the treatment of complicated proximal humeral fracture with artificial shoulder replacement.METHODS:A computer-based online search of VIP was performed for related articles published between January 1998 and October 2009 with the key words"shoulder joint,prosthesis replacement,proximal humeral fracture".Articles with high correlation were included.and repetitive studies were excluded.The data were firstly collected,the references of each article were reviewed.RESULTS:A total of 18 articles were included The treatment of complicated proximal humeral fracture remains controversial.Early semi-shoulder ioint replacement is an appropriate method.Shoulder replacement includes humeral head replacement and total shoulder replacement.Neer and Biligani type are typical artificial humeral head.The fixation patterns of humeral head prosthesis shaft include cemented and cementless.Bone cement includes total bone cement and proximal cement fixation,and cementless includes pressure fixation and compaction bone grafting.Complications following shoulder replacement involve prosthesis instability,nodule heterotopia,heterotopic ossification,prosthesis loosening,periprosthetic fracture,infection and nerve injury.CONCLUSION:Majority of complications can be prevented by appropriate location of prosthesis,reconstruction of greater and lesser tubercle.and in combination with postoperative rehabilitation treatment.
6.Effect of long-term alcohol intake on field potential of cerebellar granule layer in mice and its mechanism

Objective:To investigate the effect of long-term alcohol intake on sensory information synaptic transmission of mossy fiber-granular cells in the cerebellar cortex of mice.Methods:Twenty healthy male ICR mice aged 6 to 8 weeks were divided into normal saline group(control group) and alcohol intake group(alcohol group) according to random number table, with 10 mice in each group. The mice in alcohol group were injected intraperitoneally with 20% alcohol and the mice in control group were injected with the same amount of saline for 28 days.After the injection, the scalp, muscle tissue and skull were removed in turn, and the dura mater was removed to fully expose the crus II area of cerebellum. The mice were stimulated by air blowing at 30 mm of the ipsilateral tentacle pad with a gas jet device.When the the maximal response site was determined, the NMDA receptor antagonist (D-APV), metabolic glutamate receptor 1 antagonist (JNJ16259685) and N-methyl-D-aspartic acid (NMDA) were perfused on the brain surface of mice. Each drug was perfused for 20 minutes and ACSF was used between the two drugs until the waveform was recovered. Patch clamp amplifier was used to record the changes of potential waveform in mouse cerebellar granule layer. The data were analyzed by the softwares of Clampfit 10.3 and SPSS 22.0.Results:After exposure to wind stimulation, the latency of field potential response in granular layer of mice in alcohol group (11.8±0.7)ms was significantly longer than that in the control group (10.1±0.2)ms ( t=-8.041, P<0.05), and the amplitude of N1 (1.2±0.1) MV was significantly lower than that in the control group (0.6±0.1) MV ( t=-12.728, P<0.05). Compared with the control group, the rise time of P1 waveform((4.4±0.2)ms, (3.2±0.2)ms), duration ((12.1±0.5)ms, (10.3±0.2)ms), extinction time((7.8±0.2)ms, (6.9± 0.2)ms), volume under waveform ((7.3±0.2)ms, (4.3±0.2)ms) were significantly increased in the alcohol group ( t=16.100, - 11.840, -11.673, -35.576, all P<0.05). There were no significant differences in the amplitude, half width, rise time and decay time of Roff wave between the two groups ( t=-1.909, -0.910, -0.789, 1.462, all P>0.05). When JNJ16259685 was perfused on the brain surface of mice in alcohol group, the amplitude of field potential evoked by five blowing stimuli had no significant difference compared with that before administration (all P>0.05). When D-APV was perfused into the brain surface of mice in the alcohol group, the amplitude of P1 ((42.3±1.5) Mv)was significantly lower than that before administration ((101.1±0.9)mV) and after elution ((100.1±2.2) mV) ( t=106.762, - 69.605, both P<0.05), and the area under waveform of P1 ((42.6±1.3)%) was also significantly lower than that before administration ((100.6±1.6)%) and after elution ((97.6±2.2)%) ( t=88.862, -67.791, both P<0.05).The ratio of N2 / N1 (0.3±0.1) was significantly lower than that before administration (0.4±0.1) and after elution (0.3±0.1) ( t=2.242, 2.121, both P<0.05). When NMDA was perfused on the brain surface of mice in the control group, compared with before administration and after elution, the amplitude of P1 ((110.7±3.2) mV, (100.1±0.9) mV, (102.0±1.7) mV, t=-10.173, 7.669, both P<0.05), the area under the waveform of P1 ((127.9±3.5)%, (100.0±3.1)%, (115.0±5.3)%, t=-18.698, 6.447, both P<0.05), the ratio of N2 / N1 ((0.5±0.1), (0.3±0.1), (0.3±0.1), t=-5.669, 5.669, both P<0.05) were all significantly increased. When D-APV was perfused on the brain surface of mice in control group, the field potential evoked by blowing stimuli had no significant difference compared with that before administration and after elution (all P>0.05). Conclusion:Long-term alcohol intake significantly suppresses the synaptic transmission of excitatory glutamate in MF-GC, and enhances the inhibitory response mediated by GABAA receptor in cerebellar cortex. The inhibitory component is enhanced by NMDA receptor, but not by type 1 metabolic glutamate receptor.
7.Effect of Different Dosage of Radial Extracorporeal Shock Waves on Fracture Disunite and Bone Nonunions

Objective To explore the effect of different dosage of radial extracorporeal shock waves on fracture disunite and bone nonunions.Methods 42 patients were divided into 3 groups based on the different shock dosage, every group contained 14 patients and the shock dosages were 1000, 2000 and 3000 in Groups A, B and C respectively. All patients accepted the therapy for 3 times and followed up with X rays. Results 3 months after treatment, calluses were formated iin 2(14.7%), 8(57.1%) and 8(57.1%) patients in 3 groups respectively,which was lower in Group A than in Groups B and C (P<0.05), but there was no significant difference between Group B and Group C (P>0.05). 9 months after treatment, the corresponding cure rates were 28.6%, 85.7% and 78.6% in 3 groups respectively. The cure rate was lower in Group A than in Groups B and C (P<0.05), but there was no significant difference between Groups B and C (P>0.05). Conclusion The radial extracorporeal shock waves had dose-dependence and saturation for the shock dosage, 2000 is the optimal choice
8.Effects of siRNA targeting DDR2 on hepatic stellate cells

Objective To explore the effects of inhibiting DDR2 expression by siRNA on hepatic stellate cells and evaluate the role of DDR2 gene in hepatic fibrogenesis. Methods (1) Three pairs of chemically synthesized siRNAs targeting DDR2 were respectively transfected into HSC-T6 cells for evaluation of silence efficacy, and the most effective siRNA was used. (2) HSC-T6 cells were divided into three groups, group A served as normal controls, group B served as negative control and group C was RNA interference DDR2 (siRNA-DDR2) expression of HSC. The most effective RNA interference sequences targeting DDR2 gene was chosen to transfect HSC-T6 cells by plasmid transfection. The tendency of DDR2, α-smooth muscle actin(α-SMA) and collagen-Ⅰ mRNA expression were estimated using RT-PCR, and the protein expression of DDR2 was evaluated by Western blot. Meanwhile, MTT assay was employed to analyze the proliferation of HSC. Results (1) DDR2 siRNA, which began at nt 868, inhibited DDR2 gone expression stronger than the other two siRNAs. (2) After transfection of siRNA-DDR2, the mRNA expression of DDR2 (P<0.01) and α-SMA (P<0.01) significantly decreased compared with the normal group, and the protein expression of DDR2 also significantly decreased (P<0.01). In addition, the proliferation of HSC was also markedly suppressed as compared with the normal group (P<0.01). However, compared with the negative control group, none of them was markedly suppressed. Conclusion SiRNA targeting DDR2 significantly suppresses the activation, proliferation of HSC, and thus attenuates hepatic fibrogonesis in vitro.
9.Synthesis and anti-inflammatory activity of α-aryl-3,5-dimethoxyphenylpro-penamides

Aim: To search for new compounds with stronger anti-inflammatory activities and less gastrointestinal side effects. Methods: 2-(2, 4-Dichlorophenyl) -3-(3, 5-dimethoxyphenyl) - propenoic acid(Ⅰ) was converted to amides(Ⅱ). Their anti-inflammatory activities against xylene-induced mice ear swelling were evaluated. Results: Twelve target compounds (Ⅱ_(1-12)) were synthesized, and their structures were confirmed by IR,~1H NMR and HR-MS.Ⅱ_3 exhibited significant anti-inflammatory activity compared with CMC-Na ( P < 0. 01) and aspirin ( P < 0. 05). Conclusion: Compound Ⅱ_3 deserves further investigation.
10.Effects of crocetin on the apoptosis and the changes of its related regulating proteins caspase-3 and Bcl-2 induced by H_2O_2 in myocardial cells

AIM: To observe the effect of crocetin on the apoptosis and the changes of its related regulating proteins caspase-3 and Bcl-2 expression induced by hydrogen peroxide (H_2O_2) in cultured cardiomyocytes. METHODS: Changes of cellular morphology were detected under microscope. Apoptosis rates of the cells were analyzed by PI staining with flow cytometry. Expressions of caspase-3 and Bcl-2 proteins in the cells were determined by immunofluorescence with flow cytometry. RESULTS: In the concentrations used, more severe morphological changes with higher apoptosis rate of the cultured myocardial cells were seen in each H_2O_2 group than that in control group.
