1.Differentiation of CEA and CEA-Related Substances by Western Blot Technique in Biliary and Gastric Diseases

Objective To eliminate the interference of CEA-related substances in CEA measurement and increase the specificity of CEA in the detection of malignant digestive diseases.Methods CEA level of peripheral blood and digestive juice(bile,gastric juice) from patients with benign or malignant digestive diseases was measured by ELISA,and semi-dry electrophoretic transfer method of Western blot technique to distinguish CEA and CEA-related substances.Results In malignant diseases,the CEA level of digestive juice was significantly higher than that in the blood,and there was no difference of CEA level in digestive juice and blood in benign diseases.Meanwhile,the CEA level of digestive juice and blood in malignant diseases were significantly higher than that in benign diseases.A specific band(molecular weight about 210?10~3) was detected in all malignant diseases except four cases whose CEA level was too low(less than 5 ?g/L),whereas no one of benign diseases had this specific band no matter how high or low the CEA level was.Conclusion The specificity of CEA detection in malignant digestive diseases can be improved by using digestive juice as sample and combining with Western blot technique.

3.Laparoscopic cholecystectomy combined with duodenoscopic procedures for selective common bile duct stones

Objective To investigate the value of combined use of laparoscopic cholecystectomy (LC) and duodenoscopic procedures in the treatment of selective common bile duct stones. Methods We retrospectively analyzed clinical data of 32 cases of selective common bile duct diseases treated with laparoscopic cholecystectomy combined with endoscopic retrograde cholangiopancreatography (ERCP) from January 2002 to December 2003. Results Of 24 cases receiving ERCP before LC, 19 cases were found having common bile duct stones, 3 cases having stricture at the lower end of the common bile duct, and 2 cases normal. These cases underwent ERCP and endoscopic sphincterotomy (EST) following by LC. Of 8 cases receiving ERCP after LC, 4 cases were found having common bile duct stones, 3 cases having stricture at the lower end of the common bile duct, and 1 case normal. Except for 1 case of conversion to open surgery because of a failed stone removal, all the cases were cured with EST. Conclusions combined use of LC and ERCP is a highly useful approach in the treatment of selective common bile duct stones, which has broadened the utilization of minimally invasive treatment for bile duct diseases.
4.THE EXPRESSION OF CDC25B IN COLORECTAL CANCER AND ADENOMA AND ITS CLINICAL SIGNIFICANCE

To explore the role of CDC25B in pathogenesis and development of colorectal cancer and to investigate the relation between the expression of CDC25B and the clinical pathological character of colorectal cancer and the prognosis of patients. The expression of CDC25B in 168 patients with colorectal cancer and 25 patients with adenoma, and normal colorectal mucosa from 20 individuals was assayed by immunohistochemistry (S-P method). The expression of CDC25B was not detected in adenoma and normal colorectal mucosa. The expression rate of CDC25B in colorectal cancer was 71.4%(120/168). The expression of CDC25B had a positive correlation with distant metastasis and the increase of CEA,while it had no relationship with the tumor stage, grade, lymph node metastases, gender, age or the size of tumor . The patients in whom expression of CDC25B was detected had a markedly low 5-year survival rate. The results suggested that expression of CDC25B in colorectal cancer might accelerate the transformation of cell cycle,which promoted metastasis to distant organ, indicating that the expression of CDC25B might play a role in the development of colorectal cancer. The expression of CDC25B was a risk factor of poor prognosis.

6.Analysis of the effect of expanded deltopectoral skin flaps in repairing scar contracture deformity of faciocervical for 24 cases

Objective To discuss the effect of expanded deltopectoral skin flaps in repairing scar contracture deformity of facio-cervical .Methods Twenty-four patients with scar contracture deformity of faciocervical were included into the study .We expanded the deltopectoral skin flap with dilator .8-10 weeks later ,the deltopectoral skin flap were transferred after resection of faciocervical scar .3-4 weeks later ,the pedicles were cut off ,and then the remaining wound were repaired .Results They had not exudation .Ex-pander exposure ,local hematoma and local infection respectively were 1 ,3 ,4 cases ,all did not affect operation effect by symptomatic treatment ,and obtain good postoperative effect .Deltopectoral flap was for 27 cases ,blood supply obstruction low temperature and skin flap necrosis respectively were 4 ,2 ,1 cases ,all get good healing after corresponding .Followed up for 0 .5 year ,all flaps were survived well ,no ectropion or mental cervical adhesion ,its color and texture were similar with surrounding normal skin appearance , so they were got high satisfaction .Conclusion Application of expanded deltopectoral skin flaps to repair scar contracture deformity of faciocervical can achieve good effect and may be an ideal method to repair ,which is worthy of clinical application .
7.Effect comparison of minilaparotomy and laparoscopic-assisted right hemicolectomy on immune function of patients with colon cancer

Objective To explore the cell-mediated immune function in patients with colon cancer undergoing minilaparotomy or laparoscopic assisted right hemicolectomy. Methods From January 2009 to August 2014, the colon cancer patients receiving right hemicolectomy were retrospectively analyzed. According to the operation mode, the patients were divided into minilaparotomy group and laparoscopic-assisted group. The clinical and pathological data was analyzed. Cell counts of total CD3, CD4, CD8, CD19 as well as NK cells in venous blood samples were compared between 1 day before surgery and postoperative days (POD) 1 and 5. Measurement data with normal distribution was compared using the t test or Q test. Count data was analyzed usingχ2 test or Fisher exact probability. Results There were 408 patients with colon cancer undergoing right hemicolectomy, 26 patients of whom were excluded. The remaining 382 patients were recruited in the research, which were divided into minilaparotomy group (182 cases) and laparoscopic-assisted group (200 cases). There was no significant difference in the age, gender, body mass index, TNM staging, histological type, blood loss, return of bowel function, tumor location, hospital stay and postoperative complications between the two groups (all P> 0.05). The operating time in minilaparotomy group [(131.53 ± 22.57) min] was shorter than that in laparoscopic-assisted group [(167.53 ± 22.04) min], and there was significant difference (t=15.76, P= 0.00). Compared with prior to surgery, cell numbers of CD3, CD4, CD8, CD19 and NK cells were lower on POD 1 and POD 5 (all P< 0.05), but there was no difference between minilaparotomy group and laparoscopic-assisted group (all P>0.05). Conclusion The minilaparotomy and laparoscopic-assisted right hemicolectomy have same effect on cellular immune function of patients with colon cancer.
8.Diagnosis of Pancreatic Cancer by Detecting K-ras Gene Mutations

Objective To detect plasma K-ras gene mutation by using CED-RFLP/PCR to diagnose pancreatic cancer. Methods CED-RFLP/PCR technique was used to detect K-ras gene mutation in the plasma specimens of pancreatic cancer patients, patients with benign pancreatic diseases and healthy subjects. Results In pancreatic cancer patients the positive rate of plasma K-ras gene mutation was 73%, without false positivity, and higher than that in the pancreatic juice and duodenal juice, but lower than that in the fine-needle aspirates. Plasma K-ras gene mutation was not found in patients with benign pancreatic disease and healthy subjects. Conclusion The detection of plasma K-ras gene mutation by CED-RFLP/PCR is simple and effective, and could avoid the faults of other detection methods. It is helpful for the diagnosis and identification of pancreatic cancer.

10.Design and Implementation of a Programmable Wireless Neural Stimulation System.

The paper proposes and realizes a programmable wireless neural stimulation system which can be used as a solution of functional electrical stimulation to treat neural diseases. The system is composed of two parts: controller and neural stimulator. The controller can transmit pulse parameters to the stimulator wirelessly, and the stimulator can generate bidirectional pulses with charge balance. The simulator takes use of ADCs to sample on the bidirectional pulse output, which compared with preset amplitude to the DAC output voltage to realize the voltage calibration. Through the test, the whole system works stably and the output of the biphasic charge balanced circuit is definite. The stimulator output ranges from 0 to 5 V ajustably, and the frequency ranges from 1 Hz to 200 Hz ajustably, while the pulse width ranges from 500 μs to 1500 μs ajustably. The duration of the stimulation can be set from 10 s to 10 min.
