1.Breast-conserving modified radical mastectomy for breast cancer:a report of 21 cases

Objective To explore the indication,surgical procedure and the postoperative treatment of breast conserving modified radical mastectomy for early stage breast cancer .Methods The clinical data of 21 patients with early stage breast cancer underwent breast conserving modified radical mastectomy were analyzed retrospectively. Results All patient recovered uneventfully. All the external configuration of the breast were fine. There were no recurrence and no complications in this series. Conclusions The breast conserving modified radical mastectomy is recommended for the early stage breast cancer. The external configuration of breast is fine postoperatively, and patients have higher survival quality.But follow up is necessary in order to find and treat the recurrence of breast cancer.
2.Correlation between serum adiponectin concentration and hyperuricemia on Han in Hunan

Objective To explore the relationship between serum adiponectin concentration and hyperuricemia in Han people in Hunan.Methods Cluster random sampling method was used to carry out the survey during October to December 2008 in Changsha Health Checkup Center of the Second Xiangya Hospital.All subjects completed the questionnaires,physical examination,biochemical measurements,and the data were analyzed by t test,Pearson's correlation analysis and multiple linear regression analysis.Results The Age,WC,TG,SBP,DBP,SUA,BMI were significantly higher in hyperuricemia group compared with the normal uric acid group.The concentration of adiponectin in hyperuricemia group was significantly lower than that in the normal uric acid group (5.0±2.7 vs 6.8±4.2 μg/ml,t=3.961,P<0.05).In the normal serum uric acid group,the serum adiponectin concentration in female was significantly higher than that in male (t=4.99,P<0.05).Pearson's correlation analysis showed that serum adiponectin concentration was negatively correlated with the serum uric acid level,but the adiponectin level was not significantly correlated with Age and blood pressure.Stepwise analysis showed that the main factors that could affect the adiponectin level were SUA,gender and BMI (P<0.05).Conclusion The decreasing of serum adiponectin concentration might be one of the mechanisms of hyperuricemia,therefore,detecting serum adiponectin concentration may provide the basis for the prevention and treatment of hyperuricemia.
3.Protective Effects of Ademetionine 1,4-Butanedisulfonate on Radiation Injury

Objective This study aimed to investigate the protective effects of ademetionine 1,4-butanedisulfonate (SMT) against radiation injuries.Methods ICR mice were randomly divided into 7 groups,including normal control,irradiation-only,SMT administration-only,low-,medium-and high-dosages (250,500,1 000 mg·kg-1) of SMT pre-irradiation and high-dose of SMT post-irradiation in experimental groups.Blood and immunological experiments,organs index experiment and 30-day''s survival experiment were carried out to observe the protective effects of SMT on peripheral blood and immune system,organ index and the whole body injuries.Results Compared with irradiation-only group (4.23±1.16) ×109·L-1,the number of nucleated cells in bone marrow was (11.20±4.63) ×109·L-1 in the high dose of SMT pre-irradiation.The difference between two groups was significant.Compared with irradiation-only group (19.25±9.36),the colony forming unit-spleen was (39.00±7.57) in the high-dose SMT pre-irradiation group,there was a significant difference between the two groups.The index of liver,spleen,kidney and pancreas were significantly higher than those of the irradiation-only group in SMT administration groups.The survival rate of mice treated with SMT was increased,especially for the high dose group (46% lifted) when compared with irradiation-only group.Conclusion SMT can protect mice from radiation injuries.
4.Epidemiological study on metabolic syndrome in elderly people in Changsha city

Objective To assess the prevalence of metabolic syndrome (MS) and explore the associated risk factors with MS among 35 ~ 80 years elderly people in Changsha.Methods The survey used by cluster random sampling method (from October to December,2008 ) was carried out by the Physical Examination Center of the Second Xiangya Hospital,Changsha.All subjects aged from 35 to 80 years old had the questionnaires,physical examination,and biochemical measurements.MS was defined by IDF 2005.Results The prevalence of MS was 24.3% among elderly people in Changsha.The prevalence of MS in male and female was 27.9% and 19.3%,respectively; which in male was significantly higher than in females ( P <0.01).According to census of 2000 in Changsha,the age-adjusted rate of MS was 24.4% among elderly people,27.8% in male and 21.9% in female,respectively.The metabolic abnormalities and the prevalence rate of MS were different in gender and age.Except the essential component of the central obesity in the 194 cases of MS patients,there are 80 cases include other two kinds of metabolic abnormalities,accounting for 41.2%,most of them with elevated blood pressure + sugar metabolism disorders; the most common three kinds of metabolic abnormalities of the 78 cases,accounting for 40.2% of the people with MS,most of them with high TG + elevated blood pressure + sugar metabolism disorders; With 4 kinds of metabolic abnormalities of 36 patients,accounting for 18.6% of the MS patients.Increasing age,male,overweight and (or) obesity,high LDL-C hematic disease,high uric acid hematic disease,diabetes were all the risk factors of the elderly people in Changsha city.Conclusions Elderly people in Changsha have a relatively high prevalence of MS,especially for male.The prevalence of MS is considerably high in Changsha,which may continue to rising with the social economic development and population aging.There is an urgent need to take actions to reduce the burden of MS.
5.Expression of acetyl coenzyme A synthetase 2 in colorectal cancer and its biological role.

OBJECTIVETo investigate the expression of acetyl coenzyme A synthetase short-chain family member 2 (ACSS2) in patients with colorectal cancer (CRC) and its biological role.
METHODSA total of 74 CRC tissue samples and 40 normal colorectal tissues were tested by immunohistochemical staining to detect the expression of ACSS2 (cell staining intensity score: 0 points: without staining, 1 points: weak staining, 2 points: intensity staining, 3 point: strong staining; the percentage of positive cells in tumor or negative score: 0 points: negative, 1 point: <25% positive cells, 2 points: 25%-50% positive cells, 3 points: 50%-75% positive cells, 4 points: >75% positive cells. The product of above two scores was the final score.). Association of ACSS2 expression with clinicopathological characteristics was analyzed. Small interfering RNA (siRNA, including A and B group) was used to knock down the expression of ACSS2 in colorectal cell lines (Lovo, HCT116) and their proliferation, migration and epithelial-mesenchymal transition (E-cadherin and Snail as markers) after knocking down were observed.
RESULTSThe expression of ACSS2 was significant higher in CRC tissue than that in normal colorectal tissue (tumor average score 6.284, normal tissue average score 3.625, P<0.01) and the percentage of positive cell was higher than that in normal tissue (tumor 69.9%, normal tissue 45.1%, P=0.000). The use of ACSS2 siRNA successfully knocked down the expression of ACSS2 in Lovo and HCT116 cells. A mild suppression of cell proliferation was observed 5 days after planked (A450 value: Lovo-NC 1.758±0.041, Lovo-ACSS2-siA 1.485±10.026, Lovo-ACSS2-siB 1.371±0.049; HCT116-NC 2.609±0.038, HCT116-ACSS2-siA 2.260±0.042, HCT116-ACSS2-siB 2.295±0.029). While a remarkable ability decline of cell migration was found (Lovo-NC 225±5/field, Lovo-ACSS2-siA 40±5/field, Lovo-ACSS2-siB 79±3/field; HCT116-NC 198±7/field, HCT116-ACSS2-siA 96±7/field, HCT116-ACSS2-siB 77±9/field, P<0.05). Real-time quantitative PCR detection showed that in Lovo cells, expression of E-cadherin up-regulated and expression of Snail down-regulated, while in HCT116 cells, E-cadherin up-regulated slightly [E-cadherin: Lovo NC 1.000±0.211, Lovo-siA 3.403±0.207, Lovo-siB 2.658±0.420 (P<0.05); HCT116 NC 1.000±0.121, HCT116-siA 1.349±0.197, HCT116-siB 1.528±0.147(P>0.05); Snail: Lovo NC 1.000±0.085, Lovo-siA 0.468±0.030, Lovo-siB 0.499±0.088 (P<0.05); HCT116 NC 1.000±0.118, HCT116-siA 0.265±0.020, HCT116-siB 0.194±0.017 (P<0.05)].
CONCLUSIONCRC tissues have high expression of ACSS2, which may be associated with cell migration and epithelial-mesenchymal transition.
6.Antioxidant activity of oil in seabuckthorn seed and optimization of its extraction process

Objective To optimize the process conditions of the oil extraction ofseabuckthorn oil and to evaluate its antioxidant activity by anti-free radical action.Methods The extraction time and particle size of sea-buckseed oil were optimized by using the response surface software design-expert.Its antio xidant activity was studied through its anti-dpph free radical reaction.Results The best process of seabuckthorn seed oil was extracting time 3 h,material liquid than 1:8,extraction temperature 80 C,about 30 mesh size,the yield is highest at 11.13%.The optimum reaction time was 8 min in control,and with the increase of concentration,seabuckthorn oil antioxidant activity increased,when the addition amount of 4.00 ml sample,clearance rate as high as 77.62%.Conclusions This method is simple and reliable,the extraction rate is high,and the test results show that the oil has obvious anti-oxidative effect,which can be used as whitening and wrinkling products to delay the aging of human body.
7.Clinical evaluation of endoscopic variceal ligation

Objective To evaluate the safety and efficacy of endoscopic variceal ligation (EVL) as an alternative to sclerotherapy for the treatment of esophageal variceal bleeding and rebleeding.Methods From January 1991 to January 1995, we treated 250 patients with esophageal varices by EVL. The patients were aged 49 years on the average, and 82% of them were male. Portal hypertension was caused by post-hepatitis cirrhosis in 168 patients. Forty-five patients belonged to Child-Pugh class A, 85 class B, and 120 class C. All but 16 patients had one or more previous episodes of variceal bleeding. At the time of treatment, 78 patients had active bleeding. Seventy-nine patients had undergone devascularization or shunt surgery or had experienced postoperative rebleeding. EVL was repeated at two-week intervals until the varices were eradicated or reduced to grade one.Results The varices were eradicated or reduced to grade one in 81% of the 250 patients. Eradication of varices required a mean of 4 sessions of treatment. The active bleeding was controlled in 73 (93.6%) of the 78 patients. No major EVL-related complication was observed. Follow-ups ranged from 4 to 48 months (average: 25 months). Bleeding recurred in 15.6% of the 250 patients. Thirty-two patients died during the study. Of them, 13 died from hepatic failure, 12 from advanced hepatoma, and 7 from uncontrollable massive bleeding. Conclusions EVL is safe, effective and simple in controlling and preventing esophageal variceal bleeding. It has few complications but broad indications. In particular, it is indicated for patients with poor liver function who cannot tolerate a surgical procedure and/or recurrent bleeding after devascularization or shunt surgery.
8.Coaxial needle technique assisted percutaneous liver biopsy in patients with coagulation function abnormalities: a multicenter study

Objective:To investigate the effectiveness and safety of the coaxial needle technique in percutaneous liver biopsy for patients with coagulation function abnormalities.Methods:Clinical data of 210 patients who underwent percutaneous liver biopsy using the coaxial needle technique under ultrasound guidance from December 2018 to May 2021 in 3 centers were collected. A retrospective analysis was conducted to compare the puncture success rate, number of samples obtained, pathology qualification rate, intraoperative and postoperative bleeding rates between the group with coagulation function abnormalities and the group with normal coagulation function.Results:After propensity score matching, there were 105 patients in each group, with a puncture success rate of 100% in both groups. The pathology qualification rate was 100% for all samples.Intraoperative bleeding occurred in 78 cases (74.3%, 78/105) in the coagulation function abnormalities group and in 64 cases (61.0%, 64/105) in the normal coagulation function group, with a statistically significant difference between the two groups ( P=0.006). Postoperative bleeding occurred in 3 cases (2.9%, 3/105) in the coagulation function abnormalities group and in 0 case in the normal coagulation function group, with no statistically significant difference between the two groups ( P=0.081). Conclusions:The use of the coaxial needle technique for percutaneous liver biopsy in patients with coagulation function abnormalities not only allows for obtaining an adequate tissue sample but also demonstrates good safety.

Medicinal plants and microorganisms show a close relationship,both of which are from the plantmicroorganism symbiont system and interact mutually.Microorganisms are closely related to the growth,reproduction and secondary metabolic activities of medicinal plants.The symbiotic relationship between medicinal plants and microbes is the hotspot of medicinal plants in recent years.This paper summarized the relationship between medicinal plants and microorganisms.It reviewed the effects of medicinal plant species and root exudates on the microbial community composition and diversity,the effects of medicinal plant rhizosphere microbes on plant growth,the specificity of microorganism and the molecular recognition mechanism of medicinal plants and microorganisms.This article also provided the basis for excavation and utilization of the mechanism of interaction between medicinal plants and microorganisms,which were significance for the effective use of microbial growth mechanism to improve the stress resistance of medicinal plants,to increase the yield and to improve the quality of medicinal plants.
10.Efficacy and safety of single high-dose versus multiple low-dose ATG-Fresenius induction in de novo renal transplantation

Objective To evaluate the efficacy and safety of single bolus high dose (SD group) ATG-Fresenius induction therapy in kidney transplantation vs.multiple low dose (MD group) administration.Methods A multiple center,prospective,randomized and controlled clinical study was performed on 280 de novo renal transplant recipients from 19 centers.Patients were randomized into 2 groups as follows:SD group,a single high dose (7-9 mg/kg) of ATG-F infused as an induction agent before the vessel anastomoses;MD group,2 mg/kg of ATG-F daily administrated in postoperative 4 days.All the patients accepted maintenance immunosuppressive protocol including tacrolimus,mycophenolate and prednisone.Patients were assessed and data were collected at regular schedule clinic visits on the day 1,3,7,14,30,90,180,270 and 365.The primary end point of efficacy was therapeutic failure rate [the number of death,grafts loss and acute rejection (AR)].The event first occurred should be used in the classification of patients.The non-inferiority evaluation of the two treatment regimens was done based on treatment failure rate.The secondary end points of efficacy were the incidence of AR,delayed graft function (DGF),1-year survival rate of patients and grafts,and serum creatinine at each visiting point.The indicators for safety evaluation included hemotologic variation and incidence of adverse events.Results The therapeutic failure rate in SD group was non-inferior to the MD group (17.24% vs.23.08%).AR was the major cause of therapeutic failure and there was similar incidence of AR between SD gronp and MD group (12.07% vs.21.37%).There was no significant difference in the incidence of DGF between SD group and MD group (12.07% vs.6.84%,P =0.1721).The 1-year patient's survival rate and 1-year graft survival rate in SD group and MD group showed no significant difference (96.55% vs.98.29%,P =0.6714;94.83% vs 98.29%,P =0.2750).The serum creatinine level showed no significant differences between two groups at each visit point.There was also no significant difference in total incidence of adverse events between the two groups.In addition,there was also no statistically significant difference in the incidence of concerned and drug-related adverse events between the two groups,including infection,hemotologic abnormality,liver or renal dysfunction,gastrointestinal disorder,etc.After ATG--F administration,peripheral blood lymphocytes in the SD and the MD group immediately decreased but nearly restored to the normal level on the postoperative day 30 and 90 respectively.No severe granulocytopenia,erythropenia or thrombocytopenia occurred in both two groups.Conclusion The efficacy and safety of single high dose of ATG-F induction are non-inferior to multiple low dose ATG-F induction,moreover,single high dose of ATG-F induction is administered more conveniently and economically.
