1.MRI Diagnosis and Differential Diagnosis of Hemangioblastomas

Objective To study the MRI manifestations of hemangioblastomas.Methods MRI data of 27 cases of hemangioblastomasproved by surgery and pathology were analyzed retrospectively.Results Hemangioblastomas were often located in hemispherium of cerebellum.All cases could be divided into three types : cyst-nodule type cyst-solid type and purely solid mass type.The large cyst and small nodule sign were characteristic manifestations of the cyst-nodule type,the signals of the fluid of the cyst were higher than that ofcerebrospinal fluid on MRI.The nodules or solid potions of the mass were showed as equal or slight long T_1 and slight long T_2 signalintensity,and were obviously enhanced.The blood vessel flow-void-sign around and inside the solid parts of the masses could be seen,and had a close relation with surrounding meninges.Conclusion MRI is a effective method in diagnosing hemangioblastomas,but the atypical cases should be differentiated from cystic astrocytoma,meningioma and metastesis.
2.Change of prolactin in SD rats with experimental autoimmune thyroiditis

Objective To verify the effect of prolactin (PRL) on the development and progression of experimental autoimmune thyroiditis (EAT) in SD rats and the inhibitive effect of bromocriptine (BRC) on the development of EAT. Methods SD rats were injected with BRC to inhibit the development of EAT and the change of PRL were observed. Results High level of serum PRL was found in the EAT rats [(8.70±0.42)μg/L, P<0.01 vs. control (6.46±0.67) μg/L] , and there was also high levels of thyroid autoantibodies (thyroglobulin antibody, thyroid peroxidase antibody) . However, in rats receiving BRC, the levels of thyroid autoantibodies decreased as the PRL level declined (5.30±0.81)μg/L and the thyroid lesions were mild. Conclusion Hyperprolactinemia plays a role in the development of EAT and control of the hyperprolactinemia with BRC may afford therapeutic benefit.

4.Comparative Study Between Pedicle of Vertebral Arch Tenor System and Harrington Device Fixation in Treatment of Thoracolumbar Fracture

Objective To evaluate the different effects of the treatment of thoracolumbar fractures by pedicle of vertebral arch Tenor system and Harrington device.Methods The clinical data of 35 cases of thoracolumbar fractures were analyzed retrospectively, 23 of them were operated with Harrington device and 12 with Tenor system.Results These cases were followed up for 6 months to 2 years. The fractures treated with Tenor system got satisfied reduction, reached about 96 7% of normal vertebral height. The deformities were completely corrected and complications were significant lower than those derived from Harrington device treatment, and no delayed neurologic damage was found. Most cases got reduction with one to three Frankel grades of neurologic recovery.Conclusion The Tenor pedicle system is more effective in treatment of thoracolumbar fracture than non-pedicle device. It can get satisfied fracture reduction , reliable fixation and less complications.
5.Effect on Pancreatic Carcinoma Tumogenicity and its Metastasis by Tumorself Vaccine Encoded by Adenoviral Mediated Gene Transfer of SLC in Nude Mice.

Objective To evaluate the enhancement effect of SLC(secondary lymphoid tissue chemokine)gene mediated by a repli- cation deficient recombinant adenovirus(Ad)on human pancreatic cancer in vitro and vivo and to investigate the mechanism of action of SLC.Methods Human pancreatic carcinoma cell line ASPC-1 was infected with Ad-SLC and Ad-CFG,and compared with phosphate buffered saline(PBS).MTT(3,-[4,5-dimethyhhiazol-2-yl]-2,5-diphenyhetrazolium bromide)assay was used to estimate the proliferation of ASPC-1 cells;tube formation assay and choriallantoic membrane assay were used to evaluate angiogenesis in vivo and in vitro.Xenografted nude mice with pancreatic cancer were established to observe in vivo tumour growth suppression.Microvessel density re- vealed by CD34 immunohistochemical staining was measured and TIL in tumor region was also evaluated.Results Growth and tube forma- tion of ASPC-1 cells infected with Ad-SLC were suppressed significantly compared with cells infected with Ad-CFG or ceils treated with PBS.Neovascularisation in the Ad-SLC group was less than that in the PBS and Ad-CFG groups,based on chorioallantoic mem- brane results.Volumes of pancreatic tumours in the Ad-SLC group were significantly smaller than those in the PBS and Ad-CFG groups at the end of the treatment period.Microvessel density in the Ad-SLC group was significantly lower than that in the Ad-CFG and PBS groups and TILs in tumor region in the Ad-SLC group was significantly risen much more than that in the Ad-CFG and PBS groups when the mice transferd the same number of TILs from spleen of health mice.Conclusions The SLC gene mediated by adenovirus is efficient for gene therapy for pancreatic carcinoma.Suppression of SLC on proliferation of vascular endothelium cells,attracting TIL to tumor region and angiogenesis may account for its effect.
6.Comparisons of clinical features of female recurrent major depression patients with and without suicidal ideation

Objective To explore the clinical features of female recurrent major depression(MD) patients with and without suicidal ideation.Methods 301 female recurrent major depression patients were interviewed by specially trained interviewers using computer evaluation system and divided into suicidal ideation group and non-suicidal ideation group according to suicidal ideation.The comparisons between the two groups were investigated on items of demographic, clinical data, assessment of psychopathology, Eysenck neuroticism questionnaire, stressful life events.Results 66.4% of MD patients have suicidal ideation.Compared with non-suicidal ideation group, there were more total number of MD symptoms (8.58±0.77, OR=2.476, P=1.154× 10-9) and melancholia symptoms (6.74±1.07, OR=1.546, P=0.001) and it was more common in the factors of less appetite (OR=2.201, P=0.011), lose weight (OR=2.030, P=0.004), early morning awakening (OR=1.774, P=0.045), worthless (OR =2.352, P=0.014), impaired decision-making (OR=2.044, P=0.018), hopelessness (OR=7.130, P=6.651 × 10-9) ,helpless (OR=2.046, P=0.049) in suicidal ideation group.Also in suicidal ideation group the scores of neuroticism dimension (OR=1.631, P=0.024) were higher, and they were more suffered from stressful life events (OR=1.302, P=0.008).Binary logistic regression analysis indicated that total number of MD symptoms (OR=4.840, P=1.154×10-8) ,stressful life events (OR=1.392, P=0.014) and hopelessness (OR=3.528, P=0.008)were significantly associated with suicidal ideation.Conclusion Specific clinical features are associated with suicidal ideation among female patients of recurrent depression disorder.Hopelessness, stressful life events are risk factors of suicide ideation in female recurrent major depression patients.Integrated clinical assessment should be taken for recurrent major depression patients with suicidal ideation.
7.Effect of osteopontin on the expression of hyaluronic acid in human knee osteoarthritic chondrocytes

BACKGROUND:Both osteopontin and hyaluronic acid involve in the pathological process of osteoarthritis, resulting in the abnormal expression levels of various cytokines and enzymes. However, the relationship between the high expression of osteopontin and hyaluronic acid in chondrocytes remains unclear.
OBJECTIVE:To investigate the effect of osteopontin on the expression of hyaluronic acid in human knee osteoarthritic chondrocytes in vitro by modulating the level of osteopontin.
METHODS:Chondrocytes from human knee osteoarthritic cartilage were cultured in vitro, and were then divided into three groups:blank control group without any treatment;osteopontin group and and pontin siRNA group were treated with 1 mg/L recombinant human osteopontin and osteopontin siRNA, respectively. Expression levels of osteopontin, hyaluronic acid synthase 1, 2 and 3 mRNA were detected by real-time PCR, and the levels of hyaluronic acid were measured using ELISA.
RESULTS AND CONCLUSION:Compared with the blank control group, the mRNA expressions of hyaluronic acid synthase 1, 2 and 3 were remarkably increased in the osteopontin group, while siRNA made the significantly inhibitory effects on the hyaluronic acid synthase 1, 2 and 3 mRNA expressions (P<0.05). The level of hyaluronic acid in chondrocytes in the osteopontin group was significantly higher than that in the other two groups (P<0.05). Our results suggest that osteopontin induces the synthesis of hyaluronic acid in osteoarthritic chondrocytes through upregulating the hyaluronic acid synthases expression levels.
8.Diagnosis and treatment of primary gastric malignant lymphoma

Objective To explore the diagnosis,treatment and prognosis of primary gastric malignant (lymphoma)(PGML).Methods The diagnosis and treatment data of 21 cases of PGML admitted in our(hospital) during 8 years was retrospectively analyzed.The diagnostic methods included barium meal(examination),gastroscopy,B type ultrasonography,and CT scan.All patients underwent operative treatment and most of the patients received adjuvant chemotherapy.Results Upper abdominal pain,digestive tract bleeding,emaciation and abdominal mass were common clinical manifestations of PGML.The resectabilty rate of PGML was 90.5% while the 5-year survival rate was 57.1%.Conclusions The key to a satisfactory prognosis was early diagnosis,radical curative operation and combined treatment.
9.Choose of Drainage for Pancreatic Abscess

Objective To sum up the therapeutic experience for pancreatic abscess complicated with severe acute pancretitis (SAP) and to compared the methods of drainage according to its classification , so as to guide the clinical work. Methods Altogether clinical datas of 58 patients with pancreatic abscess were collected in the latest 20 years, pancreatic abscess were divided into 3 groupes according to its size,locationa and figure.Four methods of drainage including open operation drainaging,percutaneous puncture drainaging, small incision drainaging at lower location but not into peritoneal cavity and “F” tube drainaging were adopted.Results 29 cases drainaged by open operation ,among them 10 adopted second look operation and 4 cases third look operation 5 died of the severious complications such as overwelming blooding and intestinal fistula;10 cases through percutaneous puncture drainage ,6 cases cured and the rest converting to open operation and then cured ; 12 cases by small incision through lower location but not into peritoneal cavity ,all cured.Conclusions The results sugggest the effects of drainge related directly to the choose of methods of drainage for pancreatic abscess complicated with SAP,pancreatic classification and to choose an appropriate way according the aforementioned standard will benefit clincial work.
10.Diagnosis and Surgical Treatment of Chronic Lymphocytic Thyroiditis

Objective To discuss the diagnosis and surgical management of Hashimoto's disease.Methods Clinical and pathological datas of 36 cases with Hashimoto's disease from 1995 to 2006 were studied retrospectively.Results 36 cases were all confirmed to be chronic lymphocytic thyroiditis by pathological examination,6 cases,5 cases and 3 cases were apartly accompanied by nodular goiter,thyroid adenoma and thyroid cancer among them;The clinical manifestations before operation included thyroid nodules in 29 cases,thyroid seriously enlarged in 5 cases,thyroid pain in 2 cases;Great partial/subtotal thyroidectomy in one or both part were taken in 23 benign cases,or together with isthmus excision,3 malignant case received total and subtoal thyroidectomy in either part with isthmus and homolateral neck dissection.Conclusions Surgery is actively to be advised when carcinoma under suspicion,thyroid enlarged with pressure symptoms or pain,medication is ineffective.The mode of operation should be selected according to intraoperative frozen section.A long-term substitute medication of thyroxin can help to obtain a good prognosis after operation.
