1.Mean absorbed dose calculation at cellular level for targeted radiotherapy using Auger-electron-emitters

Objective To calculate the mean absorbed doses at celluar and subcellular levels for uniformly and non-uniformly distributed Auger electron emitters. Methods The energy deposited in cell or nucleus by Auger electrons was analytically calculated using the polynomial representation of energy loss.S-values were subsequently computed for several target-source combinations.The absorbed dose and dose distribution were calculated for spherically symmetric radionuclide distributions depending linearly and exponentially on the radial position.The dose profile as a function of the source radial coordinate was also evaluated for typical cell sizes.The contributions of photon radiation to absorbed dose in cells were ignored. Results The mean absorbed dose and dose distribution depend largely on the size of target cells,the radiation spectrum and intracellular localization and distribution of radionuclides.The contribution of intranuclear radionuclides to mean absorbed dose was larger than that of extranuclear ones. Conclusion Auger electrons can produce high local energy deposition in cells because of their very low energies and extremely short ranges.Our dose calculation method is simple.The results are reliable and can be used in many fields.
2.Primary repair of tetralogy of Fallot and double-outlet right ventricle with anomalous coronary arteries

Objective: To review the results with an approach of primary repair for tetralogy of Fallot (TOF) and double-outlet right ventricle (DORV) with anomalous coronary arteries. Methods: From June 1995 to June 2002, 12 patients with TOF and DORV associated with anomalous coronary arteries crossing an obstructed right ventricle outflow tract underwent primary surgical repair. To avoid injury of the anomalous coronary arteries, some modified surgical techniques were used. Main pulmonary artery translocation in 2 patients, repair under a mobilized left anterior descending coronary artery in 3, displaced ventriculotomy with subcoronary suture lines in 5, and the right ventricle outflow tract was repaired via the arteriotomy and/or pulmonary incision in 2. Results: There were no early or late death, and no myocardial infarction. Follow-up ranged from 3 months to 6 years, there were no residual leakage and obstruction of right ventricle outflow tract. Conclusion: Primary repair of TOF with anomalous coronary arteries can be done with excellent results. A proper surgical technique should be used during operation.
3.Research on the Anti-Calcification Feature of Polydiol Post-Treatment of Glutaraldehyde Tanned Bovine Pericardium

Objective The polydiol, such as 1,2-propanediol, 1,3-propanediol,2,3-butanediol, was first used for the post-treatment of G-tBP,in order to find new fixative, for anticalcification, improving durability and clinical use. Method G-tBP with the polydiol post-treated were implanted sabcutaneously in back of weanling SD rats.60 days after the treatment,calcium concentration analysis,histologic examination,electron microscopy,heat shrink temperature and tissue humidity were tested.Results The polydiol-post treated bovine pericardium showed less calcification significantly as compared with the G-tBP after 60 days implantation(mean tissue clacium content(1 817?3 091) versus (176 32?43 27)?g/mg dry weight, P0 05). Conclusions ⑴G-tBP with the polydiol post-treated appears less calcification. ⑵The effect of polydiol post-treatment on anticalcification is correlated to the tanned temperature, at 25℃ is more effective than at 4℃.
4.Immunogenicity of Glutaraldehyde Tanned Bovine Pericardium with Polydiol-post Treatment

Objective In this comparative study, we used a new immunocellular chemistry technique,Biotin- Streptavidin-Peroxidase(S.P) method to test the immunologic response to glutaraldehyde-pretreatment or polydiol-post treatment bovine pericardium in vitro.Method S. P was used to measure the immunologic responses to three group of bovine pericardium.Results The results of S. P in fresch bovine pericardium group showed strongly iummunogenicity in vitro. Glutaraldehyde-pretreated or polydiol-post treated bovine pericardium showed less immunogenicity in vitro.Conclusions ⑴The immunogenicity of G-tBP with polydiol post-treated was reduced significantly.⑵ S.P is a new technique and sensitive method for testing the immunogenicity of bioprosthesis in vitro.
5.Clinical application of an improved uterooperator in the interventional treatment of infertility

Objective To evaluate the effectiveness of an improved uterooperator in the interventional treatment of tube obstructional infertility, and to make a comparison with other motheds. Method One hundred cases of infertile women with tubal obstruction were divided into 3 groups and treated separately under TV fluoroscopy with 3 different methods and follow-up examination was made up to 24 months. Among the 100 cases, 60 cases were treated with improved uterooperator(109 tubes), 20 with Cook cupped coaxial catheter (36 tubes), and 20 with emulsoid double-cavity tube (20 tubes). Result Among the improved uterooperator group, Cook cupped coaxial catheter group, and emulsoid double-cavity tube group, the successful rate of selective catheterization was 92.7%, 80.6%, and 80.0%, respectively. The successful rate of recanalization was 72.3%, 72.4%, and 71.4%, respectively. The pregnancy rate was 36.4%, 35.7%, and 36.4%, respectively. 6.The Protective Effects of Nitric Oxide Inhalation on Rabbit Lung Ischemia/reperfusion Injury

Objective To investigate the protective effects and mechanism of nitric oxide(NO) inhalation on rabbit lung ischemia/reperfusion injury. Methods Total 32 Japanese white rabbits were randomly divided into four groups, each group containing 8 rabbits. In group A, animals received only thoracotomy. In group B, after animals left chest were opened, their left main pulmonary arteries were occluded for 2h, then reperfused for another 2h. In group C, animal inhaled 20 ppm NO for 30min since the eleventh minute after reperfusion. In group D, animal inhaled 60ppm NO for 30min since the eleventh minutes after reperfusion. The levels of blood PaO 2, cGMP, slCAM-1, XOD and MDA were measured before operation and after reperfusion. The pathologic changes of left lung was observed. 7.Analysis of prognostic factors of systemic lupus erythematosus patients admitted to the intensive care unit

Objective To analyze the outcomes and prognostic factors associated with the death of systemic lupus erythematosus (SLE) patients admitted to the intensive care unit (ICU). Methods During June 1996 to June 2007, all SLE patients admitted to the ICU were included. Patients were excluded if the diagnosis of SLE was established at or after ICU admission. A multivariate logistic regression model was applied using variables that were associated with death in the univariate analysis. Results A total of 101 patients meeting the criteria were included. The mortality rate was 48.6%. The most common causes of admission was lung disorder with acute respiratory distress syndrome (ARDS). Multivariate logistic regression analysis suggested that SLICC/ACR DI>7.7 (OR=6.87), APACHE Ⅲ≥21 (OR=29.8), lung disorders with ARDS (OR =55.81 ), septic shock (OR =32.22 ), intracranial haemorrhage (OR =57.35 ), hypocytopenia (OR = 5.89), mean equivalent prednisone dose>25 mg/d (OR=7.65) and prolonged tracheal intubation (OR=5.98) were signi-ficantly associated with death. Whereas sex, age, SLEDAI >27, gastrointestinal bleeding, the cumulative dosage of CTX higher than 1.0 g, pulse intravenous methylprednisolone therapy were not associated with death. Conclusion The mortality rate of critically ill SLE patients in ICU is very high. SLICC/ACR DI> 7.7, APACHE Ⅲ≥21, lung disorders with ARDS, septic shock, intracraniai haemorrhage, average prednisone equivalent dosage higher than 25mg/d and prolonged tracheal intubation (longer than 4 days) are negative prognostic factors in SLE patients admitted to the ICU.
8.Experimental measurements of dosimetric parameters of Ir-192 new source

Objective To perform thermoluminescent measurements of radial dose function and anisotropy function of a newly designed 192Ir microSelectron-HDR source, and establish evaluation methods for HDR dosimetry.Methods A polystyrene phantom with sylindrical holes in different measure positions was designed to accommodate thermo luminescent dosimeter (TLD) rods and radiation source.TLD rods with similar sensitivity were placed on a semicircle around the source center which had been precisely localized by either radiographic means or dose measurement.Dose-rates were measured at θ = 90° and r = 1-14 cm to calculate radial dose function (gL (r)), while measured along polar angles θ = 0° -165°(interval 15°) at radial distances r = 1 cm, 3 cm and 5 cm to calculate anisotropy function (F(r,θ)).All data were compared with those calculated by Monte Carlo (MC) method.The t-test was used for the statistics.Results The values of gL (r) were 1.007 -0.681 by calculation and 1.007 -0.598 by measurement with r = 1 - 14 cm(t = 0.00 - 2.89, all P > 0.05).The values of F (r,θ) were 0.631 - 1,0.660 - 1 and 0.696 - 1 by calculation, while 0.701 - 1, 0.668 - 1 and 0.629 - 1 by measurement at 1 cm, 3 cm and 5 cm with θ=0° -165°(t=0.40 -1.63, 0.02 -2.10, 0.08 -2.03, all P>0.05).Conclusions The results of radial dose function and anisotropy function measured by TLD measurement are in consistent with those calculated by MC calculations, which can be applied in treatment planning system as original input.
9.Effect of leflunomide on the superflcial costimulatory molecules spectrum of peripheral blood T lymphocytes in patients with lupus nephritis

Objective To investigate the effect of leflunomide on the superficial costimulatory molecules expression of T lymphocytes in patients with lupus nephritis ( LN ). Methods The peripheral blood mononuclear cells (PBMCs) of female active LN patients and healthy female were separated by density gradient centrifugation, and cultured by phytohemagglutinin (PHA) or leflunomide active metabolites(A771726).The CD28, CD40L, CTLA-4 and LFA-1a expressions on the peripheral blood T lymphocytes were detected by double-colored flow cytometry. The differences of the means were tested by analysis of variance( ANOVA ) and SNK q test. Results Comparing with healthy controls, there were significantly higher expressions of CD28,CD40L, LFA-1a and CTLA-4 on the peripheral blood T cells in active LN patients (CD28:33.4±6.5 vs14.4±3.2; CD40L: 13.2±3.2 vs 5.4±2.3; LFA-1a: 8.5±2.3 vs2.2±1.1; CTLA-4:4.6±1.5 all P<0.01) as well as CD28 and CD40L expression on the peripheral blood T cells from healthy controls induced by PHA (CD28:26.8±6.7 vs14.4±3.2; CD40L: 13.9±4.9 vs 5.4±2.3 all P<0.01 ), but not CTLA-4 and LFA-1a expression.However, CD28, CD40L, LFA-1a and CTLA-4 expressions on T cells stimulated by PHA increased in active LN patients(all P<0.05 ). A771726 could significantly inhibit over-expression of LFA-1a and CD40L on the T cells from active LN patients (CD40L:8.2±2.0 vs13.3±3.2;LFA-1a:5.1±1.3 vs8.5±2.3 all P<0.01 ), but not CD28 and CTLA-4 expression. A771726 did not inhibit CD28, CD40L, LFA-1a and CTLA-4 expression on the T cells in healthy controls. Furthermore, A771726 could markedly inhibit the over-expression of all of the above molecules induced by PHA on the T cells of active LN patients (all P<0.05). Conclusion One of the major mechanisms for LEF treatment of LN is that LEF can down-regulate CD40L and LFA-1a expression but not CD28 and CTLA-4 expression on the peripheral blood T cells in active LN patients.
10.The anti-fibrosis mechanism of peroxisome proliferator activated receptor γ in connective tissue disease related interstitial lung disease

Objective To study the anti-fibrotic function and mechanism of peroxisome proliferator activated receptorγ(PPARγ) in connective tissue disease-interstitial lung disease (CTD-ILD).Methods The expression of PPARγin lungs was analyzed in 37 cases with CTD-ILD and 20 control cases by immunohistochemistry.Changes in α-SMA levels were analyzed by Western blotting,and acetylation of Smad3 and Smad3 or PPARγ combined with P300 were analyzed by IP-WB.The data was analyzed by one-way ANOVA,t test or Mann-Whitney test.Results PPARγ' expression in the lung of CTD-ILD was lower than the controls [0.92%(1.44%),3.50%(1.94)%,respectively; Z=-8.924,P<0.01].Different concentration of PPARγ (0,1,5,10,20,40 pmol/L) ligandinhibited the marked elevation of the protein α-SMA induced by TGF-β1 in a concentration-dependent manner (0.918 ±0.062,0.852±0.042,0.725 ±0.057,0.678 ±0.042,0.418 ±0.022,0.456±0.029; P<0.05 or P<0.01).However,this response was blocked by a selective antagonist PPARγ signaling GW9662 (0.946±0.087 vs 0.538±0.120,P<0.01).Acetylation of Smad3 expression was increased when TGF-β1 was putted into lung fibroblasts after 60,90 and 180 min (0.565±0.047,1.127±0.101,0.873±0.022,0.614±0.407; all P<0.05).The combination of Smad3 with P300 was also increased (1.46±0.12,0.98±0.09; P<0.05),compared with the controls.But the ligand of PPARγ could block this effect (0.62±0.10,1.46±0.12; P<0.05).Meanwhile,the combination of PPARγ and P300 was increased (0.94±0.05,0.76±0.22; P<0.05).Conclusion PPARγ may play a physiologic role in the regulation of anti-fibrosis response.Its function may be realized by its competition with Smad3 combined with P300.
