1.A contrastive study on chemiluminescence immunoassay and ELISA for detecting HIV antibody

Objective To compare the effects of Chemiluminescence immunoassay ( CLIA) and ELISA for detecting the HIV antibody in order to explore the feasibility of CLIA.Methods 3,542 AIDS high-risk people were randomly selected from February 2013 to March 2015 in our city, the serum was collected and HIV antibodies in the serum were detected by CLIA and ELISA.The detection results were confirmed by the Western blot.Results By ELISA assay, 56 serum samples were positive with a positive rate of 1.58%, while CLIA 61 were positive with a pos-itive rate of 1.72%.The detection results on 11 serum samples were inconsistent among the 3,542 serum samples by ELISA and CLIA.The positive predictive value of CLIA was 96.72%, the detection sensitivity 98.33%and specific-ity 99.94%, while the positive predictive value of ELISA was 96.43%, the detection sensitivity 90.00%and speci-ficity 99.94%.The positive predictive value, sensitivity, specificity and other indicators showed no significant differ-ence between ELISA and CLIA(P>0.05).Conclusion CLIA shows high sensitivity, specificity and accuracy and therefore it can be used for detecting HIV antibody.

Objective To propose an improved C-means segment method based on Gibbs random field accelerated by GPU.Methods The parallel computation of pixel shades was used to take the place of the classical point-by-point method of CPU.By this way,the efficiency was higher than merely using the CPU computation.Results The efficiency of computation was improved over 400%.The load of CPU was reduced and the effect of accelerator was obvious.Conclusion The improved C-means segment method based on Gibbs random field accelerated by GPU enhances the clinical application of image segmentation,the computer rate of which is improved distinctly and closely to real time.[Chinese Medical Equipment Journal,2008,29(2):6-9]
4.Study on formation process of vacuum negative pressure of micro power negative pressure technology

Objective To explore the formation process of vacuum pressure differential of micro power negative pressure technology to facilitate its clinical application.MethodsThe formation of negative pressure differential and changes of negative pressure values in enclosure space were studied which was formed by liquid aspiration after the compressive polyvinyl alcohol foam material was enclosed.Micro power negative pressure technology was applied to the treatment of the right ear necrosis patient after focal cleaning,and the efficacy was observed.Results Medical adhesive film was raised gradually during the expansion of polyvinyl alcohol foam due to liquid aspiration,and then the enclosure space extended increasingly to form local vacuum negative pressure.The range of maximal negative pressure was between-110 and-200 Pa,and the mean maximal negative pressure value was-132.7 Pa.Micro power negative pressure technology behaved well in healing the wound.Conclusion Micro power negative pressure technology gains advantages in reliability and easy operation over other technologies and products,and may be a new choice for healing of refractory wounds.

Objective To assess the diagnostic value of MR imaging of the wrist synovitis in early rheumatoid arthritis.Methods 60 wrists in 30 patients underwent MR imaging examination,including T1WI,T2WI,STIR and contrast-enhanced fat-suppressed 3D fast-gradient-echo sequence.MRI manifestations were analysed in comparison with clinical and laboratory test data.Results All changes including synovial hyperplasia and pannus formation(30 cases),articular cartilage destruction(21 cases),bone erosion(16 cases),joint effusion(25 cases) and tendon lesion of wrists could be showed clearly by MRI.The signal intensity of the pannus before and after administration of contrast agent and the enhanced curve of the pannus could be used to assess the lesions whether or not activation.Conclusion MRI is helpful in diagnosing early rheumatoid arthritis.
7.A study on the implementation of family physicians contracted service

Family physicians contracted service is an important choice to deepen the reform of China′s healthcare reform and achieve the strategic goal of " health for all" . In order to effectively promote the contracted service, the central government has successively issued a number of policy documents, which are echoed by the localities in their proactive explorations, achieving remarkable results. But there are also multiple roadblocks hindering the contracted service to progress. This article referred to the three-circle theory and studied policy implementation from the three dimensions of policy value, policy resources, and policy impetus. A thorough analysis of the problems in the implementation process revealed such main problems as insufficient family physicians, inadequacy of supporting policies and low level of policy acceptance among the young healthy age groups. The follow-up work will promote the long-term implementation of the family physicians contracted service policy by enriching the family physicians team, enriching supporting policies, and focusing on promotions to key age groups.
8.Factors influencing the usage of medical and health care APP among female college students

Objective:
To investigate the usage of medical and health care APP among female college students, and to understand possible influencing factors, so as to provide reference for medical health care behaviors and personal health among college students.
Methods:
A stratified random sampling method was used to select 891 female college students from 3 colleges in Weifang. A questionnaire survey on their concentration on their own health, the knowledge, cognition and behavior of medical and health care APP, and Logistic regression analysis was used to analyze the influencing factors.
Results:
There were 55.4% female college students who had used medical and health care APP, and most of them (290) had used fitness and weight loss APP. There was statistical significance of the different usages of students of different degrees, majors and grades(χ2=26.55, 7.72, 13.96, P<0.05). Logistic regression analysis showed that educational background, annual medical costs, awareness of medical and health care APP and attitude towards APP prospect were the factors influencing the utilization of medical and health care APP (OR=0.44,0.69,1.36,1.23,P<0.05).
Conclusion
The usage rate of medical and health care APP is general, and sustained utilization rate is low, the APP should enrich menstrual period consumption and figure report, add video recommendation, interactive dating, and other functions, improve female students medical and health care APP cognition, female college students to change health care APP attitude.
9.Analysis of Single-operation-hole Thoracoscopic Lobectomy in 113 Clinical Cases

Background and objective Video-assisted thoracoscopic surgery (VATS) lobectomy is generally ac-cepted for patients with lung cancer. hTe aim of this study is to explore the feasibility of the single-operation-hole thoraco-scopic lobectomy in the treatment of non-small cell lung cancer. Methods To review and analyze the single-operation-hole thoracoscopic lobectomy performed in our hospital for 113 non-small cell lung cancer (NSCLC) cases from October 2010 to October 2013. hTe incision for observation was 1.5 cm the eighth intercostal at the rear of the midaxillary line and the incision for operation was 2.0 cm-4.0 cm at the fourth or iftfh intercostal of the anterior axillary line. hTe operations were performed through the single-operation-hole. Result hTe operation processes were smooth for all the patients without any operative mor-tality occurrence. Only in 5 cases was the operation hole expanded because of the occurrence of massive hemorrhage during the operation;3 patients with postoperative complications underwent thoracoscopic lobectomy again, including 2 cases of de-layed hemorrhage and 1 case of chylothorax. hTe average surgical duration was (178.24±31.17) min, the average blood loss was (213.56±62.38) mL, and the number of lymph nodes dissected was from 5-22. All diagnose were conifrmed by pathology atfer operation. hTe average length of stay was (8.17±2.93) d. All cases recovered well during the follow-up of (2-38) months, only 5 cases had recurrence or metastasis. Conclusion hTe single-operation-hole thomcoscopic lobectomy for lung cancer is safe and feasible, further reducing the trauma, and can be used as a conventional treatment for early-or medium-term NSCLC.
10.Long-term efficacy analysis of salvage re-irradiation therapy for patients with locally recurrent esophageal cancer after definitive chemoradiotherapy

Objective:To analyze clinical features, short-term efficacy and side effects of salvage re-irradiation therapy for patients with locally recurrent esophageal cancer after definitive chemoradiotherapy, to investigate the prognostic factors of re-irradiation with precise radiotherapy techniques.Methods:A retrospective analysis was performed on patients with locally recurrent esophageal squamous cell carcinoma after definitive chemoradiotherapy treated in the Fourth Hospital of Hebei Medical University from January 2008 to December 2016. The patients underwent re-irradiation therapy (re-RT) or re-irradiation therapy concurrent chemotherapy (re-CCRT). The main observation index was after-recurrence survival (ARS), which was calculated by Kaplan-Meier method for survival analysis. Univariate analysis was conducted by log-rank test, and multivariate analysis was performed by Cox regression model.Results:A total of 109 patients were included, with a median age of 66 years (43-89 years), and a median follow-up time of 120.8 months (79.0-176.5 months). The objective response rates (ORR) and dysphagia improvement rates (DIR) in all patients were 64.2% and 63.0%, respectively. The median ARS and 1-, 3-, 5-, 8-year survival rates in all patients were 7.8 months and 32.1%, 9.2%, 7.3% and 2.3%, respectively. The median ARS and 1-, 3-, 5-years survival rates were 10.8 months and 45.9%, 13.5%, 10.8% for patients with time to recurrence (TTR) ≥24 months, significantly longer than those of 5.7 months and 25.0%, 6.9%, 5.6% for patients with TTR<24 months ( χ2=7.99, P=0.005). The median ARS in groups with re-irradiation dose of ≤50 Gy,>50-54 Gy, and>54 Gy groups were 5.7, 10.0 and 8.1 months, respectively ( χ2=6.94, P=0.031). The 1-, 3- and 5-year survival rates were 30.4%, 5.1%, and 3.8% for re-RT versus 36.7%, 20.0%, and 16.7% for re-CCRT ( χ2=2.12, P=0.145). Multivariate analysis showed that TTR ( HR=0.607, 95% CI=0.372-0.991, P=0.046) and lesion length ( HR=0.603, 95% CI=0.371-0.982, P=0.042) were the independent factors for ARS. There was no significant difference in ≥2 grade pneumonitis and 2-3 grade radiation esophagitis between the re-RT and re-CCRT groups ( χ2=0.25, P=0.619; χ2=0.51, P=0.808). The morbidity of ≥2 grade myelosuppression in the re-RT group was significantly lower than that in the re-CCRT group (3.7% vs. 36.7%, χ2=18.15, P<0.001). Conclusions:Precise re-irradiation therapy for patients with locally recurrent esophageal cancer after definitive chemoradiotherapy can alleviate dysphagia, but ARS remains poor. Re-irradiation dose range from>50-54 Gy may be suitable for locally relapse patients as salvage treatment. Patients with TTR≥24 months and lesion length ≤5 cm obtain favorable prognosis.
