Traditional Chinese medicine market is a typical market with information asymmetry, which may lead to "lemons" problem. Third-Party Certification is efficient path to solve the problem of information asymmetry. For this reason geo-authentic and commodity classes certification seems to be very important. But as we know, information asymmetry happen in the stages of traditional Chinese patent medicines market. So a signaling transfer mechanism, which can deliver the quality signal, is badly needed in TCM market. This paper analysis and design of signaling transfer mechanism based on Third-Party Certification, which called quality prestige index of TCM enterprises (QPITCM). QPITCM can display quality information as a signal run through the TCM market.

Objective To assess the safety and efficacy of the transohturator tape (TOT) for surgical treatment of female stress urinary incontinence (SUI). Methods Clinical data of 206 TOT procedures for female urinary incontinence from Jan 2007 to Dec 2009 was retrospectively analyzed.Thirty-one women were identified as having mixed stress urinary incontinence. The mean age was 54.29 years(range,32-81 years),themean disease coursewas 10.31 years (range,1-40 years),and the mean follow-up time was 17.3 months (range,3-39 months).Therapeutic effects were assessed by Grouts-Blaivas outcome score. The results of the operation, complications and changes of urodynarnics were observed. Results The TOT operations were conducted under local anesthesia. The mean operative time was 23.9 min (range,16-35 min),Mean blood loss was 17.8 ml(range,15-30 ml).Short-term complications were as follows:thigh pain 14.08%,dysuria 5.83%,urinary posture change 0.97% and urinary retention 0.49%.The long-term complication rate was 0.97%,including vaginal wall defect and tape exposure in one case:the burr of tape exposure and sex life influence in one case.Long-term symptoms disappeared completely in 202 cases. The cure rate was 98.06%,and the rate of satisfaction was 100%. Conclusions Our results demonstrate that TOT procedure is a minimally invasive, safe and effective surgical treatment for stress urinary incontinence.

Chinese Journal of Rheumatology 2010;14(9):620-623
Objective To investigate the role of RANKL/RANK/OPG system in bone metabolism of ankylosing spondylitis (AS) by detecting bone mineral density, bone metabolism factors such as osteoprotegerin (OPG), soluble receptor activator of nuclear factors-κB ligand (sRANKL) and the expression of membrane-bound (mb) RANKL in the peripheral blood T lymphocytes. Methods Bone mineral density of AS patients were measured by dual-energy X-ray absorptiometry (DEXA) and serum levels of OPG, sRANKL,tartrate resistant acid phosphatase 5b (TRACP-5b) and bone alkaline phosphatase (BALP) were determined by enzyme-linked immunosorbent assay (ELISA). The percentages of CD4+/RANKL+ and CD8+/RANKL+ in the peripheral blood were detected with flow cytometry. T-test, x2-test were used for statisical analysis. Results ① The incidence of osteopenia and osteoporosis in AS was 47% and 37% respectively. ② Serum RANKL,TRACP-5b levels and RANKL/OPG ratio were higher in AS patients than those in normal controls (P<0.05).But there was no significant difference in OPG and BALP between AS patients and normal controls. ③There were positive linear correlation between serum levels of RANKL and OPG, sRANKL and TRACP-5b, OPG and TRACP-5b in AS (P<0.01). ④ The prevalence of CD4+/RANKL+ cells in the peripheral blood of AS patients was significantly higher than that in the normal controls (P<0.05). Conclusion There is a high incidence of bone loss in AS patients. Increased bone resorbtion is the feature of bone metabolism in AS.RANKL/RANK/OPG system may play an important role. The imbalance of RANKL/RANK/OPG system may be one of the bone loss mechanisms of AS. CD4 + T lymphocyte may play an important role in osteoclasts differentiation and bone resorption in AS by up-regulating the expression of RANKL.
