Objective The relationship between fever and the recent curative effect of transcatheter arte -rial chemoembolization ( TACE) and the related factors of fever after transcatheter arterial chemoembolization in large hepatocellular carcinoma is discussed in this paper .Methods One hundred and twenty patients with large liver cancer were divided into two groups according to the fever after TACE .The fever in group A ,and there was no fever in group B after TACE .The changes of the lesions in the two groups were compared after the first inter-ventional treatment .The factors affecting the fever after TACE were analyzed .Results The CT was performed at about one and half month after the first intervention .The curative effects of A and B were evaluated by RECIST criteria. Two groups of CR,PR,SD,PD were 0,11.11%,71.11%,17.78%and 0,0,33.33%,66.67%,respec-tively.The difference of the efficiency of OR was statistically significant (P=0.049).The probability of fever of four types of iodine oil deposits are as follows:100%,93.33%,81.93%,0.Blood supply type, middle and severe hepatic arteriovenous fistula, the use of gelatin sponge, tumor necrosis, and the use of iodine oil were more than 25ml may be factors affecting postoperative fever .Conclusion The clinical symptoms of fever after TACE sug-gest that the lesion is well embolization and iodine oil deposit is good ,and the lesion is not easy to progress ,and short-term curative effect is better .The analysis of the factors that affect the postoperative fever may help to eval-uate the curative effect of patients with large hepatocellular carcinoma .

Objective To investigate symptom characteristics and their their prevalence in terminally ill patients with ovarian carcinoma. Methods A retrospective study was carried out based on clinical data of 98 terminally ill patients with ovarian carcinoma who died in our hospital during January 1995 to December 2004.Fifteen most common symptoms were analyzed with a focus on the followings:symptom incidence, survival time after symptom occurrence, regularity of symptom cluster, and common causes of death.Fifteen symptoms were:pain, cachexia, pleural effusion and ascites, dyspnea, fever, intestinal obstruction, renal failure, bone marrow depression, lung infection, hemorrhage, deep venous thrombosis (DVT),intestinal or pancreatic fistula, mycotic infection, jaundice and emergency conditions. Results (1)The most prevalent symptom was pleural effusion and ascites(63%),followed by pain(60%), cachexia(59%),dyspnea(52%)and intestinal obstruction(49 %).(2)The symptom which lasted longest survival time was mycotic infection(77 days),followed by intestinal or pancreatic fistula(75 days), intestinal obstruction(67 days),pain(60 days)and eachexia(60 days).Symptoms such as bone marrow depression, renal failure, dyspnea and emergency conditions were comparatively critical associated with shorter survival times(14,13,12,7 days,respectively).(3)Terminal symptoms occurred typically in clusters, with 4.9?1.5 symptoms per case.Of 98 cases,84 cases(86%)had 4 or more symptoms, with the median survival time of 63 days from the last day of anti-cancer therapy, and a slow death process. The remaining 14 cases(14%)with 3 or fewer symptoms survived only 25 days,of which 10 cases(71%)died of emergency diseases. The survival time for two groups was significantly different(P

Objective To explore the clinical efficacy in bilateral lumbar foraminal stenosis (LFS) after treatment with bilateral facetectomy combined with pedicle screw fixation and interbody fusion. Methods A total of 41 cases of patients with bilateral LFS underwent bilateral facetectomy combined with pedicle screw fixation and interbody fusion from February 2010 to August 2013 in Department of Spine Surgery, the People's Hospital of Tianjin City, were retrospectively analysed. The clinical efficacy was assessed by Oswestry disability index (ODD questionnaire and visual analogue scale (VAS) before and after operation, anterior and posterior disc height and L1-S1 angle were measured as well. Then the ODI and VAS scores, and changes in anterior and posterior disc height and L1-S1 angle were calculated at the time of the last patient follow-up visit. Results All 41 patients were followed up for 12 to 36 months, with an average of (26.2±2.4) months.Compared with preoperation,at the time of the last follow-up visit the back pain VAS score, leg pain VAS score and ODI were decreased, while the anterior and posterior disc height were increased, there were statistically significant differences (P<0.05).No statistically significant difference was found in L1-S1 angle between preoperation and postoperation (P>0.05).Conclusion The short-term clinical curative effect of posterior bilateral facetectomy combined with pedicle screw fixation and interbody fusion in the treatment of bilateral LFS is satisfactory.

Objective To study the pathological characteristics of skeletal muscle in patients with polymyalgia rheumatica(PMR).Methods Thirteen patients were diagnosed PMR according to the diagnostic criteria described by Bird in 1979.The onset of disease was between 49-78 years of age (median 60.3 years). All of them showed continuous symmetric myalgia with markedly elevated erythrocyte sedimentation rate(ESR).Five patients had mild fever and 8 patients had elevated C-reactive protein(CRP).Electromyogram(EMG)showed myogenic pattern in 2 patients.Only 1 patient showed anemia and 2 patients had mild elevated creatinine phosphokinase.None of them showed evidence of temporal arteritis.Biceps brachii muscle biopsies were performed in all patients before corticosteroid therapy. Results The typeⅡmuscle fiber atrophy was observed in all patients.Moth-eaten changes appeared in 8 cases, increased intramuscular lipid drops in 8 cases and ragged red fibers(RRF)in 3 cases of them. After steroid therapy, all patients showed improvement of myalgia and normal ESR as well as CRP.Conclusions The main symptoms of the patients are myalgia and abnormal ESR.Some patients have high level of CRP and abnormal EMG.Anemia and temporal arteritis are less frequent. The main myopathological changes were typeⅡmuscle fiber atrophy. Some patients have moth-eaten changes.More lipid drops and RRF indicate abnormal metabolism of skeletal muscle.

Objective To evaluate the relationship between interleukin-18(IL-18) and insulin resistance, measured by euglycemic hyperinsulinemic clamp, in women with polycystic ovary syndrome (PCOS). Methods Forty-two young women with PCOS and 38 age-and body mass index (BMI)- matched control women were recruited in this study. A complete hormonal assay was performed and serum IL-18 level was determined in each subject. And euglycemic hyperinsulinemic clamp test was completed in 41 of the above subjects. Results Serum IL-18 levels were increased in the PCOS women, as compared with the control (P=0.033). When all subjects were divided into lean and obese groups, the IL-18 levels were slightly increased in the obese subjects (P=0.902). IL-18 levels were negatively correlated with all the clamp parameters [mean glucose infusion rate (M), M-to-insulin ratio (M/I) and glucose metabolic clearance rate (MCRg), r =-0.419,-0.396,and-0.405,P=0.006,0.010 and 0.009 respectively], but were positively associated with HOMA-β index(r=0.334, P=O.035). Conclusion Serum IL-18 level was significantly increased in PCOS women and was strongly associated with the parameters obtained from the euglycemic hyperinsulinemic clamp, indicating that IL-18 may be an important mediator between inflammation and insulin resistance.
