OBJECTIVESTo provide a cumulative data about the complications of second or third generation ankle prostheses in the literature, and to provide a summary high-grade complications associated with implant failure.
METHODSA comprehensive search for all relevant articles published in English from January 1995 to December 2010 was conducted. Two reviewers evaluated each study to determine whether it was eligible for inclusion and collected the data of interest. Meta-analytic pooling of results across studies was performed for the complications and failure rate.
RESULTSThirty-five primary studies with 4395 implants were identified. The three highest complications of total ankle arthroplasty were aseptic loosening (12.51%), intra-operative bone fracture (11.97%) and bony impingement (11.27%). The three high-grade complications associated with implant failure were aseptic loosening (45.00%), infection (33.00%) and malalignment (29.00%). The pooled mean failure rate was 10.98% (95%CI: 8.80% - 13.16%), and the pooled mean failure rate of STAR implant was 14.20% (95%CI: 10.64% - 17.76%).
CONCLUSIONSIt is found that aseptic loosening, infection and malalignment are high-grade complications associated with implant failure in total ankle arthroplasty. The orthopaedic surgeons should be more careful in the operation, and the patients should coordinate with the post-operative rehabilitation plan.

7.Epidemiological analysis for Quzhou pesticide poisoning of year 2008 to 2010.
Chinese Journal of Industrial Hygiene and Occupational Diseases 2012;30(9):676-678
OBJECTIVETo provide scientific evidence of making measures for prevention of pesticide poisoning, the investigation on the condition of pesticide poisoning was carried out in Quzhou.
METHODSRegistration data of pesticide poisoning from 2008 to 2010 in Quzhou were collected and statistically analyzed by SPSS 12.0.
RESULTSDuring the three years, there were 1222 cases reported for pesticide poisoning. Among them, the number of occupational poisoning was 225 (1 case died), with fatality rate of 0.44%. The number of non-occupational poisoning was 997 (77 cases died), and its fatality rate was 7.72% . The incidence of occupational poisoning and non-occupational poisoning accounted for 18.4% and 81.6% respectively. Male patients were in the majority in occupational pesticides poisoning (accounting for 76.4%), female patients in non-occupational poisoning (accounting for 52.1%). The pesticide poisoning mainly occurred from July to September. Occupational poisoning and non-occupational poisoning cases mainly concentrated in over 65 age group, accounting for 36.0% (81 cases) and 26.3% (262 cases) respectively. Insecticide ranks the first in the terms of total poisoning cases caused by pesticide, and organophosphate poisoning ranks the first in all insecticides.
CONCLUSIONPesticides poisoning has badly threatened the public health in Quzhou, attention should be paid to non-occupational poisoning. The pesticides poisoning was mainly caused by organophosphate insecticides.

Objective To investigate the correlation between cytotoxic T lymphocyte associated antigen 4 (CTLA - 4 ) and primary nephrotic sysdrome(PNS) with the pathologic type of mesangial proliferative glomerulonephritis(MsPGN) of glucocorticoid(GC) resistance. Methods The polymerase chain reaction - restriction fragment length polymorphism( PCR - RFLP) analysis was used to investigate the genotype of position - 318 promoter of CTLA- 4 gene of 36 patient children and 30 healthy controls. Results The frequences of genotypes at position - 318 promoter of CTLA- 4 gene in patients were 38.9% for CC,61.1 % for TC and 0% for TT. The frequences of alleles at this position were 69.4% for C allele and 30.6% for T allele. The frequences of genotypes and alleles in children were not significantly different from those in controls. Conclusion CTLA-4 promoter ( -318)C/T dimorphism was not associated with PNS- MsPGN of GC resistance, which hinted that the polymorphism may be not inlolved in pathogenensis of PNS-MsPGN and the mechanism of GC resistance.

Background Researches determined that the alteration of A69S locus of age-related maculopathy susceptibility 2 ( ARMS2 ) gene is closely associated with the pathogenesis and progression of age-related maculopathy ( AM D ).However, the location of ARMS2 protein in normal eye tissue is still in controversy, therefore, its function is below understanding up to now. Objective The goal of this laboratory work was to investigate the distribution, expression and location of ARMS2 protein in normal adult retina and choroid as well as in retinal pigment epithelial (RPE) cells and lay a basis for exploring further its function in the protein level. Methods Ten donor eyeballs of normal adult male with the age from 28-42 years were collected in eye bank of Qingdao Eye Hospital. The frozen sections of the retina and choroid were prepared for the detection and location of ARMS2 in 3 eyes by immunofluorescence under the confocal laser microscope. The retina was isolated for the primary culture of RPE cells using explant culture method. The cells were then identified by CK32 antibody by immunofluorescence. The distribution and expression of the ARMS2 protein in retina, ehoroid and RPE cells were determined by immunofluorescence technique. Results ARMS2 protein was strongly expressed in retinal vessel,RPE cell layer,Bruch membrane and choroidal vessel, but weak expression was in retinal ganglion cell layer, inner nuclear layer, outer plexiform layer, outer nuclear layer and inner plexiform layer in the normal eyes. The primarily cultured cells appeared the polygon shape with the abundant pigment in cytoplasm. The immunofluorescence of the cells showed the positive response for CK32,exhibiting the green fluorescence granules in the cytoplasm. The positive expression of ARMS2 protein also was seen in the cytoplasm of RPE cells, appearing the red fluorescence. Conclusions ARMS2 protein mainly distribute and locate retinal and choroidal vessels,RPE cells and Bruch membrane in normal eye.

Background The tissue-engineered cornea is becoming the hot spot in the ophthalmologic field, while the research of corneal substitute is in the ascendant, because it is more similar to the corneal morpha and easy to survive in vivo. Objective This study was to investigate the biocompatibility of recombinant human type-Ⅲ collagen/poly9 ( 3-( methacryloylamino ) propyl dimethyl ( 3-sulfopropyl ) ammonium hydroxide ) ( PMPDSAH ) interpenetrating polymer network (IPN) (RHC-Ⅲ/PMPDSAH IPN) hydrogel as a tissue-engineered cornea in rabbit eye and its feasibility as the corneal substitute. Methods One hundred and eight rabbits were randomly divided into experimental group( 90 rabbits) and normal control group ( 3 rabbits),and 15 rabbits ( 30 eyes ) used as the donor corneas.RHC-Ⅲ/PMPDSAH IPN,NGF PMPDSAH IPN and corneal grafts were lamellarly transplanted into the right eyes in RHC-Ⅲ/PMPDSAH IPN group,NGF PMPDSAH IPN group and allograft group respectively. The corneal transparency and neovascularization were examined and scored under the slim lamp and compared among three groups using Kraskal-Wallis H test. The corneal epithelization time was observed and compared among these three groups using one way analysis of variance and LSD-t test. The histological examination of corneas was performed at the 3rd day,1st and 2nd week,1 st,3rd and 6th month after the surgery. The immunohistochemistry was used to detect the expression of K3 in cornea at the 6th month. Results The grafts were well attached in RHC-Ⅲ/PMPDSAH IPN group,NGF PMPDSAH IPN group and allograft group, and no rejection reaction was found throughout 6-month following up.Compared with normal control group,no significant differences were found in the scores of corneal opacification and neovescularization in these three groups (x2 =4.34,P =0.23 ;x2 =2.60,P =0.46 ) at the 6th month.NGF PMPDSAH IPN group achieved reepithelialization in (4.97±0.63) days and was obviously shorted than that in RHC-Ⅲ/PMPDSAH IPN group and allograft group ( t =11.97,P =0.00; t =5.80,P =0.00).The re-epithelialization time in RHC-Ⅲ/PMPDSAH IPN was (6.86±0.71) days, and that of allograft group was (5.87±0.43 ) days, showing a significant difference ( t =6.32,P =0.00).Hematoxylin-eosin staining results demonstrated that implanted materials integrated into the host corneal tissue well and support corneal epithelialization.Part of the material degraded at the 2nd week and degraded completely 1 month later.Regular alignment and distribution of collagen fibers were seen in the regenerated cornea and were similar to those of the normal stroma in 6 months.Immunohistochemistry showed the positive expression of keratin-3 in corneal epithelial cells. Conclusions RHC-Ⅲ/PMPDSAH IPN has a good biocompatibility without toxicity to corneal tissue.Furthermore,NGF can promote the corneal wound-healing and re-epithelialization.The material can be used as safe and reliable corneal substitute after improving the mechanical strength.
