1.Genome-wide association studies of type 2 diabetes mellitus

Recently, genome-wide association study (GWAS)has achieved great success in illuminating genetic basis of complex diseases. GWAS of type 2 diabetes is regarded as a paradigm of genetic dissection of complex human diseases, which are caused by both genetic and environmental factors. Some basic facts of GWAS,as well as implementation of GWAS in type 2 diabetes, are reviewed in this article.
2.Hypoxia increases IRAK-4 expression in cultured murine N9 microglia cells

Objective To explore the mechanism of the microglia inflammatory response through observing the changes of interleukin-1 receptor associated kinase-4(IRAK-4) expression in murine N9 microglia cells under hypoxia.Methods N9 cells were exposed to 3%O2,5%CO2,92%N2 for 1,3,6,12 and 24 h respectively.The IRAK-4 protein level was detected by Western blotting and the celluar change was observed by laser scanning confocal microscope(LSCM).The TNF-? level was measured by ELISA.Results After hypoxia,the expression of IRAK-4 protein was increased in a time-dependent manner.The elevation began at 1 h after hypoxia,increased significantly at 3 h(P0.05).The fluorescence intensity of IRAK-4 also increased with the extension of anoxic time.The TNF-? level change were similar.There were significant positive correlations between the expression of IRAK-4 and TNF-?(P

4.Functional hemodynamic monitoring should be emphasized in intensive care for burn and trauma patients.

Hemodynamic monitoring is a very important measure for critically ill patients with burn and trauma, and it should be carried out throughout the course of treatment. Functional hemodynamic monitoring consists of the assessment of the dynamic interactions of hemodynamic variables in response to a defined volume change. Accordingly, response of fluid volume can be assessed in a quantitative fashion by measuring variation of both arterial pulse pressure and left ventricular stroke volunime during positive pressure breathing, or the change in cardiac output response to passive leg raising maneuver. Functional hemodynamic monitoring, contrary to that of static condition in order to realize dynamic and individual monitoring, is related to response to treatment, and it is a useful complement to static (routine) hemodynamic monitoring. At present, in the care of sepsis, shock, and mechanical ventilation, etc. related to burn injury and trauma, functional hemodynamic monitoring is more and more accepted and applied by medical personnel of ICU in burn and trauma departments. Therefore, further study on functional hemodynamic monitoring should be emphasized and practised.

7.Gastrointestinal stromal tumors:CT diagnosis

Objective To investigate the specific CT findings of gastrointestinal stromal tumors (GIST).Methods The CT imaging of 12 cases of GISTs confirmed operatively and pathologically were reviewed and analyzed.The gross pathologic findings of the tumors were compared with the manifestations of CT study.Results Two of the GIST were from the stomach,2 from the duodenum,4 from jejunum,and 1 from the junction of jejunum and ileum,ileum,rectum and mesenterium respectively.5 of 8 cases with GIST from intestine had history of hemafecia or positive of fecal occult blood test.9 of the 12 GISTs were malignant,1 of them was potentially malignant,1 was not sure to be malignant or benign.CT studies of 11 of the 12 tumors showed as exogenous mass of 3-20 cm,only 1 with less than 5 cm and 8 of them with over 5 cm.in the max diameter the tumors were cystic-solid with inhomogenous attenuation.The solid potion of tumors enhanced moderately to markedly after the contrast media administered.Ulcer was seen in the tumors of 2 cases,and the diameter of both of them were over 12 cm.Punctate calcification was seen in 1 GIST.Conclusion CT findings of GIST were specific to a certainty.CT study is helpful for locating the tumor and observation of the relationship of the tumor and surrounding structures.
9.Understanding of International Sleeve Gastroectomy Expert Panel Consensus Statement and our experience.

An international symposium on the standard surgical treatment of morbid obesity and type 2 diabetes mellitus was held in Florida, USA in March 2011. An expert panel from all over the world attended the meeting. The expert panelists discussed and established International Sleeve Gastroectomy Expert Panel Consensus Statement : best practice guidelines based on experience of more than 12,000 cases, which made a very commendable attempt in standardized surgical pathway of sleeve gastroectomy. Based on clinical experience over 10 years and the understanding of the consensus , this paper discusses the learning curve, preoperative preparation, indications/contraindications, surgical technique, management and prevention of complications, and post operative management.
