4.Subjective perception and analysis of physical assessment technique in clinical nurses

Objective To understand the subjective perception and influential factors of physical assessment technique in clinical nurses. Methods Using self-designed physical assessment skills questionnaire to investigate nurses in the six provinces of China, 1115 questionnaires were valid. Results There were 11 items in this questionnaire. The first four were: physical assessment is an essential skill of nurses (88.1%),the importance of continuous learning (84.8%), lack of maturity assessment tools (79.8%), heavy workload,lack of time (60.8%). The result of multiple regression analysis (stepwise) showed that hospital-level and regional were the main factors affecting the subjective perception. Conclusions The development of physical assessment reies mainly on rational allocation of human resources structure, establishing the physical assessment framework, and make sure different departments have their own characteristics and the importance of continuing education.
5.Physical assessment skills of clinical nurses: application status and analysis

Objective To understand the application status of physical assessment in clinical nurses. Methods Using self-designed physical assessment skills questionnaire to investigate nurses in six provinces of China, 1115 questionnaires were valid. Results There were 171 items in this questionnaire,4.0% of them were used regularly, 19.3% used frequently, 76.7% used occasionally. The result of Spearman correlation showed that there was positive correlation between the frequency of physical assessment skills and the subjective needs of nurses, regional, educational background, hospital level. There was also negative correlation with title, age and seniority. Conclusions The main factors lead to imbalance development of physical assessment skills are regional, hospital-level and education background; the assessment skills of specialist hospitals and wards need to be standardized immediately.
6.Clinical value of serum CA19-9 levels in preoperative evaluation of resectability of pancreatic carcinoma

Objective To evaluate the value of serum CA 19-9 levels in predicting resectability of pancreatic cancer. Methods Serum CA 19-9 levels of 52 patients with pancreatic cancer who were possible to be curatively resected according to the imaging were determined; the gold standard was respectability. An receiver operating characteristic (ROC) curve was constructed for the CA 19-9 levels; the point that lies closest to the upper left-hand comer of the graph is chosen as the cut-off point, the sensitivity, specificity,positive and negative predictive values were calculated. Results There were 29 (55.8%) resectable and 23 (44.2%) inoperable cases of pancreatic cancers. The CA19-9 level in resectable patients was (159.6±170.9)U/ml, and it was (944.4 ±773.4) U/ml in inoperable cases. The area under the ROC curve was 0. 918 ( > 0.9, P < 0.01 ), 95% confidence interval were 0.843 ~ 0.992. The CA19-9 level was (353.2) U/ml according to the cut-off point, the sensitivity and specificity was 93.1% and 78.3%, respectively; positive and negative predictive values was 84. 4% and 90. 0% , respectively. Conclusions Preoperative serum CA19-9 level may be a useful marker for further evaluating the respectability of pancreatic cancer who are possible to be resected according to the imaging studies.
7.Effect of low molecular weight heparin combined with psychological intervention on prevention of deep venous thrombosis of lower extremity after operation for varicose great saphenous vein

Objective To investigate the clinical efficacy of low molecular weight heparin combined with psychological intervention in the prevention of deep venous thrombosis (DVT) in patients with varicose great saphenous vein. Methods Two patients underwent surgical varicosis of great saphenous vein saphenous vein ligation and stripping surgery group, with low molecular weight heparin to prevent deep venous thrombosis, the study group with low molecular weight heparin combined with psychological intervention, prevention and control of postoperative deep venous thrombosis, the incidence of records of two groups of patients with postoperative lower extremity deep vein thrombosis. Results The incidence of deep vein thrombosis (2.04%) in the study group was significantly lower than that in the control group (14.29%) (P<0.05), suggesting that the study group had better effect on prevention of deep vein thrombosis after operation. Conclusion The combination of psychological intervention and low molecular weight heparin can achieve more ideal prevention effect of deep vein thrombosis after operation of great saphenous varicose vein, which is beneficial to protect the quality of life and life safety of patients.
8.The expression of p53, p16 and Ki-67 in penile cancer and their significances

Objective To evaluate the expression of immunohistochemistry markers p53, p16 and Ki-67 in penile cancer and penile intraepithelial neoplasia (PIN). Methods 33 patients who suffered penile cancer and received organ-sparing surgery from Jan 2004 to Dec 2013 were retrospectively analysed. The expression of p53, p16 and Ki-67 were detected by EnVision/HRP two steps immunohistochemistry. Results The positive expression rates of p53 in normal penile epithelium, low-grate penile intraepithelial neoplasia, high-grate penile intraepithelial neoplasia, well-differentiated squamous cell carcinoma and poor-differentiated squamous cell carcinoma were 0, 16.7 %, 38.5 %, 30.0 % and 66.7 % respectively. The expression rates of p16 were 20.0 %, 50.0 %, 53.8 %, 70.0 % and 33.3 % respectively. The expression rates of Ki-67 were 0, 16.7 %, 61.5 %, 65.0 % and 75.0 % respectively. Conclusions The combined detection of p16 and Ki-67 can contribute to discover PIN and penile carcinoma much more earlier. p53 may be related to the occurrence and development of low differentiated squamous cell carcinoma, and p16 may be related to the occurrence and development of well-differentiated squamous cell carcinoma.
10. Effect of estrogen and its receptors on glutamate-induced neural cell injury

It has been proven that glutamate-induced excitatory toxicity can bring damage to nerve cells. Previous studies indicate that glutamate excitotoxicity-associated neuron damage play an important role in the pathogenesis of a number of nervous system diseases, and estrogen can protect neurons from glutamate excitotoxicity-associated neuron damage, which may be mediated by estrogen receptor. This paper reviews the latest progress regarding the protective effects of estrogen and its receptors against glutamate-induced neuron damage.
