1.The study of autologous periosteum wrapping tendon graft improves tendon healing inside a bone tunnel

Objective The experiment was designed to evaluate whether the tendon healing inside the bone tunnel improved when the periosteal graft was wrapped around the tendon. Methods Sixty normal and healthy New Zealand white rabbits were used in the experiment. These rabbits were randomly divided into three groups. Each group included 20 rabbits. Group Ⅰ, periosteal graft was wrapped around the tendon with the cambium layer facing toward the bone; Group Ⅱ, periosteal graft was wrapped around the tendon with the fibrous layer facing toward the bone; Group Ⅲ, as control group, no periosteal graft was used with the tendon. In control group, the rabbit flexor hallucis longous tendon with calcaneus was designed as tendon-bone model. At 1, 2, 3, 4, 6 weeks after the operation, the specimens in three groups were got respectively and the structural integrity of the tendon bone interface were observed using optical microscope. The histological changes and the biomechanical parameters at varies interval were recorded. Results Three weeks after operation, there was significant osteochondral ossification of the periosteal cambium layer in the group of the cambium layer facing toward the bone, but no new bone appeared in the other two groups. Four weeks after operation, the newborn trabecular in the group of the cambium layer facing toward the bone was more than the other two groups. Six weeks after operation, the amount of the newborn trabecular and the tight interdigitation between the tendon graft and the newly formed bone in the group of the cambium layer facing toward the bone were superior to the group of the fibrous layer toward the bone and the control group. Biomechanical testing, according to a one-factor ANOVA and Student-Newman-Keuls multiple range test, there was significant difference in statistics between the group of the cambium layer facing toward the bone and the other two groups (P0.05). Conclusion Periosteal graft was wrapped around the tendon with the cambium layer facing toward the bone, may shorten the time of osteochondral ossification of the periostum and improve the tendon healing in the bone tunnel.

This is valuable medical book in recent time and clinically, it has profound inductive significance for later generations.

3.Low-Dose Erythromycin Treatment on Intractable Allergic Rhinitis Associated with Bronchus Asthma

Objective To explore the therapeutic effect and medication safety of low-dose erythromycin treatment on intractable allergic rhinitis(AR) associated with bronchus asthma.Methods Totally 173 cases of children received outpatient treatment because of AR associated with asthma,their ages ranging from 3 to 14 years.Among them,78 cases developed intractable AR with symptoms of asthma having been controlled or satisfactorily controlled after 2 courses of conventional treatment.Seventy-six children with intractable AR received full follow-up and were randomly divided into observation group and control group.The control group was given different second-generation antihistamines,when necessary,supplemented by nasal glucocorticoids.In the observation group,the same treatment as it was done in control group was continued,plus oral treatment with erythromycin enteric-coated capsules(10 mg?kg-1?d-1,which were taken 3 times a day for 1 month) to the observation group.Both observation group and control group were in accordance with the norms of the treatment of asthma.Results The improvement rate,inefficiency and the total efficiency were different between observation group and control group,and the diffe-rence was statistically significant(?2=12.629,8.412,8.412,Pa0.05).Their liver function was also monitored and was found normal before treatment and after the replacement of drugs for 1 month,including alanine ami-notransferase,aspartate aminotransferase,albumin,globulin,and were found normal.Conclusions On the basis of conventional treatment,low-dose erythromycin treatment of intractable AR is effective and safe.However,the treatment must be limited to the refractory cases,and the appropriate indications must be strictly observed.
4.Reconstruction of oropharyngeal defects after resection of tonsillar cancer

OBJECTIVE To study the methods and curative effects of reconstruction of oropharyngeal defects after resection of tonsillar cancer. METHODS Nineteen patients with tonsillar cancer underwent operation. The choice of surgical procedure was decided on the size of the lesions. The tumor was resected through the hyoid approach and mandibular swing approach. Surgical defects were repaired immediately with tongue flap, temporalis myofascial flap, pectoralis major myocutaneous flap. RESULTS The functions of respiration, chewing, deglutition, and speech were restored satisfied in all patients. Partial necrosis of the pectoralis major myocutaneous flap occurred in one patient, and mild difficulty of open mouth in 3 patients after repaired with temporalis myofascial flap. CONCLUSION Choosing what is optimum from multiple feasible surgical methods is prerequisite for better oropharyngeal function and quality of life in patients with tonsillar cancer.
5.Changes of Blood Flow Velocities in Middle Cerebral Artery with Rasmussen Encephalitis: 8 Cases Report

ObjectiveTo discuss clinical significance of changes in cerebral blood flow velocities with Rasmussen encephalitis. Methods8 cases diagnosed as Rasmussen encephalitis were measured the blood flow velocities in middle cerebral artery (MCA) with transcranial Doppler in ictal and interictal seizures. ResultsIctal mean flow velocity in the ipsilateral MCA to the epileptogenic foci increased 14.02%~48.14% to interictal one, while it was -0.74%~22.63% in the contralateral MCA. ConclusionAn increased flow velocity has been found in the ipsilateral MCA to the epileptogenic foci during seizure in Rasmussen encephalitis patients.
6.Findings of Transcranial Doppler during Simple Partial Seizures with Rasmussen Encephalitis: 8 Cases Report

Objective To analyze the blood flow velocities in middle cerebral artery with Rasmussen encephalitis. Methods8 patients with Rasmussen encephalitis were detected bilateral middle cerebral artery in both period of onset and intermission during simple partial seizures with Transcranial Doppler. ResultsThe mean velocity variance between focus side and non-focus side was minimums among the 3 indexes observed. The mean velocity of MCA displayed uneven increase during seizures. ConclusionThe mean velocity was the most stable index for judging.
7.Comparison of genetic damage in mice exposed to black carbon and ozone-oxidized black carbon

Objective:To make an assessment on the genotoxicity caused by black carbon ( BC ) and ozonized black carbon (O3-BC).Methods: In this study, 74 healthy male ICR mice [weighed (28 ± 1.5) g] were randomly divided into 7 groups, including one phosphate buffer solution ( PBS) control group and six particles exposed groups by intratracheal instillation with either BC or O 3-BC at the doses of 50, 100, 200 μg/mouse, respectively.There were 12 mice in the groups of 200μg/mouse and 10 mice in others.The mice were sacrificed 24 h after four intratrachealinstillations .The activities of catalase ( CAT) in serum and the levels of malondialdehyde ( MDA) in lung tissue homogenate were measured . As the DNA damage mark , 8-hydroxyguanosine ( 8-OHdG ) in urine and serum were quantified with ELISA method.Micronucleus test was used for potential genotoxicity of BC and O 3-BC.Hematoxylin and eosin staining was used to stain lung paraffin section .Results:The mice were in good condition during instillation , and the liver coefficient of the test groups was significantly lower than that of the control group (P<0.05).The activities of CAT in serum significantly increased in the 100 μg/mouse and 200μg/mouse groups after being exposed to these two kinds of particles .The micronucleus rate in allthe BC and O3-BC exposed groups increased ( P <0.05), but there was no statistically significant difference among the groups in the levels of 8-OHdG in serum and urine and MDA in lung tissue homogenate .In-flammatory response was found in the lung tissue under the microscope after exposure to BC and O 3-BC. Conclusion:Intratracheal instillation of BC and O 3-BC induced increasing of oxidative stress and genetic damage in mice .But there was no significant difference between these two particles in toxicity .Whether the genotoxicity of O 3-BC is higher than that of BC or not is uncertain .Further research is needed .
8.The tangential excision of razor graft for the giant congenital naevi in children.

OBJECTIVETo study the therapeutical effect of the tangential excision of razor graft for the spacious giant congenital naevi in children.
METHODSThe surface layer of the giant naevi, mainly razor graft, was excised tangentially using electrical dermatome. Then the left naevi was removed by dermabrasion.
RESULTSThe satisfactory effects were obtained in 9 of 16 cases. The colour of operative area approached the normal skin, occasionally with scattered pigmentation and hypertrophic scars. In 6 cases, the colour appeared lighter than before. One case relapsed after half a year.
CONCLUSIONSThe congenital giant naevi in children could be treated using the tangential excision of razor graft. The short-term results might be expected.

10.Latest update on immunotherapy of Epstein-Barr virus-associated post-transplantation lymphoproliferative disease.

Epstein-Barr virus (EBV)-associated post-transplantation lymphoproliferative disease (EBV-PTLD) is a potentially life-threatening complication after hematopoietic stem cell transplantation or solid organ transplantation. In the last decade, the survival of patients with EBV-PTLD has been significantly improved by immunotherapeutic interventions among high-risk patients. The immunotherapeutic interventions for EBV-PTLD include reduction in immunosuppression, CD20 monoclonal antibodies (rituximab) as monotherapy or in combination with chemotherapy, and adoptive immunotherapy with EBV-specific T cells. This paper reviews the latest update on the high-risk factors, clinical manifestations and immunotherapy of EBV-PTLD.
