1.CAP and ISO15189 accreditation requirements of verification and validation tests for clinical molecular diagnostic assays

College of American Pathologists (CAP) and International Standard Organization (ISO)15189 accreditation provides requirements to the verification and validation tests for clinical molecular diagnostic assays.The article will introduce the requirements in these aspects including the differences between the two accreditation systems,the differences between the verification and validation tests,the essential factors and standardized framework of the verification and validation tests for clinical molecular diagnostic assays.
2.Phacoemulsification and low powered posterior chamber intraocular lens implantation in high myopia cataract

Objective To evaluate the clinical effect of phacoemulsification and low powered posterior chamber intraocular lens implantation in high myopia. Methods Phacoemulsification was performed on 128 eyes of 84 patients with cataract in high myopia through scleral tunnel incision. Average phaco time was 201s and ultrasonic energy was 33.8%. Results The visual acuity 0.4 or better on the first day, in the first week and first month after the operations was in 71.1%,75.8%,79.7%respectively. Three months after surgery, 22.6% reached the vision of 1.0. The main cause of the poor result is due to the pathologic changes of the fundus in high myopia. The major complications were corneal edema (4.3%), fibrinous exudate in anterior chamber (1.6%) and rupture of posterior capsule (2.3%). Conclusion Phacoemulsification and low powered posterior chamber intraocular lens implantation offers advantages of better rehabilitation of visual acuity, proper correction of ametropia, prevention of retinal detachment and is an unitable technique for highly myopia cataract patients.
3.Curative effect on three non-invasive modes of mechanical ventilation in treatment of neonatal respiratory distress syndrome

Objective To investigate the curative effect on non-invasive CPAP and SIMV and BiPAP mechanical ventilation in treatment of NRDS. Methods A retrospective study was initiated on clinical data of 162 neonates, who were hospitalized from March 2010 to March 2014 in Jiangmen Central Hospital and underwent the above three modes of non-invasive mechanical ventilation treatment. All cases met the setting standard, and their curative effects were contrasted. Results 53 cases were treated by non-invasive CPAP, 60 cases were operated by non-invasive SIMV, 49 cases were done by non-invasive BiPAP. Among the three groups, the comparison on sex ratio,gestational age of birth and birth weight showed no difference (P > 0.05). The number of effective cases in three groups was 38, 53 and 44 respectively, and the number of invalid cases was 15, 7 and 5 respectively. Among three groups, the curative effect was significant (P > 0.05), curative effect of non-invasive SIMV group showed no difference (P > 0.05) compared with that of non-invasive BiPAP group, curative effect of non-invasive SIMV group and non-invasive BiPAP group was better than that of non-invasive CPAP group (P > 0.05). Conclusion Curative effects of non-invasive SIMV and non-invasive BiPAP for NRDS showed no difference. However, their curative effect is better than that of non-invasive CPAP, which indicates that they can be better choices.
4.Analysis of Utilization and Adverse Drug Reaction of Aminoglycoside Antibiotics in Our Hospital During 2005-2008

Objective:To investigate the use and adverse drug reaction of aminoglycoside antibiotics(AmAn) in our hospital during 2005-2008,and to valuate its rationality for reducing its adverse drug reaction.Methods:The data about AmAn's consumption quantity and consumption sum in our hospital were analyzed with the DDD method.Results:Etimicin dominated the first place among AmAn in our hospital,and replaced Gentamicin.The quantities of Netilmicin,Isepamicin and amikacin were stable.In the same period,the main adverse drug reactions were allergic-like reactions,skin rash,ototoxicity and local numbness.Conclusion:Careful selection according to etiology,genetics testing,stronger monitoring of blood concentration and rational dosage treatment will provide a good prospect in the use of AmAn for the multifaceted gram-negative bacteria infection.
5.A STUDY OF THE NUMBER AND DISTRIBUTION OF LANGERHAN'S CELLS IN HUMAN ESOPHAGEAL EPITHELIUM

By using ATP-ase histochemical and OKT6 colloidal goldsilver immunohi- stochemical methods, we have studied the location and distribution of Langer- han's cells (LCs) in human esophageal epithelium. The results of our study showed that the dendritic LCs scatter in the basal and middle layers of the esophageal epithelium. The morphology and density of LCs are variable as the age grows and the location changes, more LCs are found in the adult esophageal epithelium than in the foetus, and more LCs are found in the lower segment of esophagus than in the upper segment.

7.Evaluation of pediatric sequential organ failure assessment score in diagnosis and prognosis of children with infection in pediatric intensive care unit

Objective:To evaluate the predictive value of the pediatric sequential organ failure assessment (pSOFA) in predicting the prognosis of infected children.Methods:We performed a retrospective analysis of children with infection or suspected infection who were hospitalized at PICU from January 1, 2018 to December 31, 2018.The worst physiological and laboratory indicators detected within 24 hours after admission were collected.The pSOFA and systemic inflammatory reaction syndrome(SIRS) score were used for diagnosis.Patients with pSOFA≥2 and SIRS score≥2 were included.Pediatric multiple organ dysfunction score(P-MODS), pediatric logistic organ dysfunction(PELOD-2), and pediatric critical illness score were performed for those patients who met the criteria.Survival or death during hospitalization was used to determine the prognosis, and death conditions with different scores of pSOFA and SIRS were analyzed.Receiver operation characteristic curve was used to evaluate the predictive value of pSOFA for the prognosis of children with infection.Results:A total of 755 cases were eligible for infection or suspected infection, among which 303 children were eligible for SIRS and pSOFA score ≥2, 165 cases were eligible for SIRS score ≥2 alone, 85 cases were eligible for pSOFA score ≥2 alone, and 53 cases were eligible for both.The in-hospital fatality rates of pSOFA 2, 3, 4, 5-6 and 7-9 points were 8.97%, 15.38%, 26.09%, 57.64% and 100%, respectively.With the increase of pSOFA, the fatality rate increased gradually.The in-hospital fatality rates of SIRS score 2, 3 and 4 were 3.64%, 12.64% and 4.76%, respectively.The area under the receiver operating characteristic curve of pSOFA was 0.74 (95% CI 0.62, 0.86), which was significantly higher than the other four scores.At a cut-off value of 4 points, it had a sensitivity of 56% and a specificity of 82% to predict prognosis. Conclusion:The pSOFA score has the highest accuracy than the SIRS score, P-MODS, PELOD-2 score and pediatric critical illness score in predicting the hospital mortality of PICU-infected children.

Spinal cord injury affects cerebral cortex in many respects,including morphology and gene expression of neurons,reorganization in function and structure,regional brain blood flow,expression of neurotrophic factor and its receptor.These changes may affect rehabilitation care and functional recovery of the patients with spinal cord injury.Therefore,studies targeting these changes are vital for our further understanding of the mechanisms of spinal cord injury and help to explore new therapies.
9.Treatment of Graves Hyperthyroidism by Jiakangling Capsule Combined with Reduction of 131I: an Efficacy Observation.

OBJECTIVETo observe the clinical efficacy of Jiakangling Capsule (JC) combined with reduction of 1311 in treatment of Graves hyperthyroidism.
METHODSTotally 387 Graves hyperthyroidism patients were randomly assigned to the treatment group (200 cases) and the control group (187 cases). Patients in the treatment group took JC combined with reduction of 131I. The 131I dosage per gram of thyroid tissue was 50-80 microCi. They additionally took JC one week after taking 1311 for one consecutive month. Patients in the control group took 131 routinely as one disposable treatment. The 131I dosage per gram of thyroid tissue was 70-120 microCi, without using JC or other anti-thyroid drugs. All patients were reexamined after 24-month treatment. Whether hyperthyroidism was cured, incurred, or permanent was observed. Efficacies of thyroglobulin antibody (TGAb) and thyroid microsome antibody (TMAb) were compared between the two groups.
RESULTSCompared with the control group, the incurred ratio increased in the treatment group [3.2% (6/187) vs. 16.0% (32/200), P < 0.01], the incurred ratio of strong positive TGAb and TMAb patients increased [3.5% (2/57) vs. 27.1% (16/59), P < 0.01], the permanent hypothyroidism ratio decreased [21.1% (12/57) vs. 3.4% (2/59), P < 0.05 ].
CONCLUSIONJC combined with reduction of 1311 was superior in treating Graves hyperthyroidism induced permanent hypothyroidism than routine 1311 treatment, especially for strong positive TGAb and TMAb patients.

10.DIAGNOSIS OF CEREBRAL ANGIORETICULOMA WITH CT (REPORT OF 3 CASES)

To improve the CT diagnosis of cerebral angioreticuloma, 3 cases of pathologically proven cerebral angioreticuloma were reviewed. The results showed that the image of CT of the first case revealed an irregular, parenchymatous mass located in the right temporallobe and middle cranial fossa.In the second patient,the image showed a annular mass with cystic consistency,surrounded by an area of edema, situated in the right frontal lobe,while in the third patient there was an irregular high density mass with an area of edema lying in the left frontal lobe and along the middle line. Pathological diagnosis of the first patient was mixed angioreticuloma and glioblastoma, and that of the others were angioreticulomas. the results suggested that if the mass in the cerebrum was obviously enhanced in CT, the diagnosis of angioreticuloma should be considered.
