1.Relationship between Serum BNP and Heart Failure in Patients undergoing Maintenance Peritoneal Dialysis

Objective To investigate the relationship between Serum BNP and heart failure in patients un-dergoing maintenance peritoneal dialysis ( MPD) .Methods 80 uremia patients who underwent MPD and 40 healthy volunteers were enrolled in the study.In the morning of treatment, we assessed their cardiac function according to in-terrogation, physical examination, chest X-ray and echocardiography while detecting BNP, Scr, BUN levels in the fasting venous serum of upper extremity.Results 80 MPD patients were divided into 2 groups:a heart failure group (Group A, n=42) and a normal cardiac function group (Group B, n=38).40 healthy volunteers served as a con-trol group ( Group C ) .There were no significant differences in age, weight and sex ratio among 3 groups ( P>0.05).The time period of maintenance peritoneal dialysis in Group A and B was similar (P>0.05).Compared with Group C, the levels of Serum BNP, Scr and BUN were significantly higher in Group A and B( P<0.05) .There were no significant differences of Scr and BUN between Groups A and B ( P>0.05) .The level of Serum BNP was signifi-cantly higher in Group A than that in Group B (P<0.05).Conclusion The level of Serum BNP may be closely re-lated to the cardiac function and it may be used as a predictor for heart failure in MPD patients.
2.The clinical analysis of hospital acquired acute renal failure

Objective To expolre the clinical characteristic of hospital acquired acute renal failure(HA-ARF).Methods To retrospectively analyse the clinical datas of 103 patients who were suffered from acute renal failure in the duration of hospital stay within 5 years,which was compared with the homeochronous community-acquired acute renal failure(CA-ARF).Results In the 103 cases of HA-ARF,the incidence rate was 32.04% in the major age range from 60 to 74 years old,infection was the most important factor(33.01%),then other factors were cardial and cerebrovascular diseases(17.48%),acute hypovolemia(14.56%) and operation(9.71%);The complicating other organ nonfunction(34.95%),and the mortality rate was 39.80%,all the factors in HA-ARF were higher than that in CA-ARF.Conclusion The main factors of origin in HA-ARF are infection,cardial and cerebrovascular disease,acute hypovolemia and operation.The mortality rate of this disease is higher.Etilogical treatment promptly and dialysis regimen are the main means to rescue these patients.
3.Effects of genistein and daidzein on cholesterol metabolism in ovariectomized rats

AIMTo study effects of genistein and daidzein on chole sterol levels in serum of ovariectomized rats. METHODS70 Wistar rats were randomly divided into 7 groups: control group, model group, estrogen g roup, genistein groups of high dose and low dose, daidzein groups of high dose a nd low dose with 10 rats in each group. All rats were ovariectomized except for that of the control group. One week after operation, the soyisoflavones were ad ministrated with different dose of genestein and daidzein for 6 weeks. Six weeks after operation, the rats were killed, with serum and liver taken, and the leve ls of total serum cholesterol (sTC), low-density lipoprotein cholesterol (LDL- C), high-density lipoprotein cholesterol (HDL-C) and total liver homogenate ch olesterol (hTC), were measured. RESULTSThe level of sTC, hTC, LDL-C in serum of ovariectomized rats of model group increased significantly, c ompared with control group. Eestrogen reduced the levels of sTC, hTC and LDL-C, but had no effects on the levels of the HDL-C. Genistein reduced the levels of sTC, hTC and increased the level of the HDL-C, but had no effect on that of LD L-C. Daidzein reduced the levels of sTC, hTC, LDL-C, but had no effect on the level of the HDL-C. CONCLUSIONGenistein and daidzein suppressed the increase of cholesterol levels of serum in ovariectomized rats through deff erent pathways. The effect of daidzein on serum cholesterol level of rats is mor e potent than that of genistein.veloftheHDL C .CONCLUSION Genistein
4.Recent progress of protein glycosylation characterization utilizing native conformer-resolved mass spectrometry

Glycosylation of proteins, one of the most prevalent and complex post-translational modifications occurring in nature, plays a crucial role in regulating protein net charge, conformation, binding properties and, ultimately, biological function.Traditional structural techniques are not amenable for glycoproteins due to the inherent heterogeneity of oligosaccharides.With the advances in analytical technique, mass spectrometry displays an increasingly crucial role in elucidating the structure of glycoproteins.Mass spectrometry-based proteomic technique can dissect the chemical composition and site information of low-abundance glycosylation at the peptide level.Instead, native mass spectrometry (nMS) can analyze intact glycoproteins while maintaining the information for glycan heterogeneity, and the insights into the regulatory effects of glycosylation on protein higher order structures and interactions with other proteins or ligands.As a representative structural mass spectrometry tool, ion mobility-based nMS strategy is powered by its conformer-resolving capability and by the feasibility of conformer manipulation through collision-induced unfolding.Consequently, native IM-MS analysis can provide rich information of dynamic protein conformations, allowing for the rapid identification and differentiation of protein isoforms in an unprecedented manner.In this review, we briefly introduced two emerging native IM-MS analytical modes, dynamic conformer-resolving mode and glycoform-resolving mode.Besides, we also discussed the recent progress of conformational and topological characterization of intact glycoproteins with three typical model systems based on two above-mentioned emerging modes of native IM-MS.
5.Clinical study on the treatment of postoperative pain and edema caused by mixed hemorrhoids by atomization fumigation and washing of Jiedu-Shengji Decoction combined with Maren soft capsule

Objective:To evaluate the clinical effect of Jiedu-Shengji Decoction combined with Maren soft capsule in the treatment of postoperative pain and edema of mixed hemorrhoids. Methods:110 patients with circular mixed hemorrhoids in the People's Hospitalof Lujiang County from January 2017 to January 2019 who met the inclusion criteria were divided into two groups according to the random number table method, with 55 patients in each group. Both groups were treated with external dissection and internal ligation operation of mixed hemorrhoids. The control group was treated with Maren soft capsule after the operation. The study group was treated with atomization fumigation and washing of Jiedu-Shengji Decoction on the basis of the control group. Both groups were treated for 14 days. VAS was used to evaluate the degree of anal pain. The anal edema was observed and graded. Serum levels of VEGF, basic fibroblast growth factor (bFGF) and platelet-derived growth factor (PDGF) were measured by ELISA. 14 days after the operation, to observe and evaluate the complications, the clinical effect was evaluated. Results:The total effective rate of the study group was 92.7% (51/55), and that of the control group was 74.6% (41/55), the difference was statistically significant ( χ2=6.643, P<0.01). The VAS score and anal edema score of the study group on the 7 d ( t values were 9.144 and 8.541, respectively, all Ps<0.01) and 14 d ( t values were 13.720 and 12.819, respectively, all Ps<0.01) after operation were significantly lower than those of the control group. The level of VEGF, bFGF and PDGF in the study group on the 7 d ( t values were 7.491, 2.972 and 5.170, respectively, all Ps<0.01) and 14 d ( t values were 6.117, 3.031 and 4.599, respectively, all Ps<0.01) after operation were significantly higher than those in the control group. The incidence of postoperative complications was 7.3% (4/55) in the study group and 3.6% (2/55) in the control group, there was no significant difference between the two groups ( χ2=0.705, P=0.401). Conclusions:The clinical effect of atomization fumigation and washing of Jiedu-Shengji Decoction combined with Maren soft capsule in the treatment of postoperative pain and edema of mixed hemorrhoids is significant, which can significantly improve symptoms, accelerate the formation of new blood vessels, promote wound healing, and have fewer complications.
6.Comparison of different stone scoring systems in predicting the stone-free rate after RIRS for upper urinary tract lithiasis

Objective:To compare the accuracy of different stone scoring systems for predicting the stone-free rate (SFR) after retrograde intrarenal surgery (RIRS).Methods:The clinical data of 227 patients with lithiasis undergoing RIRS from June 2017 to December 2020 in Affiliated Benq Hospital of Nanjing Medical University and Qingdao Fuwai Hospital were retrospectively analyzed. There were 152 males and 75 females. The average age was (53.0±10.4) years old. The average body mass index was (26.9±2.1)kg/m 2. The maximum diameter of the stone was (22.7±12.8)mm. The stone is located in left side in 133 cases and in right side in 94 cases. The stones of 44 cases were located in upper ureter, upper calyceal or renal pelvis, that of 23 cases were in medium calyceal, 157 cases in lower calyceal, and 3 cases in calyceal diverticulum.The average CT value of stone was (778.3±350.4)HU. American Society of Anesthesiology (ASA)scores: 86 cases of grade Ⅰ, 129 cases of grade Ⅱ, 12 cases of grade Ⅲ. Preoperative non-contrast CT was conducted and three-dimensional data were constructed. A single observer reviewed and entered the modified S.T.O.N.E., RUSS, modified S-ReSC, R. I.R.S., SHA.LIN, Ito nomogram, S. O.L.V.E., stone free index (SFI) scores. Logistic analysis were performed between every score and SFR. Receiver operating characteristic (ROC) curve was drawn to detect sensitivity and specificity of every score in predicting the SFR. The predictive accuracies of all scores were compared. Results:The SFR was 83.0%(189/227). There were statistically significant differences in modified S. T.O.N.E.(10.5±1.9 vs. 12.7±1.8), RUSS[1(0, 4) vs. 3(0, 6)], modified S-ReSC (8.2±5.6 vs. 11.8±6.0), R.I.R.S.(6.2±1.4 vs. 8.1±1.2), SHA.LIN (9.9±2.4 vs. 13.0±2.1), Ito nomogram (12.1±5.8 vs. 4.3±3.3), S. O.L.V.E. (6.8±1.6 vs. 8.7±1.2), SFI score (7.9±1.1 vs. 6.3±0.9) between the stone-free group and the stone remaining group ( P <0.05). Logistic regression revealed that modified S.T.O.N.E., RUSS, modified S-ReSC, R. I.R.S., SHA.LIN, Ito nomogram, S. O.L.V.E. and SFI score were significantly associated with SFR( P<0.05). There were no significant differences in the area under the curve (AUC) between the modified S. T.O.N.E., RUSS, R. I.R.S., SHA.LIN, Ito nomogram, S. O.L.V.E. and SFI score( P>0.05), but there were significant differences in the AUC between modified S-ReSC score and other score ( P<0.05). When the cutoff of SHA.LIN, SFI and R. I.R.S. score was determined as 10, 6 and 6 scores, the specificity of SHA.LIN, SFI and R. I.R.S. score was 94.7%, 92.6% and 89.5%, respectively. Conclusions:All score could predict the postoperative SFR of RIRS, while the SHA.LIN, SFI and R.I.R.S. score were more accurate than the other scores. The accuracy of the modified S-ReSC in predicting SFR after RIRS was slightly worse than other scores.
