Goiter*

Introduction: 131Iodine therapy is effective in nodular nontoxic goiter with enhanced effects using recombinant thyroid stimulating hormone (rTSH). The eventual fibrosis of the thyroid tissue and blood vessels ligates the vascular supply of the nodule. The study aims to show the successful reduction of thyroid and nodule volumes in large solitary and multinodular goiters using serial low dose 131iodine therapy (10mCi) at three to six months interval. Methods: A retrospective analytical study was done from January 2010 to December 2012 and included twenty three patients with enlarged solitary and multinodular (nodule/s ≥2cm) non-toxic goiter (females: age range 35-65yrs) given serial 131iodine therapy (eight to10mCi) at three to six-month interval. Before each course, serum thyroid stimulating hormone (TSH) was done to document hypothyroidism while thyroid gland and nodule sizes were monitored by ultrasonographic measurements serially with each 131iodine therapy. Relief of compressive symptoms was monitored on follow-up at clinic. Results: Thyroid size reduced by 38-40% from baseline after first radioactive iodine (RAI) and by 33-39% after the second RAI. Thyroid nodules reduced by 63-69% and 11-25% serially. Significant reduction was noted after the first RAI. One subject underwent third RAI with 80-85% overall reduction in nodule size. All patients developed post-RAI hypothyroidism and overall had greater than 50% increase in levothyroxine replacement dose after the last RAI. Significant relief of compressive symptoms was noted by 91% post-therapy. Four thyroid nodules disappeared which resulted in reduced total number of thyroid nodules from 29 to 25 nodules post serial RAI. Conclusion Serial 131iodine therapy proved to have thyroid and nodule size reduction by more than 70% in this study. Among patients who do not consent or have contraindications to surgery, serial 131iodine therapy may be considered a safe and effective non-surgical alternative.
Goiter

Intrathoracic goiters consist of two types: the primary and secondary type. Primary intrathoracic goiters are very rare. We report here on a case that was resected with using the thoracoscopic assist technique. The transthoracic or transsternal approach is necessary for primary goiters to remove them completely without complications.

An clinical examination and quantification of urinary iodine were performed in 1,513 Th¸i people with ages of 6-70 at Chieng Sinh commune Son La province. Results showed that the rate of goiter was 11.76% and at mild level of local goiter and the urinary iodine was at adequate level (more than microgram/dl). However, it should monitor continuously to evaluate timely the iodine deficiency

Exophthalmus is an important symptom of Basedow’s disease. CT Scan, MRI or more simply by Hertel ruler can measure the exophthalmic level. The exophthalmic level was studied in association with age of diagnosis, disease duration and gender. The exophthalmus symptom has major diagnostic value in Basedow-related ophthalmic problems. The incidence of exophthalmus in Basedow patients is 60.9%, the longer the duration of disease, and the higher the incidence of exophthalmus.

In the year period of 1998-2002, in the Hospital N07 in Hai Duong province and the Hospital Buu Dien in HaNoi. 117 patients (99 females and 18 males, aged 17-69) who were clinically diagnosed as thyroid nucleous, goiter were undergone a fine needle puncture biopsy. Results showed a consistency between clinical diagnosis and ultrasound in 92.9% of solid form, 93.1% liquid form and in mixt form, the consistency rate was lower 55.5%. Between clinical diagnosis and ultrasound, there was a consistency in the determination of 76% of the number of goiters, and the consistency rate was higher between these 2 diagnostic methods, 81/85 in solid form and 25/27 in liquid form, while in mixt form the technique of fine needle aspiration puncture notified more cases than ultrasound 11 versus 5

A study on the clinical, paraclinical and epidemiological features of 68 patients with hypertrophy simple goiter with complication in Hue hospital during 1996-1998 has shown that the disease occurred mainly in the women with ages of 31-50. The goiter accompanying with the nuclear accounted for high rate, especially multinuclear. The operation indicated for cases of goiter with pressing the neck region or with nuclear. The results of postoperative monitoring within 4 months found good (86,8%), normal (8,8%) and poor (4,4%).

The study was conducted on 593 children, 325 salt samples, 60 urinary samples and 30 fresh water samples, using the descriptive epidemiological method. Ca, Na, K, Cl, Cyanide, Fl, Cu, Pb and Iode contents were quantified. The data was processed by statistic method. The results showed that Ca and Fl contents were within acceptable limit. Iode was provided sufficiently. The content of cyanide, an anti-thyroid agent, was higher than acceptable limit
