2.Successful reduction in thyroid and nodule volumes in large solitary and multinodular goiters with serial 131 iodine therapy

Introduction:
131Iodine therapy is effective in nodular nontoxic goiter with enhanced effects using recombinant
thyroid stimulating hormone (rTSH). The eventual fibrosis
of the thyroid tissue and blood vessels ligates the vascular
supply of the nodule. The study aims to show the successful
reduction of thyroid and nodule volumes in large solitary and
multinodular goiters using serial low dose 131iodine therapy
(10mCi) at three to six months interval.
Methods:
A retrospective analytical study was done from
January 2010 to December 2012 and included twenty three
patients with enlarged solitary and multinodular (nodule/s
≥2cm) non-toxic goiter (females: age range 35-65yrs) given
serial 131iodine therapy (eight to10mCi) at three to six-month
interval. Before each course, serum thyroid stimulating
hormone (TSH) was done to document hypothyroidism
while thyroid gland and nodule sizes were monitored by
ultrasonographic measurements serially with each 131iodine
therapy. Relief of compressive symptoms was monitored on
follow-up at clinic.
Results:
Thyroid size reduced by 38-40% from baseline after
first radioactive iodine (RAI) and by 33-39% after the second RAI. Thyroid nodules reduced by 63-69% and 11-25% serially.
Significant reduction was noted after the first RAI. One
subject underwent third RAI with 80-85% overall reduction in
nodule size. All patients developed post-RAI hypothyroidism
and overall had greater than 50% increase in levothyroxine
replacement dose after the last RAI. Significant relief of
compressive symptoms was noted by 91% post-therapy.
Four thyroid nodules disappeared which resulted in reduced
total number of thyroid nodules from 29 to 25 nodules post
serial RAI.
Conclusion
Serial 131iodine therapy proved to have
thyroid and nodule size reduction by more than 70% in
this study. Among patients who do not consent or have
contraindications to surgery, serial 131iodine therapy may be
considered a safe and effective non-surgical alternative.

3.Primary Intrathoracic Goiter: A case report.

Intrathoracic goiters consist of two types: the primary and secondary type. Primary intrathoracic goiters are very rare. We report here on a case that was resected with using the thoracoscopic assist technique. The transthoracic or transsternal approach is necessary for primary goiters to remove them completely without complications.

4.Contribution to a study on the iodine deficiency among Thai minority at Chieng Sinh commune, Son La province

6.Primarily results of surgery of hypertrophy simple goiter with complication

A study on the clinical, paraclinical and epidemiological features of 68 patients with hypertrophy simple goiter with complication in Hue hospital during 1996-1998 has shown that the disease occurred mainly in the women with ages of 31-50. The goiter accompanying with the nuclear accounted for high rate, especially multinuclear. The operation indicated for cases of goiter with pressing the neck region or with nuclear. The results of postoperative monitoring within 4 months found good (86,8%), normal (8,8%) and poor (4,4%).

7.Some chemicals in the water and their relationship to goiter in a mountain commune

The study was conducted on 593 children, 325 salt samples, 60 urinary samples and 30 fresh water samples, using the descriptive epidemiological method. The CN content in drinking water source is 7,14+/- 1,9, 10 times higher than this the limited standard, the rate of goiter : 53,91%, the rate of goiter: among the used mine- water family is higher 2,28 times than this of the families does not use mine- water.

9.Role of preventive therapy for solid lesions of normothyroidic single-nodular goiter.

Normothyroidic single-nodular goiter is a common disease. Now it can be detected and intervented early so that its prognosis is improving significantly. The effectiveness of preventive therapy for nodular goiter by TST releasing inhibition with levothyroxin was examined in 110 patients. The results showed that levothyroxin therapy to inhibit the release of TST is needed for solid lesions. In younger patients with less than 1cm goiter, the result of treatment is more likely to be maximized, the lesions regress completely and the thyroid functions are normalized. For more than 45-year patients with more than 2-cm goiter, the result of treatment is poor. Use levothyroxin with cautions in over 60-year patients who have cardiovascular diseases and multiple nodular goiter.

10.Research for exophthalmic level in Basedow patients by Hertel ruler.

Exophthalmus is an important symptom of Basedow’s disease. CT Scan, MRI or more simply by Hertel ruler can measure the exophthalmic level. The exophthalmic level was studied in association with age of diagnosis, disease duration and gender. The exophthalmus symptom has major diagnostic value in Basedow-related ophthalmic problems. The incidence of exophthalmus in Basedow patients is 60.9%, the longer the duration of disease, and the higher the incidence of exophthalmus.
