Median raphe cyst is known as congenital lesion of the perineum and genitalia, but its etiology is unclear. Most investigators believe that the median raphe cyst represent defects in the embryologic developenient bf the male genitalia. Simple surgical excision is effective in most cases. We report our experience with two cases of median raphe cyst without specific symptoms. Even though median raphe cyst is asymptomatic, surgical therapy is worth applicable because it relieve a patient from cosmetic and psychotic problem.

4.Two cases of Fournier's gangrene.

In 1883 Fournier, a French venereologist, described 5 patients with unexplained fulminating gangrene of the male genitalia. Fournier emphasized 3 characteristics 1) the abrupt onset in a young, healthy male subjects, 2) the rapid progression to gangrene and 3) the absence of a discernible cause. As currently used by many authors, Fournier's gangrene describes a widely destructive, gangrenous process of the genitalia, with little regard for Fournier's original tenets of patient's age or definable etiology. Early identification and prompt initiation of medical and surgical therapy is imperative. We report two cases of Fournier's gangrene with brief review of the literatures.

Scrotal calcinosis is a relatively common tumor of the male genitalia but there is controversy in the literature regarding its pathogenesis. The authors of many case reports debate whether the lesions are truly idiopathic or if they arise from other conditions. Only few cases of scrotal calcinosis have been proven to have an eccrine origin so far. Herein, we report a case of scrotal calcinosis originating from a milium-like syringoma in a male Korean patient.

7.Trial of Serum 5alpha-androstane-3alpha,17beta-diol glucuronide for Androgen Therapy in Children.

Androgen treatment in boys with underdeveloped external genitalia requires close attention to avoid the side effects. The dose and duration of androgen have usually been decided according to the validation of the effects and the side effects. However, the dosage that showed the desirable effect or undesirable side effects already exceeded the optimal dosage. So the indicator to predict the effect of androgen is necessary for boys during androgen treatment. To evaluate serum 5alpha-androstane-3alpha, 17beta-diol glucuronide as this indicator we tried to obtain the correlation between the increase of penile length and serum 5alpha-androstane-3alpha, 17beta-diol-glucuronide and dihydro-testosterone during dihydrotestosterone treatment. While the mean serum level of 5alpha-androstane-3 alpha,17beta-diol glucuronide was higher than that of dihydrotestosterone in 17 patients without defects of androgen action,the opposite result was observed in 6 patients with defect of androgen action. The increase in penile length estimated by the calculation of the 8 weeks difference of deviation/normal S.D.significantly correlated with the serum 5alpha-androstane-3alpha, 17beta-diol glucuronide but not with serum DHT. Therefore the responsibility of the target cell to androgen seems to determine both the effect of androgen treatment and the serum level of 5alpha-androstane-3alpha, 17beta-diol glucuronide. In conclusion the serum 5alpha-androstane3alpha, 17beta-diol glucuronide could have a value as the indicator to predict the effect of androgen treatment in children with underdeveloped external genitalia.

8.The Successful Treatment of Intraurethral Condyloma Acuminatum with Thiotepa: Report of a Case.

Condyloma acuminatum is a common lesion of the penis and genitalia. It uncommonly involves the urethra but when it does the lesions are difficult to eradicate. 9.Anatomical study on true hermaphroditism in an Indian pig (Sus Scrofa Domesticus).

A pig was confirmed to be a true hermaphrodite on the basis of gross and histomorphological studies of the genital organs. The genitalia was consisted of left ovary, oviduct, two coiled uterine horns, body of uterus alongwith right testis and an epididymis. Vagina and vulva were absent but male urethra with prostate gland was present. Grossly the size of all the genital organs appeared to be normal. Histomorphologically, testis and epididymis were underdeveloped as there was no clearcut spermatogenia and sertoli cells but Leydig cells were normal. The ovary presents normal histological features with some portion of testicular tissue. Degeneration of uterine epithelium was observed along with normal endometrial glands.

10.Unusual Penetrating Injury to the Male Genitalia.

Various self-inflicted foreign bodies have been removed from the lower urinary tract and external male genitalia. In many cases foreign bodies were inserted or applied for autoerotic reasons which are frequently associated with mental health disorders. Here the author reports an unusual case of self insertion of a 6-cm long metal needle at penoscrotal junction of the erect penis for sexual pleasure. The stitching needle was inserted by a 20-year old male who was too ashamed to admit the motive at first. The caudal end of the needle was palpable at the root of the penis as it was below the urogenital diaphragm. The needle was successfully removed under fluoroscopic guidance with minor surgical exploration and with no complications.
