1.Damage control resuscitation: redress the coagulopathy at the early stage

After the concept of damage control resuscitation is introduced, in the most severely injured casualties, when the lethal triad of hypothennia, acidosis, and coagulopathy are present, resuscitation prac-tice for damage control usually focuses on rapid reversal of acidosis and prevention of hypothermia, while di-rect treatment of eoagulopathy has been relatively neglected. Recent studies have brought back to light the importance of treating the coagulopathy of trauma at an earlier stage. This review focused on the importance and the right time of treating this disorder, analyzed the methods of addressing the early coagalopathy in damage control resuscitation and gave some advices.
2.Treatment strategies of reoperation of benign biliary tract diseases in the era of precision surgery

Reoperation on biliary tract diseases is a kind of complex and difficult surgery,with the high risk and recurrence rate.Benign biliary tract diseases included residual and recurrence of bile duct stone,bile duct injury and benign biliary stricture,which are still the most common causes for reoperation.The common causes,application of the precision surgery and new mode of multidisciplinary team (MDT) for the reoperation on benign biliary diseases were explored in this paper.With the techniques developments of minimal invasive and precision surgery,the new mode of MDT for the reoperation on benign biliary diseases will provide the best individual diagnosis and treatment to the patients.
3.Effect of suberoylanilide hydroxamic acid combined with hypertonic saline on the hemodynamics of hemorrhagic shock rats

BACKGROUND:Studies have shown that suberoylanilide hydroxamic acid (SAHA) has protective effects in some vital organs in animals after hemorrhagic shock, and 7.5% hypertonic saline (HS) exerts significant effects on stabilizing the hemodynamics of hemorrhagic shock animals. OBJECTIVE:To evaluate the effect of SAHA combined with HS on the hemodynamics of hemorrhagic shock rats. METHODS: Fifty rats were randomly and equaly divided into five groups: sham, shock non-resuscitation, SAHA, 7.5% HS, and 7.5% HS + SAHA. Each group contained 10 rats. Except the sham group, rats in the remaining four groups were applied to establish hemorrhagic shock models. In the sham group, rats were given anesthesia catheter, not bleeding; in the shock non-resuscitation group, the bleeding was found, but rats were not resuscitated and were kiled after 60 minutes of observations; in the other three groups, rats were respectively resuscitated at 60 minutes after bleeding, through intravenous administration of SAHA within 5 minutes, 7.5% HS and SAHA + 7.5% HS within 20 minutes. Heart rate, mean arterial pressure and left ventricular systolic pressure were monitored through the femoral artery and the right common carotid artery catheter in each group. RESULTS AND CONCLUSION:At 3 hours after resuscitation, the heart rate was the highest in the 7.5% HS + SAHA group, compared with the SAHA and 7.5% HS groups (P < 0.05). After resuscitation, the mean arterial pressure and left ventricular systolic pressure were increased, with long-lasting effect and less fluctuation (P < 0.05). Experimental results show that 7.5% HS combined with SAHA has a superiority than traditional HS and simple drugs for resuscitation after hemorrhagic shock.
4.Childhood acne

According to the age of onset, childhood acne can be divided into neonatal acne, infantile acne, preschool acne, and prepubertal acne. Neonatal acne and infantile acne can serve as predictive factors for severe adolescent acne, preschool acne may be related to underlying endocrine diseases, and prepubertal acne is a sign of pubertal maturation. The treatment of childhood acne is similar to that of adolescent acne, and adverse effects of drugs and their impact on the growth and development of children should be considered.
5.Efficacy and safety of budesonide/formoterol single inhaler therapy versus budesonide inhaler plus formoterol inhaler in children with bronchial asthma

Objective To compare the efficacy between budesonide/formoterol single inhaler therapy and budesonide inhaler plus formoterol inhaler in children with bronchial asthma.Methods In randomized, open, parallel-group trial, 80 cases of children with bronchial asthma in children asthma center of Children's Hospital of Hebei Province were divided into B/F group ( budesonide/formoterol single inhaler therapy ) and B +F group (budesonide inhaler plus formoterol inhaler), 40 cases in each group.All patients in B/F group inspired budesonide formoterol sing inspirator [80μg (budesonide)/4.5μg(formoterol)/snuff], and all patients in B+F group inspired budesonide(100μg/snuff) and formoterol(4.5μg/snuff), 1 snuff/time, respectively, both groups were treated with 2 times/d,12 weeks.The visual analog scale(VAS), lung function as forced expiratory volume in one second(FEV1)%, peak expiratory flow(PEF) and clinical symptoms were investigated.Results Compared with B+F group in the same time period, PEF% and FEV1%on the 12th week were higher, VAS score on the 4th, 8th and 12th week were lower, symptoms scores on 8th and 12th were lower in B/F group, respectively(P<0.05).Conclusion The efficacy of budesonide/formoterol single inhaler therapy is better than that of budesonide inhaler plus formoterol inhaler in children with bronchial asthma, which is more convenience and has a better compliance.

7.Chronic disease management and behavioral interventions

Chronic non-communicable diseases have become one of the public health problems threatening human health.Management measures of chronic diseases were summarized,such as three level prevention and self-management of chronic diseases,and patient joint decision-making.Behavioral interventions on chronic diseases were also summarized,such as risk factors of chronic disease,information technology and telemedicine.It suggests that a variety of chronic disease management and behavioral intervention measures should be applied in practical work to improve the patient compliance while enhancing the prevention and treatment of chronic diseases.
8.Three-dimensional finite element analysis on the stress of mandibular first molar restored with different locations and numbers of gold alloy posts and cores

BACKGROUND:The design of post and core of the molar is affected by various factors,including the effect of the number of posts on the retention force and the effect of the number of posts on the dentin stress and distribution.OBJECTIVE:To make a mechanical analysis on the three-dimensional finite element models of the mandibular first molar defect restored with different numbers and locations of gold alloy posts,cores and all-ceramic crown.METHODS:Cone-beam CT based three-dimensional finite element models of post and core restored first mandibular molars were established,with seven different post designs.According to the numbers and location of gold alloy post inserted into root canal,a single post was placed at the mesial buccal root in group A,a single post at the mesial tongue root in group B,a single post at the distal root in group C,two posts at the mesial buccal root and the distal root in group D,two posts at the mesial buccal root and mesial tongue root in group E,two posts at the mesial tongue root and the distal root in group F,one post respectively at the mesial buccal root,mesial tongue root and distal root in group G.The von Mises criterion was applied for comparing the maximum yon Mises stress value of dentin and stress concentration areas in the seven models which restored with golden alloy posts and cores with all-ceramic crown under the maximum (600 N),vertical (225 N),inclined (225 N) and horizontal (225 N) loads.RESULTS AND CONCLUSION:Under vertical,inclined and horizontal direct loads,the maximum stresses on the remaining dentin in group C were significantly lowest,compared with the groups A and B.The maximum stresses on the remaining dentin changed irregularly with the variation of the numbers and location of posts,when the mandibular first molar defect was restored with all-ceramic crown with gold alloy posts and cores.Under the horizontal load,the maximum Von Mises stress value of the dentin increased remarkably.Under the horizontal load,the maximum stresses on the remaining dentin in group E were the highest.To conclude,the numbers and location of posts and cores have influence on magnitude and distribution of stress,when the mandibular first molar defect is restored with all-ceramic crown with gold alloy post and core.
9.Endoplasmic reticulum stress is involved in N-methyl-N'-nitro-N-nitrosoguanidine, benzo[a]pyrene-7,8-dihydrodiol-9,10-epoxide and mitomycin-induced cellular response in FL cells

AIM: To understand whether endoplasmic reticulum stress (ER-stress) is involved in DNA-damaging agent/carcinogen induced cell responses. METHODS: Three DNA-damaging agents/carcinogens different in the mode of action, ie, alkylating agent N-methyl-N’-nitro-N-nitrosoguanidine (MNNG), bulky adduct forming agent benzo[a] pyrene-7, 8-dihydrodiol-9, 10-epoxide (BPDE) and cross-linking agent mitomycin C (MMC) were selected. SDS-PAGE and immunoblotting were used to examine the protein levels of GRP78/BiP, GDADD153/CHOP and activation state of endoplasmic reticulum located caspase-12 in FL cells before and after MNNG, BPDE or MMC exposure. RESULTS: Immunoblotting showed that the protein level of endoplasmic reticulum specific proteins GRP78/BiP and GADD153/CHOP were significantly increased and endoplasmic reticulum located caspase-12 was activated in low concentration of MNNG (0.25 and 1 ?mol/L) and BPDE (5 and 50 nmol/L)-treated cells. MMC at all of the three concentration used (5, 50 and 500 ?mol/L) decreased the expression of GRP78/BiP, while it has no effects on CHOP and caspase-12. CONCLUSIONS: Both low concentration MNNG and BPDE could trigger the ER-stress in the exposed cells, while MMC could induce the down-regulation of the GRP78/BiP protein, which plays an important mediating role in the induction of ER-stress and may thus change the responsiveness against ER-stress inducers. It is suggested that ER-stress might partially mediate the cellular responses excited by exposure to some DNA-damaging agents/carcinogens.
