1.Diagnostic and therapeutic strategies of congenital pulmonary sequestration in children

pulmonary sequestration (PS) is a rare and serious malformation of the bronchial-pulmonary.It is often associated with cardiovascular dysplasia and diaphragmatic malformations.The most common manifestation of the disease of the lung at newborn and early infancy is respiratory distress.Later on in life,cysts usually lose this compressive character and may remain asymptomatic until infection occurs,while producing cough,fever,and thoracic pain.It also can cause severe cardiovascular and pulmonary symptoms such as dyspnea,heart failure,pleural effusion,hemothorax and hemoptysis.Its symptoms are non-specific characteristic and are easily to be misdiagnosed or missed diagnosis.Misdiagnosis of PS resulted in unnecessary treatments as well as recurrent infectious complications.For confirmation of the diagnosis need imaging procedures such as CT or magnetic resonance angiography to delineate aberrant blood apply.All PS should be treatment because of the risk of inffection and interference with lung growth even in asymptomatic patients.The classical therapeutic approach is surgical resection.Embolization of abnormal supply vessels also is effective and safe treatment for PS.
3.Effect of bronchoscopy in the treatment of children′s plastic bronchitis

Plastic bronchitis (PB) is a kind of respiratory emergency that causes severe respiratory distress in children.Bronchoscopy plays an important role in the diagnosis and treatment of PB in children.To enable more PB children to benefit from bronchoscopy examination and treatment, pediatric clinicians should improve their knowledge of bronchoscopy and be familiar with the timing of endoscopic treatment of PB, the choice of rigid bronchoscope or fiber bronchoscopy, the efficacy of bronchoscopy interventional therapy, and the safety of bronchoscopy treatment.
4.The relationship between respiratory tract allergic disease and infection

In recent years, with the changes of human living environment, the morbidity of respiratory tiact allergic diseases such as allergic rhinitis, infant wheezing, allergic asthma, and other diseases increased rapidly, thereby affecting the quality of children′s life, and increasing heavy burdens to families and society.Microbial infection, including virus, bacteria, mycoplasma pneumoniae/chlamydia, fungi and parasites, can regulate the occurrence and development of respiratory tract allergic diseases through the immune mechanism.In order to deepen the understanding of the relationship between respiratory allergy and microbial infection and its related mechanism, the research progress on the relationship between respiratory allergy and microbial infection was reviewed in this paper.
6.Pathological analysis of effect of secondary hyperparathyroidism on arteriovenous fistula in maintenance hemodialysis patients

Objective To explore the effect of maintenance hemodialysis(MHD)patients with secondary hyperparathyroidism on radial artery calcification and arteriovenous fistula( AVF) function.Methods The specimens of radial artery and cephalic vein were collected from the 35 cases of MHD patients at the first time of arteriovenous fistula operation.The specimens were collected again when AVF loss function more than 2 years.Specimens of the arteries were examined histologically for calcification with von Kossa staining.According to the parathyroid hormone levels,the patients were divided into three groups,as mild secondary hyperparathyroidism group (iPTH<300pg/L) of 16 cases,moderate secondary hyperparathyroidism group (iPTH≥300pg/L,<800pg/L) of 7 cases,severe secondary hyperparathyroidism group ( iPTH≥800pg/L) of 12 cases.The pathological results were analyzed.Results The pathological results showed radial artery calcification and calcium salt staining positive significantly increased in the 35 cases of MHD patients[9cases(25.7%)vs 2cases(8.6%)],compared with the first time of arteriovenous fistula operation when AVF loss function (χ2 =5.290,P=0.020).The artery calcification was also located in the layer of medial smooth muscle cells.The radial artery specimens with calcium salt staining positive increased significantly in severe hyperparathyroidism patients group[12cases(25.7%) vs 1case(6.3%)],compared with mild secondary hyperparathyroidism patients(χ2 =5.168,P =0.012).Conclusion Calcification risk of radial artery in AVF is increased in MHD patients with secondary hyperparathyroidism.And hyperparathyroidism may be one cause of AVF failure.
8.Research progress on lung injury mechanism of adenovirus pneumonia in children

Human adenovirus is a DNA virus and a common pathogen of respiratory tract infection in children of China.Adenovirus can trigger a variety of mechanisms, thereby dissolving and killing lung cells.Consequently, it leads to such acute lung injury as acute respiratory distress and lung consolidation, and such pulmonary sequelae as bronchiolitis obliterates, unilateral transparent lung, bronchiectasis, and interstitial fibrosis.In this article, the mechanism underlying children lung injury caused by adenovirus was reviewed.
9.Clinical application of the reversed saphenous fasciocutaneous island flap combined with the vacuum sealing drainage therapy in plate exposure of the distal tibia

Objective To evaluate the clinical effect of the reversed saphenous fasciocutaneous island flap combined with the VSD therapy in plate exposure of the distal tibia.Methods From January 2008 to July 2010,seven patients with hardware exposure following internal fixation of the distal tibia fracture were treated by VSD therapy and reversed saphenous fasciocutaneous island flap reconstruction.The VSD therapy was performed from 7 to 26 days after hardware exposure,and the flap transfer was performed 7 to 14 days after the VSD therapy.The size of the soft-tissue defect ranged from 4 cm × 2 cm to 13 cm × 4 cm.The mean follow-up was 10.6 months after the fracture (range,8-14 months).Results The average times of the VSD therapy was 1.3.After VSD was removed,the exposed hardware was covered by healthy granulation tissue in all cases.All flaps were successfully transplanted,and the size of flap ranged from 6 cm × 3 cm to 15 cm × 6 cm.Six of 7 flaps survived completely without further procedures.The necrosis of the distal margin of the flap occurred in 1 patient.A solid bony union was confirmed by the X-ray in all patients after 4-6 months postoperatively.No skin defects or fistulae were observed.Conclusion The combination of the reversed saphenous fasciocutaneous island flap and VSD therapy could save the exposed hardware and cover the soft tissue defects in the distal tibia.
10.Effect of bifidobactria on dextran sulphate sodium-induced acute ulcerative colitis in mice

Objective To investigate the effect of bifidobacteria on dextran sulphate sodium(DSS)- induced acute ulcerative colitis in mice.Methods Thirty BALB/C mice were randomly divided into nor- mal control group (n=10),0501 strain group (n=10) and c122 strain group (n=10).Fifty BALB/ C mice received 5% dextran sulphate sodium(DSS) for 7 days to induce ulcerative colitis.The mice were then divided to model group,negative control group(perfused with 0.9 NaCl solution ),positive control group(perfused with SASP of 20 mg/ml),DSS + 0501 strain group(perfused with 1?10~9 CFU/ml bifidobacteria 0501 strain solution and DSS + c122 strain group (perfused with 1?10~9 CFU/ml bifidobacteria c122 strain solution).All mice were sacrificed 9 days later.The colon specimens were measure by histoehemical staining with H-E.The expressions of interleukin-10 (IL-10) and its protein were detected by RT-PCR and immunohistochemistry respectively.Results The degree of colon inflam- mation in mice both in DSS+ 0501 strain and DSS+ c122 strain groups were aggravated and expressions of IL-10 mRNA and protein were reduced compared to model group.No colon inflammation was found in 0501 strain and c122 strain groups.Conclusion Some strain of bifidobaeteria may aggravate colon in- flammation in mice when mucosal harrier is destroyed.
