1.The efficacy of 89 dilated cardiomyopathy cases treated by the carvedilol

Objective To analyse the therapeutic effect of Carvedilol in patients with severe heart failure.Methods The clinical data of 89 Carvedilol patients treated with dilated cardiomyopathy were retrospective analysis.Results The overall therapeutic effective rate was 98.88%,because of one death related to discontinued use of medication.Before and after treatment,systolic left ventricular cavity diameter (LVDD) were (68.25 ± 2.15) mm,(56.89 ±4.21) mm,respectively,and left ventricular ejection fraction(LVEF) were (28.63 ±0.33)%,(40.56 ±3.98) %,respectively,there were significantly differences (t =7.215,16.877,all P < 0.05).Conclusion Carvedilol is the cornerstone in the drug treatments of patients with dilated cardiomyopathy.
2.A New Method for the Extraction of Tea-Polyphenols

A new method for the extraction of tea-polypbenols (TP ) from tea using AlCl3 as precipitating agents was proposed. Compared with tbe traditional extraction method using organic solvents, it is shown that the new method is simple and can be operated easily. The extraction rate of TP by the new method could reach 10.5% and the content of the effective ingredients of TP could reach over 99.5%

6.Effect of preoperative neoadjuvant chemotherapy on cervical carcinoma

Objective To study the value of neoadjuvant chemotherapy in the treatment of cervical carcinoma. MethodsA clinical prospective study was carried out from Jan.1997 to Dec.2003,118 patients with cervical carcinoma were given two cycles of combined chemotherapy before operation(neoadjuvant chemotherapy group,group A).102 patients accepted surgery alone in our hospital from Jan.1997 to Dec.2003 were chosen as the comparison group(group B).Survival rate and complications were observed and analysed. Results5 year survival rate in group A was significantly higher than that in group B(P<0.01),The 5 year survival rate was significantly higher in group A than in group B,despite of different pathological types,with or without radiochemotherapy(P<0.05).The recurrent rate was lower in group A than that in group B,which show significant difference in the 5 year recurrent rate(P<0.05). ConclusionNeoadjuvant chemotherapy is an effective method for the treatment of cervical carcinoma in early stage.Because of satisfactory short-term control rate,excision rate,long-term survival rate and low recurrent and metastatic rate,neoadjuvant chemotherapy is worth popularizing in clinical treatment.
7.Effect of different wave lengths of low power laser on fracture healing

BACKGROUND: Low power laser irradiation has positive effects on fracture healing, including shortening the time of bone union and enhancement of bone formation. OBJECTIVE: To evaluate the effects of different wave lengths in low power laser on eedy fracture healing. DESIGN, TIME AND SETTING: Randomized, controlled, animal experiment. The study was performed at the Laser Center of Dankook University Medical Center between May 2005 and May 2006. MATERIALS: A total of 60 female English Hartley guinea pigs, 9 weeks old, weighing (300~30) g, were used for this experiment. They were randomly divided into three groups control, 632, and 830 nm groups (n=20). METHODS: The right femoral middle shaft fractures were made with a bone cutter and fixed with intramedullary nails (1.4 mm diameter K-wire). The frequencies of 632 nm and 830 nm semi-conductor laser machines were used. The laser irradiations were started 48 hours after operation vertically at the fracture sites and were applied every two days, at 100 J/cm<'2> of the same irradiation doses and 2.52 cm<'2> of the same irradiation areas in different irradiation groups. Finally, 46 guinea pigs survived, including 14 in control group, 16 in 632 nm group and 16 in 830 nm group. MAIN OUTCOME MEASURES: Guinea pigs were sacdficed at 3 and 6 weeks separately. The bone healing was assessed by the Modified Zorlu Scoring System, including gross, radiologic and histologic examinations. RESULTS: In the gross and radiologic findings, both 632 nm and 830 nm irradiation groups shown a significantly greater rate of callus formation at postoperatively 3 weeks compared to the control group (P< 0.01). Quantity of callus formation in 830 nm group was more than 632 nm group, but there was no significant difference (P > 0.05). The result of histological findings showed a significant increase of osteoblastic proliferation in two irradiation groups than the control group at 3 weeks postoperatively (P < 0.01). Enhancement of osteoblastic proliferation was more obvious in 830 nm group than in 632 nm group at 3 and 6 weeks, but there was no significant difference (P > 0.05).CONCLUSION: Both 632 nm and 830 nm wave lengths are considered to be optimal irradiation wave lengths for eady fracture healing. Moreover, 830 nm irradiation is better than 632 nm irradiation for eady fracture healing.
8.Effect of intraoperative amino acid infusion on glucose metabolism in dogs

Objective To investigate the effect of intraoperative amino acid infusion on perioperative glucose metabolism. Methods Thirty-six adult mongrel dogs of both sexes weighing 12-16 kg undergoing partially small intestine resection under general anesthesia were randomly allocated to one of 4 groups (n=9 each): Ⅰ control group received normal saline (C);Ⅱ,Ⅲ,Ⅳ amino acid group (A1, A2, A3) received iv infusion of 2.85%, 5.70% and 11.4% 18-amino acid respectively at 12 ml·kg-1·h-1 during operation starting from skin incision until the end of operation. The animals were premedicated with ketamine and diazepam. Anesthesia was induced with propofol 5-10 mg/kg, fentanyl 2 μg/kg and vecuronium 0.2 mg/kg and maintained with 1%-3% isoflurane and intermittent iv boluses of fentanyl and vecuronium. The animals were intubated and mechanically ventilated. PET CO2 was maintained at 30-40 mm Hg. ECG, MAP, HR, PET CO2 and esophageal T0 were continuously monitored. Venous blood samples were collected before anesthesia (T1), 15 min after induction of anesthesia (T2), 15, 30 min and 1 h after skin incision (T3-5), when abdomen was closed (T6) and 1,2,4,8 and 24 h after operation (T7-11) for determination of plasma glucose, lactate, insulin and glucagon. Liver biopsy was performed at T6-11 and muscle biopsy at T2,6,11 for measurement of hepatic and muscle glucagon. Homa index was used to estimate the degree of insulin resistance. Results The plasma glucose and insulin concentrations were significantly increased at T3-11 as compared with the baseline at T1 in all 4 groups (P<0.05). The plasma insulin concentrations were significantly higher in group A1 (at T6), group A2 (at T3,6) and group A3 (at T3-11) than in group C (P<0.05). Homa index was significantly higher in group A3(at T3-8) than in group C. Conclusion Intraoperative amino acid infusion increases plasma insulin concentration but does not prevent glycogenolysis especially high dose amino acid infusion.
9.Clinical Characteristics of Spontaneous Bacterial Peritonitis(SBP) in Patients with Severe Type of Viral Hepatitis

OBJECTIVE To investigate clinical characteristics of spontaneous bacterial peritonitis(SBP) in patients with severe type of viral hepatitis.METHODS A retrospective review of 120 cases of SBP associated with severe type of viral hepatitis was performed.RESULTS Clinical manifestations were as follows:fever 85.0%,abdominal tenderness 51.6%. 63.3% of the total white blood cells count in the ascitic fluid under 0.5×109/L,85.0% of polymorphonuclear cell ratio above 0.50.The positive culture of the ascitic fluid 24.2%,whereas 42.9% of the isolated microorganisms were Escherichia coli.CONCLUSIONS Clinical manifestations in SBP may be non-specific or absent.The total white blood cells count in the ascitic fluid are often under criteria for diagnosis.E.coli was the most common culture isolate.PMN ratio is the reliable parameter for the diagnosis of SBP.
10.Changes of Serum Level of Leptin,Growth Hormone,Insulin Like Growth Factor-1 and Insulin Like Growth Factor Binding Protein-3 in Different Intrauterine Growth and Development Situation

Objective To explore the relationship between the serum level of leptin,growth hormone(GH),insulin like growth factor-1(IGF-1),insulin like growth factor binding protein-3(IGFBP-3)and those modulatine effects on fetal growth.Methods One hundred and thirty-five cases of newborns were divided into 4 groups according to the relationship between born weight and gestational age.Group A:premature infants small for gestational age(SGA),n=30.Group B:premature infants appropriate for gestational age(AGA),n=36.Group C:mature infants small for gestational age,n=32.Group D:mature infants appropriate for gestational age,n=37.All infants were measured se-rum leptin,GH,IGF-1 and IGFBP-3 level with radiommunosassay(RIA).Results There were significant difference in the levels of leptin,GH,IGF-1,IGFBP-3 as group C0.05).There were positive correlation between levels of leptin,IGF-1 and IGFBP-3 in group A,B and C,while,there was positive correlation between only level of IGF-1 in group D.Conclusions Leptin,IGF-1 and IGFBP-3 may play an important role in the fetal growth regulation of premature infants and IUGR infants.IGF-1 also regulate intrauterine growth of group D.Another side,in group D,Leptin,GH,IGF-1 and IGFBP-3 don't regulate fetal growth.
