Mononeuropathy multiplex is a rare disorder associated with idiopathic thrombocytopenic purpura.
Extrinsic compression due to hematoma, intraneural bleed and immune mediated nerve injury are reported
mechanisms of neuropathy. We report of a case of a girl with recurrent idiopathic thrombocytopenic
purpura with mononeuropathy multiplex, along with a brief review of the mechanism of neuropathies
in asscociation with idiopathic thrombocytopenic purpura.

Background:
The Watch Tower Society, the main governing organization of the Jehovah’s Witness (JW) faith, introduced the doctrine to refuse blood in 1945 and has been enforcing it since 1961. A member can be expelled for accepting prohibited blood components. Many reconstructive surgeries place patients at an increased risk for blood loss. There have been attempts at reducing the rate of transfusions in craniofacial surgery, even in patients not opposed to it.PresentationA 15-year-old female patient, who refused blood transfusion due to her faith, presented with a class III malocclusion, transverse maxillary constriction, and a lateral open bite. Surgery was deferred until the patient reached 18 years of age and had undergone preoperative orthodontics. A two-piece Le Fort I osteotomy was performed. Erythropoietin, oral iron, and tranexamic acid were used to minimize intraoperative blood loss.
Conclusion
Here we discuss preoperative and intraoperative management strategies to ensure a transfusion-free environment for patients with religious objections to blood transfusions undergoing operations with increased bleeding risk.
5.Evaluation of anti-apoptotic activity of different dietary antioxidants in renal cell carcinoma against hydrogen peroxide.

OBJECTIVETo evaluate the anti-apoptotic and radical scavenging activities of dietary phenolics, namely ascorbic acid,α-tocopherol acetate, citric acid, salicylic acid, and estimate H2O2-induced apoptosis in renal cell carcinoma cells.
METHODSThe intracellular antioxidant potency of antioxidants was investigated. H2O2-induced apoptosis in RCC-26 was assayed with the following parameters: cell viability (% apoptosis), nucleosomal damage and DNA fragmentation, bcl-2 levels and flow cytometery analysis (ROS production evaluation).
RESULTSThe anticancer properties of antioxidants such as ascorbic acid, α-tocopherol acetate, citric acid, salicylic acid with perdurable responses were investigated. It was observed that these antioxidants had protective effect (anti-apoptotic activity) against hydrogen peroxide (H2O2) in renal cell carcinoma (RCC-26) cell line.
CONCLUSIONSThis study reveals and proves the anticancer properties. However, in cancer cell lines anti-apoptotic activity can indirectly reflect the cancer promoter activity through radicals scavenging, and significantly protect nucleus and bcl-2.

Carotid body tumor excision can lead to various complications including vascular injury and pseudoaneurysm formation. Here we describe a case of carotid body tumor excision followed by series of complications including pseudoaneurysm formation, failure of primary surgical repair, carotid stump syndrome following parent artery occlusion, and persistent hypotension.
7.Occupational Nerve Injuries due to Metallic Foreign Bodies: A Case Series of Eighteen Patients

Introduction: Peripheral nerve injuries (PNIs) remain an
important health problem. PNIs mostly affect young men as
this age group is mostly involved in road traffic accidents
and other injuries at workplace. PNI can occur from foreign
bodies like metal chips while working in industries using
lathe machines. Among PNI’s, injuries to the ulnar nerve, the
brachial plexus and the median nerve are the most frequent
lesions encountered.
Materials and methods: This presentation is on a series of
18 cases of nerve injuries among industrial workers located
from finger level up to the arm excluding the brachial plexus
due to metallic foreign bodies entering while operating lathe
machines over a period of two years with patients being
followed-up over a one year period.
Results: Mean age in this series was 31.3 years with age
range 16-40 years and all were males. Two patients had more
than one nerve involvement and one patient had associated
vascular injury. All the patients showed functional
improvement. Most common nerve injured was median
nerve. Most common site for nerve injury was forearm.
Combined lesions most commonly involved the ulnar and
median nerves.
Conclusion: Social cost of traumatic peripheral nerve
injuries is significant since it has a higher incidence in
young, previously healthy, and economically active
people.

The coronavirus disease 2019 (COVID-19) pandemic has led to an association between COVID-19 and pediatric diabetes. Studies have indicated the increased likelihood of children with COVID-19 infection developing diabetes. Our objective was to assess not only the increase in pediatric diabetes at our hospital and identify possible risk factors, but also to correlate the psychosocial changes resulting from the pandemic with new-onset diabetes.
Methods:
We analyzed data from 58 children aged 1 to 18 years admitted to our hospital with new-onset diabetes between March 2020 and December 2021. The data included inflammatory biomarkers and severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) antibodies (Abs), as well as the results of a lifestyle questionnaire.
Results:
The average number of hospital admissions per month for new-onset diabetes increased from 10 to 18 with the start of the pandemic. Of the 58 children in our analysis, 33% had positive SARS-CoV-2 IgG Ab, 31% had type 1 diabetes mellitus, and 62% had type 2 diabetes mellitus (T2DM). More than half (54%) were experiencing diabetic ketoacidosis. Those with T2DM were older, majority African American, had higher median body mass index (BMI) percentiles, and lower vitamin D levels. There were no significant correlations between any psychosocial risk factors and either diabetes type or SARS-CoV2 Ab status.
Conclusion
Despite the increased incidence of new-onset diabetes among children in Mississippi during the pandemic, this study was unable to demonstrate a significant correlation between COVID-19 infection and new-onset diabetes. The findings of this study highlighted the correlation between increased BMI and type 2 diabetes, underscoring the significant problems of obesity and diabetes in our study region. Further research is warranted.
