1.Simultaneous determination of eight constituents in Xuebijing Injection by UPLC-MS/MS

AIM To establish an ultra performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) method for the simultaneous content determination of gallic acid,matrine,danshensu,hydroxysafflor yellow A,caffeic acid,paeoniflorin,ferulic acid and cynaroside in Xuebijing Injection (Carthami Flos,Paeoniae Radix Rubra,Chuanxiong Rhizoma,etc.).METHODS The analysis of 5% acetonitrile extract of this drug was performed on a 40 ℃ thermostatic ACQUITY UPLC(C) BEH C18 column (2.1 mm × 50 mm,1.8 μm),with the mobile phase comprising of acetonitrile-0.1% formic acid flowing at 0.2 mL/min in a gradient elution manner,after which cluster analysis was applied to the determined contents.RESULTS Eight constituents showed good linear relationships within their own ranges (r > 0.996 0),whose average recoveries were 95%-104% with the RSDs of less than 3%.Generally the contents of various constituents in ten batches of samples were found to be stable except the certain differences contributed by gallic acid and paeoniflorin.CONCLUSION We should pay attention to the quality of different batches of Xuebijing Injection due to their uneven performance.
2.Screening Tools Based on Nomogram for Diabetic Kidney Diseases in Chinese Type 2 Diabetes Mellitus Patients

Background:
The influencing factors of diabetic kidney disease (DKD) in Chinese patients with type 2 diabetes mellitus (T2DM) were explored to develop and validate a DKD diagnostic tool based on nomogram approach for patients with T2DM.
Methods:
A total of 2,163 in-hospital patients with diabetes diagnosed from March 2015 to March 2017 were enrolled. Specified logistic regression models were used to screen the factors and establish four different diagnostic tools based on nomogram according to the final included variables. Discrimination and calibration were used to assess the performance of screening tools.
Results:
Among the 2,163 participants with diabetes (1,227 men and 949 women), 313 patients (194 men and 120 women) were diagnosed with DKD. Four different screening equations (full model, laboratory-based model 1 [LBM1], laboratory-based model 2 [LBM2], and simplified model) showed good discriminations and calibrations. The C-indexes were 0.8450 (95% confidence interval [CI], 0.8202 to 0.8690) for full model, 0.8149 (95% CI, 0.7892 to 0.8405) for LBM1, 0.8171 (95% CI, 0.7912 to 0.8430) for LBM2, and 0.8083 (95% CI, 0.7824 to 0.8342) for simplified model. According to Hosmer-Lemeshow goodness-of-fit test, good agreement between the predicted and observed DKD events in patients with diabetes was observed for full model (χ2=3.2756, P=0.9159), LBM1 (χ2=7.749, P=0.4584), LBM2 (χ2=10.023, P=0.2634), and simplified model (χ2=12.294, P=0.1387).
Conclusion
LBM1, LBM2, and simplified model exhibited excellent predictive performance and availability and could be recommended for screening DKD cases among Chinese patients with diabetes.

4.Document Visualization Analysis of Inhalation Therapy for Respiratory Diseases

OBJECTIVE:To investigate the research status and hotspots of inhalation therapy for respiratory diseases in the world,and to provide reference for further development of the filed in China. METHODS :All literatures about inhalation therapy for respiratory disease were retrieved from Web of Science database from the inception to Jan. 1st,2020. The retrieval result analysis function of Web of Science combined with visual analysis function of CiteSpace 5.1.R6SE software were used to analyze included literatures from respective of annual number of articles published ,countries,institutions,categories and co-cited authors , so as to analyze the research situation in this field. The knowledge map of key words was drawn ,then cluster analysis and burst detection were carried out ,and the research hotspot and development process were analyzed. RESULTS :A total of 1 385 literatures were included. From 1947 to 2020,the overall trend of annual number of articles published was on the rise. There were 66 countries involved in relevant research ,the United States was in the leading position (the number of articles published accounting for 33.79% of total articles published ),followed by Germany and UK (the number of articles published accounting for 10.32%, 8.88% of total articles published ),while Chinese number of articles published accounting for only 3.75% of total articles published. Institutions involved in this research field included the respiratory diseases research institution ,comprehensive universities and national institute for occupational safety health ;top ranked institutions were Sydney University ,North Carolina University, Harvard University , etc. The subject category involved molecular biology , internal medicine , pharmacology, engineering and other fields. A total of 749 authors were co-cited ,and the co-cited frequency of top two authors were 109,84 times respectively. Through the cluster analysis ,6 major researches hotspots were obtained in this field ,including the dosage form and technology design of inhaled drugs ,the mechanism of inhaled drugs ,the application of inhalation therapy for the obstructive diseases of respiratory system ,the application of inhalation therapy for the infectious diseases of respiratory system ,the application of inhalation therapy for the special population of respiratory diseases,and the risk factors in the application of inhalation (No.2017YFC0909900) therapy for respiratory diseases. Through the sudden detection , it was found that in the early 1990s,the research in this field was very active ,and there were many research hotspots ;in the early 21st century , there were several new research hotspots for a short time ,such as metered-dose inhaler ,aerosol,etc.;till the 2020s,there were many research hotspots ,such as nanoparticles,human trials ,etc. CONCLUSIONS :Researches about inhalation therapy for the respiratory diseases are steadily increasing internationally ,while the researches in this field in China is still very limited. In the future ,our country can pay attention to the international research hotspot and direction in this field ,and further strengthen the exchanges and cooperation with other countries.
5.Effects of CYP1A2 Gene Polymorphisms on Blood Concentrations of Antipsychotic Drugs :A Meta-analysis

OBJECTIVE:To systematically evaluate the effects of CYP1A2 gene polymorphisms on blood concentrations of antipsychotic drugs (haloperidol,clozapine,olanzapine),and to provide evidence-based reference for the clinical drug use . METHODS:Retrieved from Cochrane Library ,PubMed,Embase,CBM,CNKI and Wanfang data ,during the inception to Nov. 2019,cross-sectional study was conducted to investigate the effects of CYP1A2 gene polymorphisms on blood concentrations of antipsychotic drugs (haloperidol,clozapine,olanzapine)were collected. After screening the literature ,extracting the data and quality evaluation with Q-Genie tool ,Meta-analysis was performed by using Rev Man 5.3 software. RESULTS :A total of 11 cross-sectional studies were included ,with a total of 914 patients. Of these ,haloperidol was used in 2 studies,clozapine was used in 5 studies,and olanzapine was used in 4 studies. Meta-analysis showed that there was no statistically significant difference in the blood concentration of haloperidol between CYP1A2(-2964G>A)G/G type and G/A+A/A type [SMD =-0.22,95%CI(-0.66, 0.23),P=0.35]. The blood concentration of clozapine in CYP1A2(-163C>A)A/C type was significantly lower than C/C type [SMD =0.31,95%CI(0.01,0.62),P=0.04];there was no statistical significance in blood concentration of clozapine between A/A type and C/C type [SMD =0.09,95%CI(-0.21,0.40),P=0.56],between A/A type and A/C type [SMD =-0.22,95%CI(-0.55, 0.10),P=0.18],between CYP1A2(-2467delT)delT/delT type and T/T type [SMD =-0.11,95%CI(-0.75,0.52),P=0.72], between delT/T type and T/T type [SMD =0.01,95%CI(-0.33,0.34),P=0.97],between delT/delT type and delT/T type [SMD = -0.15,95%CI(-0.80,0.05),P=0.66]. The blood concentration of olanzapine in CYP1A2(-163C>A)A/A type was signifi- cantly lower than A/C type [SMD =-0.31,95%CI(-0.55, - 0.08),P=0.009];there was no statistically significant difference in the blood concentration of olanzapine between A/A liukefeng-num.1@163.com type and C/C type [SMD =-0.20,95%CI(-0.61,0.21),P= 0.34],between A/C type and C/C type [SMD =0.06,95%CI E-mail:hnmuzj@163.com (-0.35,0.47),P=0.77],between CYP1A2(-2467delT)delT/T type and T/T type [SMD =0.28,95%CI(-0.15,0.71),P=0.20]. CONCLUSIONS:CYP1A2(-163C>A)A/C type is related to the reduction of clozapine blood concentration ,and A/A type is related to the reduction of olanzapine blood concentration. CYP1A2 (-163C>A)gene polymorphism is significance for guiding individualized medication of schizophrenia patients.
6.Research hotspots and trends on potentially inappropriate medication from 2012 to 2021

OBJECTIVE To quantitatively and qualitatively analyze the progress of potentially inappropriate medication (PIM) research by means of bibliometrics, and to predict the hotspots and trends of PIM, so as to provide reference for PIM research in China. METHODS The research literature related to PIM were searched from Web of Science core database, and the publication trend of the literature was analyzed on the bibliometric online analysis platform and CiteSpace 5.8.R3 software. The research results were visualized and analyzed, and the research hotspots of PIM were summarized through gCLUTO. RESULTS About PIM research, the annual number of the literature issued from 2012 to 2021 showed a continuous growth trend totally; the United States had the largest number of literature (241 pieces); the University of Sydney in Australia was the research institution with the largest number of literature (59 pieces); O’Mahony D from University College Cork, Ireland, published the most literature (23 pieces); the research of PIM involved 57 disciplines, among which there was the largest number of literature about gerontology (384 pieces); International Journal of Clinical Pharmacy was the journal with the largest number of literature (61 pieces); Journal of the American Geriatrics Society was cited most frequently (1 197 times). The hotspots of PIM research mainly focused on seven aspects: the standard of PIM, the safety and economy research of PIM, the risk factors research of PIM, the PIM research of chronic patients, the PIM research of antipsychotics, the research of pharmacists’ participation in PIM management, and the PIM research about palliative care of tumor patients. CONCLUSIONS The heat of research on PIM has been on the rise, and there is little cooperation between countries and institutions, and more cooperation is needed. In the future, the focus of Chinese pharmacists’ research on PIM may be on the deep localization of PIM standards, PIM intervention and precise medication guidance, and PIM management of specific diseases and drugs.
7.Establishment of Evaluation Mode of Pharmacist Post Value in Dispensing Department in Hospital Based on Factor Counting Method

OBJECTIVE:To establis h the evaluation model of hospital pharmacist post value of dispensing department ,and to provide scientific basis for optimizing the performance management of pharmacists in despensing department in hospital. METHODS:Based on factor counting method ,combined with questionnaire ,Delphi expert correspondence method and so on ,the hospital pharmacist post value evaluation model was established. Based on this ,the relative value scores of the pharmacists in the dispensing department was determined. RESULTS :Hospital pharmacist post value evaluation model was established with 5 evaluation dimensions (knowledge,skills,responsibility,work autonomy ,work environment and intensity ,of which the first level weight coefficients were 0.194,0.166,0.365,0.216 and 0.058,respectively)and 31 evaluation elements. The above 5 evaluation dimensions contained 5,8,9,3,6 evaluation elements ,respectively,and the corresponding second-level weights were 0.052-0.431 (for example ,responsibility dimension with the highest weight ,including quality and safety control ,decision responsibility,responsibility for finance and assets ,the corresponding second-level weight coefficients were 0.279,0.140,0.132, respectively). The relative value scores of 13 pharmacist post were 342.9-840.4 in the dispensing department of our hospital with this model ,among which the department responsible person had the highest score (840.4)and the prescription post score was the lowest(342.9). CONCLUSIONS :The hospital pharmacist post value evaluation model constructed by factor counting method can provide a scientific and reliable theoretical basis for realizing the rewards of different positions in the dispensing department.
8.Analysis of the situation ,influential factors and their relationship of hospital pharmacy managers ’servant leadership behavior and hospital pharmacists ’job satisfaction

OBJECTIVE To investigate the situation ,influential factors and their re lationship of hospital pharmacy managers ’ servant leadership behavior and hospital pharmacists ’job satisfaction. METHODS The questionnaire survey method was adopted to stratified cluster sampling from primary ,secondary and tertiary hospitals ,five for each in Henan province. The personal basic data scale of pharmacists ,the hospital pharmaceutical service leadership behavior scale and the job satisfaction scale of pharmacists were used to conduct a questionnaire survey among hospital pharmacists. Excel 2019 and SPSS 23.0 software were used for statistical analysis. RESULTS A total of 956 questionnaires were distributed and 882 questionnaires were recovered ,including 841 valid questionnaires,with an effective recovery rate of 95.35%. The reliability coefficients Cronbach’s α of hospital pharmacy managers ’ servant leadership behavior scale and hospital pharmacists ’job satisfaction scale were 0.986 and 0.978,and the validity coefficients KMO were 0.908 and 0.977(P<0.01). The total score of hospital pharmacy managers ’servant leadership behavior was (110.73± 18.63). The total score of hospital pharmacists ’job satisfaction was (126.33±17.79). Hospital grade ,gender,age,professional title and highest education level all affected pharmacists ’recognition for managers ’servant leadership behavior (P<0.05). Hospital grade,age,professional title ,marital status ,highest education level and position all affected job satisfaction (P<0.05). The servant leadership behavior of hospital pharmacy managers was positively correlated with the job satisfaction of hospital pharmacists (correlation coefficient r was 0.521-0.698,all P<0.01). CONCLUSIONS The promotion and optimization the servant leadership behavior of hospital pharmacy managers can improve the job satisfaction of pharmacists ,stabilize the team of pharmacists ,and provide high-quality pharmaceutical care for patients ,so as to improve the core competitiveness of the hospital.
9.Application of parameter assignment method in the review of parenteral nutrition prescription in our hospital

OBJECTIVE To provide reference for improving the working efficiency of prescription review for pharmacists and ensuring the safe use of parenteral nutrition solution in patients. METHODS The review module of parenteral nutrition prescription was developed with the pharmacy intravenous admixture services(PIVAS) MATE system. According to the instructions and relevant information of parenteral nutrition solution, the parameters to the review module for parenteral nutrition solution prescriptions were assigned, thresholds and calculation formulas were set, and the prescription was automatically determined by the system. The type and quantity of the inappropriate prescriptions of parenteral nutrition solution in our hospital from January to August 2020 and from January to August 2022 were counted, and the review efficiency of inappropriate prescriptions was compared before and after the application of the review module of parenteral nutrition solution prescription. RESULTS The module realized the informatized review of basic attribute parameters and custom parameters of parenteral nutrition solution prescription, such as ion concentration, ratio of heat to nitrogen, ratio of sugar to lipid, glucose concentration, ratio of sugar to insulin, maximum dosage of compound preparation, necessary drug collocation, incompatibility, repeated drug use, age limit, etc. The module also shortened the review time greatly. It took about 33 minutes for two pharmacists to review 100 prescriptions, and only took 6 seconds after the module went online. Compared with January to August in 2020, the average approval rate of inappropriate prescriptions for parenteral nutrition solution increased from 0.582% to 1.416% during the corresponding period in 2022 (P<0.05). CONCLUSIONS The review module of parenteral nutrition solution prescription established by parameter assignment method in our hospital can greatly reduce the phenomenon of missed and wrong review by pharmacists, reduce the risk of parenteral nutrition solution dispensing, reduces the work pressure of pharmacists reviewing prescriptions, and ensures the drug safety of patients.
10.Meta-analysis of efficacy and safety of oral JAK inhibitor for alopecia areata

OBJECTIVE To systematically evaluate the efficacy and safety of oral Janus kinase (JAK) inhibitor in the treatment of alopecia areata (AA)in order to provide evidence -based reference for clinical use . METHODS PubMed,Embase, Web of Science ,the Cochrane Library ,CNKI,Wanfang and CBM were searched from the inception to March 29,2022. Randomized controlled trials (RCTs)of oral JAK inhibitors (trial group )versus placebo (control group )in the treatment of AA were collected . Two researchers independently screened the literature ,extracted the data ,and evaluated the quality of the included studies. RevMan 5.4 software was used for meta -analysis and analysis of publication bias . RESULTS A total of 2 170 patients were enrolled in 5 RCTs,including 1 619 in the trial group and 551 in the control group . Meta-analysis results showed that :compared with control group ,the ratio of the patients with the Severity Alopecia Tool (SALT)score ≤20 at 24th week [RR=6.10,95%CI (3.86,9.63),P<0.000 01] and 36th week [RR=6.59,95%CI(4.16,10.43),P<0.000 01] were both higher ;Hospital Anxiety and Depression Scale -Anxiety (HADS-A) score [RR=0.35, 95%CI (0.07,0.64),P=0.02] and Hospital Anxiety and 1009) Depression Scale -Depression (HADS-D) score [RR=0.50, 95%CI(0.22,0.77),P=0.000 4] were more decreased at 24th week . HADS-A score [RR=0.55,95%CI(0.21,0.89), 话:0371-66913047。 P=0.001] and HADS -D score [RR=0.50, 95%CI (0.16,0.84),P=0.004] were also significantly decreased at 36th week . The incidence of acne [RR=4.07,95%CI(1.83,9.08),P=0.000 6] and low density lipoprotein (LDL)elevation [RR=1.66,95%CI(1.21,2.28),P=0.002] in trial group were incr eased significantly (P<0.05). There were no significant differences in the incidence of urinary tract infection,upper respiratory tract infection , headache,nasopharyngitis and creatine phosphokinase elevation between 2 groups (P>0.05). There was little possibility of publication bias in this study based on the publication bias analysis . CONCLUSIONS Oral JAK inhibitor can significantly improve AA patients ’hair regrowth ,anxiety and depression . Acne and LDL elevation are the main adverse events .
