We should develop the activity——"Propagate the spirit of Bai Qiu' en and set up a fine image for hospital",and study frem him who had never thought about himself but always Solicituded others.He was very responsible for his work and dedicated all himself to people,and constanthy improved his skill.Studying Bai Qiu' en' s spirit in today has very important sense.Because it can help us settle the problems for medical personnel appearing in thought,style and work,help to depen the reform in sanitation,and promote the develepment of hospitals from all facets.We should insist on "Three beginnings" and accomplish "Four integrations".And we must actively develop the activity——"Propagate the spirit of Bai Qiu' en and set up fine images for hospital",make efforts to improve the service attitude,enhance the service guality and settle the popular problems which peoper reflect fiercely.We should also always salisfy people' s increasing demand for health,to health,to make satisfying hospitals which people need.
2.Effect of rehabilitation on ADL scores in patients with type 2 diabetes complicated with cerebral stroke

Objective To observe the effect of rehabilitation on ADL scores in patients with type 2 diabetes complicated with cerebral stroke.Methods According to the selected order,three hundred and twenty type 2 diabetes patients complicated with cerebral stroke were numbered in Arabic numbers 1,2,3 and so on,the odd number patients were randomly enrolled into the observation group,the even number patients were assigned into the control group.The control group was only given corresponding clinical treatments.In addition to the same clinical treatments,rehabilitation training was given to the observation group at the same time.The ADL scores,change between the two groups were compared after three months.Results After treatment,the ADL score of the observation group was significantly improved,the difference was statistically significant (t =-24.9,P =0.000,P < 0.05).The ADL score of the control group was also improved,and the difference was statistically significant (t =-15.52,P =0.000,P < 0.05).The ADL score between the observation group and the control group had statistically significant difference (t =24.24,P =0.000,P < 0.05).Conclusion Type 2 diabetes patients complicated with cerebral stroke who are early given rehabilitation training actively,car significantly improve the function of hemiplegia limbs,improve the ADL scores and the quality of life.
3.The Relationship of Polymorphism of TNF-? and Type 2 Diabetes Mellitus in Elderly Patients

Objective To study the relationship of TNF-? G308A and G238A polymorphism with type 2 diabetes mellitus in elderly patients.Methods Blood specimens were collected from 60 elderly patients with type 2 diabetes mellitus and 30 normal elderly volunteers.Genomic DNA was extracted.The primers of PCR for the SNPs of G308A and G238A were optimized and the polymorphism of amplicons was analyzed by DNA sequencing.Results There was a significant difference of TNF-?-308 gene in DM patients compared with control group,while no difference of TNF-?-238.Conclusions The 308 G-A polymorphism of TNF-? promoter gene may be associated with type 2 diabetes mellitus in elderly patients.
4.Application of quality control circle program in post stroke depression at the recovery phase

Objective To explore the effect of Quality Control Circle(QCC) on patients in the recovery phase with post stroke depression at rehabilitation therapy.Methods 80 patients in the recovery phase with post stroke depression were collected.According to the digital table, they were randomly divided into control group and QCC group,40 cases in each group.All the patients got treatment as rehabilitation medical care routine, rehabilitation exercise and medical treatement.And patients of QCC group received extra QCC management.The activities of daily living(Barthel Index Scale),severity of depression(Hamilton Depression Scale HAMD),rehabilitation exercises compliance were evaluated in the two groups.Results After intervention, the scores of ADL,HAMD and rehabilitation exercises compliance in the control group were (33.50±6.22)points,(22.23±3.30)points,(2.70±1.32)points,respectively,which in the QCC group were (54.13±8.84)points,(14.75±4.14)points,(4.45±0.90)points,respectively.The scores of activities of daily living(ADL)(t=15.06,26.03,all P<0.01) and rehabilitation exercises compliance(t=1.78,12.2,all P<0.01) had been improved, which in the QCC group was more obvious(t=12.10,6.90,all P<0.01)).The depression scores in both two groups were decreased(t=13.35,25.78,all P<0.01),and the decrease in the QCC group was significantly higher than that in the control group(t=8.93,P<0.01).Conclusion QCC application in post stroke depression at the recovery phase can effectively reduce the degree of depression, improve patients' ADL and rehabilitation exercise compliance.
5.The clinical effects of insulin glargine combined with novonorm in treatment of patients with newly diagnosed type 2 diabetes

Objective To observe the clinical effects of insulin glargine combined with novonorm in treatment of patients with newly diagnosed type 2 diabetes. Methods 112 cases of newly diagnosed type 2 diabetes patients were randomly separated into observation group(56 cases)and control group(56 cases). Observation group was treated with insulin glargine combined with novonorm. Control group was treated with novolin30R. The levels of FPG,2hPG and HbA1c before and after treatment,the control time of blood sugar,amounts of insulin and incidence of low blood sugar were observed in both groups. Results Compared with pre-treatment, FPG,2hPG and HbA1 c were significantly decreased (all P <0.05) after treatment, but there were no significant differences in both two groups (P >0. 05). The control time of blood sugar, amounts of insulin and incidence of low blood sugar in observation group were significantly lower than those in control group (all P < 0.05). Conclusion Insulin glargine combined with novonorm in treatment of patients with newly diagnosed type 2 diabetes could effectively control blood sugar,shorten the control time of blood sugar,decrease amounts of insulin,and low incidence of low blood sugar.
6.Application Study of Oligochips Detecting HBV and Lamivudine Drug Resistance

Objective To establish a oligochip method for detection of HBV Lamivudine drug resistance and evaluate the clinical role of the chip system.Methods 388 HBV DNA positive sera from patients receiving lamivudine treatment and 559 chronic hepatitis B patients not receiving lamivudine treatment,and 359 sera from HBV DNA negative controls were assayed for HBV mutations utilizing oligonucleotide microarray.Meanwhile,these results were contrasted with Quantitative PCR and DNA sequencing method.Results The results of clinical evaluation shows that for the codons 528,552 and 555,the agreements between the microarray and sequencing data are 96.6%,98.5% and 100%,respectively.In the 559 samples,which were detected positive for HBV DNA by quantitative PCR,all but three weak positive samples were positive by the microarray,demonstrating an agreement of 99.7%.All the 359 HBsAg negative samples were shown to be negative for HBV DNA by the microarray method.Conclusions The HBV-Lamivudine oligochip is eligible to detecting wild type HBV and HBV lamivudine-mutants in patient's sera.It has great potential application of administration for lamivudine treatment in HBV patients.
7.Clinical effect observation of rehabilitation therapy for hemorrhagic in-farction patients

Objective To observe the effect of rehabilitation therapy on functional recovery in patients with hemorrhagic infarction. Methods A total of 35 patients with hemorrhagic infarction were chosen as the study group, while 35 pa-tients with pure large-area cerebral infarction were chosen as the control group. The training were began after both groups vital signs were stable and nervous system signs were not progressing. Fugl-Meyer Motor Assessment Scale (FMA) was used to assess the motor function and Modified Barthel Index (MBI) was used to assess the activity of daily living (ADL).The two groups were respectively assessed before the treatment and after one-month treatment, using the statistic software to analyze the differences. Results There was no significant difference between the control group and the study group pre-treatment, but the differences were significant after one-month treatment in two groups (P<0.01), and more significant in the hemorrhagic infarction group(P<0.05). Conclusion Rehabilitation therapy could significantly improve the motor function and the ADL of hemiplegic patients with hemorrhagic infarction, and it is more significant than the patients with pure large-area cerebral infarction.
8.Relationship between hepatitis B surface antigen, HBV DNA quantity and liver fibrosis severity.

OBJECTIVETo explore the correlation between serum hepatitis B surface antigen (HBsAg) level and hepatic tissue pathological staging in patients with chronic hepatitis B (CHB).
METHODSClinical data was collected from our hospital's records for 302 CHB patients with HBsAg-positive status for more than 6 months and who had undergone hepatic biopsy. The HBsAg level,HBV DNA level and other clinical data were measured using commercial diagnostic assays. Liver histology was scored using the GS staging system. Correlation between serum HBsAg quantity, HBV DNA quantity, stage of inflammation and degree of fibrosis was assessed statistically.
RESULTSThe correlation of serum HBsAg level and HBV DNA level was notable. The serum HBsAg level was a variable affecting hepatic tissue pathological stage significantly. Serum HBsAg level appeared to be a highly specific and sensitive diagnostic marker of hepatic fibrosis. As the severity of liver fibrosis increased, the quantitative levels of platelet (PLT), HBsAg and HBV DNA gradually decreased, and the APRI index gradually increased; there were significant differences between the groups (all P<0.001). Serum HBsAg and HBV DNA levels in patients with hepatitis B e antigen-positive (HBeAg(+)) status showed strong correlation (r=0.721, P<0.0001) by Spearman analysis. HBeAg(+) patients with moderate to severe fibrosis (S2-4) exhibited significantly lower serum HBsAg and HBV DNA levels compared with patients with no or mild fibrosis (S0-1; t=5.475 and 4.826, P<0.001). ROC analysis suggested that a serum HBsAg cutoff of 4.46 log 10 IU/mL (28 800 IU/mL) would provide a theoretical sensitivity of 76.3%, with theoretical specificity of 70.5% in HBeAg(+) CHB patients. A serum HBV DNA cutoff of 7.13 log 10 IU/mL (1.35*10(7) copies/mL) would provide a theoretical sensitivity of 71.1%, with theoretical specificity of 73.4% in HBeAg(+) CHB patients. Logistic regression analysis showed that the level of HBsAg was an independent prognostic factor of moderate to severe liver fibrosis, with alanine aminotransferase, aspartate aminotransferase, HBsAg, HBV DNA and PLT (P<0.001).
CONCLUSIONHBsAg and HBV DNA levels decrease gradually along with aggravation of liver fibrosis. The cutoff values of 28800 IU/mL for HBsAg and 1.35*10(7) copies/mL ofHBV DNA provide higher specificity and sensitivity for predicting the degree of liver fibrosis in HBeAg-positive CHB patients, and the former is an independent predictor of severe liver fibrosis.
