Objective To identify related anatomical parameters and the strongest risk predictor leading to patients with recurrent patellar instability. Methods 52 young patients with recurrent patellar instability that failed of conservative treatment were retrospec-tively evaluated by MRI.Measurements included parameters of patellar tracking, which was characterized by bisect offset index (BSO),trochlear congruence angle (CA)and patellar lateral tilt (PTA),and anatomical parameters of patellofemoral joint, which were characterized by the sulcus angle (SA),lateral trochlear inclination (LTI),trochlear facet asymmetry, trochlear depth,Insall-Salvati Ratio (ISR),Caton-Deschamps index (C-D)and tibial tuberosity-trochlear groove (TT-TG)distance.The collection data were analyzed including the Pearson's correlation and multiple stepwise regression analysis. Results The patellofemoral joint ma-lalignment was significantly correlated with LTI,TT-TG distance and ISR,but not or partly with the trochlear depth,SA,trochlear facet asymmetry and C-D.Linear regression models including LTI,TT-TG distance and ISR explained 0.58,0.47 and 0.43 of the va-riance in BSO,CA and PTA,respectively (P<0.001).And the standardized beta coefficient was largest for LTI,then were TT-TG distance and ISR.Conclusion At full extension of the knee, anatomical related factors measured on MRI explain some degree of re-current lateral patella shift and tilt, the strongest predictor among anatomical related factors that could lead to maltracking is LTI, and then are TT-TG distance and ISR.

BACKGROUND:Airway epithelial regeneration can effectively inhibit submucosal hyperblastosis and the occurrence of obliteration. Studies demonstrated that ventilation could accelerate the regeneration of airway epithelium. OBJECTIVE:To establish and improve an orthotopic tracheal transplantation model and to further observe the effects of ventilation on trachea in alogeneic mice. METHODS:C57BL/6 mouse's tracheal served as donor, and BALB/c mouse's tracheal as recipient. This experiment contained two groups. In the experimental group, the membranous part of trachea was longitudinaly dissected in two donors and sutured into an enlarged trachea, which was implanted in the recipient. In the control group, donor's trachea was implanted into the recipient in situ. Samples were obtained and detected at 28 days after surgery. RESULTS AND CONCLUSION: Hematoxylin-eosin staining results demonstrated that compared with the control group, wel-differentiated ciliated epithelium was visible in the epithelial lamina of tracheal lumen, accompanying a few non-ciliated single or stratified squamous epithelium, mild submucosal fibrosis and inflammatory cel infiltration. Morphological analysis revealed that ciliated epithelial proportion in the experimental group was higher than in the control group (P < 0.05). The ratio of lamina propria to the tracheal cartilage, submucous fibrous tissuearea and the degree of lymphocyte infiltration were lower in the experimental group than in the control group (P < 0.05). Immunohistochemical staining demonstrated that the transplanted tracheal epithelium in both groups was recipient epithelial phenotype. Results verified that a modified orthotopic tracheal transplantation model was successfuly established. The increased ventilation of the tracheal alografts can accelerate the differentiation of tracheal epithelium. The wel-differentiated airway epithelium inhibited the proliferation of fibroblast.

OBJECTIVETo evaluate the applicability of immune-related response criteria (irRC) in treating non-small cell lung cancer (NSCLC) by Chinese medicine (CM).
METHODSTotally 97 stage III a-IV NSCLC patients were predominantly treated with comprehensive CM. Curative effects were evaluated by three methods such as Response Evaluation Criteria in Solid Tumors (RECIST), Oncologic Curative Effect Evaluation Criteria of Chinese Medicine in Solid Tumor (draft, abbreviated as CM criteria), and irRC. The correspondency and consistency between irRC, RECIST and CM criteria were analyzed and compared. The objectivity of irRC in evaluating curative effect of Chinese medical treatment for NSCLC was assessed.
RESULTSThe correspondency rate of irRC to RECIST was 59. 79% with Kappa value of 0. 379 (U test, P <0. 01). The two criteria had certain correspondence, but with an unsatisfactory consistency. The correspondency rate of irRC to CM criteria rate was 83. 51% with Kappa value of 0.751 (U test, P <0. 01). The two criteria had good correspondence and consistency.
CONCLUSIONSCM criteria had good consistency with CM criteria in evaluating curative effect for Chinese medical treatment of advanced NSCLC. Its results could objectively reflect features and advantages of CM for treating advanced NSCLC.

BACKGROUND: Peri-intraventricular hemorrhage (PIVH) in preterm infants is one of the most important reasons for mortality and disability. Moreover, investigative exponents may bring supported data for incidence rate of PIVH and high-risk factors of preterm infants with PIVH.OBJECTIVE: To explore the incidence rate and analyze the high-risk factors of PIVH in preterm infants.DESIGN: Survey and analysis.SETTING: Tongji Hospital of Tongji Medical College, Huazhong University of Science and Technology; Beijing Obstetrics & Gynecology Hospital, Capital Medical Universit; Qinhuangdao Maternity and Infants' Hospital of Hebei Province.PARTICIPANTS: A total of 1 122 preterm infants of 26.3-36.8 gestational age were selected from Beijing Obstetrics & Gynecology Hospital, Qinhuangdao Maternity and Infants' Hospital from January 2002 to August 2005. All infants received ultrasonic examination on skull within 1 week. There were 594 boys and 528 girls, and the birth weight was 850-4 500 g.METHODS: All infants received ultrasonic examination on skull within 1 week. New Philips 5500 and GE Healthcare Logiq 400 ultrasonic diagnosis devices were provided by Philip Company, Dutch and GE Company, respectively.MAIN OUTCOME MEASURES: The incidence rate and related high-risk factors of PIVH.RESULTS: All 1 122 preterm infants were involved in the final analysis. Among 1 122 preterm infants, 619 cases (55.2%) had PIVH; especially, 110 had severe PIVH with the degree more than Ⅲ, which was accounted for 17.8%.High-risk factors were mainly low gestational age, low birth weight, mechanical ventilation, hypoglycemia, hypercapnia, hyperlactic acidemia, acidosis, hypoxia, abnormal blood coagulation, and so on. Antenatal corticosteroid could reduce the incidence rate of PIVH. However, there was no obvious effect on preterm infants of old gestational age.CONCLUSION: Routine intracranial ultrasonic examination is useful for the diagnosis of PIVH in preterm infants.
