3.Clinical features and efficacy of different treatments on 5 cases with Landau-Kleffner syndrome with poor prognosis

Objective To analyze the clinical features and efficacy of different treatments on 5 cases with Landau-Kleffner syndrome (LKS) with poor prognosis.Methods Five patients with LKS who had poor prognosis were enrolled in the Children's Hospital of Fudan University from September of 2008 to December of 2012.The clinical features and findings of video electroencephalograph (VEEG) of these patients were analyzed.In addition,the outcomes of treatments (antiepileptic drug,corticosteroids and ketogenic diet) on LKS who had poor prognosis were studied.The outcome of epilepsy was classified by using the Engel classification scheme and the outcome of electrical status epilepticus during sleep (ESES) was classified by the decrease in spike wave index (SWI).Results All of 5 patients (2 male and 3 female) had auditory agnosia and epileptic seizure.All patients had normal background activity and epileptiform discharges,and the location of epileptiform discharge was mainly from bilateral temporal;3 patients had ESES.Four out of the 5 patients had refractory epilepsy and the other one got seizure free with antiepileptic drug.Four patients received corticosteroids for 6 months.The aphasia disappeared in 1 patient,improved in 1 patient and sustained in the other 2 patients.Three patients who had ESES did not response to corticosteroids.Among 3 patients receiving ketogenic diet,1 patient was seizure free and the ESES disappeared;in another SWI was reduced;the rest one had a significant reduction in seizure.All the patients had improvement in their aphasia and cognition,however they still had cognitive disorder of various degrees.Conclusions The patients who had multiple seizure type or special seizure type may have poor outcome.Most of these patients have refractory epilepsy or persistent ESES.Corticosteroids can improve aphasia and cognition,but the persistence of ESES can make aphasia and cognition worse.Ketogenic diet can be used as an alternative treatment for refractory epilepsy or persistent ESES in LKS.
4.Observation of Therapeutic Effects of Fleabane in the Treatment of Non-arteritis Anterior Ischemic Optic Neuropathy

OBJECTIVE:To observe the therapeutic effect of fleabane in the treatment of non-arteritis anterior ischemic optic neuropathy(AION).METHODS:38non-arteritis AION patients were divided into two groups at random:group A and group B were treated with fleabane and salvia miltiorrhiza respectively.RESULTS:The eyesight of both groups has been im-proved in a different degree and the improvement of visual field of group A surpassed that of group B.CONCLUSION:Flea-bane has certain amelioration and protection effects in the treatment of non-arteritis AION.
5.Comparison of two fixed methods for closed distal tibial fractures

Objective To investigate the role of endoplasmic reticulum stress ( ERS) in asbestos-induced Alveolar epithelial cell apoptosis.Methods A549 cells were treated with asbestos to observe the expression of ERS proteins and apop-tosis gene by immunofluorescence staining and western blotting.Results Under asbestos exposure, the expression of Bip, GRP94, IRE-1α, BAX and BAK are up-regulated.There is a positive correlativity of the expression of proteins to the ex-posure time of asbestos.Conclusion Endoplasmic reticulum stress takes part in asbestos-induced apoptosis.
7.Installation, Construction Design and Infection Control of CSSD Equipment

Objective To study installation, construction design and infection control of CSSD (central sterile supply department)equipment. Methods The merits of advanced CSSD building layout was studied as reference. International advanced disinfection & sterilization equipment was used. The laminar flows of the air in sterile room were purified. All apparatus and articles were controlled by computers. The third zone was equipped with full-automatic cleaning disinfection & sterilization apparatus. Results The quality and index all meet the requirement in construction design and infection control. Conclusion The design meets modern disinfection standards and can control the infection effectively.
8.Single nucleotide polymorphisms of metabolic syndrome-related genes in primary open angle glaucoma

AIM: To analyze single nucleotide polymorphisms (SNP) of primary open angle glaucoma- and metabolic syndrome-related genes in primary open angle glaucoma (POAG), in order to elucidate the roles of metabolic syndrome as a risk factor in POAG progress.METHODS: SNP genotypes and alleles of interleukin-6 (IL- 6), IL- 6 receptor (IL- 6R), dopamine D2 receptor (DRD2), beta-fibrinogen (FGB), peroxisome proliferator-activated receptor-γ2 (PPARG), transforming growth factor-β1 (TGF-β1), E-selectin (E-Sel), apolipoprotein A-5 (APOA5), C-reactive protein (CRP), ectonueleotide pyrophosphatase/ phosphodiesterase 1 (ENPP1), hepatic lipase (LIPC), adiponectin (ADIPOQ), paraoxonase 1 (PON1) and serine protease inhibitor E (SERPINE1) genes in POAG (n= 37) and normal control (n=100) groups were measured with ABI Prism 7900HT Fluorescence Quantitative PCR and TaqMan SNP Genotyping fluorescence probe kit.RESULTS: Genotypes and allele frequencies of IL- 6R, IL- 6, FGB, CRP, ENPP1, LIPC, ADIPOQ, PON1, and SERPINE1 in total POAG group were significantly different compared to the control group. CONCLUSION: Metabolic syndrome as a risk factor for POAG may be associated with genotypes and allele frequencies of the related genes.The corresponding gene expression and function can affect POAG progress, including roles of SERPINE1 in extracellular matrix, ENPP1 in insulin inhibition, IL- 6 in endogenous neuroprotection, IL- 6, IL- 6R and E-Sel in autoimmune response, LIPC and FGB in blood hyperviscosity syndrome, ADIPOQ in NOS/NO production, PON1 in vascular endothelial protection.
9.Problems in quality standard research of new traditional Chinese medicine compound.

The new traditional Chinese medicine compound is the main part of the research of new drug of traditional Chinese medicine (TCM), and the new Chinese herbal compound reflects the characteristics of TCM theory. The new traditional Chinese medicine compound quality standard research is one of the main content of pharmaceutical research, and is also the focus of the new medicine pharmaceutical evaluation content. Although in recent years the research level of new traditional Chinese medicine compound has been greatly improved, but the author during the review found still some common problems existing in new traditional Chinese medicine compound quality standard research data, this paper analyzed the current quality standards for new traditional Chinese medicine compound and the problems existing in the research data, respectively from measurement of the content of index selection, determine the scope of the content, and the quality standard design concept, the paper expounds developers need to concern. The quality of new traditional Chinese medicine compound quality standard is not only itself can be solved, but quality standards is to ensure the key and important content of product quality, improving the quality of products cannot do without quality standards. With the development of science and technology, on the basis of quality by design under the guidance of the concept, new traditional Chinese medicine compound quality standard system will be more scientific, systematic and perfect.
