1.Cloning of nm23-H1 cDNA gene and construction of recombinant adenovirus vector

Objective:To clone human nm23-H1 c DNA gene and to recombine it into adenovirus vector.Methods:Nm23 -H1cDNA fragment was amplified from human liver by reverse transcription-polym erase chain reaction(RT-PCR) and cloned into pMD18-T plasmid. The gene was exa mined by sequencing. Then pAd-Shuttle-CMV vector containing nm23-H1 was const ructed. Results:The results showed that the gene fragment cloned in pMD18-T was coincident with the sequence of nm23-H1.A pAd-Shuttle -CMV v ector containing nm23-H1 gene with correct order was con firmed by Kpnl digestion. Conclusion:nm23-H1 may be cloned into recombined adenovirus vector pAd-Shuttle-CMV.
2.Application of 3-D ultrasound in bladder diseases

Objective To evaluate the clinical application of 3 D ultrasound in the diagnosis of bladder diseases. Methods In 21 patients with bladder disease,3 D images were studied with a Voluson 530D diagnostic system.At first,the two dimensional images of lesions were collected,then defined 3 D volumes of interest were stored,reconstructed and analyzed. Results 3 D ultrasound provided a clear stereoscopic view and depicted the appearance of the 21 bladder diseases,displayed the internal structures,and showed invasive depth within and outside the bladder wall and also metastasis of the adjacent organs. Conclusions 3 D ultrasound can clearly demonstrate the morphologic characteristics and internal structures of the bladder disease and provide much more reliable information for the diagnosis of diseases.
3.The role of trypsinogen activation peptide in the early diagnosis of severe acute pancreatitis and the prediction of outcome

Objective To explore the role of trypsinogen activation peptide( TAP)in the early di-agnosis of severe acute pancreatitis( AP)and the prediction of outcome. Methods Eighty-nine cases with pancreatitis were collected containing 45 cases of severe AP and 44 of mild AP. 32 cases of non-AP acute abdominal disease were chosen as control group. Blood of all patients was collected within 6h to measure TAP. ROC curve was used to analyze the results. Results The TAP was different among the three groups (p<0. 01);the TAP of patients with severe AP was the highest. ROC curve indicated that 2. 78 nmol/L was the best cutoff to define AP and non-AP;the sensitivity,specificity and accuracy was 88. 8%,100%and 91. 5%,respectively. 8. 55 nmol/L was used to define severe AP and mild AP;the sensitivity,speci-ficity and accuracy was 92. 9%,95. 2% and 88. 7%,respectively. 11. 20 nmol/L was used to predict the occurrence of complications;the sensitivity,specificity and accuracy was 75. 0%,90. 4% and 82. 2%,re-spectively. Conclusion The TAP can be used to diagnose AP,especially to identify severe AP. The TAP is also beneficial for predicting the outcome of patients with severe AP.
4.Meta analysis of choice of laparoscopic operation time for acute cholecystitis

Objective To assess the operation time of laparoscopic operation for acute cholecystitis by Meta analysis.Methods The methods of computer literature retrieval and manual search were used to collect the research on the timing of laparoscopic surgery for acute cholecystitis in recent years,and the random effects model was used to conduct the Meta analyze.The quality of the literature was evaluated,and then the RevMan5.0.21 software was used to calculate the RR value and 95%CI of the operation time,bleeding volume and postoperative complications.Results A total of 13 articles were included in the Meta analysis,9 in Chinese,4 in English,2535 for a total number of cases,1401 in the early operation group,1134 in the delayed operation group,and 10 papers for low risk of bias,3 papers for bias risk.Meta analysis of the random effects model showed that the operation time of the early operation group was significantly shorter than that of the delayed operation group(P=0.00,RR=-20.25,95%CI=-24.97~15.33),and the intraoperative blood loss was also significantly reduced(P=0.00,RR=-12.58,95%CI=-29.36~5.78),then the postoperative complications were also significantly reduced(P=0.00,RR=0.63,95%CI=0.47~0.84).Conclusion The use of laparoscopic operation in the treatment of acute cholecystitis in patients with 72h could not only shorten the operation time and reduce the amount of bleeding,but also had a significant effect on reducing postoperative complications.
5.Evaluation of MRI-staging and assessment of lymphatic metastasis in cervical cancer

Objective To contrast the accuracy rate of nuclear magnetic resonance imaging (MRI)-staging and the International Federation of Gynecology and Obstetrics (FIGO, 2009) clinical-staging, and evaluate the value of MRI in diagnosis of lymph node metastasis in cervical cancer. Methods The surgical pathology was used as golden standard, the accuracy rates of MRI-staging and FIGO-staging were compared in 86 patients of cervical cancer (surgical pathological staging≥Ⅰb). The lymph nodes with slightly hyperintense signal in diffusion-weighted magnetic resonance imaging (DWI) and with minor axis≥1 cm in T2WI-TRA (T2 weighted imaging-transverse section) were considered as metastatic lymph nodes, the characteristics of lymphatic metastasis diagnosed by MRI were analyzed, and the accuracy rate, the sensitivity, the specificity, the positive predictive value and the negative predictive value of MRI were evaluated. Results The accuracy rate of FIGO-staging was 80.2%(69/86), and the accuracy rate of MRI-staging was 83.7%(72/86), there was no significant difference between them (P > 0.05). The accuracy rate of lymphatic metastasis diagnosed by MRI was 94.2%(81/86), the sensitivity was 73.3%(11/15), the specificity was 98.6%(70/71), the positive predictive value was 91.7%(11/12), and the negative predictive value was 94.6%(70/74). The true positive metastatic lymph nodes in 11 cases were located in the external iliac nodes or common iliac lymph nodes, the average short/long diameter was 0.76. The forms of lymph nodes were as follows:quasi-circular (n=3), border irregularity (n=3), huge fusion form (n=4), and 1 with central necrosis area. One case of false positive metastatic lymph node was located in the right external iliac node, with the sharp of huge fusion form in T2WI/TRA, comminution in T2WI-axial thin slices, and long strip in T2WI/SAG. Conclusion The accuracy rates of MRI-staging and FIGO-staging were both higher, which can diagnose lymphatic metastasis relatively accurately when they are combined together.
6.Hemodynamic responses in dogs exposed to blast-burn combined injuries followed by immersion in seawater

Objective A dog model of blast-burn combined injuries followed by immersion in seawater was reproduced in dog to study hemodynamic responses and their mechanism. Methods The dogs which were exposed to blast injuries produced by primer explosion and 10% second degree burn were randomly divided into seawater immersion group and blast-burn only group. The dogs in immersion group were immersed in seawater under anesthesia for 4 hours. The hemodynamics of the dogs was monitored, and blood samples were collected to assay malondialdehyde (MDA), and cultured for bacteria. After the above procedures were completed, the pathological changes in the dogs′ hearts and lungs were observed. The dogs in blast-burn group underwent the same protocol in immersion group except immersion. Results The body temperature (T), cardiac index (CI), mean arterial pressure (MAP), and heart rate (HR) in dogs in the immersion group were remarkably decreased, while the mean pulmonary artery pressure (MPAAP), systemic vascular resistance index (SVRI), and pulmonary vascular resistance index (PVRI) were markedly increased compared with those in blast-burn group. These obvious hemo dynamic disorders occurred during 3h to 6h after the dogs leaving seawater. Plasma MDA level was negatively correlated with CI. The interval between positive blood calture for intestinal bacteria and injury was shorter in immersion group than that of blast-burn group. The gross and histopathological changes in dogs′ hearts and lungs in immersion group injuries were more severe in degree than in blast-burn group. Conclusions Seawater immersion markedly aggravates the hemodynamic disorders in dogs having had blast-burn combined injury. The changes became obvious during 3h to 6h after leaving seawater. The aggravating effect might be related with declination of body temperature, inflammatory response and lipid peroxidation.
7.The clinical manifestations of olivopontocerebellar atrophy and the diagnostic value of MR

Objective To investigate the clinical manifestations and MR imaging features of olivopontocerebellar atrophy(OPCA).Methods The clinical data and MR findings of 8 patients with OPCA (OPCA group) were retrospectively analyzed,and 75 cases of healthy physical examination people were chosen as control group (they were divided into 21-30 years group,31-40 years group,41-50 years group,51-60 years group,61-70 years group,and 15 cases in each group),all subjects under went pons measurement.Results All 8 patients with OPCA were sporadic cases.The cerebellar symptom,autonomic symptoms and extrapyramidal symptoms were common manifestations.The head MR imaging of OPCA presented with cerebellum and brain stem atrophy,cross sign was the characteristic appearance.Vertical diameter and anteroposterior diameter of the pons in OPCA group patients was (20.63 ± 2.16) and (16.74 ±4.37) mm,compared with control group [(27.14 ±1.73) and (22.28 ± 1.49) mm],there was significant difference (P < 0.01).In control group,vertical diameter and anteroposterior diameter of the pons maximum in 21-30 years group and 31-40 years group,increased with age,the value had been reduced,but there was no significant difference (P > 0.05).Conclusion MR is the first choice for the diagnosis of OPCA,combine with clinical manifestations may improve the diagnostic accuracy.
8.Research progress of trans-cinnamaldehyde pharmacological effects.

Trans-cinnamaldehyde, the main component of volatile oil from cassia twig or Cinnamomum cassia, which is a traditional Chinese herbal medicine. Trans-cinnamaldehyde is a kind olefine aldehyde of organic compounds and has many pharmacological properties, such as anti-inflammatory, anti-tumor, anti-bacterial, antidiabetic, anti-obesity, and neuroprotection etc. The compound has preventive and therapeutic effects on the nervous system, cardiovascular, cancer, diabetes and other diseases. Trans-cinnamaldehyde, as a preventive care of nature medicine, has great clinical and market potential. This paper gives a review about the pharmacological effects and mechanism of trans-cinnamaldehyde researched in the latest five years. We hope to provide some basic information for further research on trans-cinnamaldehyde.

9.Effect of tiotropium on urination disorder in benign prostatic hyperplasia patients with chronic obstructive pulmonary disease

Objective To study the effect of tiotropium on urination disorder in benign prostatic hyperplasia (BPH) patients with chronic obstructive pulmonary disease (COPD).Methods In our prospective pilot study,96 BPH patients with COPD patients were enrolled as the treatment group and another 25 similar cases as the control group:In the former group tiotropium was administered and the control group was not.The two groups were compared in terms of the score by the international Prostate Symptom Score(IPSS),the quality of life by QOL,maximum flow rate (Q-max),average flow rate (Q-ave),time to Q-max (TTQ-Max),prostate volume (PVR) and bladder voiding efficiency (BVE) after six months treatment.Results As compared to the control,after six months treatment,such indexes in the treatment group as IPSS (15.1 ± 4.1,16.3 ± 3.4 and 14.7 ± 3.1,P =0.864),QOL(3.9 ± 0.8,4.0± 0.8 and 4.0 ± 0.9,P =0.992),Q-Max(ml/s) (8.5 ± 2.9,10.9 ± 2.2 and 9.0 ± 2.4,P =0.214),Q-ave(ml/s) (3.9 ±1.2,5.0 ± 1.4 and 3.8 ± 0.9,P =0.054),TTQ-Max(s) (11.1 ± 5.6,11.2 ± 4.0 and 10.4 ± 5.1,P =0.424),PVR(mL)(56.8 ± 33.3,62.3 ± 30.5 and 57.4 ± 29.5,P =0.981),BVE(%) (75.6 ± 13.8,72.7 ± 10.5 and 74.3 ± 12.1,P =0.992).showed no significant differences.Conclusion Tiotropium does not adversely affect lower urinary tract functions in BPH patients with COPD.
10.The role of CagA on H.pylori-induced IL-8 production from gastric epithelial cells

AIM: To investigate the role of CagA and the effect of small interference RNA (siRNA) on the release of IL-8 in H. pylori-infected gastric epithelial cells. METHODS: siRNA were transferred into CagA positive strain H. pylori NCTC 11637 by using electroporation. The CagA positive strain NCTC 11637 and the CagA negative strain NCTC 11639 were co-cultured with gastric epithelial cells and the level of IL-8 in the supernatant was measure by ELISA. RT-PCR and Western blotting were performed to detect CagA expression. RESULTS: The level of IL-8 induced by CagA positive strain NCTC 11637 was higher than the level induced by CagA negative strain NCTC 11639 ( 1 200.00 ?32.51) ng/L vs (100.00?8.58) ng/L, P0.05). Meanwhile, CagA mRNA decreased significantly in siRNAⅢgroup. The levels of CagA mRNA at 6, 12, and 24 h after electroporation were 31.3% (0.270/0.861), 57.6% (0.496/0.861), and 73.9% (0.637/0.861) of the control levels, respectively. Inhibition rate by siRNAⅢ was 68.7%. The Western blotting result in siRNA Ⅲ group showed that the level of CagA protein degraded to 30.7% (0.4/1.3) at 12 h after electroporation.
