1.Clinical Observation of Vitamin B12 Combined with Mouse Nerve Growth Factor in the Treatment of Neuro-genic Tinnitus

OBJECTIVE:To observe the efficacy and safety of vitamin B12 combined with mouse nerve growth factor in the treatment of neurogenic tinnitus. METHODS:270 patients with neurogenic tinnitus were randomly divided into control group 1, control group 2 and observation group. Control group 1 was treated with Vitamin B12 injection 25 μg by intramuscular injection, once a day;control group 2 was treated with mouse nerve growth factor 18 μg adding into 2 ml water for injection for dissolution, once a day;observation group was treated with Vitamin B12 injection 25 μg(the usage was the same as control group 1)+mouse nerve growth factor 18 μg(the usage was the same as control group 2). 6 d was a treatment course,and it lasted 2 courses. Clinical efficacy,and sleep SPIEGEL score and incidence of adverse reactions before and after treatment in 3 groups were observed. RE-SULTS:Total effective rate in observation group was significantly higher than control group 1 and control group 2,the difference was statistically significant (P<0.05);however,there was no significant difference between control group 1 and control group 2 (P>0.05). After treatment,sleep SPIEGEL score in 3 groups was significantly higher than before,and observation group was high-er than control group 1 and control group 2,the difference was statistically significant(P<0.05);however,there was no signifi-cant difference between control group 1 and control group 2(P>0.05). There were no obvious adverse reactions during treatment. CONCLUSIONS:Vitamin B12 combined with mouse nerve growth factor has better efficacy than only vitamin B12 or mouse nerve growth factor in the treatment of neurogenic tinnitus,with good safety.
2.Comparison of Blood Platelet Counts between Children with Cerebral Palsy and Healthy Children

ObjectiveTo explore the relationship between the changes of the blood platelet counts (BPC) in children with cerebral palsy (CP) and pather mechanism of CP in traditional Chinese medicine.MethodsBPCs of CP children and healthy children were analyzed with statistical method.ResultsThe BPC of CP children was higher than that of the healthy children ( P<0.05).ConclusionThe BPC of CP children is higher than that of the healthy children, and this is consistent with the theory of traditional Chinese medicine about pathogenic mechanism of CP that CP is related to blood stasis.
4.The effect and correlation factors of segmental pulmonary veins isolation for paroxysmal and persistent atrial fibrillation

Objective To evaluate the effect of segmental pulmonary veins isolation for atrial fibrillation (AF) and investigate the possible factors that may affect its result. Methods In 120 (105 men; age 50.0?8.6 years) consecutive patients with paroxysmal (n=99) or persistent (n=21) AF, segmental PVs electrical isolation with Lasso mapping catheters was performed. The relationship of the outcome of the initial procedure and factors such as age, sex, type of AF, left atrial diameter, case history, left ventricular ejection fraction and presence of hypertension were analyzed by statistical methods. Results 52 (52.5%) out of the 99 patients with paroxysmal AF and 6 (28.5%) out of the 21 with persistent AF were free of AF after a single ablation procedure. Univariate analysis revealed that left atrial enlargement (P=0.001), persistent AF (P=0.046) and old age (P=0.047) were related to the recurrence of AF after the first procedure. Patients with paroxysmal AF apparently had a better outcome than those with persistent AF after repeat ablation but of no statistical significance (P=0.094). Logistic regression analysis revealed that left atrial enlargement was the only independent risk factor of recurrent AF after the first procedure. Conclusion About 50% of the patients with paroxysmal AF are free of AF after single ablation. Left atrial enlargement is the only independent risk factor of recurrent AF while old age and persistent AF influence the outcome of the first procedure.
5.Both ischemic and pharmacological preconditioning decrease leukocyte/endothelial cell interactions in donor liver in rats

Objective To investigate the molecular mechanisms of delayed ischemic preconditioning (IPC) and doxorubicin preconditioning (DPC) to induce ischemic tolerance. Methods The models of sham-operation and orthotopic liver transplantation in the Wistar rats was used. IPC was administered with a 10-min ischemia followed by a 10-min reperfusion. Animals in the DPC group were pretreatd with Doxorubicin (1?mg/kg, iv). The control rats were subjected to saline treatment. The induction of HSP70 and HO-1 protein ( Western blot), the expression of ICAM-1 transcripts (RT-PCR)and ICAM-1 protein (immunohistochemistry), the activities of serum AST, ALT, LDH, and liver myeloperoxidase (MPO) , liver wet-to-dry weight ratios (W/D) were assessed.Results HO-1 expression was maximally induced at 12?h after IPC, and hardly changed until 48?h. A strong induction of HSP70 was detected at about 24 to 72?h after IPC. The levels of HO-1 and HSP70 were obviously elevated at 24?h after IPC or DPC as compared with the control ( P 0.05 ). In the control group, the transcription of ICAM-1 was significantly increased 6?h after reperfusion. Capillary endothelial cells of the livers strongly expressed ICAM-1. Activities of liver MPO were obviously elevated. IPC and DPC could significantly decrease the transcription of ICAM-1 in the livers in concurrence with the expression of ICAM-1 protein as well as the activity of MPO at 6?h after reperfusion ( P

Objective To investigate the relationship between the expression of Fas, FasL, Bcl-2 and Bax in transitional cell carcinoma of bladder and its clinical significance. Methods Immunohistochemistry method was used to detect the expression of Fas, FasL, Bcl-2 and Bax in 40 specimens of transitional cell carcinoma of bladder and 24 specimens of normal bladder mucosa tissue. Results The positive expression of Fas was detected both in transitional cell carcinonma of bladder and in normal bladder mucosa tissue, while FasL, Bcl-2 and Bax only in transitional cell carcinonma of bladder. OBJECTIVE To establish the isolation and identification system of human mesenchymal stem cells(hMSCs)in vitro, and then to establish a stable cultured system in which the hMSCs can be induced to osteoblasts, chondroblasts and lipoblasts in vitro. To study the possibility if hMSCs can be used as the seed cells in bone tissue engineering.This study would lay a foundation for the clinical application of tissue engineering. METHODS 5ml bone marrow of the patient was aspirated from iliac crest. The human MSCs were isolated using Percoll gradient centrifugation. The hMSCs were cultured in DMEM medium in vitro, FACS(fluorescence axtivated cell sorter)was used to identify the phenotype of hMSCs. The hMSCs were cultured in vitro, and induced to osteoblasts, chondrocytes and lipocytes. The changes of phenotype were tested by mmunohistochemisty and molecular biology technique. RESULTS The hMSCs were isolated by Percoll gradient centrifugation from patient’s bone marrow. The expression rate of MSC markers including CD105, CD166, CD29 were(78?6)%, (43?7)%, (69? 12)% respectively. The phenotype of osteoblasts induced from hMSCs was verified by the mineralized nodes formation when cultured in vitro and Col, and OCN expression showed by immunohistochemistryand RT-PCR. The phenotype of chondrocytes induced from hMSCs was verified by type, collagen, SOX9 expression with immunohistochemical staining technique and type, collagen, aggrecan mRNA expression with RT-PCR. The lipocytes’ phenotype was verified by positive result of oil red O staining and PPAR 2mRNA expression by RT-PCR. CONCLUSION The highly pure hMSCs can be harvested by means of density gradient centrifugation, and the hMSCs have multi-potentiality of cell differentiation via induced culture in vitro. The third passage of hMSCs can be induced to express osteoplast phenotype and meet the qualitative and quantitative demands of seed cells when bone tissue engineering was used in clinical practice.

Objective To explore the role of intravenous nutrition and enteral nutrition in the treatment of gastroplegia after gastrectomy. Methods The clinical data of 63 cases of gastroplegia after gastrectomy were retrospectively analyzed. Among the 63 cases, 30 were treated by gastrodynamic drugs and intravenous nutrition(intravenous nutrition group), and the other 33 cases were treated by gastrodynamic drugs and enteral nutrition(enteral nutrition group). Objective To explore the significance of blood lactic acid level and lactic acid clearance in evaluating the prognosis of septic shock.Methods The blood lactic acid concentration of 64 patients with septic shock EICU ICU admission 1h inside,6,12,24 and 48h was recorded,and the lactic acid clearance was calculated 12 hours after treatment,and according to lactic acid clearance of outcome,64 patients were divided into two groups,live group and death group;Then compared blood lactic acid level and lactic acid clearance between the two groups.Results The blood lactic acid concentration of live group and death group when enter the ICU was (7.2 ±5.4)mmol/L,(9.2 ±5.6)mmol/L respectivly,and was higher than normal,and of live group after 6h began to fall,and it reached normal level after 24h [ (2.2 ± 1.3 ) mmol/L ],but of death group after 48 h [ ( 8.0 ± 3.8 ) mmol/L ] was still significantly higher than normal,until death.There was significant difference between the two groups (t =3.7816,3.8257,7.5263,4.5875,5.8765,all P < 0.05 ).The lactic acid clearance of death group after 12 hours was (5.83 ±3.92)mmol/L,and significantly lower than that of live group[ ( 14.32 ±2.68) mmol/L] (t =6.2565,P <0.05).Conclusion Blood lactic acid concentration was positively related to the prognosis of patients with septic shock,and dynamic response of disease progression by detecting lactic acid clearance rate could be early,sensitive,quantitative indicators to determine the severity of critical illness.
