1.Theoretical research and clinical application of anterior cervical plating system

Anterior cervical plating has been widely used to treat cervical spine trauma, degeneration, deformity and tumor due to the advantages such as augmentation of cervical stability and promotion of bone graft fusion. This paper introduced the classification and evolution of plate-screw internal fixation system, analyzed the biomechanics of anterior cervical plate, and discussed its clinical application. Although the therapeutic effect of anterior cervical plating is favorable, there are still various complications postoperatively. Therefore, it is necessary to improve the anterior cervical plate from different aspects to expand its clinical application.
2.The Current Status of Adult Overweight and Obesity in Urban and Suburb areas of Tianjin

Objective To describe the current status of adult overweight and obesity in urban and suburb areas of Tianjin.Methods 2 283 adult people including 1 131 from urban and 1 152 from suburb were selected by multi-stage cluster random sampling.The waist circumference,weight and height of the subjects were measured.Then the BMI was calculated.Results The mean BMI of adult people in Tianjin was 25.05?5.02 kg/m2,the overweight rate was 36.9%,the obesity rate was 20.0% and the abdominal obesity rate was 63.9%.The prevalent rates of overweight,obesity and abdominal obesity in different age groups showed significant differences.The rates of overweight,obesity and abdominal obesity increased with ages in urban area.The overweight rate in 40~49 yrs and the obesity rate in 50~59 yrs reached their peak values in suburb.Besides the overweight rate in urban was significantly higher,the obesity rate and abdominal obesity rate were no significant differences between urban and suburb.The prevalent rates of overweight,obesity and abdominal obesity showed no significant differences between the adult male and female in Tianjin,but the prevalent rates of obesity and abdominal obesity in female were significantly higher than those in male in the suburb.The prevalence of obesity in the middle-ages showed significant differences between different occupational population,the retired taking the highest while the workers taking the lowest value.The prevalence of obesity showed no significant differences between population with different education background and different income situation.Conclusion The rates of adult overweight and obesity in Tianjin were higher than those of all over the country in 2002,and the abdomen obesity reached to 63.9%.Intervention should be taken to change this situation as soon as possible.
3.Study on Relationship between Life Style and Chronic Diseases

Objective To analyze the relationship between life style and chronic diseases.Method 2 886 people were selected randomly from three districts of Tianjin and surveyed with a questionnaire on their healthy condition and physical examination.2 622 people were taken a diet survey by 3-days foods intakes weighing and 24-hours diet recall,then the relative risk factors between life style and coronary heart disease,cerebrovascular disease,diabetes,hypertension,overweight and obesity were analyzed.Result The standardized prevalence rates of coronary heart disease,cerebrevascular disease,diabetes,hypertension, overweight and obesity of residents were 9.24%,4.33%,6.74%,34.05%,and 42.62%,respectively.The more the intake of energy, the more the risk for people to suffer from overweight and obesity,OR value being 1.00-1.70.With intake of more than 20 g dietary fiber per day,the risk for suffering from overweight and obesity could be reduced,OR value being 0.68.Logistic analysis showed that heavy salty taste could increase the risk for suffering from hypertension by 42%.Watching TV more than 3 hours per day increased the risk for suffering from cerebrovascular disease and diabetes,OR value being 1.802 and 1.703 respectively.Sleep less than 7 hours per day was a dangerous factor for hypertension,coronary heart disease and cerebrovasular disease,OR value being 1.517, 1.833 and 2.041 respectively.Conclusion The life style such as dietary pattern,sedentary occupation and sleep hours have strong connection with chronic diseases,such as hypertension,diabetes etc.Reasonable diet,suitable exercises,reducing sedentary position and increasing sleep hours will be good for health,and also for the prevention and control of chronic diseases.
4.Expression of CD95 on circulating endothelial cells in patients with hemorrhagic fever with renal syndrome

Objective To investigate the dynamic expressions and clinical significance of CD141, CD31 and CD95 on circulating endothelial cells (CEC) in febrile and polyuria phases of patients with hemorrhagic fever with renal syndrome (HFRS). Methods Expressions of CD141, CD31 and CD95 in the peripheral blood of patients with HFRS in febrile and polyuria phases were detected by flow cytometry. Comparisons among groups were done by one-factor analysis of variance. Results The percentages of CD141+ CD31+ cells in the peripheral blood cells from patients with HFRS in febrile and polyuria phases were 9.47% ±1.98 % and 8. 26% ±1.55 %, respectively, which were both higher than that (7.05%±1.45%) in healthy controls (F=8. 42; P=0. 000 and P=0. 029, respectively), and that in febrile phase was higher than that in polyuria phase (P = 0. 048). The mean fluorescent intensity (MFI) of CD95 on CEC of HFRS patients in febrile and polyuria phases were both significantly higher than that in healthy controls (F=19. 93; P=0. 000 and P=0. 000 respectively), and that in febrile phase was higher than that in polyuria phase (P=0. 049). In the febrile phase of HFRS,the MFI of CD95+ on CEC in patients with all clinical types were all higher than that in healthy controls (F= 17. 36; all P=0. 000), and that in severe (critical) type was the highest and higher than those in mild type and moderate type (P=0. 002 and P=0. 009, respectively). Conclusion The proportion of CEC and expression of CD95 on CEC are possibly related with the phase and severity of HFRS.

The cupping spot is considered as one kind of skin change due to cupping treatment. With literature regarding cupping spot, the influencing factors and value of cupping spot in clinical diagnosis and treatment were analyzed, which could make a further exploration on the action mechanism of cupping treatment. The literature showed that the formation of cupping spot was related with cupping temperature, pressure, cup-retaining time, cupping area, individual difference and health condition, etc; cupping spot had the ability to assist diagnosis, prevent disease, cure disease and evaluate clinical efficacy. Previous studies on cupping spot have already made some progress, and played a positive significance on finding cupping rule and studying its mechanism. However, the research for this area is still in the primary stage, which needed deeper study to reveal scientific connotations of cupping spot.

9.Protective Effect of Ulinastatin on Hydrogen Peroxide-induced Intestinal Epithelial Barrier Disruption

Background: Disruption of tight junctions between intestinal epithelial cells followed by loss of barrier function is crucial for the pathogenesis and progression of a variety of gastrointestinal disorders.Aims: To investigate the protective effect of ulinastatin on hydrogen peroxide (H2O2)-induced intestinal epithelial barrier disruption.Methods: Model of intestinal epithelial monolayer barrier was established with Caco-2 cells in vitro,and then divided into four groups: blank control group (without any intervention),H2O2 group (500 μmol/L H2O2),low-dose (500 U/mL) and high-dose (3 000 U/mL) ulinastatin groups (ulinastatin + H2O2).Level of malondialdehyde (MDA) and activity of superoxide dismutase (SOD) were detected;transepithelial electrical resistance (TEER) and flux of sodium fluorescein were measured to assess the barrier function;expression and localization of two tight junction proteins,ZO-1 and occludin were evaluated by Western blotting and immunofluorescence;ultrastructure of tight junctions was observed by transmission electron microscopy (TEM).Results: Compared with the blank control group,treatment of Caco-2 cell monolayers with H2O2 resulted in increase in level of MDA,flux of sodium fluorescein and decrease in activity of SOD,TEER and expressions of ZO-1 and occludin (P all <0.05).TEM and immunofluorescence showed that the brusher border of Caco-2 cells in H2O2 group was destroyed,the cell-cell junction was vague and the localization of ZO-1 and occludin was discontinuous and the fluorescence intensity was extremely low.While in ulinastatin groups,especially the high-dose group,all the indices above-mentioned were significantly improved (P all <0.05).Conclusions: Ulinastatin protects intestinal epithelial monolayer barrier against H2O2-induced disruption at least partially by its antioxidant activity and modulating expression and localization of tight junction proteins.
10.Surgical management of vertebral sarcoidosis.

OBJECTIVE:
To investigate the clinical characteristics, diagnostic foundation and treatment of vertebral sarcoidosis.
METHODS:
The clinical data of 13 patients with vertebral sarcoidosis who received anterior debridement and instrumentation were retrospectively analyzed.
RESULTS:
The onset of progressive pain in the pathological region was common in the 13 patients. Neurologic deficit existed in 4 cases. Radiographic study showed multiple vertebral bone destructions, and no other systemic lesions were found. Surgical indications were progressive vertebral destruction, spinal instability or neurological deficit. Anterior vertebra resection, and autologous bone grafts fusion with internal fixation were done. No operative mortality and major complications occurred. Diagnosis was confirmed in all patients by pathological exam. After the surgery, metacortandracin treatment was given routinely for 1 year. Patients were followed up for 12-52 (median 26) months, and pain and neurological symptoms were alleviated. Visual analog scale (VAS) score was 7-10 (median 8) points preoperatively, which dropped to 0-4 (median 2) points postoperatively. All patients showed successful bone fusion with no recurrence.
CONCLUSION
For vertebral sarcoidosis associated with progressive instability and/or neurological symptoms, surgical intervention combined with steroid therapy is safe and effective.
