1.Risk assessment of repeated cesarean delivery in scar uterus pregnancy accompanied by the placenta praevia

Objective To explore the complications and the treatments of repeated cesarean delivery in scar uterus pregnancy accompanied by the placenta praevia. Methods We performed a retrospective study in 6832 cases underwent first cesarean delivery,within which 201 cases were accompanied by the placenta praevia;and 337 cases underwent repeated cesarean deliver, within which 26 cases were accompanied by the placenta praevia. All subjects accepted cesarean delivery from January2006 toApril 2010 in our hospital. Results The occurrence rate of placenta previa was significantly higher in the repeated cesarean delivery than first-ever cesarean delivery (7. 72% vs 2. 94%, x2 = 22. 33, P < 0. 01 ) , especially the occurrence of complete placenta previa (42. 30% vs 0. 00%, x2 = 80. 43, P < 0. 01 ). The rates of uterus rupture, placenta accreta, postpartum hemorrhage and hysterectomy (r = 26. 92% ,23.08% ,26. 92% and 7. 69%, respectively) in repeated cesarean delivery in scar uterus pregnancy accompanied by the placenta praevia were significantly higher than those of the cases ( r = 2. 57% ,0. 32%, 1.29% and 0. 00%, respectively ) had repeated cesarean delivery in scar uterus pregnancy but no placenta previa ( x2 = 27.97,50. 41,42. 16,12. 79, respectively, Ps < 0. 01 ). Conclusion The incidence of placenta previa increased in scar uterus pregnancy, especially the complete placental previa.Scar uterus pregnancy accompanied by the placenta praevia are more likely to occur uterus rupture,placenta accreta,postpartum hemorrhage and had hysterectomy. Obstetricians should give more effort to reduce the cesarean section rate,improve the quality of medical care.
2.Effect of motivational interview on the self-care behaviors in type 2 diabetic patients

Objective To explore the effect of motivational interviewing on the self-care of type 2 diabetic patients . Methods Totally 96 inpatients with type 2 diabetes were randomly divided into the control group and the intervention group , each group containing 48 patients. The control group received routine diabetes education, while the intervention group received face-to-face and one-by-one motivational interviewing intervention for three months. The patients′self-care level were evaluated by using diabetes self-care scale after intervention. Result After intervention, the score on the self-care behaviors including diet control, exercise therapy, medication , blood glucose monitoring and foot care were significantly higher in the intervention group than those in the control group (P < 0.01). Conclusion Motivational interviewing can improve the ability of type 2 diabetic patients to control the glycemic level.

4.Disinfection effect of root canal with non-filling medication in root canal therapy

Objective: To observe the clinical disinfection effect of root canal with non-filling medication. Methods: 36 patients who required root canal therapy(RCT) for apical periodontitis (acute or chronic) or pulp necrosis were involved in the study.RCT was completed after two visits in both groups. Non-filling medication was used as disinfection in 18 teeth of 18 patients (experimental group) while FC in another 18 teeth of 18 patients (control group) randomly at first visit.One week later,root canal was regularly filled to complete RCT.From each case, bacteria specimens were collected and identified before sealing and filling. Results: Bacteria were detected in every specimen in both groups before sealing,which were mixture of aerobes and anaerobes.One week later,only one strain was detected in one specimen of each group. Conclusion: Non-filling medication can be used as an effective disinfection to sterilize root canal.
5.Expressions of SUMO1, NF-κB and TNF-α in myocardium of rats with type 2 diabetes mellitus

Objective: To study effect of small ubiquitin related modifier protein 1 (SUMO1) in inflammatory reactions mediated by tumor necrosis factor (TNF)-α and nuclear factor (NF)-κB in myocardial damage of rats with type 2 diabetes mellitus (T2DM). Methods: A total of 20 Goto-Kakizaki (GK) rats with spontaneous diabetes mellitus (DM) were randomly divided into group DM1 (pure DM group, n=10) and group DM2 (DM+high-fat diet group, n=10), and another 10 normal Wistar rats were regard as healthy control group. Expressions of SUMO1, TNF-α and NF-κB were measured by immunohistochemical method. Results: 1. Levels of blood glucose and TG in group DM1 and group DM2 were significantly higher than those of healthy control group, and those of DM2 group were higher than of DM1 group ,P<0.05 all; 2. Myocardial cells lined up in order and there was no hypertrophy in group DM1; but those in group DM2 showed cells loosely lined up and hypertrophy under light microscope; 3 Immunohistochemical assay indicated that expression of SUMO1 in group DM2 and DM1 group were significantly higher than those of healthy control group [(44.5±1.1) vs. (27.2±2.2) vs. (21.7±3.0)], and of group DM2 was significantly higher than that of DM1 group (P<0.01 all); expression of TNF-α in group DM2 and group DM1 were significantly higher than that of healthy control group [(27.5±1.5) vs. (20.2±2.7) vs. (13.1±1.6)], and of DM2 group was significantly higher than that of group DM1 (P<0.01 all);expression of NF-κB in group DM2 and group DM1 were significantly higher than that of healthy control group [(30.1±1.7)vs.40.7±1.5)vs.(16.0±2.6)], but of group DM1 was significantly higher than that of group DM2 (P<0.01 all). Conclusion: There are obvious metabolic disorders of glucose and lipid in T2DM rats, and complicated morphological changes of myocardial tissues similar to myocardial lesions in DM humans; the expressions of SUMO1, NF-κB and TNF-α significantly increase, suggest SUMO1 takes part in inflammatory reaction mediated by NF-κB, TNF-α in myocardial lesion of rat with T2DM,and may inhibit NF-κB, possesses effect of protect myocardium.
6.The community health diagnosis of life-way disease in Peking University Health Science Center

Objective: To study the basic information of life way disease and the corresponding risk factors of behavior in the community of Peking University Health Science Center, understand the background issue and social support system, and analyze the requirement for community health service so that the critical issue for health can be dealt with and the comprehensive program of prevention and treatment of the disease can be accordingly supplied. Methods: The random face to face questionnaire about life way disease and risk factors in 1 051 residents over 6 years of age in the community of Peking University Health Science Center was conducted according to the typical sampling principle in 1 primary school, 1 middle school, 1 university and its community, and the investigations of health data from the hospital and the police office in the community were also conducted through discussing about life ways risk factors. Accordingly , the data obtained by way of EXCEL were analyzed and processed using SPSS 11.5. Results: Diagnostic data of demography, epidemiology, behavior environment, education and organization were obtained concerning the public health of the Peking University Health Science Center community. Conclusion: The key health issue in the Peking University Health Science Center community is significantly influenced by the bad way of life. And some behavior risk factors, such as fatness, lack of outdoor exercise in the community members. Thus, the major necessary health service of Peking University Health Science Center residents is to carry out a comprehensive management program of life way disease for the whole people.
7.Effects of All-transretinoic Acid on Cell Apoptosis and p53 Expression in Rabbits' Carotid Atherosclerosis

ObjectiveTo observe the effects of all-transretinoic acid (ATRA) on cell apoptosis and p53 expression in rabbits' carotid atherosclerosis. Methods40 New Zealand rabbits were randomly divided 4 groups: normal group (NOR), control group (CON), surgery group (SUR) and treat group (TRE). Each group was fed with high cholesterol forage. The SUR and TRE groups were made up carotid atherosclerotic model by air desiccation injury after 2 weeks. The TRE group was fed with ATRA before surgery. On the 2nd and 4th week after operation, the target vessel was taken out. The intima hyperplasia was observed by microscopy through HE staining. Cell apoptosis was determined by TUNEL staining and p53 was observed by immunohistochemistry staining. ResultsOn the 4th week, the area of neointima in SUR, TRE and CON were (0.389±0.021) mm2, (0.113±0.038) mm2, (0.069±0.016) mm2 (P<0.01). The positive cells rate of TUNEL (8.40±0.45)% and p53 (3.02±0.38)% in TRE were higher than those of SUR (8.59±0.42)% and (2.23±0.29)%, respectively (P<0.01). ConclusionATRA may induce cell apoptosis in atherosclerotic plaque and inhibit the proliferation of intima associated with p53 in vessel injury.
8.Change of energy metabolism and body composition before and after chemotherapy in patients with colon cancer

Objective:To evaluate the nutritional status before and after chemotherapy in patients with colon cancer by measuring energy metabolism and body composition.Methods:Fifty-one patients with colon cancer were included and were investigated using self-control method (before and after chemotherapy).Resting energy expenditure (REE) was determined by bed metabolic instrument,and body composition was measured by body composition analyzer.Results:For all patients,the REE on the 2nd day after chemotherapy was decreased significantly compared with that before chemotherapy (P < 0.05).After chemotherapy,body fat percentage and body moisture percentage were increased significantly (P < 0.05),and lean body mass percentage was decreased significantly (P < 0.05).Conclusion:After chemotherapy,the REE and lean body mass percentage of the patients with colon cancer were decreased,and body fat percentage and body moisture percentage were increased.
9.Analysis of TCM Injections in the Inpatients from 4 Hospitals of Jiading District in Shanghai

OBJECTIVE:To provide reference for rational use of TCM injections. METHODS:Among the inpatients who used TCM injections in 4 hospitals of Shanghai Jiading district in 2013,100 cases were collected by systematic sampling in each hospital separately, and the data were analyzed retrospectively. RESULTS:The varieties and amount of TCM injections for blood-activating and stasis-resolving both occupied the first place. 22% inpatients used two or more TCM injections at the same time. The proportion of overdose use of TCM injections reached 54.55%. The type of unreasonable drug use mainly included drug use against differentiation of syndromes,super indications drugs and solvent error. Combined use of TCM injections with same ef-fect accounted for 20.45% of combined use of TCM injections. CONCLUSIONS:It is effective means to improve rational drug use that great impertance paid for clinical use of TCM injections.
10.Optimization of optical performance of biconvex Toric intraocular lens using a customized vision model

Background The optic quality of Toric intraocular lens (IOL)-implanted eye is affected by the residual astigmatism and individual difference of corneal spherical aberration and different magnification from steep and flat axis refraction.Whether correcting Toric IOL spherical aberration can effectively improve the image quality of individual patient is a question to be studied.Objective This study attempted to collect eye parameters of cataract patients to reconstruct the customized vision model by using Zemax optical software,and to evaluate the image performance with different Toric IOL spherical aberration.Methods A prospective study was performed.Forty-five eyes of 45 cataract patients were included in Second Hospital of Hebei Medical University from August 2012 to October 2013.Several relevant parameters were measured by Pentacam,including anterior and posterior surface height of cornea,corneal thickness,curvature radius of flat and steep meridians of anterior surface astigmatism,refractive diopter and curvature radius of posterior surface.The astigmatism of anterior and posterior corneal surface was described by Matlab 4.5 software.Corneal astigmatism model were set as aspheric state,and the effective position of Toric IOL was calculated using Holladay Ⅰ formula.Customized individual model eyes were constructed by Zemax software.The contrast sensitivity function (CSF) of different spherical Toric IOLs at different spatial frequencies were calculated and compared between 300 Td light environment with 3 mm pupil diameter (photopia light) and 0.3-1.0 Td light environment with 5 mm pupil diameter (mesopia light).This study was approved by Second Hospital of Hebei Medical University ethics committee,all the patients signed the informed consent.Results The mean astigmatism power was (1.51 ± 0.36) D and (1.49 ± 0.37) D,and the mean astigmatism meridian was (101.5 ± 59.8) ° and (101.9±58.5) ° in the model eyes and cataract eyes,respectively,without significant differences between them (t=0.886,0.652;both at P>0.05).Bland-Altman test showed a good agreement in astigmatism power and astigmatism meridian between model eyes and cataract eyes.The LogCSF values at 1.5,3.0,6.0,12.0 and 18.0 c/d spatial frequencies were significantly higher in the aspherical Toric IOL model eyes than those in the spherical Toric IOL model eyes,and the LogCSF values at various spatial frequencies were significantly higher in the Toric IOLs with spherical aberrations of-0.13 μm and-0.26 μm than those in the zero spherical aberrations in both photopia light and mesopia light (all at P<0.05).Conclusions A precise corneal astigmatism model based on cornea high data of cataract eyes was successfully constructed through special formulas with Zemax software.Aspherical Toric IOL can compensate for spherical aberration of cornea and enhance the optic quality in individual model eye.
