1.Finite element analysis of femoral prosthesis implant in total knee arthroplasty:positioning parameters and clinical optimization

BACKGROUND:Three-dimensional finite element simulation has been widely used in biomechanics. However, there is little research in knee joint replacement. The study of femoral prosthesis is less.
OBJECTIVE:To analyze the optimal positioning parameters of knee replacement and femoral prosthesis implantation, and to verify the efficacy of Gemini-PS knee prosthesis in total knee arthroplasty.
METHODS: (1) Knee finite element model in total knee replacement was constructed. Orthogonal experiment was performed in femoral osteotomy positioning parameters. The shift amount A, external rotation degree B, and valgus degree C of femoral prosthesis were selected as the relevant factors for orthogonal experiment. Three values were taken for each parameter to establish orthogonal tables. Nine experimental combined knee replacement finite element models were created and analyzed with finite element. Analysis of variance and range analysis were used by optimization. (2) Totally 42 patients (47 knees) with knee osteoarthritis received total knee arthroplasty with Gemini-PS knee joint prosthesis. Knee joint function was evaluated after replacement with Hospital for Special Surgery knee score and American Knee Society knee score. Knee pain was assessed with Visual Analogue Scale score.
RESULTS AND CONCLUSION:(1) Minimal polyethylene substrate surface compressive stress peak was translation 0 mm, external rotation of 3°, valgus 6°; the peak stress was 15.9 MPa. Among the influential factors, the effects of internal and external translation were larger than that of external rotation angle, and greater than that of valgus angle. The optimal positioning parameter combination was pan 0 mm, external rotation 3°, valgus 6° by range analysis and variance analysis. Orthogonal experiment was valid by simulation. (2) 42 patients were folowed up for 12-36 months. One patient affected subcutaneous fat liquefaction of the lower segment of the incision. Hospital for Special Surgery knee score and American Knee Society knee score were significantly higher after implantation (P < 0.05). Visual Analogue Scale score was significantly lower after replacement (P < 0.05). X-ray films showed that osteolysis, dislocation or loosening was not found. 2.Effect of melatonin on bioactivity of fibroblasts from human hypertrophic scar

Objective To investigate the effect and mechanism of melatonin on bioactivity of fibroblasts from human hypertrophic scar.Methods Fibroblasts from hypertrophic scar were cultured and incubated with melatonin,melatonin and Luzindole,and Luzindole,respectively,for 24 h,and the media as control.XTT/PMS assay was used to measure the proliferation of fibroblasts,ELISA assay to detect the TGF-β1 production of fibroblasts,and the expression of cell a-SMA,collagen Ⅰ,collagen Ⅲ mRNA were determined with real-time PCR method.Results Compared with the control,melatonin at the concentration of 10-5mmol/L,10-3mmol/L,and 1 mmol/L could inhibit the proliferation of fibroblasts in a does-dependent manner (P<0.05); melatonin at the concentration of 10-3 mmol/L could significantly decrease the TGF-β1 production and expression of a-SMA mRNA and collagen Ⅰ mRNA in fibroblasts from human hypertrophic scar (P<0.05) ; the effect of melatonin on fibroblast was significantly blocked by Luzindole (P<0.05),but melatonin could not inhibit collagen Ⅲ mRNA expression (P>0.05).Conclusions Melatonin can significantly regulate the biological activity of fibroblasts from human hypertrophic scar through a receptor pathway.
3.Screening of Protein Interacting with HCMV UL130 Protein by Yeast Two Hybrid from Human Fetus Brain cDNA Library

Objective To screen the human proteins interacting with human cytomegalovirus(HCMV)UL130 from human fetus brain cDNA library by GAL4 two-hybrid system 3 technique and analyze the corresponding coding sequences.Methods The "bait plasmid"(named as pGBKT7-UL130)was constructed.By using HCMV UL130 as the bait,a human fetus brain cDNA library was screened and the proteins interacting with UL130 protein were searched.The positive clones were sequenced and analyzed by bioinformatic methods.Results Nine clones interacting with HCMV UL130 were identified,two of them were synaptosome-associated protein(SNAP).Conclusion Some proteins interacting with HCMV UL130 in human fetus brain cDNA library were successfully screened.SNAP might play an important role in HCMV infection pathogenesis.

Objective Using yeast two-hybrid system to screen the proteins which can interact with the human cytomegalovirus (HCMV) UL128 which have two difference transcription structure from human fetus brain cDNA library, and compare the difference with structure and function of interacting proteins. Methods Two fragments of UL128 were amplified by 3'RACE and 5'RACE technology, the length are 519 bp and 642 bp, respectively. The "bait plasmid" (named as pGBKT7-UL128-519 bp and pGBKT7-UL128-642 bp) was constructed successfully. Using pGBKT7-UL128-519 bp and pGBKT7-UL128-642 bp as a bait, a human fetus brain cDNA was screened and the proteins interacting with UL128-519 bp and UL128-642 bp encoded protein were searched, and the positive clones were sequenced and analyzed by bioinformatic methods. Results EFEMP2 interacting with HCMV UL128-519 bp were identified, THY-1 interacting with HCMV UL128-642 bp were identified. Conclusion EFEMP2 and THY-1 proteins interacting with HCMV UL128-519 bp and UL128-642 bp in human fetus brain cDNA library were successfully screened, but same proteins weren't found from the proteins interacting with UL128-519 bp and UL128-642 bp protein, UL128-519 bp and UL128-642 bp protein may be play an different effect in the process of infect by HCMV.
5.Inhibition Effect of Non Custodial Terpenes-3β-Alcohol to Autoimmune Encephalomyelitis

Objective To study the inhibition effect of non custodial terpenes-3β-alcohol to experimentally in-duced autoimmune encephalomyelitis in guinea pigs. Methods Different doses (25 mg/kg, 50 mg/kg and 100 mg/kg) of non custodial terpenes-3β-alcohol were given to the experimentally induced autoimmune encephalomyelitis model of guinea pigs by gavage for 8 weeks. Plasma levels of CD4+/CD8+, IL-1, IL-2, IL-6, IL-10, neuropeptide Y (NPY), beta endorphin (β-EP) , transforming growth factor-β(TGF-β), matrix metalloproteinase (MMP-2), nitric oxide synthase (NOS) and leuko-cyte differentiation antigen CD3 were assessed. The brain neuron morphology changes was observed under light microscopy while its ultrastructure changes was observed under electron microscope. NOS expression in neurons was observed through immunofluoresce technology. Results Non custodialterpenes-3β-alcohol inhibited the increase of plasma CD4+/CD8+, IL-1, IL-2, IL-6, IL-10, MMP-2, CD3 and NPY while decrease of plasmaβ-EP, brain TGF-β. It also increase NOS expres-sion in neuronal cytoplasm and maintained neuron morphology. Conclusion Non custodial terpenes-3β-alcohol inhibit-ed the experimental autoimmune encephalomyelitis in guinea pig.
6.Dietary behavior and nutritional status among Chinese female college students residing in Korea.

The purpose of this study was to analyze the nutritional status of Chinese female college students in Korea, and to investigate changes in their dietary behavior after residing in Korea. The subjects included 114 Chinese female college students currently studying in Korea. General characteristics and dietary behaviors before and after residing in Korea were investigated. Daily nutrients and food intake were assessed using a one-day 24-hour recall. Consumption of most nutrients, namely calcium, iron, zinc, vitamin B2, vitamin C, and folate was estimated to fall below the EAR for the Korean population. The proportions of subjects whose intake were estimated below the EAR for folate, calcium, vitamin B2, iron, zinc, and vitamin C were 93.0%, 71.9%, 66.7%, 65.8%, 65.8%, and 63.2%, respectively. In the current study, the dietary behavior scores fell significantly after Chinese students immigrated to Korea (p < 0.001); increased frequency of meal skipping, less consumption of fruits and vegetables, irregular meal pattern, and imbalanced diet were some of the significant changes among Chinese students before and after residing in Korea. In addition, consumption of most nutrients, including dietary fiber, vitamin A, vitamin B group, vitamin C, and folate in the Worsened Group was significantly lower, compared with the No Change Group (p < 0.05). According to the changes in dietary behaviors, consumption of most nutrients, including dietary fiber, vitamin A, vitamin B group, vitamin C, and folate in the Worsened Group was significantly lower than in the No Change Group (p < 0.05). In conclusion, these results indicated that Chinese students practiced more unhealthy dietary behaviors after residing in Korea and Chinese students residing in Korea showed poor nutrient-based diet quality. Conduct of a follow-up study using blood profile tests is needed in order to assess the nutritional status of Chinese students. These results would be used in planning of a nutritional surveillance program for Chinese students.

7.Effects of Different Doses of Esketamine on Postoperative Anxiety and Depression in Patients with Breast Cancer

Objective To investigate the effect of different doses of esketamine on postoperative anxiety and depression in patients with breast cancer.Methods A total of 96 patients undergoing modified radical mastectomy were randomly divided into esketamine L group(0.15mg/kg),M group(0.30mg/kg),high-dose H group(0.50mg/kg),and normal saline group(C group)with 24 cases in each group which will be given during induction of anesthesia.State Anxiety Scale(SAI)score and Montgomery Depression Scale(MADRS)score were recorded 1day before surgery(D1),1day after surgery(D2),3days after surgery(D3),5days after surgery(D4),and 7days after surgery(D5).Data such as the dosage of remifentanil,the dosage of norepinephrine,and the duration of the op-eration(from the beginning of the operation to the end of the operation)were recorded during the operation.The time from the end of an-esthesia to extubation and the postoperative PACU residence time was recorded,and delirium,postoperative nausea and vomiting(PONV),dizziness,drowsiness,and pain relief rate within 24hours after surgery were recorded.Results At D3,group H had a lower SAI score than the other three groups(P<0.05);compared with D1,the SAI score at D4 and D5 in groups C,L,and M was signifi-cantly decreased(P<0.05).The SAI score of group H at D3,D4,and D5 was decreased(P<0.05);the MADRS score of group H was lower than that of the other three groups at D3 and D4(P<0.05);compared with D1,the MADRS score of all groups gradually de-creased with the extension of time(P<0.05).Remifentanil consumption in group H was significantly lower than that in the other three groups(P<0.05).There were no significant differences in the incidence of postoperative delirium,PONA,remedial analgesia,dizzi-ness,lethargy,and fever among the four groups(P>0.05).Conclusion Intraoperative intravenous injection of esketamine can reduce the SAI and MADRS scores,reduce the perioperative opioid consumption,and not increase the incidence of postoperative adverse reac-tions in breast cancer patients.
8.Establishment of a composite functional evaluation system to study the clinical effect of C3 type distal radius fractures in the elderly under single plate and double plate internal fixation

Objective:To establish a composite functional evaluation system to study the clinical efficacy of single plate and double plate internal fixation in the treatment of C3 distal radius fractures in the elderly.Methods:62 elderly patients (≥ 60 years old) with type C3 distal radius fractures who were admitted to the First Affiliated Hospital of Anhui University of Science and Technology from September 2018 to August 2021 were retrospectively selected and divided into two groups according to the surgical methods: bilateral plate internal fixation treatment group (double plate group) and unilateral plate internal fixation treatment group (single plate group); The preoperative anatomical angle, wrist mobility (flexion and extension) and radius shortening of the two groups were recorded. The anatomical angle, wrist mobility (flexion and extension), clinical Gartland-Werley, DASH Score for Upper Limb Function (DASH) , Wrist Joint Cooney Score and clinical healing time at the 3rd, 6th, 9th and 12th months after operation were followed up. A composite evaluation table was established to comprehensively evaluate the postoperative recovery of patients in the two group.Results:There was no significant difference in anatomical indexes of radius between the two groups before operation (all P>0.05). All anatomical angles and wrist range of motion were within the normal range after operation, and there was no statistical difference between the two groups in each anatomical index of radius within 3 and 6 months after operation (all P>0.05). However, the palmar inclination, ulnar deviation, wrist flexion and dorsiflexion mobility of the double plate group were relatively stable at 9 and 12 months after operation, which were better than those of the single plate group (all P<0.05). The palmar inclination and ulnar deviation of the single plate group increased with time. After operation, the radius shortening of the two groups recovered significantly, and the radius length of the double plate group was maintained better than those of the single plate group for 9 and 12 months (all P<0.05). There was no significant difference in the scores of the two groups within 3 and 6 months after operation (all P>0.05), but the Gartland Werley score, DASH score and Cooney wrist score of the double plate group were significantly better than those of the single plate group at 9 and 12 months after operation (all P<0.05). The clinical healing time of single plate treatment was (14.51±0.88)weeks, the longest was 108 days, while that of double plate treatment was (12.03±1.77)weeks, the longest was 129 days, with statistically significant difference ( P<0.05). There was no significant difference in the incidence of complications between the two groups ( P>0.05). There was no statistically significant difference between the two groups in the final composite evaluation results ( P>0.05), but the total score of the double plate group was smaller than that of the single plate group, with a difference of 39.67 points, indicating that the double plate group still had certain advantages in various scores. Conclusions:According to the composite evaluation system, both single plate and double plate treatment of C3 distal radius fractures in the elderly can achieve satisfactory results. Double plate fixation of distal radius fractures still has a certain significance in maintaining joint stability and improving joint mobility.
9.Effect of early activity on postoperative delirium for patients after craniotomy: an evidence-based protocols

Objective:To observe the effect of evidence-based early activity training on postoperative delirium in patients with brain tumor resection.Methods:This study used non-contemporary comparison method, a total of 238 patients admitted to the Second Affiliated Hospital of Zhejiang University School of Medicine in 2019, were included. 121 patients who met the inclusion criteria and signed informed consent after craniotomy during January 1st,2019 to June 31th,2019 were included in the control group, and 117 patients who met the criteria and obtained informed consent after craniotomy during July 1 to December 31,2019, were included in the experimental group. Neurosurgical postoperative routine nursing care were given to the control group. The experimental group received evidence-based early activity training. The incidence of delirium, duration of delirium, Barthel Index (BI), delirium-related adverse events and other indicators of postoperative hospitalization were compared between two groups.Results:The incidence of delirium, duration of delirium and postoperative hospital stay in the experimental group were 9.5%, 2.0 (1,3) days and 7 (5,10) days, lower than 23.3%, 3.0 (1,5) days and 8 (6,11) days in control group, and the differences were statistically significant ( χ2 value was 8.17, Z value were 2.96 and -2.01, P<0.05). BI index 1 week and 1 month after discharge in the experimental group was 90 (85, 90), 100 (100, 100), higher than the control group of 90 (86, 90) and 100 (100,100), with statistically significant difference ( Z values were -2.41 and -2.46, P<0.05), the comparison of adverse events, 0 case in experimental group, 1 case in control group, there was no statistically significant difference ( χ2 value was 0.97, P>0.05). Conclusions:Early evidence-based activities in patients with brain tumor resection can reduce the incidence of postoperative delirium, shorten the duration of postoperative delirium and postoperative hospital stay, and improve the ability of patients to take care of themselves after surgery, which is safe and effective.

Diagnosis and treatment of diabetes mellitus depend on accurate monitor of blood glucose of laboratory.Glycated albumin,the short-term indicator of blood glucose control, has special advantage and role in the monitor of diabetes mellitus and has been payed more and more attention.The standardization work of determination of glycated albumin at home and abroad is still in the primary stage at present.The consistency and accuracy of measurement results for glycated albumin is needed to be solved urgently.
