Objective To investigate the expression of matrix metalloproteinases 9 (MMP 9) and tissue inhibitor of metalloproteinases 1(TIMP 1) mRNA in the endometrium during mid luteal phase in women with unexplained infertility and its steroidal regulation Methods In situ hybridization method was utilized to detect the location of MMP 9 and TIMP 1 mRNA. Reverse transcription polymerase chain reaction (RT PCR) method was utilized to detect and quantitate MMP 9 and TIMP 1 mRNA expression levels in the endometrium during mid luteal phase. Serum concentration of estradiol (E 2) and progesterone (P) were measured by radioimmunoassay. Thirty eight patients with unexplained infertility were included as study group, and 20 women with male factor infertility or normal volunteers were selected as control group. Results MMP 9 and TIMP 1 mRNA were diffusely distributed in the cytoplasm and neuclei of glandular and stromal cells. Their staining in control group were more abundant than those in study group. 4.Effects of blood total renin and steroid concentrations on pregnancy in in vitro fertilization and embryo transfer.

Objective To observe the effects of renin - angiotensin system ( RAS ) and steroid hormones onpregnancy in in vitro fertilization and embryo transfer(IVF-ET). Methods .53 tubal infertility women underwent IV F-ET under control of ovarian hyperstimulation with GnRHa/FSH/hMG/hCG. Plasma total renin, serum estradiol(E2),progesterone(P) and testosterone(T) were measured on the day of injection of injection of hCG and 36h after hCG( before getting the oocytes). ResultsPlasma total renin level on the day of hCG in pregnant group was higher than that in nonpregnant group(P<0.05), E2 and P lower than that in nonpregnant group(P<0.05, P<0.01). Plasma total renin and E2 levels 36h after hCG in pregnant group were lower than that in nonpregnant group( P<0.05). There was no difference in other hormones between the two groups. ConclusionThe high concentration of total renin on the day of hCG injection and its low concentration 36h after hCG administration are probably conducive to pregnancy.
5.Resection of local recurrence of gastric malignancy tumor

Objective To determine the resectability of recurrent gastric malignant tumor and its clinical value, and to search an effective way to cure recurrent gastric malignant tumor. Methods The clinical data of 18 cases of sucessfully resected local regional recurrent gastric malignant tumor were analysed retropectively. Results In the 18 cases of recurrent malignant gastric tumor, 6 received total gastrectomy and lymphadenectomy, 5 received body and tail pancreatectomy plus splenectomy with lymphadenectomy, 2 got tumor resection in gastric bed, and the other 5 got periceliac tumor resection with lymphadenectomy. Pathology evidence revealed that 14 of them were gastric adenocarcinoma and the other 4 were gastric sarcoma. They were all recovered and got well when discharged. Follow up results revealed that resection of recurrent malignant gastric tumor could survive 16.4 months (7~38 months) averagely.Conclusions Patient's life quality and survival time could be improved if the local regional recurrence was resectable and other subsidiary therapeutic methods were used.
6.Clinical features of hepatocellular carcinoma during pregnancy

Objective To explore the clinical features of hepatocellular carcinoma (HCC) during pregnancy. Methods Clinical data of 4 patients with HCC in pregnancy were retrospectively analyzed.Results All 4 patients were positive for hepatitis B surface antigen. A marked increase in maternal serum a-fetoprotein (AFP) was found in 3 patients (310.1-5630.0 ng/ml ). Three patients were diagnosed at their advanced stages and died of disease in the 4th, 6th and 7th months, respectively. One patient diagnosed as having early HCC underwent curative surgery and has been without recurrence for 26 months. Conclusion The overall survival of patients with HCC in pregnancy is grim because most patients are diagnosed in the advanced stage. Surveillance with AFP and ultrasonography should be recommended for pregnant woman for the detection of early HCC, especially in hepatitis B virus carriers from high endemic areas, to improve patient survival.
7.An experimental study on the relationship between interleukin-18 and the pathogenesis of type 2 diabetes mellitus

Objective To investigate the relationship between interleukin-18(IL-18)and the pathogenesis of type 2 diabetes melli-tus.Methods 72 healthy Sprague-Dawlay male rats were randomly divided into four groups, NC group,NCS group,HF group and HFS group.At the end of the 8th week,NCS group and HFS group were injected with STZ(25mg/kg)into abdominal cavity.At the end of the 10th week,diabetic rats were screened by oral glucose tolerance test(OGTT).The blood sample was collected when the rats were killed at the end of the 14th and 20th week.The levels of serum IL-18,IL-6 and tumor necrosis factor-α(TNF-α)were assayed with ELISA.Results Most rats in HFS group were achieved the diagnostic standard of diabetic rat, and their insulin sensitivity index(ISI)were decreased.At the end of the 14th week and 20th week,the levels of serum IL-18,IL-6 and TNF-α in HFS group were significantly higher than those in NC group(P<0.01).In HFS group,the levels of serum IL-18,IL-6 and TNF-α at the end of the 20th week were higher than those at the end of the 14th week,but it had no statistic significance(P>0.05).Pearson linear correlation analysis showed that the level of serum IL-18 in HFS group was positive correlated with FBG,IL-6,and TNF-α(r=0.90,P<0.01 or r≥0.73,P<0.05),and negative correlated with ISI(r=-0.86,P<0.01).Condusions Our results show that IL-18 is related with the pathogenesis of type 2 diabetes mellitus and chronic inflammation plays an important role in the development of type 2 diabetos mellitus.
8.Effects of community-based health education in food glycemic index on dietary modification of patients with type 2 diabetes mellitus

Objective To evaluate the effects of dietary intervention with low glycemic index (LGI) and low glycemic load (LGL) on dietary knowledge and dietary modification of patients with type 2 diabetes mellitus (T2DM).Methods This study was a cluster randomized controlled study.From June to September 2010,105 T2DM patients were randomly sampled from 3 residential areas (n =35 in each) in Jin Pen Ling community,Changsha,Hunan province and divided into three groups with a random number table:whole-day dietary intervention group (group A,n =35),breakfast dietary intervention group (group B,n =35),and control group (group C,n =35).Interventions included 3-month health education and dietary intervention.Dietary knowledge,dietary oils,glycemic index (GI),and glycemic load (GL) of the three groups were compared before and after intervention.Results Before intervention,all the three groups showed a low level of knowledge about GI (57.7 %),and high levels of average GI value of each meal (69.71 ± 8.55),GL value per day (132.45 ± 16.25),and daily cooking oil intake [(39.95 ± 20.64)g].The average knowledge level about GI increased after intervention in both group A (96.3% vs.57.1%,P <0.05) and group B (94.5% vs.60.5%,P<0.05),but not in group C (54.8% vs.55.3%,P>0.05).The reduction of daily cooking oilintake in group A was larger than those in group B and group C [(7.81 ± 3.91) g vs.(-5.12 ± 1.37) g,(7.81 ± 3.91) g vs.(-3.45 ± 5.95) g,both P < 0.05];so was the average GI value of each meal [group A,(5.95±2.27);groupB (-0.85±1.87);groupC,(-2.87±1.93);allP<0.05].The changes of GL values in group A (11.31 ±4.31) was better than in group B (1.23 ±27.82) and group C (-0.43 ± 18.40)(both P <0.05).Conclusions The average GI value,GL value,and daily cooking oil intake of T2DM patients were all at a high level before intervention,while the level of knowledge about GI was at a low level.Health education providing GI knowledge may effectively improve the knowledge about GI in T2DM patients,while LGI and LGL dietary intervention may promote diet modification.Whole-day dietary intervention may be more effective than breakfast intervention.
9.Effect of Fuzheng-Peiyuan recipe assisted modified VAD regimen on M protein, myeloma cells and immune function in patients with multiple myeloma

Objective:To investigate the effect of Fuzheng-Peiyuan recipe assisted modified VAD program on M protein, myeloma cells and immune function in patients with multiple myeloma (MM). Methods:A total of 96 patients with MM from January 2017 to May 2019 in our hospital were selected as the research objects, and they were divided into two groups according to the random number table method, with 48 patients in each group. The control group was given a modified VAD regimen (vincristine + adriamycin + dexamethasone), and the observation group was given Fuzheng-Peiyuan recipe as an auxiliary VAD regimen. Both groups were treated for 3 cycles. The clinical efficacy, TCM syndrome score, bone pain score, blood creatinine, hemoglobin, blood calcium, M protein, myeloma cells, immune function [Interleukin-2 (IL-2), Interferon-gamma (INF-γ), Interleukin-4 (IL-4), Interleukin-10 (IL-10)] levels and adverse reactions of the two groups were recorded and compared before and after treatment. Results:The total effective rate of the observation group was 81.3% (39/48), and the control group was 62.5% (30/48). The difference between the two groups was statistically significant ( χ2=4.174, P=0.041). The scores of TCM syndromes ( t=4.674, 13.328) and bone pain scores ( t=4.505, 11.398) in the observation group were significantly lower than those in the control group ( P<0.01) at 1 and 3 cycles after treatment; SCr ( t=4.452, 10.039), blood calcium ( t=4.578, 4.155) in the observation group were significantly lower than those in the control group ( P<0.01); HbAlc levels ( t=5.290, 8.871) in the observation group was significantly higher than that of the control group ( P<0.01); M protein ( t=11.145, 33.812), myeloma cells ( t=6.415, 19.731) in the observation group were significantly lower than those in the control group ( P<0.01); serum IL-2 ( t=4.922, 8.789), INF-γ ( t=5.610, 8.886) in the observation group were significantly higher than those in the control group ( P<0.01); IL-4 ( t=4.709, 6.784), IL-10 ( t=5.287, 12.823) in the observation group were significantly lower than those in the control group ( P<0.01). During the treatment, the incidence of adverse reactions in the observation group was 41.7% (20/48) and that in the control group was 62.5% (30/48). The difference between the two groups was statistically significant ( χ2=4.174, P=0.041). Conclusion:Fuzheng-Peiyuan recipe assisted in improving the VAD regimen in the treatment of MM has a significant clinical effect, which can relieve the clinical symptoms of patients and reduce the degree of bone pain, and promote the reduction of myeloma cells in patients, improve their immune function, and improve the tolerance of chemotherapy.
10.Clinical analysis of 20 cases of primary colorectal lymphoma

Objective To investigate the diagnosis and the relationship between its clinicopathological characteristics,treatment and the prognosis of primary colorectal lymphoma.Methods The clinical data of 20 primary colorectal lymphoma patients who received operative treatment in the past 34 years in our hospital were retrospectively summarized,and the influence of treatment and each clinicopathological factor on prognosis was assessed.Results Tumor size,lymph node metastasis,invasion of neighboring organs and distant metastasis,and type of operative treatment were significantly related to the survival(P0.05).Conclusions Diagnosis of primary colorectal lymphoma before operation is difficult,and misdiagnosis is common.Operative treatment is the main therapeutic means for colorectal lymphoma.Radical operation combined with chemotherapy can gain a good prognosis.
