Objective To investigate the expression of matrix metalloproteinases 9 (MMP 9) and tissue inhibitor of metalloproteinases 1(TIMP 1) mRNA in the endometrium during mid luteal phase in women with unexplained infertility and its steroidal regulation Methods In situ hybridization method was utilized to detect the location of MMP 9 and TIMP 1 mRNA. Reverse transcription polymerase chain reaction (RT PCR) method was utilized to detect and quantitate MMP 9 and TIMP 1 mRNA expression levels in the endometrium during mid luteal phase. Serum concentration of estradiol (E 2) and progesterone (P) were measured by radioimmunoassay. Thirty eight patients with unexplained infertility were included as study group, and 20 women with male factor infertility or normal volunteers were selected as control group. Results MMP 9 and TIMP 1 mRNA were diffusely distributed in the cytoplasm and neuclei of glandular and stromal cells. Their staining in control group were more abundant than those in study group. 4.Interventional treatment for hemorrhage following hepatic biliary and pancreatic surgery

Interventional management was performed in 10 patients with hemorrhage following hepatic biliary and pancreatic surgery.The super-selection arterial embolization was performed in 9 patients using gelfoam pledgets (n =6 ) or metal coils (n =3 ),the procedure was combined with local infusion of reptilase; 1 case was managed by local infusion of reptilase alone.Selective angiography revealed the pseudoaneurysm in 3 cases,contrast media extravasation in 6 cases; in 1 case no obvious bleeding site was found The interventional treatment was successful and hemorrhage stopped in all patients,including one case with reptilase alone with a successful rate of 10/10.The results indicate that interventional treatment is a safe and effective method for hemorrhage following hepatic biliary and pancreatic surgery.
5.Intraoperative iodine-125 seed implantation for pancreatic carcinoma

Objective To investigate the clinical value of intraoperative iodine-125 seed implanttation in treating pancreatic carcinoma. Methods Seventy-five patients (fourty-one men, thirty-four women;median age 54 years) with pancreatic adenocarcinoma were enrolled into the study. Thirty-one patients (group A) were accepted tumor resection,eighteen patients(group B) were implanted radioactive iodine-125 seeds into the tumors by a combination of bypass surgery, twenty-six patients(group C) were treated by bypass surgery.Results Sixty-seven patients were followed up. The median survival time was 19, 12 and 7 months in group A,B,C respectively, among which the difference was significant (P < 0.05). The response rate(CR+PR) was 50 % and the effective rate of pain relieving was 80% in the group B. The 97.4 % of accordance rate of seed number was demonstrated by CT film, but the accordance rate of seed space distribution was only 56 %.Conclusion At present, the active resection of the pancreatic carcinoma, including the superior mesenteric vein and the retropancreatic fusion fascia, is essential for a curative resection. The combination of Intraoperative iodine-125 brachytherapy and bypass surgery is safe and effective for pancreatic carcinoma.The seed space distribution completed by seed computer therapeutic plan needs further study.
6.Clinical effect of preoperative visualization technique-guided pedicle screw fixation on multi-segmental lumbar fracture and dislocation

Objective To analyze the clinical effect of pedicle screw fixation in the treatment of multi-segmental lumbar fracture and dislocation under the guidance of visualization technique. Methods A total of 21 patients with multi-segmental lumbar fracture and dislocation were selected from November 2012 to November 2013. Before the screw implantation, the structure of bilateral pedicle was observed through Mimics software and the implantation parameters were measured. The position of pedicle screws by postoperative CT scan, operation time, and the satisfaction of the patients were assessed. The percentages of anterior vertebral height and Cobb′s angle were measured before operation, 2 weeks and 8 months after operation. Results All patients were satisfied with informed consent score and the way of pedicle screw and the selection of plant were more reasonable. With better screw position, shorter operative time and less blood loss and adverse reactions, pedicle screw fixation achieved good effect. Conclusion With high security and considerable clinical value, pedicle screw fixation in the treatment of multi-segmental lumbar fracture and dislocation under the guidance of visualization technique has exact and good effecct.
7.Discovery of a Residual Focus of Bancroftian Filariasis after Declaration of its Transmission Interruption in Guangxi

Objective To describe the discovery of a residual foci of bancroftian filariasis in Fuchuan County where the disease was announced to have been eliminated, and reveal its epidemiologic feature.Methods The investigation was carried out from August 2007 to March 2008 among residents in Changtang village where the first caseof filariasiswas found and the neighboring villages.They were screened with two thick blood smears.Immunochromatographic technology(ICT) was conducted for those going out but returned and those in surrounding areas.Vector mosquitoes were collected and dissected to find filaria larvae.Historical documents were reviewed and relevant people were interviewed.Results In Changtang administrative village, 1 052 residents were screened and 19 cases with microfilaremia were found in 2 natural villages, with a Mf-positive rate of 1.8%(5.1% in Gangshang and 1.4% in Yinshan respectively).No Mf-positive case was found in 4 119 residents screened in other 3 villages.The average microfilaria density in the 19 cases was 17.37/60 ?l blood.All the 19 cases belonged to 12 families, and 13 cases were relatives to each other, which showed a feature of spatial clustering and family clustering.More patients were identified in the age groups of 20-29 and 50-59, and 57.9% of them were older than 50 years.No larvae were found in 54 Culex pipiens fatigans dissected.Conclusion The Changtang village is identified as a residual focus of bancroftian filariasis with a low, limited endemicity.More cases have been among the elderly with a low average microfilaremia.
8.Effect of systematic self-management education on quality of life, anxiety and depression of patients with Type 2 diabetes in communities.

OBJECTIVE:
To explore the effect of systematic self-management education on quality of life, anxiety and depression of patients with Type 2 diabetes in communities.
METHODS:
A total of 248 patients with Type 2 diabetes from 10 communities of Changsha from February 2009 to July 2010 were randomized into an intervention group and a control group. The intervention group received systematic self-management education, while the controls received routine community education. The quality of life, anxiety and depression were measured by adjusted diabetes-specific quality of life scale (A-DQOL), self-rating anxiety scale (SAS) and self-rating depression scale (SDS) respectively.
RESULTS:
The quality of life, morbidity of anxiety or depression of the intervention and the control group were comparable at the baseline (P>0.05). One and half year later, there was significant difference between the two groups (P<0.01). Compared with the control group, the intervention group got better quality of life (P<0.01), less morbidity of anxiety (8.94% vs. 44.4%, P<0.01) and depression (23.58% vs. 56.00%, P<0.01).
CONCLUSION
Systematic self-management education can effectively improve the quality of life, reduce the level of anxiety and depression of patients with Type 2 diabetes in communities, which deserves further generalization.

OBJECTIVE:
To explore the effectiveness of systematic self-management education on blood sugar level of patients in the community with type 2 diabetes.
METHODS:
A total of 248 patients with type 2 diabetes from 10 communities of Changsha were randomized into intervention or control groups in February 2009. The intervention group received systematic self-management education, while the other received routine community education. Before and after intervention, fasting plasma glucose, 2 h postprandial plasma glucose and glycosylated hemoglobin were measured, and evaluated between the 2 groups.
RESULTS:
Fasting plasma glucose, 2 h postprandial plasma glucose and glycosylated hemoglobin levels of patients with diabetes from the two groups were generally comparable at baseline (P>0.05). After the intervention, these three indicators were all statistically significant between the 2 groups (P<0.01, P<0.05, P<0.01, respectively). After intervention, blood sugar levels in the intervention group were obviously improved (P<0.01), while in the control group, fasting plasma glucose was not statistically improved (P>0.05), 2 h plasma glucose and glycosylated hemoglobin levels were improved (P<0.01, P<0.05, respectively).
CONCLUSION
Systematic self-management education effectively encourages patients with type 2 diabetes to control their blood sugar levels, and deserves further promotion.

10.Clinical features and reasons of pressure ulcers in general hospital of western regions

Objective To analyze the clinical features and reasons of pressure ulcers in general hospital of western regions, and provide references for effective prevention and management of pressure ulcer. Methods Hospitalized patients who meet the conditions (≥18 year-old, hospital stay≥24 h ) in 51 wards of the largest tertiary comprehensive hospital in Ningxia were investigated by data collection tools that designed by European pressure ulcer advisory panel (EPUAP) and American survey instrument of pressure ulcer prevalence rate. For the cross-sectional survey on pressure ulcer, nurses were training and one day respectively in January and April 2015 were selected as the research day. Results Totally 3654 patients were investigated, among which there were 1894 male patients and 1760 female patients. 332 patients had the risk of pressure ulcer ( Braden score≤16 points ) , which accounted for 9.08%;31 cases and 35 parts were found with the pressure ulcer and the prevalence rate of pressure ulcer was 0.85%;the occurrence rate in hospital was 0.47%. Clinical features were mainly stage I and stage II, which accounted for 77. 42% ( 24/31 ); body parts with high occurrence of pressure ulcer were sacrococcygeal region, crista iliaca, ischial tuberosity and ankle. Implementation of prevention measures: patients with decompressor using accounted for 4. 93 among all researchers ( 180/3 654 ) , and accounted 54. 22% among patients with risk of pressure ulcer ( 180/332 );patients with dressing to reduce pressure on pressed parts that accounted 2. 49% among all researchers (91/3 654), and accounted 27.41% among patients with risk of pressure ulcer (91/332);patients with regular turning over accounted for 11. 49% among all researchers ( 330/3 654 ) , and accounted for 99. 4% among patients with risk of pressure ulcer ( 330/332 ) . Conclusions The prevalence rate and occurrence rate pf pressure ulcer in comprehensive hospitals of western area are all lower than those in researches home and abroad;the clinical features are similar to 12 comprehensive hospitals in our country, and the implementation rate of prevention measures should be improved. Nursing knowledge training of pressure ulcer in hospitals and the implementation rate of prevention measures should be enhanced.
