1.Clinical Analysis of Late Pregnancy Complicated with Uterine Leiomyoma

Objective:The treatment of late pregnancy complicated with utedne leiomyoma was investigated.Methods:193 Cases of Iate pregnancy complicated with uterine leiomyoma from January 2003 to August 2008 were recruited in our hospital.According to the delivery route,size and subtype of fibroid,blood loss,operation hours and postoperative inpatient period were compared.Results:104 cases of pregnancy complicated with uterine leiomyoma were diagnosed before cesarean section(CS).No significant differences on blood losses and operation hours were found between CS group and CS+myomectomy group(P>0.05).The operation heurs of leiomyoma in corpus uteri was significantly shorter than leiomyoma in lower uterine segment and cervix(P=0.007).Leiomyoma bigger than 8 cm needed significantly Ionger operative hours and lose more blood than the smaller leiomyoma.Operation hours,blood loss and postoperative inpatient period were significantly different between submucous leiomyoma and subserosal leiomyoma(P<0.05).Conclusions:Pregnancy complicated with uterine leiomyoma should be diagnosed as early as possible.During cesarean section on when leiomyoma is bigger than 8 cm,locating at lower uterine segment or cervix or submucous,the treatment should be cautious.
2.Femoral head diameter and acetabular anteversion in hip prosthesis replacement

BACKGROUND:Posterior dislocation after total hip replacement is one of the most common complications that second only to loosen, and can affect the stability of the hip joint seriously. There is controversy on the effect of femoral head diameter and acetabular anteversion on the stability of hip joint in clinical study. OBJECTIVE:To investigate the effect of the femoral head diameter and acetabular anteversion on the stability of hip joint after hip prosthesis replacement. METHODS:The femoral head diameter and acetabular anteversion that affect the posterior instability after hip prosthesis replacement were measured on the cadaveric bone. The acetabular shel was inserted at 0°-20° of anteversion at five degree intervals. The femoral heads with different diameters (28, 32 and 36 mm) were used for experiment, the internal rotation degrees to dislocation caused by hip at 90° flexion and 0° adduction and at 90° flexion and 30° adduction were recorded. RESULTS AND CONCLUSION:When the cup anteversion varied from 0° to 15°, there was an increase in the degree of internal rotation angle that could cause hip dislocation with the increasing of femoral head diameter (P<0.05). When the cup anteversion varied from 15° to 20°, the increasing trend was not significant. With the hip at 90° flexion and 0° adduction, and the cup anteversion varied from 15° to 20°, the stability of 36 mm group was significantly higher than that of the 28 mm and 32 mm groups (P<0.01);the stability of 32 mm group was higher than that of 28 mm group when the acetabular anteversion in 10° or more (P<0.05);there was no significant difference in stability between groups when the anteversion less than 10°. With the hip at 90° flexion and 30° adduction, the stability of 36 mm group was significantly higher than that of the 28 mm and 32 mm groups (P<0.01);the stability of 32 mm group was higher than that of 28 mm group when the acetabular anteversion in 10° or more (P<0.05);there was no significant difference in stability between groups when the anteversion less than 10°. The results indicate that the increasing of acetabular anteversion can reduce the occurrence of dip dislocation, but when it is difficult to choice the acetabular anteversion, large-diameter femoral head is preferred to increase the stability of hip joint.
3.Liner exchange into a well-fixed acetabular shell in revision total hip arthroplasty

Objective To investigate clinical outcomes and complications of isolated polyethylene liner exchange for revision total hip arthroplasty.Methods From April 1995 to December 2007,80 patients (93 hips) underwent revision total hip arthroplasty during which only polyethylene liner was exchanged with reservation of acetabular cup.There were 41 males and 39 females,aged from 27 to 82 years (average,53.3 years).The duration from the primary THA to the revision surgery ranged from 0.3 to 18.4 years (average,10.9 years).The reasons for liner exchange included:polyethylene wear with osteolysis (78 hips),polyethylene wear without osteolysis (5 hips),polyethylene wear with stem loosening (4 hips),recurrent dislocation (3hips),infection (1 hip),periprosthetic fracture (1 hip) and liner dislodgement (1 hip).Forty-seven liners were fixed into the old cup using cement,and 46 were fixed with the original locking mechanism.Sixty cross-linked polyethylene liners and 33 conventional polyethylene liners were used.Results All patients were followed up for 5 to 15 years (average,7 years).The average Harris hip score improved from preoperative 86.0±16.9 to 89.4±11.6 at final follow-up.Complications included dislocation (10 hips),infection (2 hips),periprosthetic fracture (1 hip) and liner dislodgement (1 hip).Ten hips underwent rerevision due to different reasons:cup exchange (5 hips),conventional polyethylene wear (2 hips),infection (2 hips) and liner dislodgement (1 hip).Using component loosening as the end point,the 10-year survival rate was 100% in the cement fixation group and 84.8% in the original locking group.Using rerevision as the end point,the 10-year survival rate was 90.4% in the cement fixation group and 65.0% in the original locking group.Conclusion Liner exchange either with cement or original locking mechanism is a safe and successful method.Highly cross-linked polyethylene has a higher wear resistance,which can reduce incidence of osteolysis and improve survival rate of prosthesis.
4.Study on pulverization and extraction of Huanglianjiangtang Tablets

AIM: To make the optimum process conditions of extracting and grinding for Huanglianjiangtang Tablets (Rhizoma Coptidis, Radix et Rhizoma Rhei, etc.). METHODS: Applying fluid jet mill to the root of Radix et Rhizoma Rhei and adopting emodin and chrysophanol as markers, degree of grinding on herbs was studied, optimum extraction of Rhizoma coptidis with Berberine as marker was investigated by orthogonal design. RESULTS: Optinum processing condition was as follow, powdered herbs with diameter less than 10?m macerated in 50% ethanol for 40min , reflux extraction three times and 2hr per time. CONCLUSION: The optimum processes result in high content of active constituents. These processes can be applied rightly in mass production.
5.Expression of adhesion molecules in polymyositis

Objective:To study the role of adhesion molecules in the pathogenesis of polymyositis. Methods:The abnormal expression of adhesion molecules on T cells in peripheral blood and muscle fibers from patients with myositis was analyzed by two colour immunofluoresence and RT PCR methods respectively. Results:The expression of adhesion molecules including lymphocyte function associated antigen 1(LFA 1 ),very late antigen 4(VLA 4) on T cells in peripheral blood and intercellular adhesion molecule l(ICAM 1) on muscle fibers from patients with myositis was markedly higher than that in the healthy control group. Conclusion: These findings suggested that adhesion molecules may be responsible for the migration of T cells and destraction of muscle fibers.
6.Effect of leptin on cytotrophoblast proliferation and invasion.

The effects of leptin on cytotrophoblast proliferation and invasion activity were investigated. Immunohistochemistry was used to determine the placental expression of leptin in first-trimester pregnancy. By using RT-PCR and quantitative real-time PCR, the expression of leptin in cytotrophoblast and the effect of leptin on cytotrophoblast secretion were detected. The potential of cell proliferation, invasiveness and migration was assessed by MTT, Transwell invasion assay and migration assay respectively when the cytotrophoblast was cultured with different concentrations of leptin. The results showed that: (1) Leptin was distributed diffusely around cell membrane, in cytoplasma, and on nuclear membrane of cytotrophoblast; (2) Leptin mRNA was expressed in cytotrophoblast. Ten ng/mL leptin could promote the secretion of cytotrophoblast significantly (P<0.01); (3) After culture with different concentrations of leptin for 24 h or longer, the proliferation of cytotrophoblast was inhibited, while in 24 h leptin could promote cytotrophoblast invasion and migration. Leptin at a concentration of 500 ng/mL could promote cytotrophoblast invasiveness and migration significantly as compared with controls (P<0.05). It was suggested that leptin could inhibit cytotrophoblast proliferation, and promote cytotrophoblast invasion and migration activity.
7.VEGF deficit is involved in endothelium dysfunction in preeclampsia.

This study examined the association of expression of vascular endothelial growth factor (VEGF), a promoter of angiogenesis, with endothelium dysfunction in preeclampsia. The level of VEGF protein and mRNA in the placenta and peripheral blood samples of 30 preeclampsia patients and 30 normotensive pregnant women was measured by immunohistochemistry, real-time reverse transcriptase-polymerase chain reaction (RT-PCR) and enzyme-linked immunosorbent assay (ELISA), respectively. VEGF expression in the human umbilical vein endothelial cells (HUVECs) was blocked by small interfering RNAs (siRNAs). The monolayer barrier function of HUVECs was determined by measuring the fluorescence intensity of BSA that crossed the HUVEC monolayers. The cell proliferation and cell-secreted nitric oxide (NO) level were detected by MTT method and nitrate reductase assay, respectively. The results showed that VEGF was expressed in the syncytiotrophoblasts and endothelial cells of vessels and capillaries in the placenta tissue. The serum level of VEGF in the preeclampsia patients was significantly decreased as compared with that in normal pregnant subjects, although VEGF mRNA expression in the placenta tissue of preeclampsia patients remained still high. Moreover, VEGF deficit could lead to endothelium cell dysfunction, and the administration of VEGF could protect endothelium cells from injury. It was concluded that lack of VEGF contributes to endothelium dysfunction, which may lead to the occurrence and development of preeclampsia.
8.Comparative Study on Pharmacokinetics of Components ofBuyang Huanwu Associated Prescriptions

Objective To study the pharmacokinetic features of ferulaic acid, senkyunolide A and ligustilide in Buyang Huanwu associated prescriptions (Buyang HuanwuDecoction andNaojianTablets).MethodsHPLC-DAD was applied for simultaneous determination plasma concentration of three ingredients with jugular venous cannula rats after intragastric administration ofBuyang Huanwu associated prescriptions. The pharmacolinetic parameters of each ingredient was calculated by DAS2.0, and then the total quantum statistical moment (TQSM) standard similarity was used to measure the overall pharmacokinetics behaviors.Results There were great differences in the three ingredients after the administration of two prescriptions, while the total quantum statistical parameters were very closely. The TQSM pharmacokinetic parameters of the three components inBuyang HuanwuDecoction andNaojian Tablets showed that AUC, MRT, VRT were 240.6 and 133.0, 3.192 min and 3.259 min, 21.59 min2and 19.75 min2, respectively.The similarity was up to 0.977 8.Conclusion The metabolic processes in vivo ofBuyang Huanwu Decoction andNaojianTablets have similarities. The efficacy of Chinese herbal compounds mostly depends on the multi-components overall contributions.

This study investigated the effect of epigenetic modification of maspin on extravillous trophoblastic function. The mRNA expression of maspin in placentae from normotensive and preeclamptic pregnant women was detected by RT-PCR. TEV-1 cells, a human first-trimester extravillous trophoblast cell line, were cultured and treated with CoCl(2) (300 μmol/L) to induce chemical hypoxia and with 5-aza (500 nmol/L) to induce demethylation. The mRNA expression of maspin in TEV-1 cells subjected to different treatments was determined by RT-PCR, and the proliferative and migratory abilities of TEV-1 cells were assessed by cell counting kit-8 (CCK-8) and Transwell assays. Our results showed that the maspin mRNA expression level in placentae from preeclamptic women was much higher than that from normotensive women. CoCl(2) or 5-aza could up-regulate the mRNA expression of maspin and significantly suppress the proliferation and migration of TEV-1 cells. It was concluded that the epigenetic modification in promoter region of maspin contributes to incomplete trophoblast invasion, which offers a novel approach for predicting and treating placental dysfunction.
