2.Modified continuous balloon dilatation substituting stenting in the treatment of benign esophageal stenosis in children

Objective To explore the feasibility of using the modified continuous balloon dilatation substituting the traditional membrane-covered stent in the treatment of benign esophageal stenosis in children.Methods A retrospective analysis of the clinical data of 25 cases with esophageal stenosis from January,2012 to February,2014 was conducted.Preoperative esophageal stenosis was confirmed by angiography,and the expansion of the stenosis segment was examined after 2-4 weeks postoperatively.According to the age and the location of the esophageal stricture,25 cases were classified into two groups.Twelve cases of group A without obvious contraindications were treated by continuous balloon dilatation,while 13 cases of group B with age of less than 3 years or upper esophageal stenosis,retrievable stent implantation was performed.All cases were followed up from 6 months to 2 years.Clinical symptoms,swallowing fluency and the degree of esophageal stricture were followed up at 1,3,and 6 months postoperatively,and the effect of treatment was evaluated.Results Two groups of patients were successfully treated,and the stents were removed successfully after 2-4 weeks.The average diameter of the stenosiswas significantly increased after treatment,swallowing was improved obviously,with no perforation,hematemesis and melena and other serious complications.The balloon moved down happened in one case,which was adjusted under perspective observation.One case in group A underwent colonic interposition for esophageal replacement because of recurrence of restenosis.The balloon dilatation was effective in 10 cases.In Group B,5 patients had stent migration or displacement,3 cases had recurrent esophageal stricture,and the treatment was effective in 5 cases.There was significant difference in complications between the group A and B (x2=5.23,P<0.05).Conelusions Continuous balloon dilatation is a simple,effective procedure with fewer complications in the treatment of children with benign esophageal stenosis.In addition to stenting,it may be another ideal choice for benign esophageal stenosis in children.
3.The effect of adipose derived stem cells on pancreatic stellate cells in vitro

Objective To evaluate a new method for the isolation of rat pancreatic stellate cells (PSCs) and to investigate the influence of adipose derived stem cells (ADSCs) on PSCs in vitro.Methods Normal rat PSCs was isolated by collagenase and Optiprep density gradient centrifugation.The coculture system of ADSCs and PSCs was set up by transwell insert.The proliferation of PSCs was tested by CCK-8 test kit.Smoothmuscle α-actin (α-SMA) expression of PSCs were tested by Western blot.The apoptosis of HSCs was tested by flow cytometer.The cytokines in the culture solution were tested by ELISA kit.Results The quantity of PSCs was above 5 × 106 cells per rat.The purity and the viability of the cells were about 90-97 percent.After coculture for 72 h,the proliferation and activation of PSCs was inhibited by ADSCs (F =223.27,P < 0.05 ; F =52.97,P < 0.05) and the apoptosis of PSCs was promoted by ADSCs (F =43.62,P < 0.05).more NGF and less TGF-β1 was secreted by ADSCs than by PSCs (NGF:14.68 ± 0.94 vs.8.31 ±0.86,t =4.67,P <0.05;TGF-β1:10.65 ±0.46 vs.70.47 ±0.99,t =21.72,P<0.01).Conclusions ADSCs inhibit the proliferation and activation of PSCs by ADSCs secreting cytokines.
4.Effect of shRNA inhibiting hTERT gene expression combined with γ-irradiation on human laryngeal cancer cells

Objective To construct an eukaryotic expression vector of human telomerase reverso transcriptase (hTERT) gene specific shRNA, and investigate the effect of pshRNA-hTERT combined with γ-irradiation on telomerase activity and DNA damage. Methods The recombinant expression plasmid pshRNA-hTERT was constructed and transfected into Hep-2 cells. The telomerase activity was examined by the PCR-hased telomeric repeat amplification protocol (TRAP). DNA single-stranded break (SSB) and the DNA double-stranded break (DSB) were detected by Comet assay. The xenograft model of human laryngeal carcinoma with the same genetic background and different radiosensitivity (Hep-2 and Hep-2R) was established in nude mice. The mixture of pshRNA-hTERT and liposome was injected to the transplanted tumor to observe the inhibition of the tumor growth. The cell apoptosis was detected by TUNEL. The hTERT protein expression was determined by streptavidin-peroxidase conjugated method (AP). Results Recombinant expression plasmid pshRNA-hTERT was successfully constructed and transfected into Hep-2 cells. The hTERT expression inhibition rate reached 60.78 %. pshRNA-hTERT not only inhibited telomerase activity of Hep-2 inehiding the increase of telomerase activity induced by γ-irradiation, but also inhibited the repair of the SSB and DSB induced by irradiation in the human laryngeal carcinoma xenograft in nude mice with the same genetic background and different radiosensitivity. The pshRNA-hTERT combined with γ-irradiation could inhibit the growth of transplanted tumor (Hep-2: EPO = 1.79; Hep-2R: EPO = 2.01) with reduced telomerase activity and hTERT protein expression. Conclusions The eukaryotic expression vector pshRNA-hTERT could enhance the radiosensitivity of Hep-2 cells in vitro and the human laryngeal carcinoma xenograft in nude mice which had the same genetic background with different radiosensitivity.
5.Analysis of coronary artery Z-scores of children with Kawasaki disease on echocardiography

Objective To investigate the clinical value of coronary artery Z-scores on echocardiography in diagnosing coronary artery abnormalities. Methods The echocardiography results of 612 patients with Kawasaki disease (KD) at the acute and recovery phase were retrospectively studied. Coronary artery luminal diameters were converted to body-surface-area-adjusted Z-scores. According to coronary Z-scores classiifcation, all the subjects were divided to four groups:415 cases with no dilation (ND), 133 cases with small coronary artery abnormalities (SCAAs), 47 cases with large coronary artery abnormalities (LCAAs), and 17 cases with giant coronary artery abnormalities (GCAAs). Clinical features (gender, age, typical clinical manifestations, fever duration) and laboratory results (CRP, ESR, WBC, PLT) were compared among all the four groups. Coronary artery diameters and the Z-scores were compared between acute and convalescence phase. Results Along with the increase of coronary Z-score, fever duration was prolonged [ND group:(7.75±3.12) d, SCAAs group (8.50±4.12) d, LCAAs group: (8.57±3.58) d, GCAAs group: (11.88±4.33) d, F=22.375, P<0.05]. With coronary Z-score increasing, PLT also increased (F=22.029, P=0.000), and the highest PLT was observed in GCAAs group. There were no significant differences in the CRP, ESR and WBC among all the four groups (F=0.236, 1.116, 0.121, all P>0.05). No significant different coronary diameters were found in ND cases between recovery and acute phase [(2.24±0.34) mm vs (2.33±0.36) mm, t=1.926, P > 0.05]. But there were significant difference in the coronary Z-scores of ND patients between recovery and acute phase (0.41±0.82 vs 1.17±0.75, t=8.332, P < 0.05). The coronary Z-scores in SCAAs group (1.32±0.89 vs 3.40±0.62, t=11.073, P < 0.05), LCAAs group (3.12±2.27 vs 6.20±1.28, t=4.579, P<0.05) and GCAAs group (11.88±6.77 vs 20.4±9.70, t=3.480, P<0.05) at recovery phase were smaller than values at acute phase. Conclusions The KD coronary Z-scores are the body-surface-area-adjusted standard value, and not subject to the influence of children growth and development. Therefore, it may accurately evaluate the severity of coronary artery abnormalities and its recovery process. Accurate quantitative of the coronary artery luminal dimensions is important in KD clinical management and prognosis prediction.
6.Mouse nerve growth factor combined with rehabilitation in the treatment of global developmental delay: a prospective multicenter clinical randomized controlled trial

Objective:To observe the clinical efficacy of mouse nerve growth factor (mNGF) combined with rehabilitation on children with global developmental delay(GDD).Methods:It was a prospective multicenter clinical randomized controlled trial (RCT) involving 120 children with GDD admitted to 5 hospitals in China from May 2020 to January 2022.They were randomly divided into mNGF group and conventional rehabilitation group using block randomization method.All children were managed by standardized rehabilitation after recruitment, and those in the mNGF group were additionally given mNGF injections.All subjects were surveyed using the Gesell Development Diagnosis Schedules(GDDS) at baseline, 90 days and 120 days after treatment, and their developmental quotient (DQ) was recorded.Clinical efficacy was analyzed by the paired t-test, rank sum test and Chi- squared test. Results:After 90 days of treatment and the continuous follow-up to 120 days, the increases in the DQ of gross motor (7.520±13.900 vs.0.450±11.459), fine motor (7.800±15.346 vs.1.250±11.581), adaptive behavior (7.730±13.428 vs.2.100±12.022) and personal-social behavior (6.780±11.651 vs.1.780±10.120) than baseline were significantly higher in mNGF group than those of conventional rehabilitation group (all P<0.05). Serious adverse events and important drug-related medical events were not reported. Conclusions:mNGF combined with rehabilitation effectively enhances the development levels of gross motor, fine motor, adaptive behavior and personal-social behavior, and continuously improves the condition of GDD in children with a high safety.
7.Hyperuricemic Nephropathy in Traditional Chinese Medicine and Integrated Traditional Chinese and Western Medicine: A Review

Hyperuricemic nephropathy (HN), a secondary renal damage common in clinical practice, is characterized by early concealing and continuous progression. The understanding of HN in traditional Chinese medicine (TCM) is from a macroscopic perspective. According to the TCM theory, HN is caused by the combination of external pathogens and internal injuries, with the main pathogenesis being root deficiency combined with superficial excess and deficiency-excess in complexity. In western medicine, the understanding of HN is from the microscopic perspective, which holds that the occurrence of HN is the result of inflammation, oxidative stress, renin-angiotensin-aldosterone system (RAAS) activation, and metabolic abnormalities. The TCM syndromes of HN include internal dampness and heat, obstruction in dampness and turbidity, deficiency of spleen and kidney, and deficiency of kidney yin. Accordingly, the prescriptions should clear heat and dampness, remove dampness and turbidity, tonify spleen and kidney, and nourish kidney yin, respectively. In addition to TCM prescriptions, single herbal medicines and their extracts, Chinese patent medicines, and external applications of Chinese medicines have played a significant role in the treatment of HN, promoting the application of TCM in the treatment of HN. Moreover, the integrated traditional Chinese and western medicine has also played a role in the treatment of HN, enriching the treatment schemes of HN. Different from common kidney diseases such as acute and chronic glomerulonephritis and nephrotic syndrome, HN with particularity should be carefully differentiated in clinical practice. This article systematically summarizes the research progress in the treatment of traditional Chinese medicine and integrated traditional Chinese and western medicine on hyperuricemic nephropathy with TCM and integrated traditional Chinese and western medicine, aiming to enrich the system and theory of HN treatment and further guide the clinical practice.
